1.Effects of ozonated water injection for patients suffered from inflammatory pain of ankylosing spondylitis

Objective To investigate the efficacy and safety of ozonated water injection for inflammatory pain of ankylosing spondylitis.Methods Forty patients suffered from inflammatory pain of ankylosing spondylit were admitted at the Xishan Coal Electricity Group Co.,Ltd Workers' General hospital from October 2013 to October 2016,and were randomly divided into Group A and Group B,20 cases in each group.Patients in Group A were treated with ozonated water injection into facet joint and sacroiliac joint,while patients in Group B were treated with diclofenac sodium controlled release table as control.Immunosuppressant was taken by all patients in both groups as the basic therapy.The VAS,ratio of VAS less than 3,Schober,Bath ankylosing spondylitis disease activity index(BASDAI),CRP,ESR and side effects were evaluated before treatment and 1 week,1 and 2 months after treatment.Results Before treatment,1st week,1st and 2nd month after treatment,the VAS in group A were (8.20 ± 0.92) points,(2.60 ± 0.79) points,(2.30 ± 0.67) points,(1.50 ± 0.53) points respectively,the Schober were (2.55± 1.62) cm,(3.10± 1.59) cm,(3.35±1.99) cm,(4.10t 1.37) cm,and the BASDAI were (6.30±0.64),(2.68±0.46),(2.08±0.40),(1.56±0.48).Before treatment,1st week,1st and 2nd month after treatment,the VAS in group B were (8.30± 1.06) points,(4.00± 1.05) points,(2.50± 1.18) points,(1.80± 1.03) points,the Schober were (2.50 ± 1.80) cm,(2.85± 1.49) cm,(3.95 ± 1.19)cm,(4.05±0.89) cm,and the BASDAI were (5.64±0.47),(2.98±0.29),(2.20±0.37),(1.70±0.45).The repeated measured repeated measures analysis of variance showed that among three indexes of VAS,Schober and BASDAI,the difference within group was statistically significant (P < 0.05),but there were no significant difference between groups and interaction (P > 0.05).The VAS and BASDAI of patients in both groups were significantly lower at 1st week,1st and 2nd month after treatment as compared to those before treatment,and the Schober of patients in group A at 1st week,1st month,2nd month and that of patients in group B at 1st month,2nd month significantly higher as compared with that before treatment(P<0.05).The ratio of VAS less than 3 of patients in group A were 80%,100% and 100% at 1st week,1st and 2nd month after treatment,that in group B were 40%,80% and 90%.The ratio of VAS less than 3 of patients in group A were significant higher than that of patients in group B at 1st week,1st month after treatment(P<0.05).The CRP in group A were (43.3±12.6)mg/L,(8.46±6.92) mg/L before treatment,1st week after treatment,and that in group B were (43.1 ± 16.9)mg/L,(9.92±6.98) mg/L.The ESR in group A were (38.3±9.4) mm/h,(14.2±5.5) mm/h before treatment,1st week after treatment,and that in group B were (32.0± 10.1) mm/h,(12.8±6.5) mm/h.The CRP and ESR after treatment in two groups significantly lower as compared with those before treatment (P<0.01),but there was no significant difference between group(P>0.05).Four patients (20%) in group A and 11 patients(55%) in group B were suffered from gastrointestinal side effects after treatment,the ratio of group A was significant lower than that of group B (X2 =5.23,P< 0.05).Conclusion Ozonated water injection into facet joint and sacroiliac joint compound immunosuppressant improved pain and joint function,and exhibited a low gastrointestinal side effects.
2.Sorafenib combined with radiotherapy for primary hepatic carcinoma

Conformal radiotherapy is an important therapeutic strategy for primary hepatic carcinoma,which has high local control rate.However,the frequency of intra- and extra-hepatic recurrences is very high.Sorafenib is the only drug that has been demonstrated effectiveness to increase overall survival in advanced or metastatic hepatocellular carcinoma.The combination about radiotherapy and sorafenib can increase the radiosensitiveness,and inhibit the angiogenesis,and relieve the hypoxia,as well as synchronize the cell cycle.A lot of biological studies of sorafenib combined with radiotherapy have been camied out,and the phase Ⅰ -Ⅱ clinical trials are is in progress.

5.Arthroscopic reconstruction of anterior cruciate ligament by Moriya’s method

Objective To report arthroscopic reconstruction of anterior cruciate ligament (ACL) with iliotibial tract autograft and gracilis tendon by Moriya s method and to evaluate its clinical results. Methods The iliotibial tract autograft of about one third length was dissected with the tibial attachment kept. Then the trimmed gracilis tendon was wrapped in the iliotibial tract to reconstruct ACL. 16 patients (12 males and 4 females) were treated in this way. The anterior drawer test and the Lachman s test were all positive before operation, indicating that anteromedial and posterolateral bundles of ACL were totally broken, which was proved by arthroscopy. Results All patients were followed up for 4 to 17 months (mean 9 months). According to the criteria for knee ligament function evaluation set up by Japanese Association of Orthopaedics( JOA) , 10 patients scored ≥ 90 points, 5 patients 80 to 90 points, and 1 patient ≤ 80 points.The excellent and good rate was 93.75% . Conclusion Iliotibial tract autograft transfer with gracilis tendon to reconstruct ACL under arthroscopy boasts convenience in harvesting grafts, simplicity in performance and excellent results, without evidence of damaging extension and flexion tendons of the knee or causing patellofemoral pain. 6.Effect of recombinant growth hormone on the aged patients after abdominal surgery

Objectives: To research the effects of recombinant human growth hormone(rhGH) with total parenteral nutrition(TPN) on the old patients following abdominal surgery. Methods: 26 old patients receiving TPN after major abdominal surgery were selected and distributed to study group(rhGH+TPN, n =13) and control group(TPN only, n =13).For 7 days after operation, rhGH (8 U/d) or normal saline(in control group) were used. The patients' weight, serum albumin, right hand grip, sleep time and the time of incision cicatrized were determined. Results: The increase of weight, level of plasma albumin, grip power of right hand and sleep time were improved more in study group than those in control group. The incision cicatrized time was also shortened in study group. Conclusions: Growth hormone can promote protein synthesis, accelerate incision healing and reduce the postoperative fatigue syndrome.

8.Bone mesenchymal stem cell transplantation for treatment of cerebral infarction in rats

Objective To observe the protective and repairing effects of bone mesenchymal stem cells(BMSCs) transplantation on cerebral infarction in rats and to study the different effect of transplantation at different time points. Methods Middle cerebral artery occlusion(MCAO) models were set up,and the rats were divided into a control group,a group with PBS transplantation and five groups with BMSCs transplantation 3,6,12,24 and 72 h after MCAO,respectively.The volume of infarction area and the neurological severity score(NSS) in all the groups were compared. 9.Prognosis of surgery in multilocular cystic renal cell carcinoma (report of 22 cases)

Objective To evaluate the prognosis of surgery in multilocular cystic renal cell carcinoma (MCRCC). Methods The characteristics of prognosis of surgery in 22 MCRCC cases out of 482 cases of renal cell carcinoma were reviewed.The MCRCC cases accounted for 4.56% of the renal cell carcinoma cases with a male-to-female ratio of 2.67∶1 in the same period.The mean age of the 22 patients was 47 years (age range,32-74 years). Results Of the 22 cases,18 underwent radical nephrectomy and 4 partial nephrectomy.The diameter of the tumors ranged 1.8-11.0 cm (mean 4.8 cm).Histological examination showed that 21 cases were of clear cell carcinoma and 1 of clear cell carcinoma mixed with granular cell carcinoma.Pathological staging showed pT 1N 0M 0 in 19 cases (86.4%),pT 2N 0M 0 in 2(9.1%) and pT 3bN 0M 0 in 1(4.5%).The classification by tumor nuclear grading system showed 5 cases (23%) of G 1 and 17 (77%) of G 2.Twenty patients were followed up for 9-56 months (mean,27.6 months) and 2 were lost of follow-up.Of the 20 patients,19 survived with no evidence of cancer and 1 died of intercurrent disease (upper gastrointestinal massive hemorrhage due to hepatocirrhosis). Conclusions MCRCC represents a distinct subtype of renal cell carcinoma.It is usually with low grade,low stage and is found incidentally.Our results indicate that MCRCC can be cured by surgery.Regardless of tumor size,the prognosis of MCRCC is favorable.
10.Characteristics of pedicle screw fixation for thoracolumbar burst fractures among middle and old aged patients

Objective To discuss methods of pedicle screw fixation for thoracolumbar burst fractures among middle and old aged patients with different degree of osteoporosis, and to improve the clinical result by individualized treatment according to fracture characteristics. Methods There were totally 25 patients, 17 males, and 8 females. These patients with a mean age of 52 years (range, 43-63 years), had suffered from thoracolumbar burst fractures consisted of 1 T11 fracture, 6 T12 fractures, 8 L1 fractures, 7 L2 fractures, 3 L3 fractures. According to Jikeis osteoporosis grading system, 5 patients were of early stage, 7 of stage Ⅰ, 13 of stageⅡ. Posterior stabilization with the short-segment pedicle screw system were given to all the patients, among which 6 RF, 8 AF, 5 USS, 6 Socon were performed. Fifteen patients received anterolateral decompression, and ten patients accepted no decompression at all. Some measures were taken in order to enhance the fixation stiffness for pedicle screw system, such as increasing the screws diameter, using the vertebral body reducer to reduce the depressed vertebral body and so on. To enhance vertebral body strength after fracture union, transpedicular bone graft was also performed. Results All the patients achieved fracture union during the follow-up with a mean period of 28 months (range, 9-48 months). All of the fractures were reduced satisfactorily, SI (sagittal index) was improved from 53% to 89% after operation. No fracture of screws or rods occurred, also no screw loosened. No reduction loss was found. Conclusion In order to decrease failure rate of pedicle screw fixation for thoracolumbar fractures among middle and old aged patients, osteoporosis of vertebral body should never be neglected and, of course, treatment should be given with individualized fixation.
