Objective Tetralogy of Fallo( TOF) is one of cyanotic congenital heart disease, which is difficultly diagonosed because of its complicated texture.The aim of this study was to evaluate the manifestation and value of echocardiography in diagnosis of TOF. Methods The fetal sonograms of 99 fetuses with prenatal diagnosis of TOF in 5791 gravida were reviewed and analyzed from January 2010 to December 2012. Results Ninty nine fetuses with TOF were all diagnosed accurately by prenatal ultrasonography, including 11 cases with pulmonary atresia, 2 cases with complete atrio-ventricular septal defect and 4 cases with permanent left superior vena cava.The key echocardiographic features of TOF were ventricular septal defect, overriding aorta and pulmonary stenosis.TOF was accurately diagnosed by prenatal echoeardiography combined with visualization of TOF in four chamber view, outflow tract view and three vessels view.Sixty eight fetuses were confirmed to be TOF by autopsy or operation after birth, 12 cases refused to autopsy and 19 cases lost of follow up in the total 99 cases.None of TOF was reported to us in other 5692 gravidas. Conclusion The echocardio-graphy has important clinic value in diagnosis of fetal TOF with high sensitivity and specificity.

OBJECTIVETo study the effect of Compound Ezhu Powder (CEP) on serum levels of CA125 and CA19-9, and the expression of cyclin D protein in endometriosis patients, thus providing theoretical evidence for clinical application of CEP.

METHODSTotally 69 all endometriosis patients underwent surgical treatment at Department of Gynecology and Obstetrics, Tianjin Nankai Hospital from January 2011 to January 2013 were randomly assigned to group A (35 cases) and group B (34 cases). Meanwhile, 30 patients with uterine fibroids who prepared for surgical treatment during the same period were recruited as the control group. Patients in group A took EZP 3 months before surgery. No treatment was given to patients in group B and the control group. The serum CA125 level and the expression of cyclin D in the ectopic endometrium and the eutopic endometrium were detected in the 3 groups before surgery.

RESULTSThe expression of cyclin D was higher in group A and group B than in the control group (P < 0.05). The serum levels of CA125 and CA19-9 were significantly lower in group A than in group B (P < 0.05). The expression of cyclin D in the ectopic endometrium was lower in group A than in group B, but with no statistical difference (P > 0.05). The expression of cyclin D in the eutopic endometrium was significantly lower in group A than in group B with statistical difference (P < 0.05). Meanwhile, the serum CA125 level was positively correlated with the serum CA19-9 level (r = 0.45, P < 0.05).

CONCLUSIONSThe expression of cyclin D obviously increased in endometriosis patients, which was associated with the occurrence of endometriosis. CEP could lower serum levels of CA125 and CA19-9, and down-regulate the expression level of cyclin D, indicating its roles in inhibiting the cell cycle.

The family of Kazal-type serine protease inhibitor(SPINK)correlates with chronic pancreatitis,Netherton syndrome,esophageal carcinoma,and other humane diseases.The functions of SPINK1,SPINK5 and SPINK7 have been identified.However,other members remain to be explored.In this review,we summarized locations of SPINK genes,protein structures,functions of SPINK proteins,and the relationship between SPINK family and humane diseases.

[Objective]To investigate the clinical value of OCM approach of minimally invasive total hip arthroplasty(MIS-THA).[Method]From February 2005 to December 2006,18 cases of MIS-THA were performed with OCM approach technique.The mini-incision through the intermuscular interval provided a good exposure for total hip replacement,preserved muscle integrity and kept the posterior capsule intact.A two-stage osteotomy of femoral neck was conducted.Specialized acetabular and femoral instruments were applied to fix the prosthesis.[Result]The average length of skin incision was 9.2 cm,the blood loss and drainage were 230 ml and 90 ml,respectively.At 6 months after operation,the mean Harris scores of hip increased from 46.2 to 87.1 for all cases.One case of femoral fracture around prosthesis and one of excessive anteversion were observed.[Conclusion]OCM approach brought satisfactory clinical outcomes when applied in MIS-THA.This surgical technique shows an obvious learning curve,but requires specialized instruments.

Objective To analyze the overseas imported malaria situation of Shijiazhuang City from 2012 to 2015,so as to provide the evidence for exploring the prevention and control strategy. Methods The data of overseas imported malaria were collected and analyzed with the descriptive method including Plasmodium species,population characteristics,epidemic focus distribution,onset time,diagnosis and treatment in Shijiazhuang City from 2012 to 2015,and the time from the onset to first medical visit,time from first medical visit to being diagnosed,and time from onset to being diagnosed of different Plasmodium species were analyzed respectively with the statistical analysis method. Results A total of 92 overseas imported malaria cases were reported in Shijiazhuang City from 2012 to 2015,including 88 cases from African countries(falciparum malaria taking 53.41％),and 4 cases from Southeast Asian and other countries(vivax malaria taking 50％). Eighty-nine cases were distributed in 28 counties(districts)of 6 cities in Hebei Province,except 2 persons with foreign nationalities and 1 with Anhui Province cen-sus register. The male and young adults were dominant. The median time from the onset to seeing a doctor was one day and the median time from seeing a doctor to being diagnosed was five days. Most cases were reported by the Fifth Hospital of Shijia-zhuang which was the sentinel hospital. Totally 42.39％of the cases were misdiagnosed when the first visit to a doctor. All of the cases were laboratory confirmed and 100％of them received the standard treatment after diagnosis. Conclusions The overseas imported malaria cases are increasing rapidly with years and the malignant malaria cases were more than other malaria cases in Shijiazhuang City. It is necessary to further strengthen the long-term cooperation mechanism between the medical institutions and the entry-exit inspection and quarantine department. The technician training should be strengthened to avoid the severe cas-es and death cases.

Objective To explore the effects of microwave irradiation on the proliferation of keloid-derived fibroblasts so as to analyze the expression of collagen-1 and the activation of the signaling pathway involved.Methods Cells from a human keloid scar were cultured in vitro and randomly divided into a control group withont any intervention,a 10 mW/cm2 microwave irradiation (10-MI) group and a 20 mW/cm2 microwave irradiation (20-MI) group.Aliquots of the latter 2 groups were irradiated at their corresponding intensities for 5 min,15 min and 30 min.The growth of fibroblasts was evaluated using MTT assay.The expression of collagen-1 and changes in the phosphorylation of protein JNK were detected using western blotting.Results Compared with the control group,no significant differences in the average growth of the keloid-derived fibroblasts were observed in the 10-MI group,but significant differences were observed in the 20-MI group and among the three sub-groups irradiated for different durations.The expression of type 1 collagen was significantly down-regulated after irradiation in a time-dependent manner.After microwave radiation at 20 mW/cm2,JNK was significantly activated compared to the control group at the different time points.Conclusions Microwave irradiation at 20 mW/cm2 can significantly inhibit the proliferation of keloid-derived fibroblasts and the down-regulalion is correlated with the irradiation's duration.It can also significantly inhibit collagen-1 expression and relieve scar formation through activating the JNK signal pathway.

Objective: To investigate the effect of type-A character on endothelial function in the elderly. Methods:According to type-A character scale of Zhang Baiyuan, type-A character group and non-type-A character group were screened from the normal elderly, 60 eases for each respectively. Flow-mediated diastolic function of brachial artery was examined by high-frequency eehocardiography, which was reflected by diastolic inner diameter changing rate (D %). Other parameters included height, weight, body mass index (BMI), systolic and diastolic blood pressure (SBP and DBP), fasting blood glucose (FPG), triglyceride (TG), total cholesterol(TC), low-density lipoprotein-cholesterol (LDL-C), nitric oxide (NO), endothelin (ET)and angiotensin Ⅱ (AngⅡ). All of the parameters and the scores of type-A character were compared and analyzed between the two groups. ResultS:(1) The levels of SBP and DBP were significantly higher in type A character group than those of non-type A character group (P＜0.05). There were no significant differences in values of FPG, TC, TG and LDL-C between two groups (P＞0.05). (2) The values of D% and NO were significantly lower in type A character group than those of non-type-A character group (P＜0.05 ). The values of Ang Ⅱand ET were significantly higher in type-A character group than those of non-type-A group (P＜0.05 ). (3)Pearson correlation analysis indicated that the scores of type-A character were positively correlated with Ang Ⅱ, ET, SBP and DBP (P＜0.05, P＜0.01), but negatively correlated with D% and NO (P＜0.05). Multiple linear stepwise regression analysis showed that only Ang Ⅱwas an independent risk factor for type-A character. Conclusion:h was showed that the increased Ang Ⅱinduced by abnormal activation of reunin-angiotensin-aldosteerone system (RAAS)may be the main mechanism of endothelial dysfunction in the eldedy with type-A character.

Objective To investigate the relationship between self-efficacy and positive degree in patients with cervical spondylosis.Method A questionnaire survey was conducted among 98 patients with cervical spondylosis.Results The self-efficacy score was 6.30±1.56,the score on the positive degree was 53.76 ±12.53.The self-efficacy and positive degrees were positively correlated (r=0.531,P＜0.001).Conclusions The patient's self-efficacy and positive degree are at medium level.Nursing staff should take corresponding measures to improve self-efficacy of cervical spondylosis patients,in order to improve its positive degree and achieve the purpose of effective self-management and health promotion.

ObjectiveTo investigate the effects of type-A behavior pattern on intervention of benasepril in patients with essential hypertension.MethodsAccording to the type-A behavior score patients with essential hypertension were divided into two groups:type-A group(n=80)and non-type-A group(n=80).Benazepfil was given to all patients for 3 months with gradual increase of dosage till the blood pressure (Bp)was controlled under 140/90 nlln Hg(1 mm Hg=0.133 kPa).The plasma levels of angiotensin II (Ang Ⅱ)was measured by radioimmunoassay and left yentrieular mass index(LVMI)was examined by ultrasonic echoeardiography(UCG)before and after treatmenL ResultsBefore benazepril treatment.plasma AngⅡ levels and LVMI in type-A group were higher than those in non-type-A group(P＜0.05),but there was no difference in Bp between two groups.Type-A behavior value were positively correlated with Ang Ⅱ and LVMI in two groups,but only Ang Ⅱ level was independent risk factor.Ang II levels were positively correlated with LVMI in two groups and that was more significantly in type-A group(P＜0.05-0.01).After treatment,Bp,plasma Ang Ⅱ and LVMI were all decreased significantly in two groups(P＜0.05-0.01)and Ang Ⅱ and LVMI were decreased more markedly in type-A group(P＜0.05).The variations of Ang Ⅱ and LVMI before and after treatment were positively correlated in two groups.but more markedly in type.A group (P＜0.05-0.01).CondusionCharacters may affect left ventricular hypertrophy in patients with essential hypertension through some stress factors and benazepril has better intervening effect on it.
