1.The Predictive Factors of IFN Therapeutic Effectiveness in Hepatitis C

Objective: Our purpose was to study the predictive factors of interferon(IFN) therapeutic effectiveness. Methods:Genotypes of HCV, HCV RNA quantities, β2 microglobulin, 2′- 5′oligoadenylate synthetase, and peripheral blood lymohocyte subgroup were detected by using specific primer PCR assay, energy transference technique of signal primer, sheet chromatography, radioimmunoassay, and APAAP immunoenzyme bridge technique respectively in 20 blood samples of patients with chronic hepatitis C. Results:(1)The patients who had lower level of HCV RNA before treatment and had continuing decreased HCV RNA level after treatment had good response to IFN treatment in follow-up survey. (2)The patients with HCV-Ⅲ type had complete response much more than those with HCV-Ⅱ type (P<0.05).(3)The patients with lower level of 2′- 5′oligoadenylate synthetase before treatment might have good response to IFN treatment. (4) There was no significant change in β2 microglobulin level before and after treatment and in different response groups before treatment.(5) The CD+3, CD+4, and CD+4/CD+8 in peripheral blood increased after treatment. The patients in complete response group had higher level CD+3, CD+4, and CD+4/CD+8 than those in part response and non-response groups, but there was no significance in statisitcs. Conclusion:The HCV RNA quantities, HCV genotypes, 2′- 5′oligoadenylate synthetase level before treatment were predictive factors of IFN therapeutic effectiveness. The changes of HCV RNA level in treatment and follow-up survey had important significance in therapeutic evaluation, β2 microglobulin level, and lymphocyte subgroup before treatment were not used as predictive factors for IFN therapeutic effectiveness.

Furthering of the health reform calls for better health care system,greater public financing,and deepened reform of public hospitals.Introduced in this paper are the actions taken and outcomes of the health reform in Changchun city,including the essential medicines system to minimize drug prices,improvement of the primary healthcare system to minimize people's financial burden,greater public spending to enhance public welfare nature of medical institutions,enhanced competencies of primary medical institutions to introduce rational classification of patient flow,and enhanced equity of public services to promote people's health.On such basis,further reform proposals were made for Changchun's health reform.
4.Expression and signification of NF-κB, EGFR and MUC1 in hepatolithiasis associated with intrahepatic cholangiocarcinoma

ObjectiveTo investigate the role of nuclear factor kappa B (NF-κB), epidermal growth factor (EGFR) and Mucin 1 (MUC1) in hepatolithiasis associated with intrahepatic cholangiocarcinoma. MethodsSpecimens were taken from 90 patients who underwent hepatectomies from August 1989 to June 2009 at the Second Affiliated Hospital of Sun Yat-sen University. The specimens were stained immunohistochemically for NF-κB, EGFR and MUC1. There were 33 patients who had hepatolithiasis associated with intrahepatic cholangiocarcinoma (the experiment group). 32 patients with hepatolithiasis served as the control group, and 25 patients with normal intrahepatic bile ducts taken at 1-2cm distal to benign hepatic neoplasm served as the blank group. The immunohistochemical staining were performed on tissue slices. Results NF-κB positive rate was 51.5% (17/33), 25%(8/32) and 4% 1/25) in the experiment group, the control group and the blank group respectively,P<0. 01 ; EGFR positive rates were 57. 6% (19/33), 31.3% (10/32) and 0 (0/25) respectively,P<0. 01; MUC1 positive rates were 54. 5% (18/33), 28. 1 % (9/32), 0 (0/25) respectively,P<0. 01. There were significant differences for high level expressions of EGFR and MUC1 among histopathologic grading, tumor invasion and metastasis. The survival rates of patients with EGFR and MUC1 expressed tumor were lower than of patients with non-expressed tumout (P<0. 01). ConclusionsNF-κB is probably involved in the early stage of intrahepatic cholangiocarcinogenesis. Overexpressions of NF-κB, EGFR and MUC1 play coordinately and important roles during intrahepatic cholangiocarcinogenesis. High level expressions of EGFR and MUC1 are related to the malignant degree of cholangiocarcinoma and to worse prognosis.
5.Study on Association between Interleukin-1 Beta Gene Polymorphism and Childhood Asthma

Objective To evaluae possible association between interleukin-1 beta (IL-1?) gene exon 5 polymorphism and childhood asthma.Methods The study was conducted in two different groups: asthmatic children(n=55) and healthy children(n=35). The IL-1? gene exon 5 polymorphism was genotyped by polymerase chain reaction-restriction fragment length polymorphism analysis (PCR-RFLP). Results Frequencies of CC,CT and TT genotypes were 92.7%,7.3%,0, and frequencies of C,T allele were 96.4%,3.6% in asthmatic group. However, frequencies of CC,CA and AA genotypes were 85.7%,14.3%,0, and frequencies of C,T allele were 92.9% ,7.1% in healthy group. There were no significant difference in distribution of genotypes and allele frequencies between two groups.Conclusion IL-1? gene exon 5 polymorphism may not be associated with childhood asthma.
6.Evaluation of the Method Change from JSCC to IFCC for ALT,AST,GGT and LDH Detection

Abstact:Objective To evaluate the method change from JSCC to IFCC for ALT,AST,GGT and LDH Detection.Methods The accuracy,precision,linearity and reportable range of the new detection method for ALT,AST,GGT and LDH,and the comparison analysis on the two different reagents were evaluated.Results All the accuracy bias of the testing items were within the required 1/2TEa,and all the within-run precision and between run precision were within the required 1/4TEa and 1/3TEa respectively.The linear verification results got the regression equation of the theoretical and measured valuesY =aX+b,in which a was within the range of 0.97~1.03,b was within an acceptable range.The reportable range verification re-sults showed that after the samples being diluted by different proportions,the measured/expected values were all between 90% and 110%,indicating that within a certain range of sample dilution the test esults were reliable.The comparison results showed the R 2 closed to 1.Conclusion The evaluation of the method change for ALT,AST,GGT and LDH detection met the basic requirements of the experiments in clinical diagnosis.
7.Induced Cardiomyogenic Differentiation of Bone Marrow Mesenchymal Stem Cells in Vitro

Objective To investigate the effect of 5-azacytidine on cardiomyogenic differentiation of bone marrow mesenchymal stem cells (BMSCs). Method BMSCs were isolated from the marrow of adult SD rat’s femoral /tibial bones.Different concentration of 5-azacytidine were added to primary BMSCs on 3 d and cultured for different times.Cardiomyogenic differentiation of BMSCs was observed by immunohistochemistry and RT-PCR. Result After treated with 5-azacytidine, BMSCs proliferated slowly, became spindle-shaped after 10 d and aligned in a striated pattern after 20 d.TnT positive cells were showed by Immunohistochemistry and they expressed two cardiac-marked genes GATA-4 and ?-MHC.Thus 5-azacytidine induced cardiomyogenic differentiation of BMSCs in a time and concentration-dependent manner. Conclusion Our study suggests that bone marrow mesenchymal stem cells can differentiate into cardiomyocytes in vitro. They are ideal donor cells in cellular cardiomyoplasty for treatment of myocardial infarction.
8.Systematic review of the effectiveness of intratracheal injection of pulmonary surfactant in the treatment of neonatal respiratory distress syndrome

Objective To evaluate the effectiveness of intratracheal injection of pulmonary surfactant (LISA) combined with continuous positive airway pressure (CPAP) on the treatment of neonatal respiratory distress syndrome (RDS). Methods The databases of PubMed, EMBASE, Cochrane, China National Knowledge Infrastructure (CNKI), Wanfang Database, Weipu Database and China Biology Medicine disc (CBM) were searched, and all of the randomized controlled trial of LISA combined with CPAP for treatment of RDS were included. RevMan 5.2 was used to perform meta-analysis. The differences of mortality, bronchopulmonary dysplasia (BPD), retinopathy of prematurity (ROP), intraventricular hemorrhage (IVH), periventricular leukomalacia (PVL), and other complications between LISA group and control group were compared in <34 weeks gestational age preterm infants with RDS. Results Five randomized controlled trials were included with 759 infants in total. Compared with control group, the rate of mechanical ventilation (RR: 0.32,95% CI: 0.13-0.82) and the incidence of BPD (RR: 0.61,95%CI: 0.42-0.88) were significantly decreased in LISA group. There were no differences in mortality and the incidences of ROP, IVH, and PVL between two groups (P>0.05). Conclusion The mechanical ventilation and the incidence of BPD is significantly reduced in the treatment of neonatal respiratory distress syndrome by LISA, but it has no effect on the incidences of other complications and mortality.
9.Family lifestyle factors related to children's congenital heart defects in China: a case-control study

Objective To explore the multiple risk factors for family lifestyle of children with congenital heart defects (CHDs) in Shaanxi Province, China. children and their parents. The univariate and multivariable logistic regression models were used to analyze the influence of risk factors related to parents' lifestyle on CHDs. Results Several possible risk factors were found for CHDs, including fever (OR=4.465, P=0.017), pesticides contact (OR=2.234, P=0.083), passive smoking during pregnancy (OR=20.529, P=0.007) and father's smoking (OR=3.342, P=0.005); fever (OR=2.428, P=0.012) and passive smoking during pregnancy (OR=1.201, P=0.037) were also correlated with ventricular sepal defect (VSD). Conclusion Fever, pesticides contact and passive smoking are associated with CHDs during pregnancy. We should focus our attention on health care during pregnancy to avoid the above-mentioned risk factors and call on parents to hold on to a good healthy lifestyle.
