Fifty-six fasting women undergoing elective caesarean section received one of the following five glucose regimens before delivery respectively, group Ⅰ:0.9% saline 800ml as control; group Ⅱ:120ml of 5% dextrose(6g) in normal saline and 680ml of 0.9% saline;group Ⅲ:240ml of 5% dextrose (12g) in normal saline and 560ml of 0.9% saline group Ⅳ:480ml of 5% dextrose(24g) in normal saline and 320ml of 0.9% saline;group Ⅴ:800ml of 5% dextrose in normal saline(40g) The results showed that the maternal and umbilical cord venous blood sugar at delivery were significantly increased in group Ⅳ and Ⅴ. In group Ⅴ,neonatal acidosis(pH=7.19?0.02) was induced, PaCO_2 and lactate elevated to 58.5?2.8mmHg and 26.1?2.5mg% separately, neonatal hypoglycaemia (36?6.8mg% )occured 2 hours after delivery. There was higher incidence of neonatal jaundice in infants of group Ⅴ than in thoseof group Ⅰ and Ⅱ. It is suggested that the larger amounts of intravenous dextrose to the maternal is harmful to the neonate during caesarean section.

The bacterial strain CSS-4-2 isolated from the spleen of Carassius auratus of penze(♀) ? Cyprinus acutidorsalis (♂) with septicemia was proved to b e a pathogen by artificial infection. This bacteria has short rod-shaped cells , Gram negative and motile, with single polar flagellum, and with not capsule. No spore was formed , and it was resistant to 0/129. It could utilize glucose, mal t ose ,mannite, sucrose and inositol, but could not utilize malonate, lactose, xyl ose, raffinose, sorbin and adonitol. Oxidase and arginine dihydrolase was posit ive.While urease, lysine decarboxylase and ornthine decarboxylase was negative .Bacteriological identification by VITEK-AMS-60 showed the strain CSS-4-2 w as Aeromonas caviae. Drug sensitivity assay showed that this strain was sen sitive to s treptomycin, tobramycin, chloromycetin, rifampicin, norfloxacin and nitrofuranto in, but resistant to penicillin G.

The growth property of pathogenic bacterium Aeromonas caviae strain CSS-4-2 which was isolated from the spleen of Carassius auratus of penze(♀) ? Cyprinus acutidorsalis (♂) that diseased septicemia was studied.The growth curve of CSS-4-2:lag phase was in 0～3h,log phase was in 3～30h, stationary phase was in 33～36h,decline phase was after 36h when CSS-4-2 was cultured shaking in tryptone liquid medium. It's optimum growth temperature ranges from 25℃ to 30℃, optimum growth pH ranges from 5 to 10, and optimum salinity ranges from 0% to 1% Nacl concentration .

Objective:To study the significance of QT dispersion(QTd) changes in electrocardiogram(ECG) after coronary artery bypass graft. Methods:Forty eight patients scheduled for elective coronary artery bypass graft were studied. Objective To explore the diagnostic value of bronchofiberscopic washing cytology (BWC) and bronchoalveolar lavage fluid (BALF) ThinPrep cytology test (TCT) in the diagnosis of lung cancer.Methods Three hundred and eighty-five lung cancer patients who completed the BWC and BALF TCT were selected.The sensitivity was compared between the conventional smear and the two TCT methods.The sensitivity and accuracy of BWC and BALF TCT in the diagnosis of lung cancer were analyzed.Results The sensitivity of TCT was 76.1％ (293/385) in the diagnosis of lung cancer,and conventional smear was 54.8％ (211/385).The sensitivity of TCT was significantly higher than that of conventional smear (P ＜ 0.05).In the diagnosis of central lung cancer,the sensitivity of BWC TCT and BALF TCT was 74.4％ (131/176) and 70.5％ (124/176),and there was no statistical difference (P ＞ 0.05).In the diagnosis of peripheral lung cancer,the sensitivity of BALF TCT and BWC TCT was 83.3％ (174/209) and 44.0％ (92/209),and there was statistical difference (P ＜ 0.05).In BWC TCT,the accuracy of TCT and histology in squamous cancer was 92.7％ (127/137),in adenocarcinoma was 92.6％ (151/163),in small cell cancer was 96.8％ (61/63),and total accuracy was 91.9％ (354/385).In BALF TCT,the accuracy of TCT and histology in squamous cancer was 92.0％ (126/137),in adenocarcinoma was 94.5％ (154/163),and in small cancer was 98.4％ (62/63),and total accuracy was 92.7％ (357/385).Conclusions TCT has more diagnostic value in lung cancer.BWC and BALF TCT show different sensitivity in the diagnosis of lung cancer of different kinds.BWC TCT combination with BALF TCT is helpful to improve the diagnosis of lung cancer.

Objective To survey the current status of job stressor and exhaustion among nursing staff in the catheter room of grade Ⅲ-A general hospitals and to look for regularities.Methods 178 nursing staff coming from catheter room,operating room,and outpatient department of Shenzhou Hospital and Fengtian Hospital accepted questionnaires of Chinese Nursing Stressor Scale and Exhaustion Scale in Spring 2010.Results Chinese Nursing Stressor Scale indicated that scores of subitem and total score of job stressor in the catheter room were significantly higher than those in the outpatient department; however,there were no significant differences in scores of subitem and total score of job stressor between catheter room and operating room.On the other hand,Exhaustion Scale showed that scores of emotional exhaustion and job unconcerned feeling in the catheter room were significantly higher than those in the outpatient department,but scores of job achieved feeling in the catheter room were significantly lower than those in the outpatient department.There were no significant differences in Exhaustion Scale between catheter room and operating room nurses.Conclusions Job stressor and exhaustion were serious among nursing staff in the catheter room of grade Ⅲ-A general hospitals,which should cause the attention of management department.

Objective To evaluate the efficacy of circinal stapled suture and manual suture in cervical esophagogastric anastomosis in esophageal resection.Methods The clinical data of 187 patients with esophageal cancer who were admitted to the Cancer Hospital of Sichuan Province from January 2010 to January 2012 were retrospectively analyzed.All the patients were divided into the stapled suture group (98 patients) and manual suture group (89 patients).The time of anastomosis,operation time,time for dieting,duration of hospital stay,the incidence of postoperative complications and positive rate of esophageal remnant cancer cells of the 2 groups were compared.All data were analyzed using the t test or chi-square test.Results The time of anastomosis,operation time,time for dieting and duration of hospital stay were (7.8 ± 1.4) minutes,(227 ± 60) minutes,(6.3 ± 0.9) days and (14 ±4)days in the stapled suture group,which were significantly shorter than (28.5 ±2.3)minutes,(301 ±81)minutes,(8.4 ± 1.0)days and (22 ±9) days in the manual suture group (t =75.44,7.14,7.71,7.41,P ＜0.05).The incidence of anastomotic fistula was 1％ (1/98) in the stapled suture group,which was significantly lower than 8％ (7/89) of the manual suture group (P ＜ 0.05).The incidence of anastomotic stricture was 5％(5/98) in the stapled suture group,which was lower than 7％ (6/89) in the manual suture group,but no significant difference was detected (P ＞0.05).The positive rate of esophageal remnant cancer cells was 0(0/98),which was signifiantly lower than 4％ (4/89) in the manual suture group (P ＜0.05).Conclusion Circinal stapled suture in esophagogastric cervical anastomosis not only reduce the time of anastomosis,operation time and duration of hospital stay,but also decrease the incidence of anastomotic fistula and the positive rate of esophageal remnant cancer cells.

Objective To establish the weights of risk factors indicators for lower limbs deep vein thrombosis through the scientific research methods.Methods 21 experts were consulted for two rounds of questionnaires survey with Delphi method,according to the experts'opinion,analytic hierarchy process (AHP) was used to calculate the weights of indicators in grade one and the experts'evaluation was used to calculate the weights of indicators in grade two.Results The weight coefficients of grade one including vascular endothelial injury,blood high condensation state and slow blood stream were 0.3325,0.5278 and 0.1396 respectively.The items with maximal weights of the 21 indicators in grade two was vascular trauma,in particular vein injury(0.0770).The Kendall's W of the indicators in grade two was 0.422.Conclusions Through qualitative and quantitative research methods,the risk factors indicators which provide the reference for the prevention of lower limbs deep vein thrombosis are scientific and reliable.

Objective To determine the 95 ％ effective target plasma concentration (EC95) of remifentanil for tracheal tube tolerance during the recovery period from anesthesia following cervical spine surgery.Methods Thirty ASA Ⅰ or Ⅱ patients,aged 18-60 yr,weighing 50-80 kg,scheduled for elective cervical spine surgery under total intravenous anesthesia,were enrolled in this study.Anesthesia was induced with iv injection of propofol,sufentanil and rocuronium.The patients were tracheal intubated and mechanically ventilated.Anesthesia was maintained with iv infusion of propofol and target-controlled infusion of remifentanil.The target plasma concentration (Cp) of remifentanil was set at 4-6 μg/L.BIS value was maintained at 40-60.Infusion of propofol was stopped at the end of surgery.Participants were allocated to a dose of remifentanil by 3-patient cohorts.Six Cps were selected from 1.0-3.5 μg/L before beginning and they were 1.0,1.5,2.0,2.5,3.0,3.5 μg/L.The Cp of remifentanil was 3.0 μg/L in the first cohort.After completion of the trial in each cohort,the posterior probability of each concentration was calculated according to the condition of sedation/analgesia and anterior probability of each concentration.The concentration with the posterior probability closest to 95 ％ was chosen as Cp in the next cohort.The concentration-probability curve was made according to the posterior probability of each concentration,and then EC95 and 95 ％ confidence interval of remifentanil were calculated.Results The EC95 and 95 ％ confidence interval of remifentanil were 2.77μg/L (2.65-2.83 μg/L) for tracheal tube tolerance during the recovery period from anesthesia following cervical spine surgery.Conclusion The EC95 of remifentanil for tracheal tube tolerance during the recovery period from anesthesia is 2.77 μg/L in patients undergoing cervical spine surgery.
