Objective To report arthroscopic reconstruction of anterior cruciate ligament (ACL) with iliotibial tract autograft and gracilis tendon by Moriya s method and to evaluate its clinical results. Methods The iliotibial tract autograft of about one third length was dissected with the tibial attachment kept. Then the trimmed gracilis tendon was wrapped in the iliotibial tract to reconstruct ACL. 16 patients (12 males and 4 females) were treated in this way. The anterior drawer test and the Lachman s test were all positive before operation, indicating that anteromedial and posterolateral bundles of ACL were totally broken, which was proved by arthroscopy. Results All patients were followed up for 4 to 17 months (mean 9 months). According to the criteria for knee ligament function evaluation set up by Japanese Association of Orthopaedics( JOA) , 10 patients scored ≥ 90 points, 5 patients 80 to 90 points, and 1 patient ≤ 80 points. The excellent and good rate was 93.75% . Conclusion Iliotibial tract autograft transfer with gracilis tendon to reconstruct ACL under arthroscopy boasts convenience in harvesting grafts, simplicity in performance and excellent results, without evidence of damaging extension and flexion tendons of the knee or causing patellofemoral pain. Purpose To evaluate the application of fluorescence in situ hybridization ( FISH) in detection of EWSR1 gene translocation in formalin-fixed paraffin-embedded tissue samples of Ewing family tumor ( EFT) . Methods Four cases of EFT were collected and their clinical pathological features were obsered. Interphase FISH was carried out in paraffin-embedded tissue of EFT cases, using a EWSR1 dual colar break-apart probe. Fifteen cases of other soft tissue tumor were selected as negative control. Results Translocation of EWSR1 was detected in four EFT cases, which may be a adjuvantly diagnositic value for the diagnosis of atypical cases. Conclu-sions FISH may serve as an adjunctive diagnostic tool in problematic cases of EFT, however, FISH results should be interpreted con-cerning clinical pathological features, morphological patterns and immunohistological results.

Objective To investigate the clinical efficacy and indication of surgical treatment of L5S1 tuberculosis us?ing a medial edge of rectus abdominal muscle and retroperitoneal approach. Methods From Jan. 2010 to Nov. 2014, totally 18 cases of L5S1 tuberculosis were surgically cured by debridement, fusion and internal fixation via a medial edge of rectus ab?dominal muscle and retroperitoneal approach. There were 10 males and 8 females, with an average age of 35.6±10.1 years (21 to 55). The mean course of disease was 5.3±2.2 months. The primary destruction lesions were located at L5S1, and one patient of whom L4 was also involved, three cases S2 involved, and one S3 involved. Abscess pre sacrum was noticed in all cases. Six patients were combined with abscess in vertebral canal. Neural deficiency was detected in 3 patients, all grading as Frankel D. Dubousset's lumbosacral angle was 104°-126°, mean 114.6°±6.0°. Formal anti?tuberculosis medicine treatment was given for at least 2-3 weeks. All patients were treated using a medial edge of rectus abdominal muscle and retroperitoneal approach. Af?ter debridement, a tri?cortical iliac bone was used for structural inter?vertebrae fusion. The iliac bone was fixed to the sacrum by one cortical screw. Operation time, volume of blood loss, and surgery complications, including wound infection, looseness and shift of internal fixation, sinus occurrence, vascular injury, retrograde ejaculation in male patients were evaluated. The X?ray and 3D CT of lumbosacral spine, together with ESR and CRP were used to evaluate the control of tuberculosis during fol?low?up. The Dubousset's lumbosacral angle, visual analogue score, and the Frankel grade were compared between preoperative and postoperative. The bone graft fusion were also observed. Results The operation time was 147±16 min in average. The mean blood lose was 275 ± 77 ml. No vascular or neurological injury was noticed. There was also no retrograde ejaculation happened in this group. All patients were followed up from 18 to 28 months. Dubousset's lumbosacral angle was 122.4°±3.8° post?operatively and 121.4°±3.7° at the final follow?up. There was a significant difference between the lumbosacral angles pre and post operation, whereas no statistical difference between the post operation and the final follow?up. Bone fusion was noticed in all cases. All pa? tients have intact neurological function at the final follow?up. No screw breakage or graft bone migration occurred. VAS was 5.6± 1.09 pre?operation and 0.83±0.71 at the 3rd month's follow?up, and there was a significant difference between them. Conclusion Surgical treatment of L5S1 tuberculosis by debridement, fusion and internal fixation via a medial edge of rectus abdominal muscle and retroperitoneal approach is an effective method in terms of minimal trauma, low rate of modality, good ability in restore and maintenance of lumbosacral junction alignment and high fusion rate.

Objective To explore the multiple risk factors for family lifestyle of children with congenital heart defects (CHDs) in Shaanxi Province, China. children and their parents. The univariate and multivariable logistic regression models were used to analyze the influence of risk factors related to parents' lifestyle on CHDs. Results Several possible risk factors were found for CHDs, including fever (OR=4.465, P=0.017), pesticides contact (OR=2.234, P=0.083), passive smoking during pregnancy (OR=20.529, P=0.007) and father's smoking (OR=3.342, P=0.005); fever (OR=2.428, P=0.012) and passive smoking during pregnancy (OR=1.201, P=0.037) were also correlated with ventricular sepal defect (VSD). Conclusion Fever, pesticides contact and passive smoking are associated with CHDs during pregnancy. We should focus our attention on health care during pregnancy to avoid the above-mentioned risk factors and call on parents to hold on to a good healthy lifestyle.

BackgroundCorneal graft reject is a major cause of corneal transplantation failure.Although many immune-suppressing drugs have been utilized to reduce the reject response, their adverse effects on organ and tissue are still insoluble. The tolerance induction of natural killer T (NKT) cells is currently under investigation. However, the study on the application of NKT cells in high risk corneal transplantation is seldom.ObjectiveThe present study was to explore the effects of α-GalCer-activated NKT cella on allografts survival after high-risk corneal transplantation surgery via retro-bubar injection. Methods The lymphocytes were picked up from the spleen of SPF Lewis rats and cultured in RPMI 1640 medium with 100 mg/L α-GalCer.After one week,NKT cells were sorted by the FACSVantage system as CD161+ TCR-α+ cell from the lymphocytes with the cell densities 5×106/ml.Ten SPF Fisher344 rats were used to prepare the donor corneas, and 20 Lewis rats served as recipients. The high risk corneal transplantation models were created by corneal suturing in 20 recipient rats.Penetrating keratoplasty (PKP) was performed in the model rats.0.1 ml NKT cells or the same volume of normal saline solution were retro-bubarly injected at the end of surgery respectively. The corneal allografts were observed and scored based on Holland criteria at the three-day interval under the slit lamp for 30 days. Two weeks after surgery, three rats from each group were sacrificed by excessive anesthesia method and the eyeballs were obtained for histopathological examination. The inflammatory cell infiltration ( CD4+ and CD8+ ) in grafts was evaluated by immunochemistry and flow cytometry. The use of the animals complied with the Statement of ARVO.ResultsThe mean survival time of the allografts was (7.90± 1.37) days in normal saline solution group and (14.70± 1.49) days in NKT cell group, showing a statistically significant difference between the two groups ( t =10.61,P =0.00 ).Two weeks after surgery, all the allografts showed the severe opacity with lots of new blood vessels and edema in normal saline solution group. However, the corneal grafts were clear in NKT cell group.Abundant CD4+ and CD8+T lymphocytes were seen in the allografts in normal saline solution group, but the inflammatory cells were obviously less in NKT cell group. The percentage of NKT cells in the spleen was (5.67±0.25)% in NKT cell group and ( 1.21±0.19)% in normal saline solution group ( t =8.43,P =0.00 ).Conclusionsα-GalCer-activated NKT cells can prolong the survival time of allografts in high-risk corneal transplantation.Retro-bubar injection of α-GalCer-activated NKT cells probably is a new approach to the prevention of the rejection of corneal transplantation.

Chinese Journal of Integrated Traditional and Western Medicine 2007;27(6):545-547
OBJECTIVETo study the effect of Shenqi Fuzheng Injection (SQFZI) on erythrocyte immune function in patients after cardiopulmonary bypass (CPB).
METHODSTwenty patients ready for receiving CPB were randomly assigned to two groups, 10 in each group. Patients in the SQFZI group were treated with 250 mL SQFZI via intravenous dripping starting from 5 days before operation and after ending CPB once a day. Peripheral venous blood samples of all patients were collected at the time points of before anesthesia, and 6 h, 24 h, 72 h and 7 days after CPB, which were anticoagulated with heparin for dynamically detecting the changes of RBC-C3bRR and RBC-ICR.
RESULTSRBC-C3bRR and RBC-ICR in the SQFZI group were significantly higher at 6 h, 24 h, 72 h, and day 7 after operation than those in the control group respectively at the corresponding time points, and they restored to the normal levels at day 7, suggesting the erythrocyte immune function after CPB in the SQFZI group was superior to that in the control group (P < 0.05).
CONCLUSIONApplication of SQFZI in the peri-operative period can significantly improve the hypo-immunological function of erythrocyte caused by CPB, and promote the recovery of erythrocyte immunity.
