1.Quality evaluation on different specifications of cervi cornu pantotrichum with its effect on ovariectomized osteoporosis model rats.

OBJECTIVETo study the effect of eight specifications of Cervi Cornu Pantotrichum on the osteoporosis of ovariectomized rats and grade the eight different specifications of Cervi Cornu Pantotrichum.
METHODTotally 100 SD female rats were divided randomly into 10 groups, namely the normal group, the model group and eight Cervi Cornu Pantotrichum groups of different specifications. Their bilateral ovaries were excised to reproduce the osteoporosis model. Meanwhile, the rats were given the eight different specifications of Cervi Cornu Pantotrichum for consecutively 12 weeks. Subsequently, the effects of the different specifications of Cervi Cornu Pantotrichum on bone mineral density and serum biochemical indicators of rats were observed. A clustering analysis was made for the eight specifications of Cervi Cornu Pantotrichum, with the serum content of ALP, BMP-2 and BGP as influencing factors.
RESULTAfter 12 weeks, the eight different specifications of Cervi Cornu Pantotrichum could significantly improve ALP, BMP-2, BGP in serum and bone mineral density of ovariectomized rats. And the cluster analysis showed similar results to the quality classification of traditional commercial herbs Cervi Cornu Pantotrichum.
CONCLUSIONDifferent specifications of Cervi Cornu Pantotrichum can antagonize the osteoporosis of ovariectomized rats, and their effects are related to the quality of commercial herbs.

2.Drug Resistance Related Gene of Carbopenem Resistant Pseudomonas aeruginosa and Type Ⅰ Integrating Enzyme Gene

OBJECTIVE To sudy drug resistance related gene of carbopenem resistant Pseudomonas aeruginosa and type 1 integrating enzyme gene.METHODS With PCR method to detect and analyze carbopenem resistant P.aeruginosa pertinent metal ?-lactamase IMP,VIM,SPM,GIM genes and cell membrane protein oprD2 gene and six main drug resistance genes of type Ⅰ integrating enzyme gene and five others.RESULTS Fifty one strains of imipenem and meropenem resistant P.aeruginosa SPM,GIM metal enzyme genes were detected to be negative,16 strains of IMP and 5 strains of VIM type metal enzyme were all positive,14 strains of carbopenem resistant P.aeruginosa oprD2 gene were positive,other 37 strains of P.aeruginosa oprD2 gene were negative,7 strains produced metal enzyme and cell membrane protein oprD2 gene were deleted at the same time,49 strains with type Ⅰ integrating enzyme gene intⅠ1 were positive.CONCLUSIONS It is indicated the gene deletion of P.aeruginosa outer membrane protein OprD2 is important mechanism of carbopenem resistant P.aeruginosa,the second is metal enzyme,type Ⅰ integrating enzyme exists largely in P.aeruginosa.This hints the monitoring that drug resistance gene spreads and propagates in bacterium strain must be reinforced.
3.Application of transvaginal sonography and human chorionic gonadotropin for the final diagnosis of pregnancy of unknown location

Objective To evaluate the outcomes of pregnancy of unknown location (PUL) with serial of serum β-human chorionic gonadotropin (β-hCG) and transvaginal sonography.Methods A retrospective study was performed to analyze the relationship between serial serum β-hCG and transvaginal sonography among 315 women who were diagnosed with nonvisible intrauterine pregnancy during Jan 2015 to Dec 2015.Results The pregnancy outcomes of 315 women included 254 intrauterine pregnancies,43 ectopic pregnancies,and 18 spontaneously resolving miscarriages or spontaneously resolving ectopic pregnancies.The serial serum β-hCG curve was classified as rising hCG levels,declining or plateauing hCG levels.Different pregnancy outcomes could exhibit as rising or declining hCG curves.Conclusions It is critical to perform two or more serial serum β-hCG examinations followed by transvaginal sonography to establish the correct diagnosis for PUL.

5.Clinical effect of ulinastatin in preventing pancreatitis after endoscopic retrograde cholangiopancreatography:a Meta -analysis

Objective To investigate the clinical effect of ulinastatin in preventing pancreatitis after endoscopic retrograde cholangiopancre-atography (ERCP).Methods The Cochrane Library,PubMed,EMBASE,CNKI,VIP,and Wanfang Data were searched for randomized controlled trials (RCTs)on ulinastatin for the prevention of post -ERCP pancreatitis published from 1970 to June 2016.Two researchers se-lected RCTs,extracted data,and evaluated methodological quality independently,and RevMan 5.3 software was used for the meta -analy-sis.The chi -square test was used for the heterogeneity analysis of RCTs included,and the funnel plots were used to evaluate publication bi-as.Results A total of six RCTs with 923 patients were included in this analysis.Compared with the placebo,ulinastatin had significantly better effects in preventing post -ERCP pancreatitis (OR =0.26,95%CI:0.13 -0.53,P =0.000 2),hyperamylasemia (OR =0.47, 95%CI:0.33 -0.67,P <0.001),and abdominal pain (OR =0.56,95%CI:0.34 -0.91,P =0.020).Compared with gabexate,uli-nastatin had similar effects in preventing post -ERCP pancreatitis,hyperamylasemia,and abdominal pain (P =0.52,0.13,and 0.79);low -dose ulinastatin also had similar effects as gabexate in preventing post -ERCP pancreatitis and hyperamylasemia (P =0.49 and 0.25).The funnel plots based on the effect of ulinastatin in preventing post -ERCP pancreatitis were slightly asymmetric,which suggested the presence of publication bias.Conclusion Ulinastatin (≥15 ×104 U)can effectively prevent post -ERCP pancreatitis,hyperlipi-demia,and abdominal pain in the general population and it is recommended to start using this drug before surgery.
6.Tiotropium and salmeterol xinafoate and fluticasone propionate powder for inhalation combined with non-invasive ventilation in respiratory failure

Objective To explore the clinical efficacy of tiotropium and salmeterol xinafoate and fluticasone propionate powder for inhalation combined with noninvasive ventilator in the treatment of respiratory failure, and to provide reference for clinical treatment.Methods 72 patients with respiratory failure were randomly divided into treatment group (n=36) and control group (n=36).The patients were randomly divided into treatment group and control group.Patients in the control group were treated with conventional noninvasive ventilators and treatment group were treated with tiotropium and salmeterol xinafoate and fluticasone propionate powder for inhalation on the basis of control group.Then compare the exercise tolerance, arterial carbon dioxide partial pressure, heart rate, pH, respiratory rate, blood oxygen pressure between two groups.Results After the treatment, condition of the patients in the two groups have different degrees of improvement.The final results showed that the average hospital stay was significantly reduced, respiratory frequency was(28 ±5) times/min in treatment group, heart rate was(98 ±16) times per minute, PaO2 also increased to(72 ±3) mmHg, PaCO2 was(57 ±17) mmHg, pH value increased to (7.42 ±0.03), compared with the control group, with statistically significant difference (P<0.05).Conclusion In patients with clinical implementation of tiotropium and seretide combined with non-invasive ventilator treatment of respiratory failure, can significantly improve the patient's condition, so that patients with respiratory function was significantly improved, so that the indicators of patients tend to normal.Its can effectively reduce PaCO2 , increased PaO2 and speed up its cure rate.
7.Toxicological Safety Assessment on Artificial Mineral Water Pills

Objective Artificial mineral water pills were made by loading many kinds of trace elements into the ceramic carrier.Pure water could be mineralized after dipping the pills into the water.To assess the possible hazardous effects on the human health induced by the pills,a series of hygienic and toxicological experiments on the pills were carried out.Methods Acute oral toxicity test,micronucleus test of mice bone marrow cells,sperm abnormality test in mice and Ames test were con-ducted.Contents of trace elements,heavy metals and radioactivity in the mineralized water were determined according to the examination methods in Standard Examination Methods for Drinking Water and Examination Methods for Drinking Natural Min-eral Water.Results The contents of trace elements,heavy metals and radioactivity in mineralized water met the requirements of GB5749-1985Sanitary Standard for Drinking Water and GB8537-1995Sanitary Standard for Drinking Natural Mineral Wa-ter.Mineral pills were classified as actually non-toxic and the results of micronucleus test of mice bone marrow cells,sperm ab-normality test in mice and Ames test were negative.Conclusion Mineralized drinking water treated by artificial mineral pills were safe.
8.Pathological changes of pancreas and changes of serum amylase and lipase in septic rat

Objective To systematically observe the damage of pancreas and changes of serum amy-lase and lipase in septic rat at different time points after the septic rat model being established. Methods Fifty-four male rats were randomly divided into sham group,sepsis group and normal control group. In the sepsis group,rats were given cecal ligation and puncture so as to produce a model of the rat′s intestinal septic shock. In sham group,after opening the abdominal cavity of the rats,only the cecum was pulled out of the abdominal cavity,then the abdominal cavity was satisfied and stitched. The arterial blood and pancreatic tis-sues of the rats in both sepsis and sham groups were collected for detection of serum amylase and lipase and observation of damage of pancreas by light and electron microscopy at 3 h,6 h,12 h,24 h after the model be-ing established. Results The levels of serum amylase of sepsis group showed a rising trend over time,and peaked at 24 h(2 779. 83 U/L), which showed no difference among 4 time points. There was no significant difference of serum amylase among 3 groups at each time point except for 12 h value. The levels of serum li-pase in both sepsis and sham group were higher than that in normal control group at 3 h(P<0. 05),while de-creased to the level of normal control group at 24 h. By light microscope,pathological changes of pancreas in the sham groups were that lobule was slightly widened; mild interstitial edema,vasodilation and a little in-flammatory cell infiltration were occasionally found;most of the lobular structure was complete. In the sepsis group,pancreatic pathological changes aggravated with time prolonged. Pancreatic lobules gap widened,inter-stitial were infiltrated by the inflammatory cell,flocculus brim structure was fuzzy,focal necrosis was found occasionally,islet structure was complete at 3 h. Pancreatic lobule was significantly wider,more inflammatory cell infiltrated,necrosis enlarged at 6 h,and pancreatic lobule was obvious edema,more inflammatory cell in-filtration and focal necrosis were found at 12 h. Vascular of pancreatic lobule was dilatation and congestion, pancreatic lobule and islet edge blurred,more lobular necrosis at 24 h. The modified Schmidt scoring of sham group and sepsis group at 3 h,6 h,12 h,24 h were 2. 0,3. 6,3. 4,3. 6 and 4. 6,6. 0,6. 8,7. 6,respectively. By transmission electron microscope,the ultrastructure of pancreatic cell in sham group was normal,mitochondri-al and lysosomal swelling were found occasionally. When in the sepsis group,the nucleus was normal,some mitochondria was swelling and cracking, lysosomes was swelling at 3 h, nucleus was complete, a part of swelling mitochondria and lysosomes appeared vacuolated necrosis at 6 h,and karyopyknosis and chromatin condensation were found,more mitochondria and lysosomes were swelling necrosis and vacuolar degenerated, rough endoplasmic reticulum was swelling at 12 h,cell karyopyknosis and chromatin condensation were obvi-ous,a large number of mitochondria and lysosome, endoplasmic reticulum were swelling, dilatation and vacuolar necrosis at 24 h. The modified Schmidt scoring showed no significant correlation with the levels of serum amylase and lipase in sepsis group(r=0. 472,P=0. 199;r=0. 260,P=0. 499). Conclusion The pancreas was damaged in the septic rat. The pathological changes of pancreas were obvious at 6 h after sepsis onset. The characteristics of the pancreas ultrastructure change includes the changes of the mitochondria and lysosome in the early period,clear vacuoles necrosis of mitochondria and lysosome,and destruction of nucleus and the endoplasmic reticulum. There is no significant correlation between pancreatic pathological changes and serum amylase as well as lipase,serum amylase and lipase can′t indicate the extent of pancreas damage in sepsis.

The research frontiers in foreign medical informatics were analyzed according to the co-occurrence net-work map, hotspots density map and cluster tree map that were plotted for the PubMed-covered scientific papers on medical informatics from 2010 to 2014 , which showed that the research frontiers in foreign medical informatics were the natural language processing and data mining of electronic health records, medical decision-making assistant system, computer-aided drug therapy, clinical decision-making support, health service attitude, health service quality, health service model, medical records system and its linkage, information system and its integration.
