1.Diagnosis, treatment and prognosis of patients with acute pancreatitis

Acute Pancreatitis Classification Working Group published Classification of Acute Pancreatitis-2012:Revision of the Atlanta Classification and Definitions by International Consensus in 2012,which redefined the diagnosis,severity,local complication,local infection of acute pancreatitis (AP).Under the guidance of revised Atlanta classification (RAC),the constituent ratio of severe acute pancreatitis (SAP) decreases significantly,which is between 3.7% ~ 25.4%,and less than 10.0% is reported in most researches.The mortality of SAP is between 10.0% ~ 52.9%,and more than 20% is observed in most publications.The constituent ratio of mild severe acute pancreatitis (MSAP) is between 10.9% ~ 40.1%,the mortality is between 0 ~ 4.8%,and the incidence rate of organ failure is between 4.6% ~ 34.0%.The percentage of patients with SAP who need ICU care and surgery intervention is higher than that of MSAP,and the total hospital stay of SAP are longer than that of MSAP.RAC could better reflect the severity and predict the outcome of AP than Atlanta classification published in 1992,but RAC also has some potential limitations.For example,the “local infection” is not included in this classification.This paper veviewed the relavent issues.
2.The surgical treatment of infection secondary to pancreatic and (or) peri-pancreatic necrosis

Several minimally invasive techniques were used to treat infection secondary to pancreatic and (or peri-pancreatic) necrosis recent years,including percutaneous catheter drainage (PCD),endoscopic transluminal treatment,min-incision surgery,video-assisted minimally invasive technique and laparoscopic surgery.We should grasp the indication of each technique.PCD/ endoscopic transluminal surgery usually acted as initial therapy.Open surgery is still effective,and acts as a rescue operation when minimally invasive techniques fail.

4.Application of octavius 4D system in volumetric modulated arc therapy dose verification

Objective To study the feasibility of dose verification with Octavius 4D system in volumetric modulated arc therapy (VMAT).Methods Twenty patients were treated with the VMAT.Their treatment plans were performes and then acquired the measured data with the Octavius 4D system.In addition,the treatment plans are used in the CT phantom for Octavius 4D system,and acquire the simulated data.We use the γ pass rate to analysis the measured and simulated results for the conditions:the dose deviations are 2%3%5%,the displacement deviations are 2 mm/3 mm/5 mm and the thresholds are 5% 10%,and got the Gamma pass rate and fitness of profile curve.Results The distributions of measured and simulated dose are matched well,and the fitness of profile curve are also matched well.The averaged Gamma pass rate is 97.78% in the case 3 mm 3% 10%.Conclusions Octavius 4D system can meet the dose verification for VMAT treatment.The measured plane is always following the rotating frame and perpendicular to the beam.
5.Study on Stability of Zhiling Liumiao Oral Liquids

Objective:To investigate the stability of Zhiling Liumiao oral liquids. Methods: The active ingredients in Zhiling Li-umiao oral liquids were determined by HPLC, and the identification was studied by TLC. Meanwhile, the other indices including char-acter, relative density, pH and microbial limit were determined as well. Results:The active ingredients in Zhiling Liumiao oral liquids could be influenced by strong light, while the changes were within the range of the quality standards, and the samples were stable in the accelerated test and long-term test. Conclusion:The designs of preparation process and package are rational to keep the stability of the preparation. The shelf time may be set at 12 months.
6.Clinical significance of detecting levels of serum and ascitic PCT in patients with liver cirrhosis complicating spontaneous bacterial peritonitis

Objective To study the changes of serum and ascitic procalcitonin (PCT ) level in the patients with liver cirrhosis complicating spontaneous bacterial peritonitis (SBP) and to investigate the value in its early diagnosis .Methods 92 patients with cirrhotic and ascites were detected the levels of serum and ascitic PCT by the immunofluorescence method and the detection results were compared with the ascites routine and the bacterial culture results .Results Serum levels of PCT in the SBP group and the non-SBP group were obviously higher than those in the control group ,the difference was statistically significant (P<0 .05);Serum and ascitic levels of PCT in the SBP were significantly higher than those in the non-SBP group ;The sensitivity of serum PCT for di-agnosing peritonitis was 89 .3% ,the sensitivity of ascitic PCT for diagnosing peritonitis was 71 .4% .Conclusion The levels of ser-um and ascitic PCT have the important value for the early diagnosis of liver cirrhosis ascites ,moreover serum PCT level has higher value for early diagnosing liver cirrhosis ascites .
7.Feasibility of integrated detection for coronary heart disease-associated protein factors by coding microsphere suspension chip

AIM: Coding microsphere suspension chip is a novel protein chip technology, which is considered as the developing direction of medical tests. This study investigated the feasibility of coding microsphere suspension chip to detect coronary heart disease (CHD)-associated protein factors. METHODS: The serum samples of 30 patients with CHD were collected including 19 males and 11 females, with average age of (63?10) years in the Department of Cardiology, Zhujiang Hospital of Southern Medical University from August to September 2007. Patients did not have hepatic or renal dysfunction, cerebrovascular accident, acute infection or any anticoagulation medication. The regional ethics committee approved the study protocol and the informed consents were obtained from all subjects. The integrated detection method was established by coding microsphere suspension chip to detect CHD-associated protein factors including high sensitivity C reactive protein (hs-CRP), lipoprotein a (Lpa) and adiponectin (ADPN). Meanwhile, these three factors from 30 CHD cases were detected by the enzyme-linked immunosorbent assay (ELISA). The detection results by both methods were compared. RESULTS: The serum samples of 30 CHD cases were all included in final analysis. The detection results by coding microsphere suspension chip integrated detection method were hs-CRP (2.54?1.97) mg/L, Lpa (102.41?76.74) mg/L and ADPN (15.43? 9.87) mg/L. The detection results by ELISA were hs-CRP (3.93?2.52) mg/L, Lpa (208.63?81.23) mg/L, and ADPN (16.63? 8.21) mg/L. 8.Curative effect of Salvia miltiorrhiza ligustrazine on the protection of intestinal barrier in severe a-cute pancreatitis

Objective Dynamic observation of plasma endotoxin (ET),plasma TXA2 and PGI2 levels and clinical symptoms in patients with severe acute pancreatitis (SAP),the protective effect of Salvia miltiorrhiza ligustrazine on the intestinal barrier in severe acute pancreatitis was studied.Methods The 60 cases of severe acute pancreatitis treated in our hospital from August,2014 to October 2015 were studied as the research object,and were randomly divided into the control group and the danchuan group.Two groups of patients received conventional treatment,such as diet,gastrointestinal decompression,early active fluid resuscitation,maintenance of stable electrolyte,anti infection,inhibition of pancreatic secretion,parenteral nutrition and other support treatment,Dan Chuan group on the basis of conventional treatment combined with 10 ml of compound Salvia miltiorrhiza ligustrazine injection of 5% glucose injection,250 ml intrave-nous drip,1 times a day.Two groups of patients with 14 days for a period of treatment,the clinical symp-toms of the two groups were observed after the end of treatment,the clinical symptoms and the changes of la-boratory index.Results ⑴In the group of Dan chuan,the time of patients relieve intestinal pneumatosis or intestinal obstruction,abdominal pain were less than the control group,the difference was statistically sig-nificant (P <0.05).⑵ The recovery time of plasma endotoxin,serum amylase,and leukocyte in the Dan Chuan group were shorter than those in the control group (P <0.05).⑶The incidence,mortality and inci-dence of complications in the control group were significantly lower than those in the control group,but the difference was not statistically significant (P >0.05).Conclusions Salvia miltiorrhiza ligustrazine has a significant effect on severe acute pancreatitis.It can improve the intestinal microcirculation by correcting the balance between PGI2 and TXA2 ,and can promote the recovery of intestinal function,reduce the incidence of intestinal bacterial and endotoxin,and reduce the incidence of complications.
9.Dendritic cell induced cytotoxic lymph cells for the therapy of nude mouse transplanted tumor by pancreas cancer cell line Bxpc-3

Objective To study the antitumor effect of dendritic cell(DC) induced cytotoxic T lymphocyte (CTL). Method CTL induced by DC extracorporeally, were co-cultured with Bxpc-3 cells, CTL activity was observed by counting the killing of Bxpc-3 cells in vitro. Nude mice with Bxpc-3 cell transplant tumors were treated by injection of CTL on the edge of tumors, and kinetics of tumor growth was recorded, RT-PCR-ELISA was used to determine the telomerase of transplant tumor. Result CTL activity was 71.6%. Thirty-one days after transplantation tumor size and telomerase activity were not statistically different among therapy group and control group, whereas after fifty-five days tumor size (38?6)mm 2 , and telomerase activity (1.33?0.03) in CTL group were statistically different from that of ( 74? 33)mm 2 and (4.16?0.32) in control group. ConclusionDC induced CTLs suppress the experimental pancreatic tumor growth, providing an evidence for clinical immunotherapy of pancreatic cancer.
10.Nursing strategy of primary PCI in the aged with acute coronary syndrome
