3.Detection of platelet activation marker and blood clotting activity in patients with hypertension and its clinical significance

Objective To investigate the changes of molecule markers of pre-thrombotic state,expression of CD62P,concentration of plasma fibrinogen(Fib),prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT) in the patients with essential hypertension for providing theoretical evidence for diagnosis and therapy of thrombus complication in early clinical stage.Methods Fifty-three patients with hypertension(grade Ⅰ 17 cases,grade Ⅱ 16 cases,grade Ⅲ 20 cases)were selected and twenty-eight people with medical examination in good health were used as control group.The expressions of CD62P and CD61 were measured by flow cytometry.The concentration of Fib and the levels of PT,APTT,TT were measured by Class coagulation method.Results The expression of CD62P and the concentration of plasma Fib in essential hypertension group were higher than those in control groups(P0.05).Conclusion The essential hypertensive patients are in pre-thrombotic state,the activation of platelet and blood clotting activity increase.Molecular markers should be measured in essential hypertensive patients for prevention and treatment of thrombotic disease.
4.Incision Infection in Surgical Operation: Investigation and Strategy

OBJECTIVE To explore the infection situation of surgical operation and enhance postoperative(observation) and rational use of antibiotics.METHODS Totally 11 208 cases after surgical operation were(investigated)(retrospectively).The diagnosis standard was based on the Diagnosis Standard of Hospital(Infection)(Draft) published by Ministry of Health,the People′s Republic of China in Jan 2001. RESULTS There were(11 208) cases after surgical operation from Jan 1,2000 to Dec 31,2004 in our hospital and 275 cases suffered(incision)(infection,) there(infection) rate was 2.45%.The pathogenic bacteria of incision infection were confirmed as G~-(bacilli)(72 strains,78.26%),and G~+ cocci(20 strains,21.74%).CONCLUSIONS The key points of prevention of incision infection are sterile operation,protection of operation field when incising cavity organ,inside(sterilization),flushing of abdominal cavity and incision,change of contaminated apparatus,gloves and dressing promptly,(selection) of suture line and suture technique,besides the serious operation preparedness and skin(sterilization).
5.Imaging of Cisterna Chyli and Thoracic Duct by Nonenhanced MR Lymphography

Objective To investigate the depiction rate of normal cisterna chyli and thoracic duct by nonenhanced MR lymphography and to describe their appearances on MR imaging.Methods Special MR hydrography sequence was added to the MR imaging protocols of 112 patients undergoing MR examination of the thorax and upper abdomen.MR imaging sequences included:①Respiratory-gated HASTE T2W sequence;②Breath-hold FLASH T1W sequence;③Respiratory-gated TSE 3D T2W sequence(3D MR hydrography sequence)in coronal plane.One hundred cases who met the inclusion criteria were included into the study for observation of the depiction rate,location and morphology of cisterna chyli and thoracic duct.Results On TSE 3D T2W imaging:①Cisterna chyli was visualized in 71/100(depiction rate 71.0%),morphologically including single-tube type 43.7%(31/71),bifurcation type(2-3 tubes)23.9%(17/71),plexus type 32.4%(23/71).Average length of the cisterna chyli was 4.5 cm.②The depiction rate of the lower segment of thoracic duct was 57.0%(57/100),average ductal diameter was 0.23 cm.③The depiction rate of upper segment of the thoracic duct was 31.0%(31/100).Conclusion As a noninvasive method for depicting the lymphatic system,nonenhaced MR lymphography(TSE 3D T2W sequence)demonstrated a high depiction rate for cisterna chyli and lower thoracic duct.Combined with axial images of HASTE and FLASH sequences,the location and morphology of these larger lymphatic ducts can be defined.
6.Virtual non-contrast of the second-generation dual-source CT:the capability to display the adipose tissue of gastric serosa

Objective To evaluate the capability of virtual non-contrast (VNC)of the second-generation dual-source CT (DSCT), and to display the adipose tissue of gastric serosa.Methods 48 patients with gastric cancer undergoing preoperative DSCT and sur-gery were enrolled.The true non-contrast (TNC)values and VNC values of perigastric adipose tissue were measured and compared with conventional non-contrast scanning and VNC of arterial phase and venous phase.The VNC values of perigastric adipose tissue were compared between serosal involvement group and non-serosal involvement group.Results There was statistical difference be-tween TNC values and VNC values of perigastric adipose tissue (P <0.05),the absolute differences were under 13 HU.The differ-ence of VNC values between serosal involvement group and non-serosal involvement group was significant (P <0.05).Conclusion VNC of the second-generation DSCT can clearly display the adipose tissue of gastric serosa.It has certain capability in judging sero-sal involvement.VNC has the proficiency to replace conventional non-contrast scanning.
8.Mechanism of anti-proliferation effect of ginseng Rh_2(G-Rh_2) on mouse MFC gastric cells

Aim To investigate the molecular mechanism of anti-proliferation effect of Ginsenoside Rh2(G-Rh2)on mouse MFC gastric cells.Methods The cell viability was determined by MTT method and the state of cell proliferation was analyzed.The morphologic changes were observed by invert microscope and fluorescence microscope(Hoechst 33258).Early apoptotic rate was detected by Annexin Ⅴ-FITC through flow cytometry.Western blot was used to detect the p-JNK and p-c-jun activity in MFC cells.Immunocytochemistry staining was used to detect caspase-3 positive cells.Results G-Rh2 could inhibit proliferation of MFC cells in a dose-and-time-dependent manner.The morphological changes of apoptosis were observed in MFC cells after G-Rh2 treatment.The apoptotic ratio was increased.The phosphorylation of c-Jun and c-Jun NH2-terminal kinase(JNK)activity increased with time within 4 h compared with that of control.The expression of caspase-3 positive cells was increased.But all the above could be partly inhibited by pre-treatment with SP600125(5 ?mol?L-1)for 2 h.Conclusion G-Rh2 can decrease cell viability by inducing apoptosis and the process may be associated with the activation of JNK transduction pathway and expression of caspase-3.
9.The effects of autoimmune regulator(AIRE) TLRs on TLRs expression

Objective:To explore the effect of autoimmune regulator to TLRs expressions on peripheral antigen presenting cells(APC).Methods:①pEGFPC3-AIRE plasmid was transfected with liposome.②Confocal microscopy was used to observe the effect of transfection.③RT-PCR assay was used to detect the expressions of AIRE and TLR1-9 in RAW264.7cells at 36,48,72,96 h after transfection.Results:①The plasmid was transfected into RAW264.7 cells successfully,and the efficiency of transfection was 60~70%.②AIRE transfected RAW264.7 cells were achieved,and the best time was 72 h.③At 72 h after transfection,the expressions of TLR1,4,5,9 increased,and TLR3,7,8 reduced.The expression of TLR2,6 increased at 96 h.Conclusion:AIRE may regulate the immune response by control TLR expression in APC.It maintain the effective response to pathogen and tolerance state to self tissues through the effects to different TLRs.
10.Perioperative management of the elderly patients undergoing coronary artery bypass grafting

Objective To introduce the strategies of coronary artery bypass grafting (CABG) surgery for elderly patients over 65 years old. Methods Three hundred and ninety four elderly patients were retrospectively studied, 92.4% of whom were complicated with other diseases. Off-pump CABG (OPCAB) was used in 248 cases (62.4%). For the remaining patients with severely compromised cardiac function or small target vessel with diffuse lesion, conventional CABG (CCABG) was selected. Left internal mammary artery (LIMA) was harvested with extrapleural harvesting technique and grafted to left anterior descending artery (LAD) in 96.2% of patients. Saphenous vein or radial artery was utilized for the other coronary arteries. Transit-time flowmeter (TTFM) was utilized to make sure that grafts were patent with satisfactory blood flow. Perioperative blood sugar level was controlled at 6-10mmol/L by insulin. Results Grafts of per patient were 3.14?0.55 for on-pump CABG and 2.40?0.81 for OPCAB. Three patients died resulting in a mortality of 0.76%. The incidence of complication was 2.28%. The rest recovered uneventfully. Intubation time of OPCAB was significantly shorter than that of CCABG. Patients were discharged within 11.84?4.95 days after operation. Conclusion Good short-term result, namely low incidence of mortality and complication, could be obtained in elderly patients over than 65 years old undertaking CABG.
