1.Sinomeuine and sodium ferulate prevent complement-induced granulocyte aggregation

Complement-induced polymorphonuclearcytes (PMN) aggregate in microvasculature of vital organs, produce microemboli, and block microcirculation blood flow. Activated PMN release oxygen free radicals, prostaglandins and proteases, in turn, damage endothelial and parenchyma cells. This is an important factor in pathogenesis of multiple system organ failure. We have found that Sinomeuine and Sodium Ferulate prevented PMN aggregation induced by activated complement. Adding the drugs into PMN suspension treated with cytochalasin B, the zymosan activated plasma, PMN aggregation was prevented. The changes of light transmission were smaller than those of control without drugs recorded with platelet aggregometer. The 50% of PMN aggregation inhibition with Sinomeuine was 0.76mg/ml (1.gmmol/L), and 0.82mg/ml (3.8mmol/L) with Sodium Ferulate.

The nursing educations in many developed countries thought great important for nursing students to learn the gene relating knowledge, and emphasized the importance of setting up gene relating course in nursing education system.Our school integrated genetics and genomics for nursing course into the undergraduate nursing curriculum system during the period of international course weeks.This article introduced the basic theories and practice needs of genetics and genomics for nursing course,the settings of this course in the foreign and our country′s undergraduate nursing curriculum system, the concrete practice of our school carrying out this course,and analysed the challenges of carrying out this course in our country,put forward the corresponding suggestions.
3.Comparison of sufentanil and fentanyl in total intravenous anesthesia for liver resection: A double blind randomized clinical trial

Objective To compare fentanyl and sufentanil in total intravenous anesthesia for liver resection. Methods 100 patients undergoing liver resection were randomly received either sufentanil (group S, n = 50) anesthesia or fentanyl (group C, n = 50). Hemodynamic variables, time to spontaneous eye opening and extubation were recorded. VAS-scale was used to assess postoperative pain. The incidence of nausea & vomiting, agitation and respiratory depression was recorded and compared. Results Patients in group S had a more stable hemodynamic variables, and the incidence of postoperative pain (VAS: group S 4. 2± 1.1, group C 6. 1 ± 1.2), agitation (group S 2 cases, group C 11 cases) and respiratory depression (group S 1 case, group C 5 cases)was lower. Conclusions Both sufentanil and fentanyl were safe and efficacious in total intravenous anesthesia for liver resection. Compared with fentanyl, Sufentanil had higher hemodynamic stability, and lower incidence of postoperative pain, respiratory depression and agitation.
4.The discussion of the methods on troubleshooting of networking malfunction for mindray iPM-9800 central monitoring system

Objective:Common troubleshooting methods of networking malfunction for central monitoring system were discussed in order to realize its safe application of networking central monitoring system.Methods: Time calibration server and necessary conditions of network for central monitoring system were discussed and analyzed. In addition, practical troubleshooting and maintenance methods were proposed to solve the networking malfunction between a bedside monitor of Mindray iPM-9800 and its central workstation.Results: Smooth operation of networking central monitoring system could simultaneously obtain physiological parameters of several patients, thus largely improving the efficiency of medical staffs.Conclusion: Safe operation of networking central monitoring system could ensure the accuracy of real-time monitoring information of patients and improve hospital’s management level of network and informatization.
5.Diagnosis and treatment of severe perioperative infection complicated with DIC

Objective To discuss the early diagnosis of severe perioperative biliary infection complicated with DIC and efficacy of comprehensive treatment for it. Methods Comprehensive treat-ment was conducted in 21 patients who had intensive biliary infection complicated with DIC. The he-moglutination function was monitored before and after treatment. Results Of the 21 patients, 19 were cured and 2 dead. The curative rated was 90.5 %. The hemorrhage tendency was controlled, abdomi-nal bleeding stopped, prothrombin time (PT) obviously reduced, content of fibrinogen (Fbg)in-creased, level of D-dimer and FDPs obviously dropped. Conclusion The patients who have biliary in-fection in the perioperative period are easy to suffer from DIC and the mortality is high. Early diagno-sis and comprehensive therapy, especially the application of the cryofibrinogen and heparin, are very important.
6.The changes of multifocal electroretinogram of the affected and fellow eyes of the patients with central serous chorioretinopathy

ObjectiveTo evaluate the retinal function with central serous chorioretinopathy (CSC) and the fellow eyes (mf-ERG).MethodsTwenty patients (40 eyes) with unilateral CSC, diagnosed by fluorescein angiography, were examined on each eye of the patients by mf-ERG multifocal electroretinography. The patients aged from 31 to 53 (13 men, 7 women; mean age, 41 years), and the CSC eyes had visual acuities of 0.2 to 0.8. None of the patients had significant abnormality in the fellow eyes with visual acuities of 0.8 to 1.5. Twenty-four healthy people (29 normal eyes with visual acuities of 1.0 and more and without any retinal diseases) were studied simultaneously as the normal control. The stimulus matrix of 61 hexagonal elements were grouped into 5 concentric rings from the fovea outward to 30? of the degree of visual field. The first-order responses of mf-ERG were recorded. The first negative wave (N 1-wave) and positive wave (P-wave) of the mf-ERG were measured. Amplitude intensities and latechies for the two configurations were calculated and compared.ResultsCompared with the control eyes, the responses were not only depressed severely in P 1- and N 1-wave amplitude intensities in the ring 1~3 in the CSC eyes with CSC (P0.05). There were no significant differences in N 1- and P 1-wave latencies in 5 concentric rings among the control eyes, the fellow eyes or the CSC eyes.ConclusionsCSC is a kind of macular disease involving both eyes. It may cause the decrease of retinal function in macular area of CSC eyes and also influence the retinal function in macular area of the fellow eyes. mf-ERG is a sensitive way in examining visual function.
7.Clinical complication understanding of early postoperative enteral nutrition in gastrocolonic surgery

Objective To evaluate if the enteral nutrition have effect on the immune function and in- flammatory reaction after operating about gastric carcinoma.Methods 58 postoperative patients suffering from stomach cancer and colon cancer were randomly divided into the EN group and the TPN group.On the first postoperative day,nutrition fibre were given via nasal intestinal tube,increasing the capacity and drop- ping speed day by day until patients can eat liquid diet.While patients in the TPN group didn't eat anything until enterokinesia completely recovered.Observing on preoperative day 1 and on postoperative day 3 and day 8 respectively to check IgA,IgG,IgM,C3,C4,CRP,LYM,LYM%,TP,ALB,PA.Results The results showed that on the postoperative day 8,the target ascension extent was higher than that in the PN group.The statistical significance was very obvious(P
8.Neuroprotective Effect of Sevoflurane on Controlled Hypotension in Patients with Coronary Heart Disease Undergoing Craniocerebral Surgery

Objective To study the neuroprotective effect of sevoflurane on controlled hypotension in patients with coronary heart disease undergoing craniocerebral surgery. Methods Twenty-six patients with coronary heart disease undergoing craniocerebral surgery were randomly divided into two groups,receiving either inhaled 2%-5%sevoflurane plus intravenous sodium nitroprusside (treatment group,n=13) or intravenous sodium nitroprusside 3-8 μg·kg-1 alone (contorl group,n=13) for blood pressure control. The hemodynamic changes were recorded during the operation. Patient satisfaction with surgeons and the duration hospital stay after surgery were recorded. The levels of cardiac troponin I (TNI),creatine kinase-MB (CK-MBM),neuron specific enolase ( NSE) and S100 calcium binding proteinβ( S-100βprotein) in serum were detected at one day pre-operation,the end of the operation,and one day post-operation. Results The duration of hospital stay after surgery was significantly shortened in treatment group [(20. 3±3. 8) versus (23. 9±4. 6) d,P<0. 05) compared with control group. The average heart rate significantly decreased, patient satisfaction significantly increased, and serum levels of NSE and S-100β protein one day post-operation significantly decreased as compared to control group (all P<0. 05). Compared with the day before operation,serum levels of NSE and S-100βprotein in the two groups increased significantly at the end of surgery (P<0. 05),and the levels of TNI,CK-MBM,NSE and S-100βsignificantly elevated one day post-operation (P<0. 05). Compared with the end of operation,serum levels of NSE and S-100βin contorl group incarcerated remarkably (P<0. 05) Conclusion Sevoflurane plays an important neuroprotective role,as evidenced by improving patients' satisfaction,reducing hospital stay after surgery,and maintaining the balance of myocardial oxygen delivery and consumption in craniocerebral surgery patients with coronary heart disease during controlled hypotension. However,it can not prevent postoperative myocardial injury in these patients.

A rapid analysis method of piperazine ferulate tablets by optic-fiber sensing technology with UV-vis absorption spectrum was established.Qualitative and quantitative data were obtained and compared by maximum and minimum wavelength,absorbance and contrast spectra.Similarity method was used to identify authenticity of drugs.The difference of contents measured by this method and UV determination method in China Pharmacopoeia showed no statistical significance (P>0.05),while the similarity can be used as a parameter to identify the authenticity of drugs.

In the clinical practice,critically ill patients are in great risk of delirium because of the impact of psychological,surroundings,illness,and medications,with the incidence rate as high as 45%-87%.Delirium could result in bad prognosis and clinical consequences of cognitive function in late stage.Dexmedetomidine (DEX) is a novel and highly selective 2-adrenoceptor agonist.It offers beneficial pharmacological properties and has been widely used in intensive care unit (ICU).In order to provide scientific and effective references for clinical practice,this paper reviewed the development of DEX intervention for the prevention of delirium in ICU both in China and abroad refereed to the pharmacological characteristics and the efficiency in clinic and the influence factors ofthe efficiency;and also discussed the possible mechanisms,such as modulating the neurotransmitters,and attenuating stress response and inflammation.
