1.Study on Relation between Diet,Physical Activities and Chronic Diseases

Objective To analysis the relation between diet,physical activities and chronic disease.Methods Collecting and analyzing the data of 3 673 Tianjin residences selected randomly as sample in 2002 China National Health and Nutrition Examination Survey.Dietary pattern and prevalence of chronic diseases were obtained,as well as the relative risk of diet,physical activities on chronic diseases including obesity,hypertension,diabetes and hyperlipidemia.Results The prevalent rates in Tianjin adults were 26.8% for obesity,3.07% for hypertension,32.2% for hyperlipidemia and 3.7% for diabetes.Main dietary problems among them were higher intake of oil and salt,lower intake of vegetable and fruit.The percentages of energy from fat ≥30% and watching TV≥3 hours/d were risk factors for overweight/obesity,hypertension,diabetes and hyperlipidemia with OR values as 2.04(95%CI:1.53~3.04),2.00(95%CI:1.13~3.57),2.22(95%CI:1.66~2.98),1.64(95%CI:1.18~2.31);1.89(95%CI:1.32~2.70),1.79(95%CI:1.48~2.16),1.47(95%CI:1.33~1.78)and 1.45(95%CI:1.28~1.65).Physical exercises ≥30 minutes /d was a protective factor for those four chronic diseases with OR values as 0.42(95%CI:0.28~0.68),0.60(95%CI:0.40~0.89),0.50(95%CI:0.35~0.80),0.61(95%CI:0.44~0.93)respectively.Intake vegetable and fruit ≥500 grams /d could reduce the risk for obesity and hyperlipidemia,OR values were 0.60(95%CI:0.40~0.89)and 0.47(95%CI:0.30~0.72).Conclusion There was a strong relation between diet,physical activities and chronic diseases in Tianjin sampled people.It should strengthen health promotion with balanced diet and good physical activity for the chronic diseases prevention.
2.Study on Relationship between Life Style and Chronic Diseases

Objective To analyze the relationship between life style and chronic diseases.Method 2 886 people were selected randomly from three districts of Tianjin and surveyed with a questionnaire on their healthy condition and physical examination.2 622 people were taken a diet survey by 3-days foods intakes weighing and 24-hours diet recall,then the relative risk factors between life style and coronary heart disease,cerebrovascular disease,diabetes,hypertension,overweight and obesity were analyzed.Result The standardized prevalence rates of coronary heart disease,cerebrevascular disease,diabetes,hypertension, overweight and obesity of residents were 9.24%,4.33%,6.74%,34.05%,and 42.62%,respectively.The more the intake of energy, the more the risk for people to suffer from overweight and obesity,OR value being 1.00-1.70.With intake of more than 20 g dietary fiber per day,the risk for suffering from overweight and obesity could be reduced,OR value being 0.68.Logistic analysis showed that heavy salty taste could increase the risk for suffering from hypertension by 42%.Watching TV more than 3 hours per day increased the risk for suffering from cerebrovascular disease and diabetes,OR value being 1.802 and 1.703 respectively.Sleep less than 7 hours per day was a dangerous factor for hypertension,coronary heart disease and cerebrovasular disease,OR value being 1.517, 1.833 and 2.041 respectively.Conclusion The life style such as dietary pattern,sedentary occupation and sleep hours have strong connection with chronic diseases,such as hypertension,diabetes etc.Reasonable diet,suitable exercises,reducing sedentary position and increasing sleep hours will be good for health,and also for the prevention and control of chronic diseases.

4.Role of Kupffer Cells in High Fat Diet Induced Hepatic Insulin Resistance

Objective To explore the role of Kupffer cells in high fat diet induced hepatic insulin resistance. Meth-ods Eighty-four C57BL/6J mice were divided into two groups: normal chow (NC) group (7 subgroups) and high fat diet (HFD) group (7 subgroups). Glucose tolerance tests were performed at feeding time of 1,2,4,8,12 and 16 weeks in one NC group and one HFD group. The mice livers at the same feeding time were obtained in other 6 NC groups and 6 HFD groups respectively. The expression levels of IRS1-ser307, Akt-ser473, S6K1-thr389 and p-JNK were detected by Western blot as-say. The values of MCP-1,TNF-α,IL-1βand IL-10 mRNA were examined by qRT-PCR. Results Compared with NC group, the impaired glucose tolerance was found from the first week in HFD group (P < 0.05). The hepatic expressions of IRS1-ser307, S6K1-thr389 (4-16 weeks ) and p-JNK(2-16 weeks ) increased and Akt-ser473(8-16 weeks )decreased in HFD group than those of NC group. The same results were gained by analysis of protein relative expression (all P<0.05). The hepatic pro-inflammatory factor MCP-1,TNF-αand IL-1βmRNA expressions were higher in HFD group than those in NC group during 2-16 weeks (all P < 0.01). The anti-inflammatory factor IL-10 mRNA was significantly lower in HFD group than that in NC group during 4-16 weeks (all P<0.01). Conclusion High fat diet maybe play a role in the hepatic insulin resistance by stimulating M1 Kupffer cells to secrete pro-inflammatory factor and inhibiting M2 Kupffer cells to se-crete anti-inflammatory factor.
5.Continuous resting energy expenditure measurement in ventilated critically ill children

Objective To monitor the changes of resting energy expenditure in ventilated critically ill children,to compare the results of standard equations and indirect calorimetry (IC) in predicting energy expenditure,and to investigate the possible influence factors of the metabolic status of the critically ill children.Methods From September 2012 to September 2013,56 critically ill children on assisted ventilation and fitting the requirements of IC in pediatric intensive care unit of Shanghai Children's Medical Center were enrolled in this prospective study.IC measurements were performed using metabolic cart on day 1,4,7,10 after trachea intubation.General clinical data of these children were recorded.Results 130 IC measurements were performed in the 56 children.The measured resting energy expenditure (MREE) did not exhibit significant differences among day 1,4,7,and 10 (P =0.379).Although there were no significant differences between MREE and energy expenditure predicted with Schofield and WHO equations (P =0.917,P =0.995),the agreement was poor between the measured and predicted values (R2 =0.185,R2 =0.322).The metabolic status of the children on day 1 of ventilation was only correlated with age (P =0.000) and height (P =0.027),not with severity of underlying diseases or clinical outcomes.Conclusions MREE of IC method in ventilated critically ill children did not significantly change over time in this study.A poor agreement was observed between equationpredicted energy expenditure and MREE.IC measurement of resting energy expenditure is recommended for guiding individual nutritional support among critically ill children so as to improve clinical outcome.
6.Correlation Between Diffusion Weighted Imaging Parameters and Protein Content in Fluid:An Experimental Study

Purpose Protein is the main influencing factors for diffusion weighted imaging (DWI) signals and apparent diffusion coefficient (ADC), it results in hyperintensity on DWI and low ADC, but not fully matched in clinic. This paper aims to investigate the effect of protein type and concentration on the signal intensity (SI) and ADC of DWI. Materials and Methods Different concentrations of albumin, globulin solution and the mixed solution were created in vitro. DWI was performed on GE 1.5T superconducting nuclear MRI system. Results ① There was a linear negative correlation between the ADC value and the concentrations of protein solution (at 37℃, ra= - 0.849, Pa<0.05; rg= - 0.843, Pg<0.05; at 40℃, ra= - 0.894, Pa<0.05; rg= - 0.819, Pg<0.05);there was a linear positive correlation between the SI of DWI and the concentrations of the albumin solution (at 37℃, r=0.753, P<0.05; at 40℃, r=0.845, P<0.05). There was no correlation between the SI of DWI and the concentrations of the globulin solution (at 37℃, r= - 0.222, P>0.05; at 40℃ , r= - 0.270, P>0.05). ② SI of the albumin solution was significantly higher than the globulin solution at the same concentration and temperature (t=3.96, P<0.001); the ADC values were not statistically different between the albumin and the globulin solution (t=0.61, P>0.05). Conclusion The nature of the cystic fluid can be understood preliminarily through quantitative analysis of the cystic fluid DWI and ADC values, so as to provide theoretical basis for the qualitative diagnosis of cystic lesions in vivo.
8.Expression and Immunogenicity Evaluation of Ectodomain and Subdomains of Plasmodium berghei Apical Membrane Antigen 1

Objective To express and evaluate the immunogenicity of ectodomain and its subdomains of Plasmodium berghei apical membrane antigen 1(PbAMA-1).Methods Sequence of PbAMA-1 gene was isolated from the genome of P.berghei,and was redesigned and divided into three overlapped fragments according to its subdomain structure.The codon-optimized DNA fragments of PbAMA-1 were synthesized and inserted into vector pET32a for expression in E.coli and the recombinant proteins were purified by Ni-NTA column,followed by refolding in vitro.Mice and rabbits were immu-nized with the recombinant proteins formulated with Freund adjuvant.Titer of the specific antibodies was detected by ELISA and IFA.The immunized mice were challenged by P.berghei to evaluate protective efficacy in vivo.Results The sequence of the PbAMA-1 gene was shown to be identical to that published before.PbAMA-1 sequence was redesigned via codon optimization and synthesized.Both ectodomain and its subdomains of PbAMA-1 were successfully expressed in E.coli after induction.The proteins were isolated with the purity of more than 90% after Ni column purification and refolding in vitro.Immunization of mice with the recombinant proteins induced high level of specific antibodies.The antibody titer to ectodomain E after the 3rd immunization showed a strong immunogenicity at(34.4?0.15)?10-4.The antibodies interacted with the parasites by indirect fluorescence.The immunized mice were partially protected from the challenge of P.berghei.Conclusion The recombinant PbAMA-1 is highly immunogenic and induces protective immunity against the challenge of P.berghei.
9.Investigation of application and administration of hyperbaric oxygen chambers in Beijing

Objective To investigate the application and administration of hyperbaric oxygen chambers in Beijing.Methods Forty-eight hospitals with hyperbaric oxygen chambers in Beijing were investigated by field investigation,letter investigation or telephone investigation.The investigation includes grades of hospital,personnel,equipment,therapy of severe cases,time of therapy and treatment plans in questionnaire.Results Hyperbaric oxygen was popular in Beijing,which ensured emergency treatments.But the scale was little relatively and the number of full-time technicians was less,besides,the therapeutic system and plans were diversified,which should be standardized.Conclusions Something must be done to improve the conditions of hyperbaric oxygen medicine in Beijing,such as cultivating personnel,standardizing therapeutic system,standardizing treatment plans,realizing computer control and so on.
10.Practice of PBL combined with evidence-based medicine in the standardized rotary residency train-ing in oncology department

Standardized rotary residency training is an important part of clinical medical education. Traditional clinical teaching can't meet the rapid development of oncology medicine. In the rotary residency training in oncology department, we put forward the problems encountered in clinical practice, stimulate the interest and initiative of residence, the PBL teaching model is combined with the evidence-based medicine in the teaching process through the relevant training, literature review and discussion. By standardizing the treatment concept the residence's understanding of the basic theory and frontier knowledge of oncology was improved, the thinking innovation and clinical practice ability of the residency doctors were enhanced.
