1.Peripheral blood stem cells transplantation in patients with heart failure after myocardial infarction: their efficiency and safety

Objective To compare the efficiency and safety of intracoronary transplantation of peripheral blood stem cells (PBSC) between elderly and younger patients with heart failure after myocardial infarction (MI). Methods Twenty-five patients with heart failure after MI were divided into aged group(≥60 years,n=13) and non-aged group (<60years,n=12) to receive intracoronary PBSC transplantation (PBSCT) following bone marrow cells mobilized by granulocyte colony-stimulating factor (G-CSF). Clinical data including coronary lesion characteristic, left ventricular shape, infarct region area and cardiac function, as well as adverse side effects between the two groups were compared. Left ventricular function was evaluated before and 6 months after the treatment by single photon emission computed tomography (SPECT). Results At 6 months, the left ventricular ejection fraction (LVEF) and 6 minute walk test (6MWT)distance increased, while the left ventricular diastolic diameter (LVDd) decreased significantly in both groups. There were no significant difference between the two groups in absolute change in the cardiac function parameters. Conclusions The present study demonstrated that autologous intracoronary PBSCT might be safe and feasible for both old and younger patients with heart failure after MI and left ventricular function is significantly improved.
2.MICROARRAY ANALYSIS OF DIFFERENT GENE EXPRESSION OF HUMAN CERVICAL CANCER SUBCLONE CELL LINES

Objective To examine the differentially expressed invasion-related genes in two anchorage-independent uterine cervical carcinoma cell lines derived from the same patient using a cDNA array. Methods Two human uterine cervical carcinoma subclonal cell lines CS03 and CS07 derived from a single donor line CS1213 were established by limited dilution procedure. The two cDNA samples retro-transcribed from total RNA derived from CS03 and CS07 cells were screened by a cDNA microarray carrying 234 human cell-cycle related genes and 1011 human signal transduction and membrane receptor -associated genes, scanned with a ScanArray 3000 laser scanner. Results The cDNA microarray analysis showed that 12 genes in CS03 were up-regulated compared to CS07, and 24 genes in CS07 were up-regulated. The function of a number of differentially expressed genes was consistently associated with cell-cycle, cell proliferation, migration, apoptosis, signal transduction and tumor metastasis, including p34cdc2, TSC22, plasminogen activator inhibitor I (PAI-1)and desmosome associated protein(Pinin). Conclusion Multiple genes are differentially expressed in uterine cervical carcinoma cell lines even came from the same patient. It is suggested that these genes are involved in the different phenotypic characteristics and development of cervical carcinoma.
3.Establishment of rat colon injury model with acetic acid in vitro and the protective effect of melatonin

Aim To build a simple experiment model of rat colon injury induced by acetic acid in vitro and to observe the effects of melatonin on this model.Methods On the basis of the establishment of rat colon injury model with acetic acid in vitro,we observed the colon mucosal damage caused by different concentrations of acetic acid(0,0.1%,0.2% and 0.4%) for different time(5,10,15,20,25 and 30 min),determined the content of lactic dehydrogenase(LDH),malondiadehyde(MDA) and mucus in medium,and examined the histological changes of colon mucosa by Alacian Blue method.On the basis of the establishment of this model,the experiment was designed into normal group,model group(acetic acid of 0.4%,time for 30 min) and melatonin treatment group(the final concentration 0.1,1.0、10 mmol?L~(-1)),and the indicators described above were detected to investigate the protective effects of melatonin.Results The LDH content of medium elevated gradually and the colon mucosal epithelial cells were injured by acetic acid in dose-and time-dependent manner,the mucosal edema,colon-wall depth and epithelium lose were observed at the same time,the MDA content of medium enhanced and mucus reduced correspondingly,but no significant change of the mucin expression in mucosal epithelial cells were observed.Pretreatment with melatonin reduced the release of LDH and MDA while promoted the secretion of mucus.Conclusion Colon mucosa of rats was injured by acetic acid in dose-and time-dependent manner in vitro.Acetic acid can impair the mucosal-mucus barrier by oxidating injured cell memberane.Melatonin can strengthen the barrier function of colon mucosa by its anti-oxidant action,attenuate the direct damage on colon mucosa of acetic acid.
4.Clinical features of moderate to severe asthma exacerbation in school-age children

Objective To understand the clinical features and current therapies on asthma exacerbation,so as to provide practical evidence for managing asthma episodes. Methods Two hundred and fourteen school - age(≥6 years old)cases who were admitted for asthma exacerbation in Beijing Children's Hospital,from January 2005 to De-cember 2013,were enrolled. All the cases were categorized into 2 groups:asthma with pneumonia group(group A,n =88)and asthma group(group B,n = 126). Results There were 214 cases enrolled,the median age was 9(6 - 18) years old,and the male - female ratio was 1. 5: 1. 0,78. 0%(167 / 214 cases)of the patients enrolled had moderate asthma exacerbation,and 22. 0%(47 / 214 cases)were defined as severe episodes,7. 0%(15 / 214 cases)were on me-chanical ventilation. Compared with group B,group A had more systemic corticosteroids use(53. 4% vs 38. 9% ,χ2 =4. 416,P = 0. 036),higher proportion of cases needing mechanical ventilation(12. 5% vs 3. 3% ,χ2 = 5. 476,P =0. 019),longer durations in hospital(9. 0 d vs 8. 0 d,Z = - 3. 063,P = 0. 002),and higher hospitalization expenses (10 000 yuan vs 9 000 yuan,Z = - 2. 058,P = 0. 000). The prescription of Aminophylline didn't help to shorten the duration of wheeze days(5. 7 d vs 5. 9 d,t = - 0. 233,P = 0. 817). Twenty - four cases received bronchial lavage through bronchoscope during wheezing days,in which 22 cases(91. 7% )of wheezing disappeared within 3 days after the lavage. There were more sensitized allergens detected in those patients who showed hypersecretion status under bronchos-copy(3. 3 ±1. 4 vs 2. 2 ±1. 2,t =2. 190,P =0. 036),compared with those without such signs. Conclusions Pneumonia may increase the demand for systemic use of corticosteroids in asthma exacerbations. The early use of systemic corticoste-roids may have an effect on reducing the severity of asthma attack,especially those with pneumonia. Multi - sensitization may contribute to the hypersecretion in bronchi. The bronchial lavage may help improve the bronchial ventilation caused by mucous plugin. The clinical usage of Aminophylline is still controversial,and it needs to be optimized.

7.Investigate the use of PPI to stress ulcer prophylaxis in 1 745 cases of surgical patients receiving aseptic-contaminative and contaminative incision operation

Objective To study the rationality in the use of proton pump inhibitors (PPI) to stress ulcer(SU) prophylaxis in pa-tients receiving aseptic-contaminative and contaminative incision operation .Methods A retrospective analysis was performed on the use of PPI to SU ,which taken from surgical patients received aseptic-contaminative and contaminative incision operation and hospi-talized from January 2013 to April 2013 .Results Among 1 745 patients in the study ,540(30 .95% ) received a stress ulcer prophy-laxis with PPI .The rate of prophylaxis to ICU ,general surgery ,urology ,thoracic surgery and neurosurgery was higher than other departments(except for between ICU ,thoracic surgery ,neurosurgery and orthopedic) .A total of 232 patients had risk factors , 72 .41% of the patients who received prophylaxis .A total of 1 513 patients did not have risk factors ,24 .59% of the patients who received prophylaxis ,the rate of prophylaxis to general surgery ,thoracic surgery and urology was higher than other departments (except for ICU ,and between thoracic surgery with orthopedics ) .In prevention of SU ,the usage rate of Lansoprazole was higher than that of Omeprazole and Pantoprazole .Preoperative prophylactic usage rate was 14 .44% .Excessive dosage was 0 .93% .Preven-tive medicine treatment longer accounted for 12 .78% .Conclusion The phenomenon such as non-risk factors ,inappropriate of medi-cation time and long course of treatment are existed ,when physicians use the PPI for SU prophylaxis in surgical patients receiving aseptic-contaminative and contaminative incision operation .Therefore ,treatment recommendations for SU prophylaxis are needed to restrict PPI use for justified indications .
8.Comparative Study on the Processing Technology of Psoralea corylifolia by Salt Frying and Salt Steaming

OBJECTIVE:To compare the contents of 2 index components in salt frying and salt steaming samples of Psoralea corylifolia,and select better processing technology. METHODS:Under the conditions that ratio of salt and medicinal materials was 1:50,4-5 times salt of adding water,brine run time was 2 h,the amounts of psoralen and isopsoralen in salt frying samples after 10,12,14 min of frying under 150 ℃,170 ℃,190 ℃ and in salt steaming samples after 1,1.5,2,2.5 h of steaming were re-spectively investigated,and the effects of 2 processing technology were compared. RESULTS:Under fixed condition,the contents of index components was the highest in salt frying samples by fried for 12 min under 150 ℃ and in salt steaming samples by steamed for 1 h;the content in salt steaming samples (1.49%) was higher than that in salt frying samples (1.29%)(P=0.011). CONCLUSIONS:According to the comparison of index components'contents in processing samples,salt steaming is superior to salt frying of P. corylifolia.
9.Alloantigen specific TCR transgenic CD8~+T cells require IL-2 to mediate allograft rejection

Objective To study the role of IL-2 in regulating allograft rejection mediated by alloantigen-specific CD8~+ T cell.Methods T cell proliferation in vivo at a single cell level was examined using the CFSE dilution assay. IL-2 expression by activated CD4~+ versus CD8~+ T cells was determined by intracellular cytokine staining. The ability of alloantigen-specific CD8~+T cells in mediating allograft rejection was studied using the islet transplantation model.Results CD8~+ T cells divided vigorously in vivo in the allogeneic hosts regardless the presence or absence of CD4~+ T cells. CD4~+ T cells, but not CD8~+ T cells, were the primary source of IL-2 when both subsets were present. However, CD8~+ T cells could express high levels of IL-2 in the complete absence of CD4~+ T cells. In 2C TCR transgenic (Tg) mice in which the 2C TCR transgene was selectively expressed on the CD8~+ T cells that specifically recognized alloantigen (Ld) of Balb/c origin, islet allografts from Balb/c mice was promptly rejected by the 2CTg recipients with mean survival time of only 8 days. In contrast, in 2CTg mice with a genetic deletion of the IL-2 gene (2CTg-IL-2KO mice), the alloantigen specific CD8~+ T cells failed to mediate the islet allograft rejection and all the Balb/c islets survived for more than 50 days.Conclusions CD8~+ T cells appear to be very plastic in producing and utilizing IL-2. In the presence or absence of CD4~+ T cells, CD8~+ T cells can use CD4~+ derived or self derived IL-2 for proliferation and effector function respectively. In an alloantigen specific TCR transgenic model, the effector function of CD8~+ T cells is strictly IL-2 dependent. Thus, in situations where graft rejection is mediated solely by the CD8~+T cells, blocking IL-2/IL-2R pathway may be critically important in preventing transplant rejection.
