1.Multiple endocrine neoplasia type 2B:4 cases report and literature review

Objective To investigate the clinical characteristics and diagnostic methods of multiple endocrine neoplasia ( MEN) 2B. Methods The clinical data collected from 4 cases with multiple endo-crine neoplasia type 2B, who had been diagnosed and treated from July .1980 to Jan.2012, were retrospec-tively analyzed.Of the 4 patients, 3 males were diagnosed at the age of 14, 24, 21, respectively, and 1 fe-male was diagnosed at age of 23.Two cases had the first diagnosis of thyroid mass during the physical exami-nation, and the adrenal tumors and hypertension were first diagnosed in other two cases , respectively.All four cases had multiple mucosal neuromata .Of whom, three cases had medullary thyroid carcinoma and pheochromocytoma , two cases had Marfan syndrome figure , and one case had pheochromocytoma and Marfan syndrome.Two cases were accompanied by paroxysmal hypertension , and the highest systolic blood pressure was over 200 mmHg (1 mmHg=0.133 KPa).Two cases of pheochromocytoma were found during follow up . Three cases had bilateral adrenal tumors .The thyroid mass was diagnosed by ultrasonography , and the pheo-chromocytoma was confirmed by ultrasonography , CT and 24-hour urine catecholamine test .Adrenalectomy and thyroidectomy were carried out during the treatment process .To clarify the RET mutation status , exons and adjacent introns from 10th to 16th of RET proto-oncogene were sequenced in two patients . Results Gene analysis of two cases confirmed the activating mutation at the sites of 918 in exon 16 of RET proto-on-cogene in which methionine was replaced by threonine ( ATG-ACG) .Among the 3 cases with medullary thy-roid carcinoma , two patients were treated by total thyroidectomy and neck lymphadenectomy , one was opera-ted by subtotal thyroidectomy .For the treatment of pheochromocytoma , adrenal tumor resection was carried out in 3 cases, but not in the other one .Patients were followed up for 2 to 15 years postoperatively , 1 case without tumor recurrence and metastasis , 2 cases with multiple metastases . Conclusions The diagnosis of multiple endocrine neoplasia type 2B mainly depends on endocrine tests , ultrasonography and CT .Surgery is the primary treatment .When pheochromocytoma coexists with other tumors , pheochromocytoma should be re-moved first .

Objective To summarize the appearances of MS lesions on DWI, and to quantitatively study the ADC and AI values in different clinical MS subgroups Methods One hundred and eighty seven lesions were studied in 18 cases of MS at different clinical stages Routine MR and DWI were performed in all cases ADC and AI were calculated in the lesions,NAWM, and normal control group Results Hyperintensity (edema like on T 2WI) was demonstrated on DWI at acute primary progressive MS lesions Hyperintense ring sign was found at acute relapse MS lesions ADC increased in all subgroups of MS Significant difference of ADC was demonstrated among MS lesions, NAWM, and normal control groups Significant decrease of AI was demonstrated at later stages Hyperintensity was found in all MS lesions on T 2WI The hyperintensity lesions on DWI had enhanced or non enhanced appearances on CE T 1WI The ADC value of enhanced MS lesions was less than that of the non enhanced lesions ( t =4 19, P 0 05) Conclusion DWI could give quantitative information for different pathological changes in MS lesions of different clinical subgroups Quantitative measurement of diffusion has great potential value in the diagnosis, differential diagnosis, and the prediction of prognosis in MS
3.Hypotensive Action of the Extract of Kudingchadongqingye(Ilex kudingcha)

Intravenous injection of an extract of Ilex kudingcha leaves remarkably lowered the blood pressure of anesthetized dogs. Blood pressure were also lowered when given to unilateral kidney ligated consoious rats or SHR by gastric gavage. When given orally to adult or obesic rats, their abdominal subcutaneous fatty tissue deceased markedly. These results suggested that the extract posses antihypertensive and weight reducing activities, LD5, of the extract in mice was 3.6?0.6g/kg by ip route.
4.Small interfering RNA targeting connective tissue growth factor prevents rats liver fibrosis

Objective To explore whether portal vein injection of small interfering RNA(siRNA) targeting connective tissue growth factor(CTGF) could inhibit CTGF expression in rats liver in vivo and prevent rats hepatic fibrosis. Methods Twenty four male rats were equaly divided into four groups. Rats received subcutaneous injection of 40% CCl4 (3 ml/kg) together with portal vein injection of saline every three days for 6 consecutive weeks were served as model group; CCl4 together with portal vein delivery of siRNA (0.1 mg/kg) as preventive group; CCl4 for 2 weeks followed by CCl4 and CTGF siRNA for more than 4 weeks as curative group, and only intraportal injection of saline as control group. Portal vein pressure in all rats at 3 days after the last CCl4 injection were measured, and blood and hepatic tissue from rats were harvested. Serum concentration of transaminases, hyaluronic acid(HA) and procollagen type Ⅲ (PⅢNP) were measured. Expression of CTGF mRNA and protein in rats liver was evaluated by RT-PCR and Western blot, respectively. Inflammation and fibrosis in rats liver was analyzed by H-E and Sirius red collagen staining. The fibrotic area was measured quantitatively by a computer-aided manipulator. Results Compared with model group, the expression of CTGF mRNA and protein in liver in both preventive and curative groups were markedly down-regulated. Inflammation, necrosis and fibrosis in hepatic tissue was significantly attenuated. In addition, the serum concentration of transaminases, HA and PⅢNP were greatly reduced. 6.Therapeutic effects of Fuzheng Huayu decoction in different pathological stages CCl4-induced liver cirrhosis rat model and on hepatic stellate cells activation

Objective To evaluate the influence of Fuzheng Huayu decoction on different pathological stages fibrotic liver tissue and activated hepatic stellate cells (HSCs). Methods Use a carbon tetrachloride (CCl4)-induced liver cirrhosis rat model system. The drug intervention was administered via oral-gastric irrigation in building, the end of week 2 and 4. Rats were batch sacrificed at the end of week 2 , 4, and 6. Liver fibrosis was evaluated by histology, and expression of a-smooth muscle actin (α-SMA) was determined by immunohistochemistry. Between-group comparisons were made by completely random design and ANOVA with Bonferroni correction. Results At the end of weeks 2, 4 and 6, the model group and drug intervention groups had significantly higher area ratio of liver fibrosis and higher expression of α-SMA than the normal group, but the three drug intervention groups had significantly less area ratio of liver fibrosis and less expression of α-SMA than the model group and the drug intervention 1 group showed the most robust decrease (P<0.05). Conclusion The Fuzheng Huayu decoction leads to down-regulated α-SMA expression and reduced degree of liver fibrosis on different pathological stages of fibrotic liver tissue, and the earlier drug intervention, the better the results.
7.The study on the serum proteomic fingerprints in gastric carcinoma patients by SELDI technique

Objective To detect serum protein spectrum in gastric cancer, screen the serum proteins related to gastric carcinoma and build auxiliary diagnosis model with SELDI-TOF-MS, Methods The serum proteomic fingerprints were detected by SELDI-TOF-MS and CM10 proteinchip. The sera came from 50 first visit gastric cancer patients who were diagnosed by pathological method and 16 healthy peo- ple. The data were analyzed by Biomarker Wizard Software and the proteomic fingerprint of gastric carcinoma model was developed by BPS (biomarker pattern software). Results Compared with the healthy people group, there were 34 significant different protein peaks in the gas- tric carcinoma group. And the diagnosis model composed with 6 proteins (M/Z values were 6016, 6744, 2822, 4474, 7892 and 3242) could classify the 2 groups correctly. In the test group, the sensitivity and specificity were 96% and 93.75% respectively, the accuracy was 95.45%. Conclusion SELDI-TOF-MS method show features such as microcontent, fast and high- resolution etc. It could be utilized to screen significant proteins and develop an auxiliary diagnosis model in gastric carcinoma, which may be used to diagnose this disease.

9.Effects of propofol anesthesia on long-term cognitive function in neonatal rats

Objective To investigate the effects of different doses of propofol anesthesia on the long-term cognitive function in neonatal rats.Methods One hundred 7-day-old male Sprague-Dawley rats,weighing 9-18 g,were randomly divided into 5 groups (n =20 each) ∶ control group (C group) and propofol 25,50,100 and 200mg/kg groups (groups P1-4,respectively).Groups P1 and P2 received intraperitoneal propofol 25 and 50 mg/kg,respectively.Groups P3 and P4 received intraperitoneal propofol 100 and 200 mg/kg,respectively,and after righting reflex completely recovered,an increment of propofol 50 mg/kg was given until the total amount was finished.Five animals in each were chosen and arterial blood samples were obtained immediately after the animals were fully awake for blood gas analysis.The rats were then continuously fed.When the rats were 9 weeks old,the spatial learning and memory function was tested by Morris water maze.The animals were then sacrificed and their brains were removed for detection of the expression of nerve growth factor (NGF) and Caspase-9 protein and mRNA in hippocampal tissues (by Western blot analysis and RT-PCR) and for microscopic examination of the ultrastructure of hippocampal neurons.Results There was no significant difference in blood gas analysis index between the 5 groups (P > 0.05).Compared with group C,no significant change was found in the escape latency and frequency of crossing the original platform in group P1,the escape latency was prolonged and the frequency of crossing the original platform was decreased in groups P2-4,the expression of NGF protein and mRNA was down-regulated and the expression of Caspase-9 protein and mRNA was up-regulated in groups P1-4 (P < 0.05).The escape latency was gradually prolonged,the expression of NGF protein and mRNA was gradually down-regulated and the expression of Caspase-9 protein and mRNA was gradually up-regulated in groups P1-4.The frequency of crossing the original platform was decreased in groups P2-4 compared with group P1 (P < 0.05).There was no significant difference in the frequency of crossing the original platform between groups P2-4 (P > 0.05).Nucleus condensation,chromatin condensation,nuclear fragmentation and apoptotic bodies were observed in groups P2-4.Conclusion Propofol anesthesia can impair the long-term cognitive function in neonatal rats,the effect is related to the dose,and inhibition of NGF expression and increase in the activity of Caspase-9 may be involved in the mechanism.
10.The effect of ankle strategy stability limit training on balance and gait in recovering stroke patients

Objective To explore the effect of ankle strategy stability limit training on balance and gait in recovering stroke patients with hemiplegia. Methods Forty recovering stroke patients were randomized into an intervention group and a control group.The patients in the intervention group were given ankle strategy stability limit training using visual feedback on the static long sets of a Smart Equitest Balance Master (SEBM) machine.Those in the control group practiced routine postural balance training using mirror visual feedback in parallel bars.Both groups of patients practiced balance and posture control for 30 minutes,once daily,6 days a week for two weeks. Both groups were also given routine therapy and other rehabilitation.The patients' balance function was evaluated using the Berg Balance Scale (BBS),and their gait was assessed using the walk across technique (WA). Results There was no significant difference between the two groups with regard to general information,BBS scores or WA results before treatment.After 2 weeks of treatment,BBS scores as well as the step length and pace in the WA improved significantly in both groups,but all improved significantly more in the intervention group.There was no significant difference in width of gait. Conclusion Ankle strategy stability limit training can enhance weight-bearing on stroke patients' affected foot as well as their balance and the symmetry of their steps.
