Using isoeugenol as the sole carbon source,a novel strain,producing high amounts of vanillin from isoeugenol,was isolated from soil.According to the physiological and biochemical characteristics and its 16S rRNA gene sequence analysis,it was identified as Bacillus fusiformis.The initial results showed that 4.20 g/L vanillin was obtained by bioconversion of 2% isoeugenol with Bacillus fusiformis.

Practice has showed that,compared with traditional teaching method, special topic combined with case teaching method can arouse students' leaning interest, improve comprehensive analytical ability and innovate clinical performing skills.It is an effective clinical teaching method.

Objective To evaluate the effects of ulinastatin on acute lung injury during ortho- topic liver transplantation(OLT).Methods Twenty ASAⅢ-Ⅳpatients with end-stage liver disea- ses.undergoing OLT were randomly divided into two groups.Ulinastatin group received intravenous infusion of ulinastatin(3?10~4 IU in 100 normal saline)after skin incision and every 4 h thereafter(n =10).Control group received same amount of normal saline instead of ulinastatin(n=10).Blood sam- pies were taken before skin incision,120 min after skin incision,30 min after liver was removed,5 min and 60 min after reperfusion of the graft and at the end of operation for determination of plasma IL-6, IL-8,TNF-?,MDA concentrations and SOD activity.Respiratory index(RI)[P_(A-a)DO_2/PaO_2]was cal- culated before skin incision,30 min after liver was removed,5 min and 60 min after reperfusion of the graft and at the end of operation.After anesthesia was induced,cardiac output,mixed venous oxygen saturation and central venous temperature were continuously monitored during operation.ECG,CVP, SpO_2,P_(ET)CO_2,radial artery and MPAP were also continuously monitored during operation.P_(ET)CO_2 was maintained at 35-40 mm Hg during operation.Blood temperature was maintained above 35.5℃during operation.Results In group C plasma IL-6 and IL-8 concentrations were significantly increased from 120 min after skin incision to the end of operation as compared with the baseline values(P0.05).RI was significantly lower at 60 min after reperfusion of the graft and at the end of operation in group U than in group C(P

Objective To improve the diagnosis and treatment of testicular masses associated with congenital adrenal hyperplasia(CAH).Methods A 25-year-old man presented with bilateral testicular nodules.Physical examination revealed palpable multiple nodules in the bilateral testes and epididymides as well as enlarged and hard right testis with uneven surface.Biochemical examination showed testosterone(T) of 18.7 nmol/L,17?-(OH)P＞20.0 ng/ml,positive results of rapid ACTH stimulation test and dexametha- sone suppression test.B-ultrasound demonstrated diffuse testicular lesions with nodules in parts of the bilater- al testes.Bilateral testicular nodules were biopsied,and pathological diagnosis was interstitial cell hyperplasi- a.Positive staining for?-inhibin was observed immunohistochemically.The clinical characteristics of testicular masses associated with CAH were reviewed in combination with the literature.Results Based on medical history,imaging,laboratory and pathologic examinations,the patient was diagnosed with testicular masses as- sociated with CAH.He received oral cortisol at dose of 40 mg daily for 2 weeks,and then at dose of 30 mg daily as maintenance therapy.After treatment of 8 months,B-ultrasound showed complete regression of the testicular tumors.Follow-up of 24 months showed no recurrence of the tumor.Conclusions For patient with bilateral testicular masses,endocrinological evaluation and clinical history taking are indicated to exclude testicular interstitial cell hyperplasia associated with CAH.By oral steroid replacement therapy,hormone-sen- sitive testicular masses can regress.

Objective To determine how genetic polymorphisms in norepinephrine transporter (NET) gene influence the response of antidepressant treatment and how they interact with childhood trauma and recent life stress in a Chinese depressive patients.Methods 281 Chinese Han depressive patients received single antidepressant drugs for 6 weeks.Hamilton Depression Scale-17 (HAMD-17),the Childhood Trauma Questionnaire short term (CTQ-SF) and the Life Events Scale (LES) were used to evaluate severity of depressive symptoms and the occurrence of stressful life events respectively.Three single nucleotide polymorphisms (SNPs) in norepinephrine transporter were genotyped.Associations of single locus and haplotypes with antidepressant treatment response were analyzed using UNPHASED 3.0.13.The interaction of gene and life stress was analyzed by SPSS13.0 software.Results One NET SNP rs2242446 was significantly associated with antidepressant response in this Chinese male sample(0.4118vs0.2375,x2=7.046,P=0.0079,OR=0.445,95％ CI (0.243-0.815)),as was the haplotype CG(rs2242446 and rs5569;x2 =5.886,P=0.0153,OR=0.457,95％ CI (0.198-1.054)) and another haplotype CG-G(rs2242446,rs1532701 and rs5569;x2=5.360,P=0.0206,OR=0.530,95％ CI (0.202-1.386)) of NET in male samples.The NET SNPs rs5569 demonstrated interaction with childhood trauma to influence antidepressant response(β=-2.727,SE =1.195,P=0.023,OR=0.065,95％ CI (0.006-0.681)).Conclusion Antidepressant drug response was influenced by not only NET genetic polymorphisms in norepinephrine transporter gene but also interaction between the NET genetic polymorphisms and early life stress.

Objective:To investigate differences in serum uric acid levels between elderly patients with prostate cancer and patients with benign prostatic hyperplasia(BPH).Methods:A total of 300 prostate cancer patients admitted to the urology department of our hospital between Feb.2010 and Jun.2019 were retrospectively analyzed.During the same period, 240 BPH patients and 400 elderly men with normal prostate size were enrolled as the control group.Serum uric acid and prostate-specific antigen(PSA)levels, C-reactive protein(CRP), neutrophil count and lymphocyte count were determined.Serum uric acid concentrations were monitored in prostate cancer patients with different clinicopathological characteristics.Results:CRP and Neu/Lym levels were higher in the prostate cancer group than in the BPH and control groups( P<0.05). The serum uric acid level was (327.0±58.3)μmol/L in the prostate cancer group, lower than in the BPH group(375.2±68.4)μmol/L and the control group(377.8±73.2)μmol/L( F=55.69, P<0.001). Multivariable Logistic regression analysis indicated that serum uric acid was a protective factor for prostate cancer( OR=0.593, 95% CI: 0.542-0.718, P=0.004). There were significant differences in serum uric acid levels between prostate cancer patients with different ages and pathological grades( t=-4.63, F=12.73, P<0.001). However, serum uric acid levels were not significantly correlated with clinical staging or lymph node metastasis( F=-2.72 and 0.77, P=0.068 and 0.460). Conclusions:Compared with BPH patients and healthy males, serum uric acid levels are reduced and inflammatory markers are increased in prostate cancer patients, indicating that serum uric acid may be a risk factor for the occurrence and progression of prostate cancer in the elderly.

Objective To analyze the incidence, clinical feature and late complications, and treatment for diabetes ketoacidosis (DKA) in children with type 1 diabetes mellitus.Methods Ninty children with type 1 diabetes mellitus within 10 years were retrospectively reviewed.The onset situation,clinical feature and long-term complication,and treatment of DKA were analized.Results High morbidity was found in 10 to 16 years old children.DKA was often caused by infection; late complications of diabetes mellitus was resulted from interrupted injection of insulin.Conclusions Emergency treatment for DKA may involve the injection of small dose insulin,correction of the disorder of water and electrolysis and regulation of acid-base.The education of patients and parents about diabetes mellitus and long-term injection of insulin are of importance in preventing the complications.
