1.Transmission of HIV drug resistance in antiretroviral treatment-naive HIV-infected individuals

Human immunodeficiency virus ( HIV) is commonly transmitted through blood transfu-sion, sexual contact and mother-to-child transmission. The number of patients with HIV infection has kept growing in the last three decades. Although the wide application of antiretroviral therapy ( ART) has effec-tively suppressed the replication of HIV, the emergence of drug resistant mutants compromises the efficacy of ART. What is worse is that there has been transmission of drug resistance strains in ART-na?ve HIV-infected individuals. This review describes the distribution of transmitted drug resistance strains in different regions and high risk population as well as the factors associated with the transmission in recent years.
2.Analysis on the Medical Service Management at Private Hospitals based on the Investigation of Patient Satisfaction

Objective:To investigate and analyze the current status of patient satisfaction and the influencing factors at private hospitals in Guizhou Province,and thus to provide a scientific basis for the management of medical service at private hospitals.Methods:The satisfaction scores were statistically described and analyzed by the factor analysis and multiple linear regression analysis.Results:The overall satisfaction score of patients was (3.87 ± 0.85),of which the satisfaction of medical technology,drug price,inspection fee,and medical facilities was lower.The factor analysis showed that satisfaction of patients at private hospitals could be classified into the environment,the quality of service,medical expense,and service flow.Multivariate linear regression results showed that its standardized partial regression coefficient by the parameter descending order followed as medical expense,the environment,the quality of service,and then service flow.Conclusions:Private hospitals should set up a fair medical expenditure mechanism,improve the environment and quality of medical services,and optimize service flow in order to improve patient satisfaction.
4.Comparative study on the efficacy of different ways of common sclerosing agents used in ultrasonic intervention on the treatment of ovarian endometriosis cyst

Objective:To compare the efficacy of anhydrous ethanol and lauromacrogol in the treatment of ovarian endometriosis cyst (OEC), and to provide evidence for more effective use of these two sclerotic drugs.Methods:Thirty-three cysts in 30 patients with OEC, who were admitted to Taizhou Hospital of Zhejiang Province from February 2017 to June 2018, were treated by ultrasound interventional therapy, followed by catheter aspiration, saline cavity cleaning, anhydrous ethanol rinsing and sclerotherapy, and lauromacrogol alcohol 1∶1 mixed solution retained in the capsule (mixed group). The therapeutic effects were observed 6 and 12 months after operation. A total of 30 OEC in 28 cases admitted to the same hospital from 2013 to 2015 were retrospectively analyzed after similar treatment steps, but the intracapsular retention fluid was anhydrous ethanol (ethanol group). A total of 28 OEC in 26 patients admitted to the same hospital from 2015 to 2017 were washed with normal saline, then washed with lauromacrogol and retained in the capsule with lauromacrogol (lauromacrogol group). The therapeutic effects of 6 and 12 months after treatment between the three groups were compared.Results:According to the classification of cure, marked effect and inefficacy, there was a weak correlation among the three groups. The mixed group did not tend to be ineffective, while the lauromacrogol group tended to be ineffective 6 and 12 months after treatmen.the difference was statistically significant (adjusted standardized residual >2). According to the total effective (cure+ markedly effective) and ineffective classification, the effective rate of the mixed group was significantly higher than that of the lauromacrogol group ( P<0.016 7). There was no significant difference for the effective rate between the mixed group and the ethanol group, and between the ethanol group and the lauromacrogol group ( P>0.0167). Conclusions:The retention of ethanol-lauromacrogol mixture in the capsule of sclerotic OEC, after pre-rinsing with anhydrous ethanol is helpful to improve the curative effect of OEC and give full play to the therapeutic effect of common sclerosing agents used in ultrasound intervention.
5.Research on cellular receptors associated with HBV preS 1 adhesion

Objective To seek the specific receptors associated with hepatitis B virus (HBV) adhesion by separating the binding protein of the HBV preS1 region in HepG2 and performing the mass spectrometry .Methods The immunomagnetic bead method was adopted to separate HepG2 membrane protein combined with preS1 peptide fragment and the binding protein was separated by the sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) ,then the destination strips was analyzed by LC-MS/MS mass spectrometry and retrieved by the database .Results 16 bands were separated from HepG2 membrane proteins combined with preS1 by SDS-PAGE ;14 kinds of proteins were identified from 6 bands with better repeatability separated from HepG 2 membrane proteins combined with preS1 .Conclusion Protein analyzed by the mass spectrometry is mainly related with the material transport , cellular signal transduction ,antigen presentation ,immune regulation and energy metabolism .
6.Prediction of metabolic syndrome with combination of waist-to-hip ratio (or waist circumference) and blood pressure measurements

Objective To clarify which component plays a pivotal role in metabolic syndrome (MS) which is associated with an elevated risk for cardiovascular diseases and type 2 diabetes. Methods A total of 15564 participants (aged≥25 years old) with high risk for type 2 diabetes in the 1994 National Diabetes Mellitus Surveywere included in this analysis. MS was diagnosed according to the 1999 WHO criteria, and insulin resistance (IR) was defined as being above the 75th percentile of HOMA-IR of the population aged 25-74 years with normal glucose tolerance. Multiple logistic stepwise regression analysis was used to identify the contribution of each component to MS. Results The unadjusted and age-adjusted prevalences of MS in this population was 64.35% and 59.00%, respectively. Age, sex, systolic blood pressure(SBP), diastolic blood pressure(DBP), waist to hip ratio (WHR)/waist circumference and body mass index(BMI), but not IR, plasma glucose level, and plasma triglyceride level, were independently associated with MS, with predicted concordance of 93.8%. The combination of WHR, BMI, SBP, and DBP accounted for 90.2% of the variance of MS, and if BMI was deleted, the remaining three items accounted for 86.1% of the variance. An increment of 0.05 in WHR, or 5 cm in waist circumference, or 10 mm Hg in SBP, or 5 mm Hg in DBP, or 5 kg/m 2in BMI would elevate the risk for MS by 3.35 times, 1.87 times, 1.52 times, 1.46 times, or 1.34 times, respectively. Conclusion The combination of WHR/waist circumference and blood pressure can be used as a simplepredictor for MS in clinical practice.

8.Study of the relationship between blood glucose fluctuation and type 2 diabetes mellitus with macroangiopathy

AIM:To investigate the relationship between the fluctuation of blood glucose and the macrovascular complication of atherosclerosis (AS) in diabetic patients.METHODS:The individuals with different glucose tolerance were observed by continuous glucose monitoring for 3 days including the mean blood glucose (MBG) and its standard deviation (SD),mean amplitude of glycemic excursions (MAGE) and absolute means of daily differences (MODD).In addition,the intima-media thickness (IMT),intima smoothness as well as AS scores were measured respectively in bilateral common carotid arteries by means of high resolution B mode ultrasonography.RESULTS:The incidence of macrovascular complications in the subjects with impaired glucose tolerance (IGT) and newly diagnosed type 2 diabetes (T2DM) was significantly higher than those in the subjects with normal glucose tolerance (NGT),and a significantly difference between group IGT and T2DM was observed.The indexes detected by B mode ultrasonography were all increased gradually from NGT to IGT,then to newly diagnosed T2DM.The values of glycemic excursion were higher in IGT and T2DM group,especially in the subjects with diabetes than those in NGT group.In addition,multiple regression analysis showed that MAGE was significantly correlated with mean IMT and AS scores.CONCLUSION:Blood glucose fluctuation is associated with atherosclerosis.The patients with a larger range of blood glucose excursion have higher risks for developing atherosclerotic complications.The impaired glucose stability is a possible risk factor for atherosclerotic macroangiopathy in diabetes.

10.Comprehensive yield and quality comparison of Bupleurum chinense' s seeds of different harvesting time from Qingchuan County with Topsis approach.

In order to investigate the comprehensive quality differences of the seeds harvested in different growing time, and offer theoretical guide for the optimization of seeds' production technology, we analyzed the apparent size, 1 000-grain weight, water absorbency, germination indexes, postharvest embryo rate change, relatively contents of coumarins and the yield of single plant of its seeds of different harvesting time, and compared their comprehensive quality by Topsis analysis. The results showed that there existed obvious differences in yield and quality between seeds of 3 harvesting times. While the harvesting time postponed, the yield of single plant raised, and the shrunken seeds rate decreased, the quality of seedlings increased, while the contents of coumarins showed a steady increase, and the germination rate decreased. The comprehensive quality of the seeds harvested in the black ripe time rank the first place, followed by the brown ripe time and the yellow ripe time. As the harvesting time delays, the seeds' comprehensive quality increases, therefore, we could put off the seeds' harvesting time properly for the high efficient seed production.
