1.Sounding Resonance Characteristics of Pre-and Post-Tonsillectomy in Patients with Tonsil Hypertrophy

Objective To study the influence of tonsillectomy on resonance characteristics of four resonance cavity in patients with tonsil hypertrophy (II or III degree) .Methods Using multi-channel voice analysis system to test the cavity resonance characteristics and energy distribution of 38 tonsil hypertrophy male patients (including pre- and one month of post-operative) and 40 normal males when they speak /a:/.The frequency spectrum was 4 000 Hz(FR1 ,FR2 ,FR3 ,FR4 ) .Results FR2 ,FR3 energy values of the head cavity and mouth cavity ,and FR2 energy values of the thoracic cavity increased to the level of the normal control group after tonsillectomy(P<0 .05) .FR1 energy values of head ,mouth and thoracic cavity decreased to the level of the normal control group after tonsillecto‐my(P<0 .05) .FR1 ,FR2 ,FR3 ,FR4 energy values of laryngeal cavity maintained the normal level ,including pre- and post - operation .Conclusion Tonsillectomy makes oropharyngeal cavity volume expanded and airflow channel broaden .Tonsillectomy mainly maks high-frequency resonance of head and mouth cavities recovered to the normal level after about one month .
2.Describing and predicting the effectiveness of physical therapy in treatment of lumbar disc herniation using a logistic curve model

Objective To establish a mathematical model for describing and predicting the effectiveness of physical therapy on lumbar disc hernation(LDH).Methods Daily comprehensive physical therapy interventions including lumbar traction,mobilization and physical agents were administered to 31 LDH patients for 10 sessions.A visual analogue scale was used to measure the pain intensity before each session.The logistic curve model was used in the regression analysis.The first 2 to 9 pain measurements were involved in different predictive models,and the predictive results were compared among different models.Results In regression analysis,the global R2 was 0.719.In predicting analysis,the more measurements involved in the predictive model,the more effective the prediction.The predictive power was low when the model included less than 4 measurements.the prediction power was high when more than 5 measurements were used in the model.Conclusion The logistic model can describe and predict the effectiveness of multiple physical therapy on LDH patients.
3.Clinical Significance of Measuring Interleukin-13 in Serum and Pulmonary Function in Infants with Bronchiolitis

Objective To evaluate the clinical significance of measuring IL-13 in serum and dynamics of pulmonary function in infants with bronchiolitis.Methods A total of 50 infants with bronchiolitis(30 in acute phase,20 in recovery phase) and 20 infants in healthy controls were recruited into this study.IL-13 level in serum were detected with Sandwich enzyme linked immunosorbent assay(ELISA),at the same time the object′s respiratory function were tested,then the above indexes were compared.Results The level of serum IL-13 in recovery phase of bronchiolitis was significantly lower than that in acute phase of bronchiolitis and those of both 2 groups were significantly higher than that in control group(all P0.05).Conclusions IL-13 plays a role in pathogenesis in infants with bronchiolitis.The reduction of level of IL-13 will contribute to recovery of infants with bronchiolitis.During the acute phase of bronchiolitis higher resistance can be seen.

Objective To study the effects of ischemia postconditioning(IPC) in reducing ischemia-reperfusion injury(IRI) in acute limp ischemia(ALI) and investigate the mechanism.Methods 45 New Zealand rabbits were treated with the method that combined high lipid diets and femoral intima injury by balloon inflation to build up limp atherosclerotic stenosis model,then they were randomly divided into three groups( each group 15 rabbits):control group;IR group and IPC group.Serum creatine phosphate kinase(CK),malondialehyde(MDA),superoxide dismutase (SOD) in three groups were measured before occlusion and 2 hours after sustaining reperfusion.The histological changes of limp skeletal muscle of experimental rabbits were analyzed and TUNEL method was used to access apoptosis of skeletal muscle cells.Results The levels of CK,M DA in IPC group were lower than IR group [ (7.49 ± 0.84)vs (8.19 ± 1.06),P<0.05],[ (3.67 ±0.36) vs (4.06 ±0.55),P <0.05] while SOD level was higher than IR group [ (420.40 ± 30.94 ) vs ( 384.73 ± 44.12),P < 0.05 ] ; The injury of skeletal muscles in I PC group was lighter than IR group and the apoptosis index of skeletal muscle cells was significantly decreased compared with that in IR group[(12.27+2.11)% vs (16.62 ± 1.44)%,P<0.01].Conclusion Applying IPC in acute limp ischemia could alleviate IRI and protect skeletal muscles.The mechanism was associated with oxidation resistance enhancing and the effects in reducing apoptosis of skeletal muscles induced by IRI.

The “vertical tongue”method was used in speech training for 12 patients with functional speech disorder of consonant /L/.Af-ter treatment,the average vocal intilligibility of the 12 patients increased from 86.3% to 98.9%(P <0.05)./L/consonant average intelligi-bility increased from 42.9% to 85.2%(P <0.05).
7.Association of microalbuminuria and insulin resistance with acute coronary syndrome

Objective To investigate relationship of microalbuminuria (MAU) and insulin resistance (IR) to severity of coronary lesions in patients with acute coronary syndrome (ACS), and its predictive value for short-term prognosis in then.Methods In total, 162 ACS patients admitted to the department of cardiology, Guangzhou First People's Hospital, Guangzhou, Guangdong province during January 2008 to June 2009 were selected and divided into three groups according to their urine albumin excretion rate (UAER): 54 cases in normal group with UAER less than 20 μg/min, 62 cases in microalbuminuria group with UAER of (20-200)μg/min, and 46 cases in mass-albuminuria group with UAER more than 200 μg/min.Difference in IR (by homeostasis model assessment, HOMA ) and severity of coronary lesions among the three groups was assessed.Effects of baseline MAU and IR on main adverse cardiac events within the recent six months were observed in ACS patients after percutaneous coronary intervention (PCI).Results IR increased and coronary lesions aggravated as UAER increasing in all the three groups of ACS patients (P<0.05 or P<0.01).MAU associated with IR, with a coefficient of correlation of 0.366 (P<0.01).Results of multivariate logistic regression analysis showed that both MAU and IR were strong independent predictors for main adverse cardiac events in ACS patients within six months after PCI.Conclusions MAU and IR associate with severity of coronary lesions in ACS patients, suggesting certain predictive value for main adverse cardiac events in short-term after PCI.
8.Preoperative selective internal iliac arterial chemotherapy for invasive bladder cancer

Objective To evaluate the short term and long term clinical effect of internal iliac arterial chemotherapy (IIAC) followed by surgery for invasive bladder cancer. Methods Fifty six patients with invasive bladder cancer were preoperatively treated by IIAC with cisplatin (100 mg/m 2) and adriamycin (30 mg/m 2).All the cases received IIAC twice with 3 to 4 weeks interval.Two weeks after the second chemotherapy,the patients were examined by ultrasound and cystoscopy.Bladder preserving operation or radical cystectomy was performed according to clinical stages and grades of the bladder cancer. Results Two weeks after IIAC,down staging was observed in 42 patients (75%),and no change or increase in volume and stage were observed in 14 patients (25%).After IIAC TURBt was performed on 21 patients,partial cystectomy on 5,and radical cystectomy on 28 depending on the rumor size and clinical stages.The 1 ,2 ,3 ,5 year disease free survival rate in evaluative 40 patients was 85.0%,70.0%,57.5%,50.0%,respectively. Conclusions IIAC as a neoadjuvant chemotherapy has the possibility of down staging the cancer,which provides the chance of bladder preserving operations for invasive bladder cancer, and the possibility of radical cystectomy for high stage (T 3b ~T 4) bladder cancer.IIAC followed by surgery can effectively improve the disease free survival rate for invasive bladder cancer.
9.The comparison among extraction methods of exosomes from T cells of rats

Objective The purification methods of the exosomes derived form T cells were established in order to get high quan-tity exosomes .Methods Exosomes from T cells culture supernatants were purified by ExoQuick Precipitation ,ultrafiltration and sucrose gradient centrifugation ,differential ultracentrifugation ,and confirmed via using transmission electron microscopy .The pro-tein expression of the exosomes were analyzed by SDS-PAGE electrophoresis .Western blotting was used to test the expression of IL-2 .Results The protein concentration of the exosomes purified through ExoQuick Precipitation ,ultrafiltration and sucrose gradi-ent centrifugation were higher than through differential ultracentrifugation (P<0 .05) .SDS-PAGE displayed the difference among the exosome purified by three methods .Three kinds of exosomes all expressed IL-2 .Conclusion ExoQuick Precipitation ,ultrafiltra-tion and sucrose gradient centrifugation technique can obtain high purity and complete exosome sample .
10.The application effect of ICU checklist of medical cooperation to prevent ventilator-associated pneumonia

Objective To explore the application effect of ICU checklist of medical cooperation on prevention of ventilator-associated pneumonia. Methods Totally 728 patients with mechanical ventilation from July 2015 to July 2016 in ICU were assigned to the experimental group with using of the checklist; and 685 patients with mechanical ventilation from June 2014 to June 2015 were set as the control group who did not use the checklist. The incidence of ventilator-associated pneumonia and medical cooperation were compared before and after the implementation of the checklist. Results The incidence of ventilator associated pneumonia were 6.72% (46/685) and 3.02% (22/728), the intervention strategy execution rates were 63.36% (434/685) and 84.89% (618/728) before and after the application of checklist, there was significant difference between two groups (χ2=10.51, 86.03, P<0.01). Medical cooperation (doctors and nurses) and satisfaction scored 18.53 ± 2.39, 21.91 ± 2.16, 30.93 ± 2.18 and 26.71 ± 2.21, 28.64 ± 2.55, 45.33 ± 1.74 before and after the application of checklist, there was significant difference between two groups (t=6.14, 14.25, 2.39, P<0.01 or 0.05). Conclusions The application of checklist can improve the medical cooperation on prevention of ventilator-associated pneumonia management, reduce the incidence of ventilator-associated pneumonia, improve medical cooperation, nursing satisfaction and the management efficiency, which is worthy of popularization and application.
