Objective To investigate the clinical significance of kynurenine (KYN) and uric acid (UA) levels in chronic myeloid leukemia patients (CML).Methods 30 CML patients (CML group) and 25 healthy controls (Control group) were enrolled in the hospital from Mar 2014 to Dec 2015.Serum KYN and tryptophan (TRP) of all subjects were determined by high performance liquid chromatography,and KYN/TRP ratio (KTI) were calculated.Leukocytes were counted by automat ic blood cell analyzer.Serum UA were detected by enzyme method.Correlation between UA and KTI,leukocytes were analyzed by Pearson correlation.Results The serum KTI of the CML group (49.76±19.06) before treatment was significantly higher than that of the control group (27.87-±-5.06,t=4.470,P=0.001).KTI cutoff value was 40 by ROC analysis.There had correlation between KTI and leukocytes (r=0.743,P=0.001) and no correlation between KTI and ages (r=0.205,P =0.276).KTI (64.7±17.8) and the leukocytes [(96.7±64.1) × 109/L] of 10 cases of KTI≥40 of CML patients before treatment was significantly higher than that of KTI (27.0± 10.9,t=7.102,P =0.001) and the leukocytes[(10.1 ± 5.2) × 109/L,t=4.285,P=0.002] after treatment.The serum UA in KTI＜40 CML patients (n=15,354.98±103.9 μmol/L) was significantly lower than that of KTI≥40 CML group CML (n=15,484.98±132.1 μmol/L,t=1.432,P=0.045).The UA in CML group was associated with KTI (r=0.573,P=0.001) and leukocytes (r=0.537,P=0.002).Conclusion The increase of KTI in CML patients suggested that an increased activity of indoleamine-2,3-dioxygenase (IDO).IDO activity may associated with CML morbidity and therapy effect.The detection of KTI may contribute to the diagnosis of CML and assessing therapy effect of CML.

OBJECTIVETo explore the perioperative cardiovascular dysfunction and its relevance to age in patients with silent coronary heart disease (or silent myocardial ischemia), and explore the clinical treatment and recovery of perioperative arrhythmias.

METHODSOne hundred and eighty cases were selected from selective surgery patients with silent myocardial ischemia (SMI). Among the cases, 130 patients older than 51 years old were divided into 51 - 60 year-old group, 61- 70 year-old group and 71 - 80 year-old group. Control group was set up by other 50 patients younger than 51 years old. Electrocardiogram data of 24 h before the operation, 24 h after the operation and 48 h after the operation were continuously monitored by dynamic electrocardiogram (DCG). The electrocardiogram data of ST shifting, arrhythmia incidences of different type and at different time were analyzed by professional doctors. At the same time, the treatment and recovery of perioperative arrhythmia were recorded.

RESULTSAs the age increase, the magnitude and duration of ST shifting appeared upward trend compared to the control group (P < 0.05, P < 0.01). The incidence of ST elevation in 71 - 80 year-old group was higher than the control group (P < 0.05). The ST depression duration in 61 - 70 and 71 - 80 year-old group and ST elevation magnitude in 71 - 80 year-old group were higher than 51 - 60 year-old group (P < 0.05). Compared to the control group, the incidence of accelerated idioventricular rhythm (AIR) in 61 - 70 year-old group and the incidence of sinus bradycardia (SB), ventricular premature beat (VPB), ventricular tachycardia (VT) in 71 - 80 year-old group were higher (P < 0.05, P < 0.01). Compared to the 51 - 60 year-old group, the incidence of atrial fibrillation (AF) in 61 - 70 year-old group and the incidence of VP, VT, AF in 71 - 80 year-old group were higher (P < 0.05, P < 0.01). The arrhythmia incidences in 24 h after operation were higher than 48 h after operation and 24 h before operation (P < 0.01). As the age increase, the recovery incidence by removing inducement was decreased, but the recovery incidences by drug and electric-shock treatment were increased (P < 0.05).

CONCLUSIONOld SMI patients have high levels of perioperative myocardial ischemia and arrhythmia, and 24 h after operation is the period of high incidence.

OBJECTIVE: To explore the correlation between syndrome differentiation and CT quantitative diagnosis of fatty liver. METHODS: One hundred and thirteen cases of fatty liver verified clinically accepted plain CT scan after syndrome differentiation (syndrome of stagnation of liver qi and spleen deficiency, syndrome of intermingled phlegm and blood stasis, syndrome of phlegm-dampness blocking internally, syndrome of deficiency of liver and kidney, syndrome of interior dampness-heat), and then the CT values of the liver and the spleen were measured respectively. RESULTS: It was shown that there was a correlation between the syndrome differentiation and the fatty infiltration in the liver. Significant differences of the distribution of fat and the ratio of liver-spleen were found among the five different syndromes (P<0.05). Diffused, mild fatty liver mainly displayed syndrome of stagnation of liver qi and spleen deficiency. CONCLUSION: Syndrome differentiation of fatty liver is correlated with CT quantitative diagnosis, which can be discriminated by the ratio of liver-spleen.

Objective To investigate the clinical characteristics,diagnosis and treatment for Graves' disease associated with papillary thyroid microcarcinoma(PTMC) by retrospectively analyzing domestic and foreign literatures.Methods 32 patients of Graves' disease associated with PTMC treated at the Department of Thyroid Surgery,the First Bethune Hospital of Jilin University from Jun.2008 to Jun.2013 were collected.32 cases of PTMC with normal thyroid function were randomly selected by a random number table method from the same period as control.Results The experimental group had 32 patients aging from 24 to 71 years old,among whom 3 were males,and 29 were females with the male to female ratio was 1∶9.7.15 cases in the experimental group had positive central lymph node metastasis and the other 17 cases were negative,while in the control group 7 cases were positive and 25 cases were negative.The mean number of positive lymph nodes was 3 ± 2.88 in the experimental group and 1.71 ± 1.25 in the control group.The ratio of positive lymph node metastasis(the number of positive lymph nodes/the total number of lymph nodes,and the total number of central lymph nodes was ＞3)was (50.34 ±27.09) ％ in the experimental group and(33.5 ±30.79)％ in the control group.For the above 3 values of central lymph node metastasis,the experimental group were all higher than those of the control group (P =0.035,0.039,0.018 respectively).Conclusions Compared with patients of PTMC,patients of Graves' disease associated with PTMC are more likely to have central lymph node metastasis,more number of positive lymph node and higher metastasis ratio.In order to prevent the postoperative recurrence of Graves' disease and thyroid carcinoma local lymph node,total thyroidectomy plus central lymph node dissection should be the ideal operation method.

Objective To investigate the effects of p-cresol on human umbilical vein endothelial cells.Methods The effects of p-cresol on endothelial cell growth,cell cycle,cell morphological change and p21 protein were detected by the CCK-8 assay,flow cytometry assay,inverted microscope and Western blotting.Results P-cresol could inhibit the growth of human umbilical vein endothelial cells in dose-and time-dependent manners (all P ＜ 0.05).The human umbilical vein endothelial cells treated with p-cresol became elongated processes,cloudy cytoplasm,and irregular shapes.The p-cresol stopped human umbilical vein endothelial cells at cell cycle G1 and had no effect on cell apoptosis.The p-cresol could increase protein expression of p21 in a dose dependent manner (P ＜ 0.05).Conclusion P-cresol can increase protein expression of p21,induce cell cycle arrest at G1 stage and inhibit the proliferation of human umbilical vein endothelial cells.

Objective To probe the X-ray imaging features and clinical manifestations of some rare diseases in male breast.Methods The data of 12 patients with some rare diseases in male breast were collected.The X-ray manifestations and clinical features of the diseases were analyzed.Results The postoperative pathology showed the lipogranuloma in 1 patient,plasma cell mastitis in 2,secondary infection of epidermal cyst in 1,intraductal papilloma in 1,breast cancer in 6 (invasive ductal carcinoma in 4,poor-differentiated adenocarcinoma in 1,and mucinous carcinoma in 1),and breast lymphoma in 1.The digital mammography showed the intramammary solitary masses in 1 1 and multiple ones in 1.10 masses were located at the rear of the areola,and other 2 were opposite.Unclear edge of the lesion was found in 9,spiculation in 1,lobulation in 1,and micro-calcification in 1.Nipple retraction and areola skin thickening in 10 and enlarged axillary lymph node in 1 were also found.The total accurate rate of X-ray diagnosis was 41% (5/12),while that of breast cancer was 83.3%.The total misdiagnosis rate was 59% (7/12),and that of benign lesion was 80% (4/5).Conclusion The diseases in male breast are rare and various,and the X-ray and clinical manifestations are not specific.
