1.Changes of the maxillary arch produced by different self-ligating appliance systems during the leveling and aligning phases

Objective:To evaluate the changes in the transverse dimension and the perimeters of the maxillary archs produced by 3 self-ligating appliance systems during the leveling and aligning phases. Methods: 45 patients who had class I skeletal pattern and upper arch crowding 3-6 mm were divided into 3 groups with 15 subjects in every group. The dental casts were analyzed. Results: Statistically significant increases were recorded of all dentoalveolar widths (with the exception of intermolar width measured lingually) and arch perimeters. The increase in arch perimeter showed a significant negative relationship with the individual perimeter of the maxillary arch before treatment. No significant change was found in arch depth. Conclusion: The self-ligating appliance systems produce statistically significant increases in the transverse dentoalveolar width and the perimeter of the maxillary arch during the leveling and aligning phases of treatment.
2.Expressions of apoptosis-related genes of retinal blood vessel in early diabetic rats

Objective To analyze the expression of apoptosis-relared genes of retinal blood vessel in early diabetic rats by gene chip technology. Methods To make diabetic rat model by intraperitoneal injection of streptozotocin (STZ). On the 6th week after blood pressure increased, 10 rats were executed in Diabetic group and normal control group respectively. 20 retinal blood vessels were extracted and the RNA was isolated. The probe was made of α-32 P-deoxyadenosine triphosphate (dATP)-labeled sample which hybridized 1176 nylon chips, and then analyzed by software. Three different expression genes were selected to verify by reverse transcription polymerase chain reaction (RT-PCR). Results On the 6th week, 136 (11.5%)genes were differentially expressed [up-regulated genes were 90 (7.6%), down- regulated genes were 46(3.9%)] in diabetic group. These genes involved into different groups according to their function. Especially in 72 apoprosis-related genes, 15 genes were differentially expressed. The up- regulated genes were some TNF receptor family members such as TNFRSF12, TRAIL, TNFRSF9, FADD;Bcl-2 family members such as bcl-w, bax, bakl and AKT. The down-regulated genes were FAF1 which related to fas. Conclusions The expression of retinal vascular gene in early diabetic rats has been changed complicatedly. In particular, the multiple apoptosis-related genes have been changed in early diabetic, and most of them are at the upstream of apoptosis pathway. These findings indicate that the development of diabetic retinopathy is associated with multiple signaling pathways leading to apoptosis, while the alterations on the level of molecular biochemistry are still limited in apoptosis induction period.
3.Effect of Sensory Integration Training on Children with Attention Deficit Hyperactivity Disorder

ObjectiveTo observe the curative effect of sensory integration training on children with attention deficit hyperactivity disorder (ADHD).Methods60 ADHD children were randomly divided into trial group and control group with 30 cases in each group. Children in trial group were treated with sensory integration training besides routine medication, and assessed with Scale for Assessment of Attention Sensory Integration after treatment.ResultsAfter treatment, scores of large muscle dis-equilibrium and ill-proprioception increased obviously (P<0.01) ; scores of improper tactile defense and insufficient development of learning ability also increased (P<0.05) compared with pretreatment. The curative effect of trial group was better than that of control group (P<0.05 ).ConclusionOn the basis of medication, sensory integration training can improve the rehabilitation effect of ADHD children.
4.Oxidative stress and interleukin-1β in acute gouty arthritis and the efficacy of probucol treatment

Objective To investigate the role of malondialdehyde(MDA),superoxide dismutase(SOD),and interleukin-1β (IL-1β) played in the pathogenesis of acute gouty arthritis and the treatment efficacy of probucol.Methods Acute gouty arthritis model in rat was established with injection of monosodium urate monohydrate (MSU) into the ankle joint cavity,then the joint swelling index was observed periodically,and MDA,SOD,IL-1β,and white blood cell count in the synovial fluid were determined by 72 h.Results The joint swelling index gradually increased,reaching a peak by 11h,and this finding was most obvious in the model group.The joint swelling index in the group treated with high-dose of probucol was significantly lower than that in the model group by 48 h(P<0.05).The joint swelling indices in the groups treated with moderate and high doses of probucol were lower than that in the model group by 72 h(P<0.05).The values of MDA in the moderate and high-dose groups were (14.45±3.11) and (11.54 ±3.10) nmol/ml respectively,being significantly lower than that in the model group[(24.46±4.27) nmol/ml,P<0.01].The amounts of SOD in the moderate and high-dose groups were (25.56±4.51) and (44.61±4.11)U/ml respectively,being higher than that in the model group[(13.32±2.02) U/ml,P<0.01].The amount of IL-1β in the high-dose group was lower than that in the model group[(15.41 ± 3.12 vs 19.87±3.99)pg,P<0.05].The white blood cell counts in the moderate and high-dose groups were (11.08±1.64)×l0~7/L and (7.43±1.52) x 10~7/L,being lower than that in the model group[(14.18±2.30)×10~7/L,P<0.01].Conclusion The levels of MDA and IL-1β in the synovial fluid were raised,while SOD was significantly decreased in acute gouty arthritis.Moderate and high-doses of probucol can effectively control acute gouty arthritis attack.The efficacy of high-dose probucol was equal to that of colchicines.
5.Comparison of the effects of two anesthetic methods on MAC-awake of enflurane in patients undergoing mastectomy

Objective To determine the awakening concentration of enflurane and evaluate the emergence from anesthesia in the presence and absence of epidural block. Methods Thirty ASA Ⅰ - Ⅱ female patients aged 35-65 yr undergoing elective radical mastectomy were randomly divided into general anesthesia group (GA, n = 15) and combined general-epidural anesthesia group (CGEA, n = 15) . The patients were premedicated with intramuscular atropine 0.01 mg? kg-1 30 min before operation. In CGEA group epidural block was performed at T3_4 before induction of general anesthesia. A catheter was inserted into the epidural space in a cephalad direction for 3-4 cm. A loading dose of 10-12 ml of 1.5% lidocaine was given after a test dose. During operation 1.5% lidocaine 5-6 ml was given via the catheter every 40-60 min. General anesthesia was induced with propofol 2.5 mg ? kg-1 and succinylcholine 2 mg ? kg-1 . The patients were intubated and mechanically ventilated (FiO2 = 100 % , FGF = 1.5 L ? min , VT = 8-10 ml? kg-1 ), Anesthesia was maintained with similar concentration of enflurane to ensure that the enflurane consumed during maintenance of anesthesia was similar in both groups. At the end of operation enflurane was maintained at 0.5 MAC for 15 min. The enflurane vaporizer was then turned off and enflurane was washed out with FGF 10 L?min-1 . The end-tidal enflurane concentration was recorded every 10 seconds and the patients were continually being asked to open their eyes. The end-tidal enflurane concentration at which the patients opened their eyes on request was recorded as MAC-awake and Steward score was evaluated (0 = deeply anesthetized, 6 = wide awake). Results MAC-awake of enflurane was 0.4% in GA group and 0.24% in CGEA group. Steward score was 4.8 in CGEA group and 4.2 in GA group. Conclusion Epidural block with lidocaine reduces the MAC-awake. Steward score is higher in CGEA group than that in GA group.
6.Analysis of outpatients' prescriptions utilization of Chinese Patent Medicines in primary health institutions from rural western China

Objective: This study aims to analyze the outpatient prescriptions in primary rural health institutions in western China to reveal the feature and disadvantages of utilization of Chinese patent medicines in rural areas.Methods: A total of 14336 prescriptions were selected from 132 health institutions in rural areas of 7 provinces in western China in 2011.The utilization of Chinese patent medicines is described and compared to the health institutions in different levels and different provinces/autonomous regions.Results: The prescriptions of Chinese patent medicines accounted for 62.47% of the total prescriptions in rural health facilities.The proportion of county, township and village medical institutions was 49.76%, 56.96% and 71.51% respectively.86.84% of the Chinese patent medicines prescriptions were prescribed by physicians without traditional Chinese medicine background, while only 13.16% of them were traditional Chinese medicine physicians.The number of varieties of Chinese patent medicines in a single prescription in county, township and village health institutions were 1.38, 1.91 and 2.54, respectively and the average drug costs of every prescription in these health institutions were 52.20 yuan, 21.89 yuan and 19.37 yuan, respectively.Conclusions: There are differences in the utilization of Chinese patent medicines in outpatient prescriptions of rural health institutions in western China.The hospitals in higher medical level use more Chinese patent medicines.The drug cost of the prescription of county health institutions is higher than that of township and village hospitals.This study suggests that health policy should be adjusted based on the specific needs and characteristics of rural health institutions at a different level and should be implemented in the management of prescription in quality control.
7.Utilization Analysis of Carbapenems in 34 Hospitals of Wuhan Area during 2007-2014

Objective: To investigate the application and tendency of carbapenems in the hospitals of Wuhan area.Methods: The application of carbapenems in 34 hospitals of Wuhan area during 2007-2014 was analyzed statistically in terms of consumption sum, DDDs, DDC and so on.Results: The total consumption sum of carbapenems increased year by year.Compared with that in 2007, the total consumption sum increased by 4-fold in 2014.The growing rate of total consumption sum of biapenem was the fastest among various varieties.Since 2009, three injections including meropenem, biapenem and imipenem / cilastatin predominated the market of Wuhan area, followed by faropenem with oral administration.The changing trend of DDDs was basically consistent with the total consumption sum.DDC of all the drugs was relatively high and demonstrated decreasing tendency, and that of biapenem decreased most notably, while still ranked the first place.DDC of meropenem and imipenem/cilastatin both exceeded 400 yuan.The price of faropenem fluctuated notably during 2007-2009, while increased by 2-fold in 2009 (453 yuan) and then remained stable.Conclusion: The special rectification measures show positive impact on the selection of antimicrobial agents in Wuhan area.The drug varieties are proper, while the average daily cost is still high.The government should continue to strengthen the management of antibiotics to ensure the rational use of carbapenems.
9.Impacts of Kechuanning on Expression of Signal Transducer and Activator of Transcription 6 in CD4+, CD8+T Lymphocytes of Athma Rats Induced by Virus

Objective To observe the impacts of Kechuanning on expression of signal transducer and activator of transcription 6 (STAT 6) in CD4+, CD8+T lymphocytes of athma rats induced by virus, and analyze the mechanism of Kechuanning in treating asthma induced by RSV. Methods The PBMC was isolated from the peripheral blood, CD4+T, CD8+T lymphocyte was separated by immunomagnetic beads. The purity and activity of CD4+T and CD8+T lymphocytes were measured by using flow cytometry, trypan-blue dye exclusion test. The protein expression of STAT6 was detected by fluorescence-activated cell sorting. Results The descending progress of the trial were supported by the purity and activity of CD4+T, CD8+T lymphocyte. CD4+T lymphocyte in the asthma model group were increased (P<0.01), however the increase of CD8+T lymphocyte were not significant (P>0.05). The expression of the protein of STAT6 in the treated group with Kechuanning decoction were decreased than that in the model group (P<0.01). Compared with Dexamethasone and Salbutamol group, there were no significant differences (P>0.05). Conclusion Kechuanning decoction could decrease the protein of STAT6 in CD4+, CD8+T lymphocyte, thereby it could regulate the disorder of Th1/Th2, thus prevent and treat the asthmatic attack induced by virus.
10.Analyze compensation patterns and compensation effects of catastrophic disease insurance in L City

Objective:This paper designs to analyze compensation patterns and compensation effects of cata-strophic disease insurance in L City, and put forward feasible suggestions to improve the compensation patterns of cat-astrophic disease insurance. Methods:We combined the relevant policy documents to analyze compensation patterns, and used benefit rate, OOP and The effective reimbursement rate to analyze compensation effects of catastrophic dis-ease insurance. Results:Catastrophic disease insurance benefit rate in L city in 2013 was 3. 2%; Rates of NCMS fund unilization was 92%, which diversed from county to county. Patients' OOP decreased significantly after reim-bursement of catastrophic disease insurance;Catastrophic disease insurance and NCMS total effective reimbursement rate reached 84.8%;The NCMS compensation rate reached 68. 9%, while fund incurred a financial deficit at the same time. Conclusion:Set deductibles, compensation rate and compensation range scientifically, and cancel ceiling level,improve the program of catastrophic disease insurance. Take measures to reduce the unfair between the districts and counties at city level. Establish effective link-up between catastrophic disease insurance and NCMS.
