3.The Role of Antidepressants in the Treatment of Functional Dyspepsla

Objective:To evalute the role of antidepressants in the treatment of functional dyspepsia.Methods:Separated 50 functional dyspepsia(FD)patients into 2 groups by random.Then treated the antidepressant group antidepressants,while the antidyspepsia group was treated with drugs to cure primary digestive system disease by individual principle.And both groups were treated with supportive psychotherapy.The therapy continued for 8 weeks.Mental and symptoms state evaluation was done before and after treatment by Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)and by symptoms.Results:There was significant difference in the scores of SAS and SDS between the FD groups and normal groups(P0.05).Conclusions:FD patients commonly have depression or anxiety.Antidepressants have good efficiency in treatment of FD patients who has emotional impediment both for improving somatic and psychological symptoms.

Her2-overexpression is associated with tumorigenesis and metastasis. Due to significant clinical effects, Her2-targeted cancer therapy especially therapeutic antibody has become the hot spot in the field of cancer treatment. Anti-Her2 antibody drugs include monoclonal antibodies, antibody-drug conjugates, bispecific antibodies and emerging "two in one" antibody. Based on structure and function of Her2, this review focuses on recent advances in active mechanisms and clinical researches of these antibodies.

6.Risk factors and clinical prediction of shoulder dystocia in non-macrosomia

Objective To investigate the risk factors,clinical prediction and intrapartum management of shoulder dystocia in non-macrosomia.Methods Totally 7 811 cases of vaginal delivery were retrospectively reviewed from Juanary 2009 to December 2013 in Shengjing Hospital.Shoulder dystocia was found in 11 cases (0.14%,11/7 811),including 1 case of macrosomia and l0 cases of non-macrosomia (shoulder dystocia group).Each non-macrosomia shoulder dystocia case was matched with 10 cases of normal delivery in the same week,which were selected randomly as the control group.The tendency and risk factors of shoulder dystocia in macrosomia and non-macrosomia were analyzed,and the following data between the two groups were compared,including the height of uterus fundus,abdominal circumference of the pregnant woman,the increasing of body mass index(BMI),fetal biparietal diameter (BPD),fetal femur length (FL),duration of every stage of labor,birth weight of the newborn,head circumference and chest circumference of the newborn,Apgar score.Results (1) There were 213 macrosomias among the 7 811 vaginal deliveries,with the incidence of 2.73% (213/7 811).Only 1 shoulder dystocia was macrosomia (0.46%,1/213); while the other 10 cases were non-macrosomia (0.13%,10/7 598).(2) From 2009 to 2013,the macrosomia happened by 24 cases (2.32%,24/1 034),42 cases (3.61%,42/1 164),46 cases (2.60%,46/1 772),62 cases (3.01%,62/2 060),39 cases (2.19%,39/1781),respectively.The incidence of macrosomia had no significant difference among these 5 years (P>0.05).The shoulder dystosia occurrence without macrosia in these 5 years were 1 case (0.10%,1/1 034),3 cases (0.26%,3/1 164),2 eases (0.11%,2/1 172),2 cases (0.10%,2/2 060),2 cases (0.11%,1/1 781),respectively.The incidence of shoulder dystocia without macrosomia had no significant difference among these 5 years (P>0.05).(3) In the should dystocia group,5 cases were complicated with premature rupture of membrane (5/10),4 cases were mother≥ 35 years old (4/10),3 cases were multipara(3/10),3 cases had gestational diabetes mellitus(3/10),3 cases were occiput posterior during the first stage of labor(3/10),3 cases had prolonged second stage of labor (3/10) and 6 cases had routine lateral incision (6/10).In the control group,3 cases were complicated with premature rupture of membrane(3/10); 1 case was mother≥35 years old (1/10); 2 cases were multipara(2/10),3 cases had gestational diabetes mellitus (3/10),1 case had prolonged second stage (1/10) and 7 cases had routine lateral incision (7/10).(4) There were no significant difference in the height of uterus fundus,BMI,BPD,FL,and duration of the first stage of labor between the shoulder dystocia group and the control group (P>0.05).Compared with the control group,the increasing of BMI [(6.8±3.1) vs (4.8± 1.4) kg/m2],the time of the second stage of labor[(86±65) vs (38±28) minutes] and abdominal circumference[(108±8) vs (101±7) cm] were significantly higher in the shoulder dystosia group (P<0.05).(5)There were significant difference in the chest circumference of the newborn [(34.0±1.6) vs (32.2±1.9) cm] and the ratio of chest circumference to head circumference of the newborn [(0.99±0.03) vs (0.97±0.03)] between the two groups(P<0.05).The 1-minute Apgar score of the newborn (7.4±2.8) was significantly lower than the control group (10.0±0.0) (P<0.01).Clavicular fracture occurred in 3 newborns and brachial plexus injury occurred in 4 newborns in the shoulder dystosia group.Conclusion It is difficult to predict shoulder dystocia in non-macrosomia.Shoulder dystocia of non-macrosomia could be predicted by measurement of the head circumference,chest circumference,the ratio of chest circunfference to head circumference by using prenatal ultrasound.The risk factors may complicated with premature rupture of membrane,abnormal occiput position during the first stage of labor and prolonged second stage of labor.

8.Application value of cerebrospinal fluid MRLCs combined with lactate dehydrogenase detection in diagnosis of central nervous system leukemia

Objective To study the apllication value of cerebraspinal fluid minimal residual leukemia cells(MRLCs) combined with lactate dehydrogenase(LDH) detection in early diagnosis of central nervous system leukemia(CNSL) to provide a laboratory basis for clinical diagnosis and evaluation of disease progression and therapeutic effect.Methods Thirty inpatiernts without blood disease and central nervous system(CNS) organic diseases,and normal CSF routine and detected biochemical indexes were selected as the control group and 96 cases of acute leukemia (AL) diagnosed by bone marrow puncture FAB morphology and flow cytometry typing served as the disease group.Flow cytometry was used to detect the MRLCs in CSF,the activity of LDH in CSF was detected by the rate method,and the detection results were performed the comparative analysis.Results Among 96 cases of AL,30 patients with CNSL had 13 cases of positive MRLCs in CSF,the positive rate was 43.33%,among 66 cases of non-MRLCs,MRLCs was not detected detected.Among 96 cases of AL,CSF LDH level in 30 cases of CNSL was (30.54±10.29)U/L,which was significantly higher (16.16±7.12)U/L in the patients without CNSL,the difference between the two groups was statistically significant (P<0.05).Conclusion The detection of MRLCs and LDH in CSF of AL can be used as a laboratory diagnostic index for early diagnosis of CNSL,and MRLCs combined with LDH detection can improve the positive diagnosis rate of CNSL.
9.The value of MSCTA in differential diagnosis between epithelial ovariancarcinoma and borderline epithelial ovarian tumor

Objective To evaluate the value of multislice spiral computed tomography angiography (MSCTA) in differential diagnosis between epithelial ovarian carcinoma (EOC) and borderline epithelial ovarian tumor (BOT).Methods The MSCTA images of 39 EOC patients and 23 BOT patients confirmed by surgical pathology were reviewed retrospectively.Main characteristics of tumor vessels were analyzed: the number of feeding arteries, the existence of dilated draining veins, whether the tumor vessels were tortuous, whether the distribution of tumor vessels were disturbed, and whether there were accompanying microaneurysms or arteriovenous malformations (AVMs).Results Two or more feeding arteries of the EOCs and BOTs were 89.7% (35/39) and 8.7% (2/23), respectively.Dilated draining veins were observed in 87.2% (34/39) of the EOCs and 4.3% (1/23) of the BOTs.The tortuosity of tumor vessels was observed in 97.4% (38/39) of the EOCs and 13.0% (3/23) of the BOTs.79.5% (31/39) of the EOCs and 8.7% (2/23) of the BOTs were complicated by microaneurysms, and 74.4% (29/39) of the EOCs and 4.3% (1/23) of the BOTs were complicated by AVMs.The characteristics of tumor vessels were significantly different between the two groups (P<0.01), with relatively high sensitivity and specificity.Conclusion MSCTA can better show the distribution, number and pattern of tumor vessels and is of great value in differential diagnosis between EOC and BOT.
10.Effects and mechanisms of raltitrexed on the growth of human gastric cancer xenograft transplanted in nude mice

Objective:This study was designed to explore the effects and mechanisms of raltitrexed on the growth of the human gastric cancer cell line (MGC-803) transplanted in nude mice. Methods: Models of MGC-803 xenograft transplanted in nude mice were established. Body weight, mental state, food, and stool of the nude mice were closely monitored, and the inhibitory action in the tu-mors was evaluated after drug intervention. Distribution and apoptosis of the tumor cells were examined through flow cytometric assay. P53 mRNA and protein expression levels were determined through reverse transcription-polymerase chain reaction and Western blot analysis. Changes among the three groups were compared. Results:Body weights and tumor volumes of the nude mice were lower in the raltitrexed and 5-fluorouracil (5-Fu) groups than those in the control group. Tumor inhibition ratios were 49.02%and 45.75%in the raltitrexed and 5-Fu groups, respectively. In the G0/G1 stage, the number of cells decreased in the raltitrexed and 5-Fu groups. Signifi-cant differences were found between the experimental and control groups (P<0.01). In the S stage, the number of cells increased in the raltitrexed and 5-Fu groups, with significant differences from the control group (P<0.01). P53 mRNA and protein expression levels in the raltitrexed and 5-Fu groups were significantly higher than those in the control group (P<0.01), but no significant differences were found between the raltitrexed and 5-Fu groups (P>0.05). Conclusion:Raltitrexed can inhibit the growth of MGC-803 xenograft trans-planted in nude mice and shows inhibitory effect similar to that of 5-Fu. Raltitrexed can induce the S-phase block of the cell cycle and cell apoptosis in the MGC-803 xenografts in the nude mice. This drug may exhibit an antitumor effect by upregulating the p53 mRNA and protein expression levels.
