2.Study on the competency of community health administrators

Objective To explore the competency of community health administrators in a survey of such administrators at large in Shanghai. Methods McClelland's Competency Dictionary was used to set two benchmark positions, while the behavioral event interviewing and questionnaires were made to survey 19 community health service centers in Shanghai. Results The competency model benchmarking on the head of a medical branch consists of 6 character clusters, 11 characteristic items, 27 evaluation dimensions and 27 typical behavior descriptions. The model benchmarking on the community team leader consists of 5 character clusters, 8 characteristic items, 21 evaluation dimensions and 21 typical behavior descriptions. Conclusion Findings of the survey provide objective criteria for competence appraisal of community health administrators, and serve as reference for formulating their development plans as well.
3.HPLC fingerprint of Fructus aurantii micropowder by methanol extraction

AIM: To provide a HPLC fingerprint of Fructus aurantii micropowder in order to create a basis for(identification.) METHODS: With the help of computer similarity evaluation,Seventeen kinds of Fructus aruantii sample were pulverized and micronized separately,then were extracted by methanol and compared correlation between both extracts. RESULTS: In methanol extraction,contents of aurantiamarin,hesperidin and neohesperidin in micropowder were more than that in pulverized powders,correlation of both HPLC fingerprint was 0.9~1.0. CONCLUSION: The method established can be used for identifying and evaluating crud drug of Fructus aurantii,and further prove that micronization is beneficial to increasing flavonoid dissolution.

AIM: To provide application and identification basis for methanol-extratis of Fructus aurantii by constructing the HPLC-FP. METHODS: The C_(18) column was used with a mobile phase of(A)(0.095%) phosphoric acid acetonitrile-(B)(0.095%) phosphoric acid gradient elution.The flow rate was(1.0) mL/min.The wavelength of detecter was set at 330 nm.Naringin was reference standard. RESULTS: By cluster analysis,the eighteen kinds of Fructus aurantii samples were classified as four clusters: the superior in producing area,the ordinary in producing area,the ordinary and the inferior.By similarity calculation,the similarity of the superior in producing area and the ordinary in producing area were(0.9)~(1.0),and the ordinary and the inferior were less than(0.9). CONCLUSION: The HPLC-FP of Fructus aurantii has been established.The method can be used to identify and evaluate the quality of Fructus aurantii.
5.Observation and Nursing to Hepatic Malignancy Patients Treated by Radio Frequency

Treating Hepatic Malignancies with radio- frequency is a new microinvasive method which has been developed in recent years. The patients treated with this method felt less painfull and recover quickly after operation. The author has reported the detailed observaticn to 39 patients with hepatic carcinomas and cooperated with the doctors for patients'psychological nursing in this article. During the treatment,the author tried to explain the aim,method and treating effect of the operation, help the patients to relieve nervousness end worries, observe the patients' inoperative response under cardiopneumograph and guide the patients for the postoperative observation of complications so that the treatment was completed successfully. The author suggests that sentimental support is an important step in the whole course of nursing
6.Arthroscopically assisted mini-incision arthrolysis for the stiff knee

[Objective]To discuss the method and effectiveness of arthroscopically assisted mini-incision arthrolysis for the stiff knee.[Method]A small lateral suprapatellar incision was made in all 26 patients suffered from stiff knee,and the scissors were introduced the joint to release the adhesive tissues of patellofemoral joint.All adhesive tissues between the medial,lateral retinaculum and the femur,and the between vastus intermedius and femur were released subsequently.After the release of the adherence between the fat pad and femoral condyle,the lateral retinaculum was dissected 1 cm lateral to the patellae,and the dissection of medial retinaculum was applied at the insertion of the vastus medialis.Eventually,arthroscopic debridement and hemostasis combined with manipulative release was applied to remove all remanent adhesive bundles and scar tissues.[Result]Follow-ups were maintained for an average of 19 months(range,from 8-32 months).An increase in the average range of motion of the knee was attained from 32 degrees preoperatively to 113 degrees postoperatively.No complications such as skin necrosis,wound dehiscence,tendon rupture and fracture had occurred during the follow-up period.[Conclusion]The advantages of arthroscopically assisted mini-incision arthrolysis for the stiff knee include simple procedure,minimal trauma,and quick rehabilitation.Otherwise,reservation of the insertion of vastus medialis is of great importance not only for the maintenance of extensive strength and stability of the knee,but also for the functional recovery and the reduction of complications.
7.HPLC fingerprint of ultramicro decoction piece of Radix paeomae Alba

AIM: To determine the HPLC fingerprint of ultramicro decoction piece of Radix paeoniae Alba. METHODS: HPLC was used to analyze the extracts of ultramicro decoction piece of Radix paeomae Alba from 10 different sources. RESULTS: The fingerprint of ultramicro decoction piece of Radix paeoniae Alba was composed of 20 peaks,among which there were 10 characteristic peaks. CONCLUSION: The fingerprmt can be used to control the ultramicro decoction piece of Radix paeoniae Alba qualities.
8.Virtual non-contrast of the second-generation dual-source CT:the capability to display the adipose tissue of gastric serosa

Objective To evaluate the capability of virtual non-contrast (VNC)of the second-generation dual-source CT (DSCT), and to display the adipose tissue of gastric serosa.Methods 48 patients with gastric cancer undergoing preoperative DSCT and sur-gery were enrolled.The true non-contrast (TNC)values and VNC values of perigastric adipose tissue were measured and compared with conventional non-contrast scanning and VNC of arterial phase and venous phase.The VNC values of perigastric adipose tissue were compared between serosal involvement group and non-serosal involvement group.Results There was statistical difference be-tween TNC values and VNC values of perigastric adipose tissue (P <0.05),the absolute differences were under 13 HU.The differ-ence of VNC values between serosal involvement group and non-serosal involvement group was significant (P <0.05).Conclusion VNC of the second-generation DSCT can clearly display the adipose tissue of gastric serosa.It has certain capability in judging sero-sal involvement.VNC has the proficiency to replace conventional non-contrast scanning.
9.Evaluation of a modified Chinese version of the National Institutes of Health-Chronic Prostatitis Symptom Index

Objective To revise the Chinese version of the National Institutes of Health-Chronic Prostatitis Symptom Index (CHN-NIH-CPSD), and evaluate its feasibility, reliability, validity and responsiveness. Methods The NIH-CPSI was translated into Chinese according to a standard methodology including forward-backward-forward technique. The CHN-NIH-CPSI was pre-tested in consecutive samples of 162 native-speaking Chinese chronic prostatitis(CP)patients. Ninety-five of 162 filled the index again on the same day and after 4-week therapy. Ninety-seven healthy men were included as evaluated. Results The recovery of the questionnaires was 100% and all the patients filled the index completely. The mean time to complete the questionnaire for the patient group was 5.2±2.4 (range 2 - 12) min. The split-half reliability was 0.82. For the overall index and each subscale, the test-retest reliability was 0.98, 0. 98, 0. 98, 0. 97, respectively(P<0.01);and the Cronbach's α coefficient was 0. 61,0. 71, 0. 59, 0. 75, respectively. The confirmatory factor analysis showed good construct validity with a goodness of fit index of 0. 85 and a x2 of 124.67(P<0. 01). Of all 162 patients, the scores of the overall index and each subscale were 23. 33±5.91. 8. 80±4.26, 5.30±2.82, 9. 23±1.90, respectively;and those of healthy controls were 1. 95±1.97, 0. 37±1.03, 0. 15±0.58, 1.42± 1.20,respectively. Of the 95 patients, the original scores were 23. 53±5.60, 9.21 ±4.04, 5.10±2.75,9.21 ±2.05, comparing with 19.47±6.36, 7.79±3.95, 3. 58±1.88, 8.11±2.50, the 4 weeks later scores. The group t-test and paired t-test showed good responsiveness. Conclusions The CHN-NIH-CPSI has high feasibility, reliability, validity and responsiveness for testing the patients with CP. It is suitable for Chinese-speaking patients and helpful for cross-cultural comparisons of men with CP in clinical and research settings.
10.Study on Preparation and Releasing Characteristics in vitro of Weiyoukang Intragastric Floating Tablet

OBJECTIVE:To prepare Weiyoukang intragastric floating tablet,and to investigate in vitro releasing characteristics of it. METHODS:Weiyoukang intragastric floating tablet was prepared by full powder compression method using HPMC-K15M (framework material),octadecanol(low density bleaching promoter),sodium bicarbonate(foaming agent),PVP-K30(release reg-ulator)and magnesium stearate(lubricant)as excipients. The amounts of HPMC-K15M,octadecanol and PVP-K30 were optimized by orthogonal test and validated with 2 h,4 h and 8 h accumulative release rate of berberine (Y2 h,Y4 h,Y8 h) as index,start time (T1)and duration(T2)of floating as adjunctive index. The releasing regularity of berberine,tetrahydropalmatine,baicalin and glyc-yrrhizic acid in Coptis chinensis,Corydalis vanhus,Scatellaria baicalensis and Glycyrrhiza uralensis were measured by release model fitting method and Peppas equarion fitting method. Similar factor(f2)method was used to investigate the similarity of 4 com-ponents. RESULTS:The optimized formulation was as follows as HPMC-K15M 40 g,octadecanol 14 g,PVP-K30 2 g for 200 tab-lets. Y2 h,Y4 h and Y8 h of berberine of preparations were 31.94%,55.88%,93.34%(RSDs were 1.2%,1.5% and 1.5%,n=3),re-spectively. T1 and T2 were about 1 min and 13 h. The drug release of four ingredients all fitted Higuchi models,and were synergis-tic action of diffusion and matrix erode. The f2 were all more than 80,indicating that the release of four ingredients were similar. CONCLUSIONS:Weiyoukang intragastric floating tablet is prepared successfully,showing fast onset,long floating,sustained-re-lease and balanced release of multiple indigents.
