1.CT manifestations of lymph node metastasis of thyroid carcinoma

Objective To study the CT manifestations of metastatic lymph node of thyroid carcinoma. Methods CT appearances of metastatic lymph node of thyroid carcinoma proved by surgery and pathology in 108 patients were reviewed. Results Of these 108 cases, metastatic lymph nodes were located at superoir and middle internal jugular chain(n=76), inferior internal jugular chain and super clavicular region(n=86), tracheoesophageal groove(n=52), and superior mediastinum(n=17). Of 84 thyroid papillary carcinoma patients, the attenuation of metastatic lymph nodes were similar to that of normal thyroid gland(n=16), with cystic formations (n=24), intracystic high attenuation papillary-like nodules (n=18), and fine granular calcifications (n=11). Of 24 follicular carcinoma, medullary carcinoma, and clear cell carcinoma patients, 17 cases had significant homogeneous or heterogeneous enhanced nodes, and the attenuation was the same as primary or recurrent thyroid tumors. Conclusion For thyroid carcinoma, the most common locations of metastatic lymph nodes were internal jugular chain, tracheoesophageal groove, and superior mediastinum regions. Marked enhancement similar to normal thyroid gland, cystic formations with intracystic high attenuation papillary-like nodules, and fine discrete granular calcifications were the characteristic manifestations of metastatic lymph node of papillary carcinoma. Marked homogeneous or heterogeneous enhancement after contrast administration and the same attenuation as the primary or recurrent thyroid tumor were found in follicular carcinoma, medullary carcinoma, and clear cell carcinoma metastases.
2.EFFECT OF OXYFEDRINE ON RABBIT PLATELET AGGREGATION AND MDA FORMATION INDUCED BY THROMBIN

Oxyfedrine (Oxy ) exhibited inhibitory effect on washed rabbit platelet aggregation and MDA production induced by thrombin (4 U /ml) . The concentration of Oxy causing 50% inhibition of platelet aggregation induced by thrombin was 254 ?mol/L . Up to 2 nig/ml, acetysalicylic acid (ASA) could not antagonize platelet aggregation induced by thrombin 。 However, ASA 50 ?g/ml reduced the generation of MDA markedly. These results suggest that anti-aggregation of Oxy may be related to inhibition of PAF formation and the effect of Oxy on MDA generation may be due to the inhibition of arachi-donic acid release.

5.Application of via-anal ileus tube drainage in the therapy of acute low colorectal obstruction caused by cancer in elderly patients

Objective To evaluate the role of via-anal ileus tube drainage in the therapy of acute low colorectal obstruction caused by cancer in elderly patients.Methods Totally 31 elderly patients with acute obstruction caused by colorectal cancer were treated by this modality.With the help of colonscopy and radiograph,the ileus tube was inserted into the proximal bowel of the obstructive side via anus.Then we irrigated and drain through the tube.At the same time we observed the patients' symptoms,abdominal girth,plain abdominal radiograph.After the relief of the acute obstruction,operations were processed.Results After 7-14 days treatment,the acute obstruction was relieved and emergency operation was avoided.The cure rate of 1-staged resection was 93.5% (29/31),the rate of 1-staged anastomosis was 68.9% (20/29),the rate of complication was 19.4% (6/31),no anastomotic leakage and no perioperative death occurred.Conclusions The via-anal tube drainage in the treatment of the elderly patients with acute obstruction is safe,effective,and the trauma is small.It would be the first choice to relieve the acute obstruction in future.

8.Prevention of oral ulcer in radiotherapeutic patients after tongue squamous cell carcinoma opera-tion

Objective To investigate the preventive effect of oral ulcer using two methods in radio-therapeutic patients after tongue squamous cell carcinoma operation. Methods Thirty-six radiotherapeutic patients after tongue squamous cell carcinoma operation were collected from May 2012 to May 2014 and divided into control group and treatment group according to the random number table method. Regular oral nursing was administered to control group, treatment group were received regular therapy plus orally vitamin B and cetylpyridinium chloride gargle for gargling, and then evaluated the efficacy according related standards. Results Less oral ulcer (1/18 vs.7/18,χ2=-2.372,P<0.05), better oral lustration (Z=-2.093,P<0.05) and overall oral nursing effect (Z=-2.166,P<0.05) were observed in treatment group compared to control group after 7 days of therapy, however, treatment group presented no significantly better comfort level at the same time (Z=-1.708,P>0.05). Conclusions Regular therapy plus orally vitamin B and cetylpyridinium chloride gargle for gargling are effective for the prevention of oral ulcer in radiotherapeutic patients after tongue squamous cell carcinoma operation.
9.Regulatory effect of vitamin D on the expression of occludin in the intestinal epithelial cells of neonatal ;rats with necrotizing enterocolitis

Objective To investigate the expression of occludin in the intestinal epithelial cells of neonatal rats with necrotizing enterocolitis ( NEC) . The influence of vitamin D on NEC and the expression of occludin was also explored. Methods Sixty neonatal Wistar rats from different litters were divided into four groups 48 hours after birth:10 in the breast-fed+ control group,10 in the breast-fed +vitamin D group,20 in the NEC+ control group and 20 in the NEC+ vitamin D group. In groups with NEC,neonatal rats were sep-arated from their mothers and treated with rat milk substitute+hypoxia+hypothermia. In groups with vitamin D treatment,paricalcitol was given through intraperitoneal injection 30 minutes before,one day and two days after the induction of NEC. The tissue was harvested from the ileum of the rats 72 hours after induction of NEC. Microscopically,the tissue was assessed according to the Nadler scoring system. The expression and location of occludin was observed after immunofluorescence staining. Western blotting was performed and quantified to compare the expression of occludin among groups. Results In NEC rats,the intestinal structure was destructed. Separation of the submucosal or muscular layer,discharge or disappearance of the villa was accompanied by necrosis of the intestinal cells. Vitamin D treatment could significantly alleviate those chan-ges,with the Nadler score 1. 70 ± 0. 21 compared to 2. 90 ± 0. 23 without vitamin D treatment ( P<0. 01 ) . The expression of occludin was decreased in NEC rats and vitamin D could partly rescue this decrease (P<0. 01). The distribution of occludin was sparse and discontinuous in NEC rats while after vitamin D treatment,the distribution was maintained even and continuous. Conclusion The expression of occludin in intestinal epithelial cells is significantly reduced in NEC rat. Vitamin D treatment may inhibit the development of NEC by maintaining the expression of occludin.
