1.Organochlorines, Glutathione S-transferase M1 Genetic Polymorphisms and Risk of Breast Cancer

Objective To study the potential effect of gene-environment interaction between glutathione S-transferase M1 (GSTM1) and serum organochlorines residues on the risk of breast cancer in women, in China. Methods Seventy newly pathologically diagnosed female patients with breast cancer from September 2006 to October 2007 were selected as the cases from five large hospitals in Tangshan. The controls were identified at the same hospital as cases. 1∶1 matched case-control study. Between the cases and controls, the difference of age was not over two years and the residence was similar. The organochlorine residues levels in the serum were measured by gas chromatography (GC). Genotypes of GSTM1 polymorphisms were analyzed by multiplex allele-specific polymerase chain reaction (PCR). Interaction indexes (?) were calculated to determine the type of gene-environment interaction. Results After confounding factors adjusted, the result showed that interaction existed in genetic polymorphisms of GSTM1 and DDT, HCH residues, and interaction indexes (?) value were 1.237 and 1.379. Conclusion GSTM1 genetic polymorphisms and DDT, HCH may present an interaction in the development of breast cancer.

3.Introduction of American Military Medical Triage

Because of lots of ethical problems,military medical triage has been unaccepted by general doctors.But it's adopted by American Army in front of facts of the war.The main principles of the military medical triage are justice and efficiency.They practice the military medical triage through tow steps.First,all patients are assigned into urgent,immediate ,delayed,minimal or ambulatory and expectant status in decreasing order of medical urgency.Second,choosing the model of triage depends on nonaustere,austere or extreme conditions.The ethical issues of American military medical triage focus on four aspects - how to deal with minimally injured patient treatment first,how to treat expectants,how to care for noncombatant casualties and who decide the model of triage.
4.Intracoronary stenting for senile patients with acute myocardial infarction

ObjectiveTo evaluate the clinical efficacy of intracoronary stenting for senile patients with acute myocardial infarction(AMI).Methods42 senile patients with AMI underwent intracoronary stenting were analysed. Before and after stenting, the changes of the patients' blood pressures, heart rates and cardiac functions tested by ultrasonography were observed.Results45 intracoronary stents were implanted in 42 patients, one patient falled during hospitalization. The blood pressure was increased significantly, the heart rate decreased and the candiac function improved after stenting.ConclusionIntracoronary stenting is a very effective and safe therapy for the senile patients with AMI.
5.Epidemiological characteristics of pulmonary tuberculosis in Taizhou City from 2016 to 202

Objective:
To investigate the epidemiological characteristics of pulmonary tuberculosis in Taizhou City from 2016 to 2021, so as to provide the evidence for formulating the pulmonary tuberculosis control measures.
Methods:
The epidemiological data of pulmonary tuberculosis cases in Taizhou City from 2016 to 2021 were retrieved from the Tuberculosis Management Information System and Infectious Disease Surveillance System of Chinese Disease Prevention and Control Information System, and the trends in the incidence of pulmonary tuberculosis and regional, population and temporal distributions of pulmonary tuberculosis were analyzed using a descriptive epidemiological method in Taizhou City from 2016 to 2021.
Results:
A total of 15 621 pulmonary tuberculosis cases were reported in Taizhou City from 2016 to 2021, with annual mean reported incidence of 42.12/105. The incidence of reported pulmonary tuberculosis reduced from 43.74/105 in 2016 to 36.31/105 in 2021, with an annual decline rate of 3.65% (χ2trend=34.172, P<0.001). There were 7 606 cases positive for pathogenic tests (48.69%), and the positive rate of pathogenic tests appeared a tendency towards a rise from 2016 to 2021 (χ2trend =470.906, P<0.001). The reported incidence rates of pulmonary tuberculosis were 57.83/105 and 25.46/105 in men and women, and a high incidence rate was reported in patients at ages of 20 to <25 years (72.80/105) and 65 years and older (84.75/105). Farmer was the predominant type of occupation (8 365 cases, 53.55%), and the cases peaked from March to August, with the greatest seasonal index in May (119.76%). A high annual incidence rate of pulmonary tuberculosis was reported in Xianju County (52.51/105) and Wenling City (49.98/105) from 2016 to 2021, and the a high constituent ratio of patients positive for pathogenic tests was seen in Sanmen County (58.71%) and Luqiao District (53.24%).
Conclusions
The reported incidence of pulmonary tuberculosis showed a tendency towards a decline, and the proportion of pulmonary tuberculosis patients positive for pathogenic tests showed a remarkable increase in Taizhou City from 2016 to 2021. The number of patients peaked in spring and summer. In addition, high incidence of pulmonary tuberculosis was seen in Xianju County and Wenling City.

Objective To analyze the medical undergraduates'employment intentions and determinants for policy recommendations on how to solve the contraction between the employment and human resources shortage working in the rural place.Methods Descriptive method was used for the employment recognition and unconditional logistic regression was employed for its factors analysis.Results 74.90% interviewees were not high for policy recognition,and only 21.16% undergraduates were willing to work in rural area.Logistic regression analysis found local culture and life environment (OR=2.668),institute performance(OR=0.630)and work challenge(OR=1.509)were key factors to affect their employment.Conclusion To give reform of primary institute performance mainly on pubic health service,take great importance on the medical service,improve work and life environment of rural workers and highly care health workers'career planning and development.
7.Study of HPV Infection, P53 Gene Mutation and Expression in Cervical Carcinoma

P53 gene (exon7~8) mutatins and p53 proteins and HPV 6,11,16,18-DNA were examined in 49 cervical carcinoma by immunohistochemistry, polymerase chain reaction (PCR) and single strand conformation polymorphism (SSCP) in order to investigate their role and mutual relation and clinical significance in the onco genesis of cervical carcinoma. The results showed that first, p53 proteins posit ive rate was 48.98%, and not outstandingly related to the differentiation and the invasive degree of cervical carcinoma(P>0.05); the defects of P53 gene (exon7~8) were not found but P53 (exon7~8) mutations were detected in 7 of 49(14.29%) cervical carcinoma; then, HPV16-DNA positive rate was much higher than HPV6,11,18-DNA positive rate respectively(P<0.001),and the different HPV-DNA was simultaneously tested in one cerv ical carcinoma; last, not all cases of P53 mutations had p53 proteins posi tive, but the cases of P53 mutations and p53 proteins negative certainly had HPV infections, and HPV positive cases were much more than its negative one in the cases of p53 proteins positive(P<0.001). These results proved that the oncogenesis of cervical carcinoma is mainly associated with HPV16 infections, and second related to P53 (exon7~8) mutations. p53 proteins positive results from P53 mutations or/and HPV infections in cervical carc inoma.
8.Effects of titanium ions on the proliferation and activation of T lymphocytes in vitro

BACKGROUND:Titanium ions have been proved to stimulate the secretion of bone remodeling-related factors from T lymphocytes;however, the effects of titanium ions on the early activation, intermediate activation, and cel cycle of T lymphocytes remain unclear. OBJECTIVE:To investigate the effects of titanium ions on the proliferation and activation of T lymphocytes in vitro. METHODS:Cel proliferation and cycle test:Jurkat E6-1 T lymphocytes in logarithmic phase were col ected and cultured in the medium containing 0 (control), 25 (low concentration), 50 (middle concentration), and 100μmol/L (high concentration) titanium ions for 24 hours to detect the cel relative proliferation rate and cel cycle. Cel activation trial:Jurkat E6-1 T lymphocytes were divided into two groups that were subdivided into four groups containing 0, 25, 50, and 100μmol/L titanium ions, respectively with or without phytohemagglutinin (PHA) pre-stimulation. The expressions of CD69 and CD25 were measured after cultured for 24 hours. RESULTS AND CONCLUSION:Titanium ions enhanced T lymphocytes proliferation in a concentration-dependent manner (P<0.05). Compared with the control group, the percentages of G0/G1 phase decreased and the proportions of cel s in S and G2/M phase increased significantly in the low, middle and high concentration groups (P<0.05). The proportion of G0/G1-phase cel s in the high concentration group was less and the proportion of G2/M phase cel s was higher than those in the middle and low concentration groups (P<0.05). With PHA pre-stimulation, the expression of CD69 in the high concentration group was higher than that in the middle and low concentration groups (P<0.05);whereas the difference of CD25 expression was not significant among four subgroups. Titanium ions promoted the expression of CD69 in a concentration-dependent manner (P<0.05), but there was no CD25 expression in each subgroup without PHA pre-stimulation. To conclude, titanium ions can significantly promote T lymphocyte proliferation and early activation in vitro, and moreover, induce S and G2/M phase arrest in T lymphocytes.
10.Imaging manifestations of hepatic epithelioid hemangioendothelioma

Objective To explore imaging manifestations of hepatic epithelioid hemangioendothelioma (HEH). Methods CT and MR images in 14 patients with HEH proven by histopathology were retrospectively analyzed. Plain and two-phase contrast-enhanced CT scan were performed in 5 cases, non-contrast and multiphase contrast-enhanced MR scan were performed in 7 cases, CT and MRI were both performed in 2 cases. Characteristics of CT and MR T2WI images were classified and analyzed. All lesions were classified into three types:multiple, diffuse and solitary form. Results (1) Multiple form of HEH:228 lesions were found in 11 patients, including 178 lesions on MRI and 50 lesions on CT. On T2WI, three or two layered-target-signs with hyperintensity core were found in 79.2% (141/178) of the lesions. Three layer-target-sign included hyperintensity core, hypointensity rim and slightly high signal halo from the inside out. Two layer-target-sign included hyperintensity core and slightly high signal halo from the inside out. Characteristics of dynamic contrast-enhanced scan included peripheral two or three layered-rim-like enhancement in 66.3%( 118/178) of the lesions;peripheral, gradual rim-like enhancement with enhanced core in 27.0%( 48/178) of the lesions;heterogeneously mild enhancement in 2.2%( 4/178) of the lesions;centripetal enhancement in 4.5%( 8/178) of the lesions. Fifty lesions were found in CT, which showed low density nodules or masses with clear margins. Two-layered-black-target sign were found in 42 lesions in contrast-enhanced images, white-target sign were found in 3 cases, and centripetal enhancement was found in 5 cases. (2) Diffuse form of HEH:in one of the two cases of this type, the lesions could not be separated from normal liver parenchyma, gradual enhancements were found along with the vessels in the center of the lesions. (3) Solitary form of HEH: one case, the lesion showed heterogeneous density in non-contrast CT images and gradual enhancement in contrast-enhanced images. Conclusions We found some imaging characteristics of HEH. Two or three layered-target-sign on T2WI and black-target sign, white-target sign on contrast-enhanced images were unique imaging features of HEH.
