1.Lumbar fusion research comparing the effect of unilateral and bilateral internal fixation for lumbar disc herniation

Objective:To compare lumbar fusion effect of unilateral and bilateral internal fixation for lumbar disc herniation, to explore more effective, reliable treatment. Methods:112 cases of lumbar selects my courtyard 2007 April-2010 year in April admitted to the disc herniation were randomly grouped, unilateral group and bilateral group with 56 cases in each group, respectively, to take unilateral or bilateral internal fixation for the treatment of lumbar fusion operation, compared two groups of patients and the therapy effect. Results:the unilateral group operation time, amount of bleeding during the operation and hospitalization expenses below the bilateral group, there was significant difference(t=8.857, t=9.620, t=12.338;P<0.05), the two groups had no significant difference (P>0.05);the two group patients after VAS and ODI were significantly decreased (P<0.05), before and after the treatment of VAS and ODI the comparison between the two groups were not statistically significant difference (P>0.05);the unilateral group at the end of the follow-up of 50 cases of intervertebral fusion, the fusion rate was 96.2%, the bilateral group interbody fusion in 52 cases, the fusion rate was 100%, two groups of patients at the end of the follow-up intervertebral fusion rate has no obvious statistical difference (x2=0.372, P<0.05);unilateral group were no significant complications, bilateral group were found 1 cases of intervertebral fusion device implant bone absorption, 1 cases highlight the root symptoms, complication rate was 3.8%, the two groups of patients with complications were no significant statistical difference. Conclusion:Lumbar unilateral and bilateral internal fixation can effectively improve the patients with lumbar disc herniation pain symptoms, and has good fusion degree, high security, and unilateral internal fixation operation time is shorter, less blood loss and the burden of patients is smaller, need operation treatment for severe lumbar disc herniation, fusion and unilateral internal fixation for the treatment of recommend the use of lumbar.
2.Nosocomial Alcaligenes xylosoxidans Infection

OBJECTIVE To investigate the clinical distribution and resistance of Alcaligenes xylosoxidans,discussing risk factors of nosocomial infection and measures of prevention and treatment.METHODS We retrospectively analyzed 1323 isolated strains of Al.xylosoxidans in our hospital from Sep 2004 to Aug 2006.RESULTS For specimen from which Al.xylosoxidans was examined,sputum accounted for 99.85%,being the highest percentage.Al.xylosoxidans was especially distributed at respiratory ward(46.94%) and intensive care unit(ICU) ward(12.77%).Then was cardiovascular ward,endocrinology ward and hematology ward.The antibiotic drugs which had the high drug-resistance rate(i.e.more than 98%) were aminoglycoside(amikacin and gentamicin),quinolone(ciprofloxacin) and cefepime.But the drug-resistance rate to carbapenems,?-lactam/?-lactamase inhibitor combinations and the third generation cepholosporins(cefoperazone and ceftazidime) was less than 5%.CONCLUSIONS It is important for clinic to strengthen the disinfection for hospital environment and to use antibiotic drugs reasonably in order to control colonization and spread of Al.xylosoxidans in hospital.
3.Clinical Study:Concurrent Interventional Cisplatin Ieucovorin Fluorouracil Hydroxy Camptothecin Chemotherapy Plus Radiotherapy in Patients with Ⅳ Stage Middle-lower Section Esophageal Cancer

Objective To investigate the efficacy of concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy in patients with Ⅳ stage middle-lower section esophageal cancer.Methods The subjects were all the patients with Ⅳ stage middle-lower section esophageal cancer.Among of them,42 patients,via artery,had been enrolled for concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy.40 patients were given irradiation.Results The incidence rate of Ⅰ grade esophageal X-ray in the former group was 57.1%,and that in the later group was 25.0%,and the parameters had significant difference(?0.05).Conclusion Concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy may be attributed to progression of Ⅳ stage middle-lower section esophageal cancer.

6.3D printing personalized implant manufactured via fused deposition modeling: an accuracy research.

OBJECTIVEThe aim of this study was to determine the accuracy of personalized implant fabricated via 3D printing and fused deposition modeling technique (FDM) and to compare the results with a real tooth.
METHODSSix prepared extracted orthodontic teeth (in vivo) were scanned via cone beam computed tomography (CBCT) to obtain 3D data and to build the data models by using Mimics 15.0 software. The extracted orthodontic teeth (in vitro) and the personalized implants designed via 3D printing and FDM were scanned via CBCT to obtain data and to build the data models at the same parameters. The 3D deviations were compared among the in vivo teeth data models, in vitro teeth data models, and printing personalized implant data models by using the Geomagic studio software.
RESULTSThe average deviations of high and low areas between date models of in vivo teeth and personalized implants were 0.19 mm and -0.16 mm, respectively, and the average deviations between in vitro and in vivo teeth were 0.14 mm and -0.07 mm, respectively. The independent t test showed that no statistically significant difference was observed between the two groups (P>0.05).
CONCLUSION1) The personalized dental implants were manufactured via 3D printing and FDM with a high degree of precision. 2) Errors between the data models of in vitro and in vivo teeth were observed at the same CBCT parameters.

7.Effect of facilitation antagonism acu-therapy in early rehabilitation of patients with cerebral thrombosis

ObjectiveTo study the effect of facilitation antagonism acu-therapy in early rehabilitation of patients with cerebral thrombosis.Methods60 patients with cerebral thrombosis were divided randomly into two groups, Group A received facilitation antagonism acu-therapy, Group B received traditional acu-therapy. Fugl-meyer(FMA), MBI, etc were applied to assess effect before and after treatment.ResultsBoth groups had a great improvement after treatment(P<0.01).The effect in Group A was better than that in Group B compared in two groups (P<0.01 or P<0.05).ConclusionThere is effective in early rehabilitation of patients with cerebral thrombosis using facilitation antagonism acu-therapy.It is better than traditional acu-therapy.
8.Extended pancreaticoduodenetomy combined with mesentery root resection in treatment of patients with pancreatic and duodenal malignancy involving root of mesentery

Objective:To search for a method for radical resection of pancreatic and duodenal malignancy involving the mesentery root and for the long post-operation survival of patients.Methods:From Jan.2004 to Aug.2006,a total of 26(16 male and 10 female. aged 27-70)patients with pancreatic and duodenal malignancy involving the mesentery root were treated in our department.The patients included 3 with duodenal malignancy and 23 with pancreatic malignancy.Curative resection was performed by the extended pancreaticoduodenetomy(Whipple procedure)combined with mesentery root resection(MRR)for all patients.The outcomes,safety and the post-operation survival rate were analyzed retrospectively.Results:Thirteen patients were treated with Whipple procedures combined with MRR,9 were treated with partial portal vein/superior mesenteric vein(PV/SMV)and reconstruction of the vessel,and 4 patients received pre-shunt between PV and SMV with artificial vessel graft before the extended Whipple and MRR procedures.The operation time was 2.5 to 7(4.4?1.1)hour,and blood loss was 300 to 5 000(1892?1414)ml with the blood transfusion of 0 to 5 600(2 100?1 586)ml.There was no death in our group and 7(27%)had post-operation complication.The post-operation hospital stay was 10 to 30 days.The pathologic examination showed negative surgical margins for all specimens.The tumor size was 4 to 10 (6.17?2.03)cm.After a follow-up of 9 to 38 months,the pain was relieved in all patients.One of the 3 patients with duodenal adenocarcinoma had liver metastasis at 10 months after operation,and the other 2 survived 10 months and 27 months without evidence of tumor reccurence.The patient with pancreatic micro-adenocarcinoma died of local reccurence 9 months after operation.The patient with neuroendocrine carcinoma died of organ failure 24 months after operation.The patient with lymphoma have survived for 24 months after operation.The 1-year and 2-year accumulated survival rates in the 20 cases with pancreatic ductal cancer were 86.6% and 45.6%. respectively.Conclusion:The extended Whipple procedure with MRR is safe and effective.It can obtain R0 resection in patients with malignant tumors(over 5 cm in diameter)in the head,neck and uncinate process of the pancreas and duodenal.
9.Study on prediction of compound-target-disease network of chuanxiong rhizoma based on random forest algorithm.

To collect small molecule drugs and their drug target data such as enzymes, ion channels, G-protein-coupled receptors and nuclear receptors from KEGG database as the training sets, in order to establish drug-target interaction models based on the random forest algorithm. The accuracies of the models were evaluated by the 10-fold cross-validation test, showing that the predicted success rates of the four drug target models were 71.34%, 67.08%, 73.17% and 67.83%, respectively. The models were adopted to predict the targets of 26 chemical components and establish the compound-target-disease network. The results were well verified by literatures. The models established in this paper are highly accurate, and can be used to discover potential targets in other traditional Chinese medicine ingredients.

10.Clinical effect of arthroscopic debridement and infusion-drainage on septic arthritis after arthroscopic anterior cruciate ligament reconstruction.

OBJECTIVETo investigate therapeutic strategy on septic arthritis after arthroscopic anterior cruciate ligament reconstruction.
METHODSThe clinical data of 6 cases of septic arthritis after arthroscopic anterior cruciate ligament reconstruction in our department from March 2005 to February 2014 were analyzed. All the patients were male,ranging in age from 18 to 36 years old. After operation, the knee joint became painful and swollen, and ESR and CRP were both increased. Culture of joint fluid allowed the recovery of staphylococcus epidermidis. The patients were dealt with arthroscopic debridement and infusion-drainage. The clinical results were evaluated by Lysholm rating system and range of motion.
RESULTSThe infection of all the patients was controlled. The ESR and CRP both recovered to normal level. The score of Lysholm rating system ranged from 85 to 95,and the range of motion was 120 to 135 degree.
CONCLUSIONArthroscopic debridement combined with infusion-drainage is effective in septic arthritis after arthroscopic anterior cruciate ligament reconstruction.
