Voiding dysfunction is a serious complication after spinal cord injury.Now we have no effective treatment to deal with it,related operations mainly belong to reconstructive methods without long-term certain results.Neuromodulation,as a kind of way to suppress overactive detrusor,supposes to be an effective method.This paper would discuss its origin,application of dorsal penile nerve modulation and its candidate mechanism to certify the feasibility of neuromodulation on spinal cord injury patients.
2.Factors influencing olfactory dysfunction in patients with chronic rhinosinusitis.

OBJECTIVE:
To evaluate the relative factors influencing olfactory dysfunction in patients with chronic rhinosinusitis (CRS).
METHOD:
Visual analogue scale (VAS) was applied to measure the severity of olfactory dysfunction of 270 patients with CRS. Patients were divided into two groups, one was that the quality of life (QOL) of patients was affected by olfactory dysfunction (VAS > 5), the other was that without QOL affected by olfactory dysfunction (VAS ≤ 5). The association between age, gender, nasal polyps, allergic rhinitis, smoking history, early nasal surgery history and other clinical factors, and serum total IgE level, peripheral blood eosinophil count, peripheral blood mononuclear cell count and olfactory dysfunction was analyzed.
RESULT:
The number of patients with nasal polyps, allergic rhinitis, previous nasal surgeries, the level of serum total IgE, and the severity of edema were significantly increased in patients with impaired QOL associated with olfactory dysfunction (P < 0.05). Sex distribution, age, smoking history, deviation of nasal septum, eosinophil and mononuclear cell count did no statistically differ between the groups with and without impaired QOL associated with olfactory dysfunctions (P > 0.05). Serum total IgE increased (OR = 1.003, P < 0.01) and severe edema (OR = 2.483, P < 0.01) were the risk factors for the impairment of olfactory function, more notably for edema; whereas previous nasal surgeries was a protective factor (OR = 0.408, P < 0.01).
CONCLUSION
Sever edema and increased serum total IgE are risk factors, whereas previous nasal surgeries history is a protective factor for the olfactory dysfunction.

3.Hypertension management for migrant workers in the community of Shenzhen city

The management model “primary care in community,team service,first contact physician responsibility and two-way referral” was implemented for migrant workers with hypertension in the community.The hypertensive patients under the management for over six months were assigned as management group; the patients who were registered but not under management for over six months served as the control group.The number of new cases detected in hypertension screening increased from 32 in 2008 to 701cases in 2010; and number of hypertensive patients under management increased from 7 cases to 348 accordingly.A questionnaire survey was conducted in 464 hypertensive patients,including 196 cases in management group and 268 cases in control group with a response rate of 94.9% (186/196) and 92.5% (248/268) respectively.The results showed that drug compliance rate in management group (168/186,90.3% ) was significantly higher than that in the control group ( 26.6%,66/248,P =0.00 ).The rate of blood pressure control in management group (87.1%,162/186 ) was significantly higher than that in the control group (25.8%,64/248,P =0.00).The overall satisfaction (98.9%) in management group was significantly higher than that in the control group (88.7%,P =0.00 ).The results indicate that implementation of the hypertension management in migrant workers improves the health seeking behavior and blood pressure control of patients.
4.The relationship between carotid intima-media thickness and coronary atherosclerosis plaque in asymptomatic individuals

Objective To investigate the relationship between carotid intima-media thickness (IMT) and coronary atherosclerosis plaque through coronary CT angiography (CTA) in asymptomatic individuals.Methods Two hundred and fifty cases with CTA screened were selected in asymptomatic individuals,and according to the inspection result they were divided into no plaque group (detection of no coronary atherosclerosis plaque,180 cases) and plaque group (detection of coronary atherosclerosis plaque,70 cases).All cases were checked carotid IMT with B-ultrasonic.Results ANOVAs analysis showed that age and body mass index (BMI) had significant correlation with coronary atherosclerosis plaque (P < 0.05).x2 test was used to show the smoking and family history of coronary heart disease had significant correlation with coronary atherosclerosis plaque (P < 0.05).Univariate Logistic regression analysis showed that carotid IMT,systolic pressure,diastolic pressure,low density lipoprotein cholesterol,fasting serum glucose and creatinine had significant correlation with coronary atherosclerosis plaque (P < 0.05).Multifactor Logistic regression analysis showed that carotid IMT had correlation with coronary atherosclerosis plaque (P < 0.05).Conclusion There is significant relationship between carotid IMT and coronary atherosclerosis plaque in asymptomatic individuals.
5.Long-term results and prognostic factors after anatomical hepatectomy for hepatocellular carcinoma: a report of 90 patients

Objective To investigate the safety and long-term therapeutic results of anatomical hepatectomy for hepatocellular carcinoma (HCC).Methods 90 patients underwent anatomical hepatectomy from January,2003 to January,2010.The tumor size was ≤5 cm in 38 patients,and >5 cm in 52 patients.The tumor number was a single lesion in 58 patients,and multiple lesions in 32 patients.Cirrhosis was present in 46 patients (55.42%); cancer embolus was present in 34 patients (37.78%).There were 64 patients in Child-Pugh A and 26 patients in B.The operating time,blood loss and blood transfusion were recorded and analyzed.Complications and liver function were monitored after surgery.The Kaplan-Meier method was used to calculate the survival rates.Log-rank test was used to analyze factors associating with postoperative recurrence.Independent factors influencing tumor-free survival and overall survival were analyzed by Cox-model logistic regression.Result There was no perioperative death.The incidence of complications was 31.1% (28/90).The recurrence rate was 51.1% (46/90).The 1-,3-,5 year tumor-free survivals were 92.2%,67.3% and 49.7%%00,respectively.Positive resection margin and satellite nodule were independent factors for recurrence (RR19.22,95 % confidence interval 5.85~63.17).The 1-,3-,5-year overall survivals were 94.4%,80.0% and 60.0%,respectively.Positive resection margin and TNM were independent factors for overall survival (RR 2.013,95% confidence interval 1.28~3.17).Conclusions Anatomical hepatectomy was a safe and efficacious procedure to treat HCC.Positive resection margin and TNM were independent factors associated with overall survival.Anatomical hepatectomy had the advantages in ensuring a lower rate of negative resection margin.
7.Effective research for systemic intervention to occupation burnout of medical staffs

Objective To study effectiveness of System intervention to occupation burnout of medical staff.Methods Specimens were classified as interventional group and control group by 1:1 partnership rule.Systemic intervention,which came from organization,family and individual,was only applied to interventional group.The objective was to establish requirement and resource mechanism,continually increase response resource,and improve matching state of individual & job.It allowed full play to the first grade prevention(development intervention),the second grade prevention(symptom intervention)and the third grade prevention(burnout intervention).It determined effectiveness of intervention via indexes of occupation burnout,mental health and satisfaction within group or internal groups.Results Before systemic intervention,there was no significant difference between intervention group and control group(P>0.05).After systemic intervention,the scores of burnout(emotion exhaustion(17.54±9.97)vs(14.30±11.87),P<0.01;depersonalization(5.29±5.01)vs(4.63±5.65),P<0.05;low personal accomplishment(6.47±4.89),(5.61±5.10),P<0.01),check list-90(SCL-90),personal satisfaction in intervention group showed apparent difference compared with that in control group.But there was no significant difference in control group before and after intervention.Conclusion On the whole systemic intervention to health care staff obtains obvious achievement,effectively reduces emotional exhaustion,obsessive symptom and terror emotion,and enhances individual satisfaction.
8.The correlation between the MRI performance of breast cancer and biological factors ER,PR,c-erbB-2,p53

Objective To explore the correlation between MRI performance and biological factors of estrogen receptor (ER),progesterone receptor (PR),human epidermal growth factor receptor 2 (c-erbB-2)and p53 through a group cases of breast cancer.Methods The MRI characteristics of 39 cases with breast cancers confirmed by surgery were analyzed .The MRI characteristics of rim enhancement,speculate, the type of time-intensity curve (TIC),the early-phase enhancement ratio,peak to time,apparent diffusion coefficient (ADC)value were recorded and the lesions’pathological results of inmmunohistochemistry:the expression of ER,PR,c-erbB-2,p53 were aslo collected.The correlation between MRI characteristics and biological factors were analyzed with statistical software SPSS1 9.0.Results Spiculate was positively correlated with ER positive expression in breast cancer.Peak time of breast cancer and positive expression of p53 was negatively correlated.ADC value of breast cancer and positive expression of p53 was negatively correlated.Conclusion There are certain correlations between MRI characteristics and biological factors of breast cancer ER,PR,c-erbB-2,p53.
9.Relationship between absent or hypoplastic fetal nasal bone and chromosome abnormalities: analysis of 187 cases

Objective To investigate the relationship between absent or hypoplastic fetal nasal bone and chromosome abnormalities.Methods From January 2010 to April 2014,187 fetuses were found to have absent or hypoplastic nasal bone by prenatal ultrasound scanning in Guangzhou Maternal and Children's Hospital.All the pregnant women should undergo interventional prenatal diagnosis for fetal chromosome abnormalities,and should be followed up for three months after the expected delivery date.The correlation between absent or hypoplastic fetal nasal bone and chromosome abnormalities,and the effects of complicating structural defects were analyzed by descriptive analysis and the Chi-square test.Results Of the 187 pregnant women,126 underwent interventional prenatal diagnostic tests,and fetal chromosome abnormalities were detected in 36 cases (28.6%),including 26 cases (20.6%) of trisomy 21,6 cases (4.8%) of trisomy 18,three cases (2.4%) of trisomy 13 and one sex chromosome chimerism.In the 126 cases received prenatal diagnosis,the incidence of chromosome abnormalities in fetuses without other structural defects was significantly lower than that with structural defects [12.7% (8/63) vs 44.4% (28/63),x2=15.556,P=0.000].Among 63 cases without other structural defects,seven fetuses were confirmed to have chromosome abnormalities in 14 women with high risk by Down syndrome screening,no chromosome abnormalities were found in 39 pregnant women with low risk by Down syndrome screening,and one sex chromosome chimerism was found in the other ten women who did not undergo Down syndrome screening.Absent or hypoplastic nasal bone detected in the first trimester resulted in a higher risk of chromosome abnormalities than that detected in the second and the third trimester [25.5% (28/110) vs 10.4% (8/77),x2=6.613,P=0.007].Conclusions When a fetus is found to have absent or hypoplastic nasal bone,it is necessary to perform Down syndrome screening and a detailed morphology scan.Women shown to have fetuses with absent or hypoplastic nasal bone with other structural defects or high risk by Down syndrome screening should undergo prenatal diagnostic tests to exclude fetal chromosome abnormalities.
10.Clinical application of neurocutaneous vascular flap on dorsal part of hand or finger in repair of distal finger injury

Objective To introduce the clinical application of neurocutaneous vascular flap on dorsal part of hand or finger for the soft tissue defect of the distal finger. Methods According to different defect sites of fingertip soft tissue, 56 flaps of 3 different kinds were devised to repair them : dorsal neurocutaneous vascular flap of the second or third metacarpal for the thumb, ulnar dorsal neurocutaneous vascular us flap of the thumb for the index, dorsal neurocutaneous vascular flap of the finger for the third to fifth finger. The largest flap was 2.1 cm×3.3 cm. Results Fifty-six flaps survived completely. The period of follow-up ranged from 6 months to 3 years. The contour and consistency of the flaps were nearly normal. Two-point discrimination was 5.4-9.6 mm. The appearance and function were satisfactory.. Conclusion The neurocutaneous vascular flap on dorsal part of hand or finger is a good flap for coverage of soft tissue defects of the distal finger.
