1.Impact of the timing of tratheostomy on patients treated with prolonged mechanical ventilation-A metaanalysis of randomized and quasi-randomized controlled trials

Objective To compare the early and delayed tracheostomy in patients treated with prolonged mechanical ventilation in respects of mortality.incidence of nosocomial pneunonia and length of ICU stay.Methods Randomized controlled trials(RCTs)and quasi-randomized controlled trials(quasi-RCTs)were performed by searching throush the Ovide MEDLINE(1996-2006.7),EMBASE(1980-2006.7),Cochrane Database(Issue 2,2006),Chinese Cochrane Centre Database and CBMdisc(1978-2006.7).The published and unpublished data and their references were searched.All RCTs and quasi-RCT of tracheostomy for critically ill patients treated withprolonged mechanical ventilation were included.Data were collected and evaluated by two reviewers independently and separately.RevMan version 4.2 software was used for data analysis.Results Two hundred and eighty-six patients from 4 RCTs and 106 cases from one quasi-RCT were taken for meta analysis.The meta-analysis showed that the early tracheostomy could reduce mortality(OR 0.69,95%CI 0.51,0.95),significantly shorten the duration of mechanical ventilation(WMD-8.49,95%CI-15.32,-1.66)and shorten the length of ICU stay as well(WMD-15.33,95%CI-24.58,-6.08)in patients treated with prolonged mechanical ventilation.but the incidence of nosocemial pneumonia was not different between two sorts of patients(OR 0.91,95%CI 0.70,1.18).Conclusions In case of prolonged mechanical ventilation,the tracheostomy performed at earlier stage may decrease the mortality,and shorten the duration of mechanical ventilation and the length of ICU stay.but cannot reduce the incidence of nosocomial pneumonia.
3.Relationship between ambulatory arterial stiffness index and extent of coronary atherosclerosis

Objective To study the relationship between ambulatory arterial stiffness index and extent of coronary atherosclerosis.Methods All subjects were underwent coronary CT angiography.Ambulatory arterial stiffness index(AASI) was measured before coronary CT angiography.AASI value between different periods in each group were calculated and compared.Results AASI between different periods in the normal control group and coronary heart disease group existed statistical difference (P < 0.05).The extent of coronary sclerosis and AASI between different periods in different coronary artery lesions groups were statistically different(P <0.05),AASI was proportional to the degree of coronary sclerosis,24 h AASI,DAASI,NAA SI were related to extent of coronary atherosclerosis lesions,the correlation coefficient between NAASI and extent of coronary atherosclerosis was the largest.Conclusion AASI night is related to coronary atherosclerosis most closely,namely the higher the value of the nighttime AASI,the greater the degree of coronary atherosclerosis.
4.Arthroscopic treatment of ulnar impaction syndrome

Objective To investigate ulnar impaction syndrome imaging characteristics and wrist arthroscopy in the diagnosis and treatment effects.Methods From September 2006 to April 2014,41 patients with ulnar impaction syndrome were treated.Of all 41 patients,28 were male and 13 female with an average age of 34.5±3.9 years old (ranging from 15 to 52 years old).The preoperative values of wrist range of motion (ROM) were 44.17°±6.22° at volar flexion,42.72°±3.37° at dorsal extension,17.45°±4.11° at radial deviation,19.12°±3.70° at ulnar deviation,66.81°±4.28° at pronation,and 70.27°±5.30° at supination.Arthroscopic exploration was conducted in all patients,aiming to observe the ulnar variance,wrist osteonecrosis,articular cartilage damage and triangular fibrocartilage complex tear.Arthroscopic synovectomy were performed in all cases;33 cases of articular cartilage injury were treated with arthroscopic chondroplasty;16 cases of osteonecrosis were treated with arthroscopic drilling decompression;30 cases with tear of TFC were treated with arthroscopic repair;26 cases with ulna positive mutation were treated with arthroscopic distal ulna resection.Patients were followed at 3,6,and 12 months after operation.At 12 months follow-up,the values of wrist range of motion was evaluated for volar flexion,dorsal extension,radial deviation,ulnar deviation,pronation,and supination.GreenO'Brien method was used to evaluate the function of wrist joint.Results Primary healing was presented in all patients without neurovascular injury and joint infection.All patients were followed up 12-36 months (mean,17.5 months).At 12 months followup,the values of ROM compared with that preoperative increased with significant difference,49.13° ±3.47° at volar flexion,47.13°±2.39° at dorsal extension,19.33°±3.29° at radial deviation,25.17°±4.89° at ulnar deviation,75.73°±7.66° at pronation,and 77.22°±4.89° at supination.According to Green-O'Brien standard,the results were excellent in 21 case,good in 17 cases and fair in 3 case,with 93% excellent and good rate.Conclusion Wrist arthroscopy dose not only contribute to the diagnosis in ulnar impingement syndrome but also can be used in treating inflammatory synovium and cartilage lesions,bone drilling decompression,TFC repair and arthroscopic distal ulna resection with improvement in the range of motion of the wrist joint and function.
5.Influence of endoscopic lumbar discectomy on limb function and quality of life in patients with lumbar intervertebral disc protrusion

Objective To explore and analyze the influence of endoscopic lumbar discectomy on limb function and quality of life in patients with lumbar disc prolapse.Methods 90 cases of lumbar intervertebral disc protrusion were selected,and they were divided into control group and observation group according to the operation method,45 cases in each group.The control group was given open lumbar disc excision treatment,the observation group was given percutaneous endoscopic lumbar disc resection treatment.The perioperative therapeutic index,physical function and quality of life were compared between the two groups.Results The intraoperative blood loss,time in bed,hospital stay,postoperative VAS score of the control group were (60.21 ± 20.32) min,(30.36 ± 14.25) mL,(26.96 ± 9.69) d,(8.21 ± 2.36) d,(5.12 ± 1.36) points,respectively,which of the observation group were (58.96 ± 19.88) min,(12.88 ± 10.28) mL,(100.69 ± 18.75) d,(12.99 ± 3.88) d,(1.98 ± 0.33) points,respectively.Compared with the control group,the observation group had less intraoperative blood loss,shorter time in bed,shorter hospital stay,lower postoperative VAS score,the differences were statistically significant (t =4.25,4.66,5.12,5.78,6.12,all P < 0.05).The excellent and good rate of the control group was 89.89%,which of the observation group was 93.33%,there was no significant difference between the two groups (x2 =7.68,P > 0.05).After treatment,the physical function,role function,emotional function,cognitive function and social function score of the control group were (7.89 ± 0.78) points,(7.99 ± 1.25) points,(7.84 ± 0.65) points,(7.65 ± 0.75) points,(7.45 ± 0.77) points respectively,which of the observation group were (8.96 ± 1.78) points,(8.75 ± 1.12) points,(8.56 ± 0.88) points,(8.42 ± 0.66) points,(8.56 ± 1.47) points,respectively.Compared with before treatment,the physical function,role function,emotional function,cognitive function and social function score in the two groups after treatment were higher.After treatment,compared with the control group,the scores increased more significantly in the observation group (t =6.55,7.56,7.88,7.56,7.89,all P < 0.05).Conclusion Endoscopic resection and open lumbar disc excision technique in treatment of patients with lumbar intervertebral disc protrusion of lumbar can obtain similar curative effect,but the former has less incision,less intraoperative blood loss,postoperative recovery fast,can better improve the patients' quality of life.
6.Diagnosis and treatment of severe obesity complicated with type 2 diabetes mellitus

The incidence of obesity increases worldwide every year,positioned as an important disease by WHO,and it has become an important public health problem.National survey shows that overweight and obesity rates were 22.8% and 7.1% respectively in Chinese adults in 2002,affecting 270 million people.At present,bariatric surgery is a safe and effective treatment for severe obese patients.In July 2011,a patient with Ⅲ degree obesity (weight 148 kg and BMI 51 kg/m2) complicated with type 2 diabetes mellitus was admitted to the Third Xiangya Hospital of Central South University.The treatment plan was made after diagnosis and treatment of muhidisciplinary team and the patient underwent laparoscopic sleeve gastrectomy.The patient was followed up to September 2012 and recovered well without severe surgical complications,and the weight and BMI were decreased to 75 kg and 25 kg/m2 respectively.
7.Research on 3D printed β-TCP/HA/PLA scaffolds for bone grafts

Objective To develop the personalized 3D printing scaffolds for bone grafts,to meet the needs of the patients with the bone defect.Methods The model of three-dimensional woodpile structure was designed by the software of computer-aided design (CAD).The β-tricalcium phosphate (β-TCP),hydroxyapatite (HA),and polylactic acid (PLA) composite scaffolds with three-dimensional woodpile structure was simulated using 3D printing method by three-dimensional air floating platform.The three-dimensional bone graft scaffolds was then vacuum heat-treated,and the residual chloroform was detected by energy dispersive X-ray spectrometer.The morphology of the β-TCP/HMPLA scaffolds was characterized by scanning electron microscope,and in vitro cytotoxicity against osteoblasts hFOB 1.19 was assessed by thiazolyl blue tetrazolium bromide (MTT) assay.Results When the extrusion pressure of printing slurry was 137.9-413.7 kPa,the three-dimensional bone graft scaffolds could be printed out.Then the scaffolds was vacuum treated at 90 ℃ and preheated at 150 ℃,which could eliminate the solvent CHC13.The three-dimensional bone graft scaffolds,with a through-hole and rough surface,was co-cultured with osteoblasts hFOB1.19 for 7 d,and it's cytotoxicity grade was grade 0.Conclusions The three-dimensional bone graft scaffolds have a through-hole and rough surface,which is favorable to the osteoblasts culture and bone induction,indicating the advantage and development prospects of 3D printing in the preparation of porous materials for bone grafts.
8.Multiple keratoacanthoma centrifugum marginatum: first case report in China

A case of multiple keratoacanthoma centrifugum marginatum is first reported in China. A 37-year-old woman was admitted to the hospital for papules and plaques on her face, which had been increasing in number for 4 months. Cutaneous examination revealed dozenes of well-marginated, pale-red or skin-colored crateriform papules of variant size, and plaques in a geographic pattern on her face. The papules presented with a central umbilication filled with grey-brown corneous material. The plaques were surrounded by dyke-like borders, covered with thick, crusted brown corneous material, and partly depressed in the center. Histopathology showed hyperkeratosis, parakeratosis, irregular strip-like extension of epidermis into dermis, keratinous cysts and squamous eddies. The tumor cells had eosinophilic and glassy cytoplasm characteristic of keratoacanthoma.Given both the clinical and histologic evidence, a diagnosis of multiple keratoacanthoma centrifugum marginatum was made. After more than 3 months of treatment with oral acitretin and topical tretinoin, the lesions faded,leaving rugosity scars. No relapse was noted during 3-year follow-up.
9.Expression of hepatitis B virus core gene in Pichia pastoris

AIM: To study the expression of hepatitis B virus core gene in Pichia pastoris and to obtain high-level expressed recombinant HBcAg with good immunoreactivity and high specificity. METHODS: HBV core gene was amplified by PCR from plasmid pHBV1 which contained HBV whole DNA sequence. The PCR product was cloned into pGEM-T vector by TA cloning strategy. After confirmed by DNA sequence analysis, the gene of interest was inserted into the yeast expression vector pPIC9. The recombinant plasmid pPIC9-cAg was constructed and transformed into GS115 by electroporation. The recombinant yeast GS115 was induced by 0.5% methanol. The expressed product was analysed by SDS-PAGE,Western blot and ELISA. RESULTS: The restriction analysis and DNA sequence analysis proved that HBV core gene had already been cloned to yeast expression plasmid pPIC9. The expressed HBcAg existed in SDS-PAGE. Good immunoreactivity and high specificity of the recombinant HBcAg have been proved by ELISA and Western blot. The titre of the recombinant HBcAg in the cell lysate was 1∶ 12 800 . CONCLUSION: The recombinant plasmid pPIC9-cAg was successfully constructed. The recombinant HBcAg with good immunoreactivity and high specificity was successfully expressed in Pichia pastoris expression system and can be applied to further developing HBcAb immunoassay. [
10.Expression of E-cadherin, β-catenin in labial salivary glands of patients with primary Sj(o)gren's syndrome

Objective To study the expression of E-cadherin( E-cad), p-catenin(β-cat) in labial salivary glands of patients with primary Sj(o)gren's syndrome (pSS) in order to explore their role in pathogenesis. Methods Biopsies of labial salivary glands were obtained from 52 patients with pSS and 30 healthy controls. The immunohistochemical staining was used to detect the expression of E-cad and β-cat. Anti-SSA and anti-SSB antibodies were measured. Semi-quantitative analysis was performed by image analysis software. Ultra-structural changes was used by electron-microscopic techniques. Results ① The area of expression, optical intensity and the accumulated optical intensity of the E-cad group [(2513±1086) μm2, 0.212±0.041, 566 ±297 ] were lower than normal controls. The expression level was reduced as the increase of lymphocyte infiltration focus. ② The area of expression, the optical density and the accumulated optical density of the β- cat group [(12 324±7883) μm2, 0.113±0.031, 566±297] was lower than those of the control group. The expression level was reduced as the increase of the lymphocyte infiltration focus. ③ The E-cad expression and the p-cat expression was positively correlated in the labial gland of patients with pSS. ④ Howev-er, there was difference in the expression of E -cad and β -cat between patients with positive SSA and negative SSA antibodies. Conclusion In salivary samples, the expression of both E-cad and p-cat in patients with pSS is lower than those of the controls. Anti-SSA/SSB antibodies are important parameters of pSS and they may be involved in the pathogenesis of pSS.
