1.Survey on psychological adaptability of medical undergraduate interns

One hundred medical undergraduate interns were randomly selected to undergo a survey using self-designed questionnaire,95 students completed the questionnaire with a response rate of 95.0%.The results revealed that all interns receiving survey had psychological adaptability problems,57.9% of the interns were not adaptive to life well,67.7% had maladjustment of learning,63.2% had social maladjustment,65.3% had poor metal adaptability.The 47.4% of the interns felt very heavy stress and 36.8 % felt stress,only 15.8 percent felt less stress.The stress sources included seeking employment and pursuing career for clinical practice or academic training.The results showed that psychological adaptability is a common problem for medical undergraduate interns and the social stress sources would intervene the clinical training and study for them.The medical educators should pay attention to this problem and psychological counseling and other measures should be taken for medical interns.
2.Influence of seawater immersion of open abdominal wound on function and pathological changes in liver in dog

Objective To investigate the influence of seawater immersion of open abdominal wound on function and pathological changes in liver in dog.Methods Twenty healthy dogs were divided randomly into two groups.A perforating abdominal wound was was made in each animal of both groups.Animals in control group(n=10)only abdominal wound was made,while abdominal wound was followed by seawater immersion in the animals of seawater immersion group(n=10).Blood samples were taken before the trauma(0 hour)and at 4,8,12,16 and 20 hours after the trauma and water immersion for both groups.Total bilirubin(TB),alanine transaminase(ALT),aspartate transaminase(AST),lactate dehydrogenase(LDH),blood ammonia(NH3),prothrombin time(PT),tumor necrosis factor-?(TNF-?),interleukin-6(IL-6)and endotoxin(ETX)were assayed.Experimental animals were sacrificed 24 hours after trauma,and the pathological changes in hepatic tissue were studied.Results TB,ALT,AST and PT were higher in seawater immersion group,than those in control group,with a marked elevation at 16 hours.In seawater immersion group,LDH was significantly higher than those in control group at 4 and 12 hours,and NH3 was increased significantly at 4 hours in experimental group(P
3.Effects of sodium arsenite on melanin metabolism of A375 and G361 cell lines

Objective To study the impact of sodium arsenite(NaAsO2) exposure on melanoma cells A375 (hereinafter referred to as the A375) and G361 (hereinafter referred to as the G361) pigment production and tyrosinase (TYR) activity and the differences of pigment metabolism capacity between the cell lines.Methods A375 and G361 cells were exposed to sodium arsenite at concentrations of 0.0(control),0.1 and 1.0 μmol/L for 72hours.Cell viability was measured by Alamar Blue assay.Melanin levels and TYR activity were measured at the same time.Results After exposure for 72 hours,the cells of 0.1 μmol/L dose groups of both of the two cell lines [A375:(103.32 + 1.26)%; G361:(104.10 + 1.76)%] showed a slightincrease of proliferation without significant differences compared with those of the control[A375:(100.00 ± 1.08)%; G361:(100.00 + 1.79)%,all P < 0.05] ;while cell viability of the 1.0 μmol/L dose group of both of the two cell lines[A375:(75.32 ± 1.59)%; G361:(78.26 ± 2.10)%] were significantly lower than those of the control (all P < 0.05).Melanin levels of G361 cell line [(7.19 ± 0.35),(7.34 ± 0.83),(8.19 ± 0.86)pg/cell] were significantly higher than that of A375[(4.35 ± 0.72),(4.54 ± 0.01),(4.60 + 0.59)pg/cell,all P < 0.05] in all the three groups.TYR activity of G361 cell line [(54.13 ± 1.21),(54.56 ± 0.21),(56.25 ± 0.85)Bq] were also markedly higher than that of A375 cell[(42.00 ±0.21),(42.90 ± 0.54),(42.91 ± 0.01)Bq,all P < 0.05] in all the three groups.The melanin levels and TYR activities of both of the two cells lines showed an increase tendency along with increased doses of arsenic exposure,but without significant differences when compared with those of the three groups (all P > 0.05).Conclusions Arsenic related pigment disorder may be associated with increased melanin levels and TYR activities induced by arsenic exposure; individual difference of pigment metabolism may be associated with different basal melanin levels and TYR activity between different individuals.
4.Apllication of scalp nerve block combined with sevoflurane in children drilling and drainage operation

Objective To study the application of nerve block scalp combined with sevoflurane in children drilling drainage oper-ation .Methods 40 children cases undergoing drilling drainage were selected and randomly divided into the scalp nerve block com-bined sevoflurane group(A ,n=20) and the sevoflurane intravenous inhalational combined anesthesia (B ,n= 20) .The changes of HR and MAP before before induction and at 5 min after induction ,skin incision ,15 min after operation beginning ,operation end , postoperative 30 ,120 min were observed and recorded .The awaking time ,occurrence rate of postoperative nausea and vomiting ,in-cidence rate of postoperative dysphoria and fentanyl dosage were compared between the two groups .Results HR ,MAP at skin in-cision ,15 min after operation beginning ,operation end ,postoperative 30 ,120 min in the group B were significantly increased than before operation(P<0 .05);the awaking time in the group A was significantly shortened compared with the group B (P<0 .05) , the fentanyl dosage was significantly reduced (P<0 .05);in the group B ,crying and screaming appeared in 16 cases ,dysphoria oc-curred in 14 cases ,the incidence rate was significantly higher than that in the group A (P<0 .05) .Conclusion Compared with sim-ple sevoflurane intravenous inhalational combined anesthesia ,sevoflurane combined with scalp nerve block used in children drilling drainage operation has rapid induction and rapid awaking ,stable vital signs ,fewer adverse reactions ,small fentanyl dosage and bet-ter anesthetic and postoperative analgesic effects ,which is safe ,feasible ,simple and effective .
5.Effect of polypeptides from ehlamys farreri on human umbilical cord stem blood cells differentiation in vitro

6.Expression of HIF-1α and GLUT-1 in endometrial carcinoma and its clinical significance

Objective To investigate the expression and significance of hypoxia inducible factor-1 alpha(HIF-1α)and glucose transporter protein 1(GLUT-1)in endometrial carcinoma.Methods Immunohistochemical(PV-9000 two-step)method was applied to detect the expression of HIF-1α and GLUT-1 in 36 cases of endometrial carcinoma,18 cases of atypical hyperplasia,12 cases of complex hyperplasia,15 cases of simple hyperplsia,atypical hyperplasia and endometrial carcinoma were 6.7%,25.0%、38.9%and 77.8%.Compared expression of HIF-1αin endomelrial adenocarcinoma with those in endometrial simple hyperplasia,complex hyperplasia and atypical hyperplasia tissue,differences were statistically significant(P<0.05).The positive rates of GLUT-1 in endometrial simple hyperplasia,complex hyperplasia,atypical hyperplasia and endometrial adenocarcinoma were 0,8.3%,44.4%and 83.3%.Compared expression of GLUT-1 in endometrial adenocarcinoma with those in endometrial simple hyperplasia,complex hyperplasia and atypical hyperplasia tissue,differences were statistically significant(P<0.05).According to Spearman Correlation analysis,there was positive correlation between HIF-1α and GLUT-1(r=0.559,P<0.01).Conclusion The positive rates of HIF-1α and GLUT-1 gradually increase in endometrial simple hyperplasia,complex hyperplasia,atypical hyperplasia and endometrial carcinoma,which indicates HIF-1α and its taget gene GLUT-1 play a very important role in the development of endometrial carcinoma.The expression of GLUT-1 can be enhanced by HIF-1α.
7.Bipolar artificial femoral head replacement for treatment of elderly unstable intertrochanteric fractures

Objective To investigate the clinical effect of bipolar artificial femoral head replacement in the treatment of elderly patients with unstable intertrochanteric fractures. Methods 82 elderly patients with unstable intertrochanteric fractures were randomly divided into two groups with 41 cases in each group. The control group was treated with dynamic hip screw internal fixation, and the observation group was treated with bipolar artificial femoral head replacement. The operation time, bleeding volume and ambulation time were compared, and all the patients were followed up for one year. Harris scores of the two groups at 1 month, 3 months, 6 months, and 12 months after operation were compared, and the incidence of complications were recorded and compared. Results The operation time, bleeding volume and ambu-lation time of the observation group were less than the control group with a statistically significant difference (P<0. 05);Harris scores of the 2 groups at 1 month and 3 months after operation were of significant difference, while there was no significant difference at 6 months, and 12 months after operation (P>0. 05). The incidence of complication of the observation group was lower than that of the control group, the difference was statistically significant (P<0. 05). Conclusion Bipolar femoral head replacement has the advantages of simple operation, less trauma and complications. It is conducive to the early ambulation of patients and the recovery of hip function, and it is suitable for elder-ly patients with unstable femoral intertrochanteric fracture.

10.Retrospective study on neurological deterioration after mild traumatic brain injury

Objective To study the causes, temporal courses, predictors and outcomes of delayed neurological deterioration (DND) after mild traumatic brain injury. Methods In a cohort of 254 patients, the incidence, causes and temporal courses of DND after mild traumatic brain injury were summed up. Univariate and multivariate (Logistic regression) analysis were performed to assess the predictors of DND. The short-term outcomes were compared between patients with and without DND. Results DND occurred in 14.2% (36/254) of the 254 patients. Intracranial causes of DND such as hematoma growth were observed in 91.7%(33/36) of patients with DND. The interval from arrival to DND was 0.5- 59.0 h. Thirty patients (83.3%, 30/36) deteriorated within 24 h. The univariate analysis revealed that dangerous injury mechanism, loss of consciousness, severe headache, more than 1 time vomiting, interval from injury to first CT scanning, first Glasgow coma scale (GCS) scores, basal skull fracture, hypocoagulability, heavy alcohol consumption were the predict factors for DND (P<0.05 or<0.01). The multivariate analysis revealed that interval from injury to first CT scanning (P=0.000, OR=0.169, 95% CI: 0.083- 0.344), first GCS scores (P = 0.004, OR = 0.355, 95%CI: 0.175- 0.721) and hypocoagulability (P=0.001, OR=0.077, 95%CI:0.017-0.354) were the independent predict factors for DND. All of the patients without DND recovered better. But in the patients with DND, 30 patients recovered better, 4 patients had slight disability, 1 patient had severe disability, and 1 patient died. The treatment outcome between two groups had significant difference (Z =-6.088, P = 0.000). Conclusions The causes of DND are mostly intracranial. Most patients deteriorat within 24 h. The interval from injury to first CT scanning, first GCS scores and hypocoagulability appear to be independent predictors of DND. DND is associated with poor short-term outcomes.
