1.Analysis of the effect factors about postoperative mechanical ventilation time in 217 children with congenital heart diseases

Objective To analyze the multiple factors affecting the postoperative mechanical ventilation supporting time of chil-dren with congenital heart diseases ,and to preferably master the offline period and offer intervention techniques about shortening mechanical ventilation supporting time and improving efficacy .Methods A total of 217 with congenital heart diseases in 2012 from January to October were retrospectively studied to seek the factor affecting the duration of ventilation .The factors including the age ,weight ,cardiopulmonary bypass time(CBP) ,aortic cross-clamping time(ACC) ,preoperative albumin ,preoperative pulmonary infection ,preoperative pulmonary artery pressure ,postoperative complications were analyzed by the stepwise Logistic regression . Results The ventilation supporting time was 0 .5 -312 .0 h ,average(28 .42 ± 2 .99)h .Multivariate stepwise Logistic analysis showed that the postoperative complications ,preoperative pulmonary infection and CPB were mechanical ventilation use time of the main influence factors .Conclusion Postoperative complications ,preoperative pulmonary infection and CPB are the main influencing factors of mechanical ventilation supporting time .
2.Efficacy of progesterone combined with Tiaojinghuoxue tablets treatment of amenorrhea induced by antipsychotic drug s

Objective To explore the clinical efficacy and safety of progesterone combined with Tiaojinghuoxue tablets in treating amenorrhea induced by antipsychotic drugs. Methods Total 100 patients were randomly divided into treatment group treated by progesterone combined with Tiaojinghuoxue tablets and progesterone group. The index including prolactin (PRL), luteinizing hormone (LH), estradiol (E2), endometrial thickness were measured before and after treatment in both groups . Also the clinical efficacy of the twogroups were compared. Results PRL, LH, E2, endometrial thickness and the recovery of menstrual flow rates in combination group were all better than the progesterone group, (P<0.05 or P<0.01). Cure rate in the combined group was 54.00%and the control group was 74.00%(P<0.05). Total effective rate of combined group was 90.00%and the control group was 80.00%(P>0.05). The rate of adverse reactions in combined group was 40.00%, less than 22.00%in progesterone group, (P<0.05). Conclusion The efficacy of progesterone combined withTiaojinghuoxue tablets is better than single progesterone treatment and have fewer adverse reactions in therpy antipsychotics drug induced amenorrhea.
3.Appraisal of efficiency and performance of public hospitals in Sanming

Objective To evaluate the performance and efficiency of public hospitals in Sanming city,for suggestions on bettering their medical resources deployment efficiency and operational efficiency.Methods Descriptive analysis was used to longitudinally describe the general performances of such hospitals,leveraging the operating data of 21 public hospitals of the city.Meanwhile,appropriate indexes were selected to construct data envelopment analysis models for analysis of hospital operational efficiency and variations.Results Longitudinal analysis showed the operation process as stable,the efficiency as improved,and the structure as optimized thanks to the reform.Horizontal analysis found three hospitals (14% of all) as efficient in general,and seven (33%) as efficient technically.Those hospitals found as inefficient technically suffer from overinvestment or output deficiency to varying extents.Three hospitals (14%) were found with scale efficiency,while those found without scale efficiency were regarded as diminishing returns on scale.Conclusions Reforms measures must be adhered to consolidate outcomes of the ongoing reform;Regional healthcare resources planning must be consolidated to improve the efficiency of resource allocation;Delicacy management must be made to enhance service delivery capabilities.
4.Systemic evaluation of biomembrane for preventing tendon adhesion

OBJECTIVE:To explore the preventive effect of biomembrane on tendon adhesion. METHODS:A total of 40 Chinese articles about randomized controlled animal experiments and clinical applications which were focus on the preventive effect of biomembrane on tendon adhesion published from 1990 to 2008 were collected. And then,those articles were summed up based on experimental animals,grouping,materials,methods,results,and conclusions. Furthermore,clinical subjects received biomembrane were generally analyzed,and following up and functional evaluation were performed to overall summarize the clinical application of biomembrane for preventing tendon adhesion. RESULTS:Six out of nine articles suggested that cock was the major experimental animals,and the biomembrane could effectively prevent tendon adhesion due to good histocompatibility and permeability,and no effect on normal healing of tendon;therefore,it was an ideal material. Eight out of twelve articles suggested that there were no significant differences in biomembrane between age,sex,and injured region. Evaluation criterion in the domain of total range of active movement and active range of each joint demonstrated that biomembrane could successfully prevent tendon adhesion. CONCLUSION:Biomembrane is an excellent anti-adhesive materials with good histocompatibility and permeability.

Objective To investigate the value of renal CT arteriography in the preoperation evaluation of patients with renal tumors.Methods From Jan 2007 to June 2012,372 patients with renal tumors took renal CT arteriography (with Philips Brilliance 16) to evaluate the variations of their renal arteries.We executed plain scan first,then artery phase,venous phase scan and delaying scan by turns after injections of Omnipaque.All images were delivered to imaging workstation and reconstructed to 3D views of renal arteries to supply imageological supports for surgeons.Results Tumors in solitary kidneys were detected in 21 patients,and masses in both kindneys were detected in 4 cases.We collected data of 376 kidneys finally.We found premature branching renal arteries on 46 kidneys (12.2%) and accessory renal arteries on 74 kidneys (19.7%).Double accessory renal arteries were seen on 10 kidneys (2.7%),and there were 84 accessory renal arteries in summary.7 accessory renal arteries (7.2%) came from arteria phrenica and 77 (91.7%) came from abdominal aorta (x2 =65.68,P < 0.01).We found the regions where the accessory renal arteries entered kidneys:65.5% (55/84) in upper poles,19.0% (16/84) in central regions and 15.5% (13/84)in inferior poles.Routine CT scans found 44 variations of renal arteries in 218 cases who underwent surgeries,and then renal CT arteriography found 63 variations in the same cases (20.2% vs 28.9%,x2 =4.47,P < 0.05).Finally,surgeons found 60 variations of renal artries in operations in the 63 cases with variations found by renal CT arteriography before surgeries.Conclusions There is limitations of routine CT scan (plain scan and with contrast medium) in detection of renal artery variations.Renal CT arteriography can supply reliable supports for establishment of surgical plans for renal tumors.
7.Expression of activating transcription factor 4 in osteogenic induction of bone marrow mesenchymal stem cells

BACKGROUND:Activating transcription factor 4 is found as an activating factor that can regulate osteogenic differentiation and function, and plays a critical role in the osteogenic differentiation.
OBJECTIVE:To investigate the expression and significance of activating transcription factor 4 in the osteogenic differentiation of bone marrow mesenchymal stem cells from Sprague-Dawley rats.
METHODS:Bone marrow mesenchymal stem cells from Sprague-Dawley rats were cultured using the whole bone marrow adherence method, and ossification revulsant was added to induce passage 3 cells. cells with no osteogenic induction served as controls. RT-PCR and western blot assay were employed to dynamical y monitor expression of activating transcription factor 4.
RESULTS AND CONCLUSION:RT-PCR results showed that activating transcription factor 4 mRNA expression increased with the increasing osteogenic differentiation, and peaked at day 16. Western blot analysis showed that the protein expression of activating transcription factor 4 tended to increase with the increasing osteogenic differentiation, peaked at day 16 and stil maintained at a higher level at day 19. Compared with the uninduced cells, activating transcription factor 4 in the induced cells exhibited a higher expression at mRNA and protein levels (P<0.05). These findings indicate that activating transcription factor 4 expression is elevated during osteogenic differentiation, showing a positive correlation with osteogenic ability of bone marrow mesenchymal stem cells.
8.Effects of immediate loadings with different forces on immediate implant-bone ossification

BACKGROUND:Animal studies showed that functional early load or immediate load on immediate implant did not affect the prognosis of implant.
OBJECTIVE:To analyze the effect of immediate loading with different forces on the binding surface of immediate implant bone ossification.
METHODS:Six adult healthy male dogs were selected. Al teeth (three premolars) between cuspid teeth of lower mandible and the first molar were pul ed out. One implant (OSSTEM GSII) was immediately implanted, total y 36 implants. The vertical immediate loading by device was used on the implants at 24 hours after implantation. Grouped by the loading force values:0, 10, 20 N on the left three implants of each dog from front to back, 30, 40, 50 N on the contralateral three implants of each dog, at a frequency of 1 Hz for 10 minutes every day. The implant stability quotient (ISQ) was tested using resonance frequency measurement instrument (OSSTELL) at 0 day, 4, 8, and 12 weeks.
RESULTS AND CONCLUSION: With increased load forces, serum osteocalcin expression increased, and peaked on 20 N, but decreased in 30 N group, and lowest in 50 N group. At 4 weeks after immediate implantation, the ISQ values were slightly less than pre-implantation in each group, especial y the 50 N group. At 8 weeks after immediate implantation, ISQ values were increased in each group to different degrees. The increased degree of the 20 N group was maximal. At 12 weeks, a peak value was detected in each experimental group. The implants could bind to bone tissues to different degrees. The range of implant-bone interface formation was positively associated with time. Results indicated that the smal force cannot impact implant-bone ossification, but promotes it in a manner, but large force value (≥ 20 N) wil affect the stability of the implant-bone ossification.
9.Impact of flushing uterine cavity with metronidazole solution at different temperatures on postpartums with high risk of infections during cesarean section

Objective To explore the effect of flushing uterine cavity with metronidazole solution at different temperatures on postpartums with high risk of infections during cesarean section.Methods One hundred and two puerperas with high risks of infections undergoing caesarean section were randomized according to random sampling into three groups equally.Groups A,B and C were treated with uterine cavity flushing with metronidazole solution at three temperatures of 22~24℃,39~40℃and 37.5~37.8℃respectively.Comparisons were done between the three groups in terms of vaginal bleeding 2h after operation,heart rates 15 min before operation and 15 min after operation and the temperatures.Results The differences in temperature and heart rate 15 min after operation were significantly different arnong the groups(P<0.05).The temperature and heart rates of group A were higher and faster than the other two groups(both P<0.05)but no differences between group B and group C(P>0.05).In group A,the temperature and heart rate after operation were significantly lower and faster than those before operation(P<0.05),while there were no differences within group B and C respectively (P>0.05).The vaginal bleeding volume 2 h after operation in groups A and B was significantly more than group C(P<0.05)and that of group B was significantly smaller than group A(P<0.05).Conclusion Uterine cavity flushing using metronidazole at the temperature of 37.5~37.8℃after cesarean section can reduce postpartum hemorrhage and relieve patient’s discomfort.
10.Effects of triptolide on the expression of androgen receptor in human prostate LNCaP cells and its mechanism of action.

To study the regulation of androgen receptor (AR) expression in human prostate cancer LNCaP cells by triptolide (TP) and the possible mechanism, by using qRT-PCR and Western blot, the AR mRNA and protein levels in TP treated LNCaP cells were detected, and the AR protein level in TP and NF-κB inhibitor treated LNCaP cells was also detected; a series of pGL3-AR promoter reporter gene vectors were built using restriction-free cloning method, and the vectors were employed to investigate the effects of TP on the transcriptional activity of AR promoter in LNCaP cells; the upstream proteins which may play regulatory roles were detected using western blot assay. After treated LNCaP cells with TP for 48 h, AR mRNA and protein expressions decreased with increasing TP concentration. The expression of AR target gene PART1 and prostate specific antigen (PSA) was also downregulated by TP treatment; a series of pGL3-AR promoter reporter vectors were constructed and validated by sequencing and luciferase activity; the results of dual luciferase reporter assay showed that TP downregulated AR at the transcriptional level; PI3K/AKT/NF-κB pathway which is associated with AR promoter activity was drowregulated by TP. In conclusion, our results demonstrated that the transcriptional activity of AR in LNCAP cells was downregulated by TP, and PI3K/AKT/NF-κB pathway may be involved in the regulation mechanism.
