2.Experience of Clinical Pharmacists Participating in the Therapy for an Ovarian Cancer Patient Complicated with Cirrhosis

Objective:To improve the safety,rationality and efficacy of medication for ovarian cancer patients complicated with cirrhosis by the participation of clinical pharmacists in the therapy. Methods:Clinical pharmacists participated in the therapy for an ovarian cancer patient with cirrhosis,and provided a rational and individualized therapeutic regimen through the drug experience of clinical pharmacists as well as the relevant medical guides and literatures. Results:The therapeutic efficacy was increased by the participation of clinical pharmacists in the therapy,the potential risk of the chemotherapy was avoided and the security of medication was assured. Conclusion:The participation of clinical pharmacists in therapeutic practice can improve the normalization of pharmacotherapy for ovarian cancer patients with cirrhosis,which also can provide ideas and methods for treating the similar patients.
3.Regional Diastolic Dysfunction of Ventricles in Hypertension Patients

LAVP2. Multiple linear regression revealed duration of hypertension, grade of hypertension, age and smoking status were independent determinants for the deterioration of diastolic dysfunction. Conclusion A progressing change in regional diastolic dysfunction from right to left ventricle was found in HT patients. The grade of hypertension and the duration of HT were the major determinants on diastolic dysfunction.
4.Application of multiplex amplification of STRs for monitoring survival of engraftment after bone marrow transplantation

Objective To evaluate the application of fluorescent multiplex amplification of short tandem repeat(STR)for monitoring survival of engraftment after bone marrow transplantation(BMT).Methods Three STR loci named D12S391,D18S865 and D20S161 in 56 cases were detected by fluorescent multiplex amplification.PCR products were separated and typed by DNA Sequencer.Results The genotypes of STR in 52 recipient after bone marrow transplantation were completely identical with those of the donors.In another 4 cases the evidences of mixed chimerism were observed.Conclusion The system of fluorescent multiplex amplification of STRs exhibited high capacity of discrimination and low cost.Its application in the detection of STR after BMT is reliable,sensitive and simple.Combined with the clinical manifestation it can be used to evaluate the effect of BMT.
5.Impact of increasing intra-abdominal pressure on digestive system during colorectal cancer patients under minimally invasive surgical procedure

Objective To investigate the impact of intraoperative intra-abdominal pressure increasing on digestive system.Methods A retrospective analysis of clinical data of 132 cases of colorectal cancer patients from January 2013 to June 2016 was made. Patients were divided into groups A, B and C according to the intra-abdominal pressure, 46 cases in each group. Group A: 46 patients, 10 mmHg (1 mmHg = 0.133 kPa), group B: 45 patients, 12 mmHg, group C: 41 patients, 15mmHg. Comparison of postoperative recovery time, complications, serum cytokine levels was made among the three groups.ResultsThe number of postoperative 6h nasogastric tube pulled out, bowel recovery time, ifrst lfatus or a bowel movement, tolerance semi-liquid diet, postoperative duration of diarrhea, postoperative hospitalization time among the three groups show no statistically difference (P > 0.05). The difference of acute gastric injury (AGI), chyle leakage, anastomotic fistula, surgical bleeding, intestinal paralysis, vomiting incidence of postoperative among the three groups has no statistically differences (P > 0.05). While postoperative IL-6 level and MAP were signiifcantly increased compare with preoperative level, the difference was statistically signiifcant (P < 0.05). Postoperative TNF-α show no statistical differences compare with preoperative (P > 0.05). Postoperative serum IL-6 levels in 25 patients with intestinal paralysis was (10.71 ± 4.37) ng/L, 107 cases had high serum IL-6 levels in patients did not occur intestinal paralysis was (10.66 ± 4.13) ng/L, the difference was not statistically significance (P > 0.05).Conclusion Intra-abdominal pressure increasing during minimally invasive surgical procedure has no signiifcant effect on the digestive system in colorectal cancer patients.
6.Effect of Yipi Rougan recipe comined chemotherapy on quality of life in intermediated or advanced-staged primary hepatic carcinoma and its efficacy

Objective To investigate the effect of Yipi Rougan recipe comined chemotherapy on quality of life in the treatment of patients with intermediated or advanced-staged primary hepatic carcinoma and its efficacy.Methods From April 2013 to December 2015 in our hospital, 97 patients with advanced primary liver cancer patients were selected, and randomly divided into traditional Chinese medicine group of 48 patients and chemotherapy group of 49 patients, two groups were treated with transcatheter arterial embolization chemotherapy, while the traditional Chinese medicine group were given Yipi Rougan recipe adjuvant therapy, 21 days for a course of treatment, with a total of four courses.Results There was no significant difference in total effective rate between traditional Chinese medicine group and chemotherapy group ( 83.33%vs.77.55%) .After treatment, the quality of life (QOL) score, Karnofsky performance score (KPS) in traditional Chinese medicine group were higher than those in chemotherapy group (P<0.05). The each symptom score in traditional Chinese medicine group was lower than that in chemotherapy group (P<0.05).Conclusion The efficacy of Yipi Rougan recipe combined with chemotherapy is not significant on patients with intermediated or advanced-staged primary liver cancer chemotherapy, but can significantly relieve symptoms and improve the quality of life of patients .
7.The Effect of Timely Infection Evaluation and Intervention on Low-body Mass Incubator Infants in Primary Hospitals

Objective] To discuss the effect of timely infection evaluation and intervention on reducing infection to low-body mass incubator infants of primary hospitals. [Method] Select 98 cases of low-body mass incubator infants of 1210~2000g as control group(routine prevention used), and another 95 cases of 1200~2010g as observation group(taking timely infection evaluation, giving pointing intervention to infants with various infective risk factors). Compare their infection and in-hospital period. [Result]In control group 98 cases, 45 cases(45.92%) had infection; in observation group 95 cases, 11 (11.58%) had infection; the infective rate of observation group was lower than control group, the difference had statistical meaning( P<0.05). [Conclusion] To take timely infection evaluation and preventive intervention can definitely reduce infection.
8.Influence on pubertal reproductive function in female rats by immune challenge in early life

Objective To investigate the long-term programming effects on pubertal reproductive function by immunological challenge in early life. Methods Female Sprague-Dawley rats were administered by endotoxin (lipopolysaccharide, LPS) at a dosage of 50 μg/kg and saline intraperitoneally on postnatal day 3 and 5. Body weight was measured weekly. Puberty onset ( vaginal opening) and oestrous cyclicity were monitored from postnatal day 30. At the age of 6 weeks, bilateral ovariectomy was performed. The histological and morphological change of the ovaries (the thickness of the theca interna and the number of different kinds of follicles) were observed and the immunoreactivity of the ovarian sympathetic nerve markers (low affinity receptor of nerve growth factor, p75NGFR) was evaluated by immune staining. Results Immunological challenge (exposed to LPS) in early life delayed vaginal opening significantly [LPS-treated (40.6 ±0.7) days versus controls ( 38. 6 ± 0. 5 ) days, P < 0. 05], decreased the percentage of normal oestrous cyclicity ( LPS-treated 26. 1% versus controls 66. 8% , P< 0. 05 ) , decreased the total number of different types of follicles (primordial follicles: LPS-treated 610 ±47 versus controls 1181 ±57, P < 0. 05; primary follicles: LPS-treated 624 ±41 versus controls 960 ± 30, P < 0. 05 ; preantral follicles: LPS-treated 183 ± 16 versus controls 260 ± 14, P < 0. 05; antral follicles: LPS-treated 32 ± 4 versus controls 79 ± 7, P < 0. 05) and increased the thickness of the theca interna [LPS-treated ( 15. 8 ±0. 4) μm versus controls (11.4 ±0. 3) μm, P < 0. 05]. The immunostaining of p75NGFR was obviously enhanced in the LPS-treated ovaries when compared with that of controls ( P < 0. 05 ) . Conclusions Immunological stress during early critical developmental windows could have long dysfunctional effects on the pubertal reproductive function. It delayed puberty onset, reduced the percentage of the normal oestrous cycles, decreased follicles reserve and increased the thickness of the theca interna which might involve the up-regulation of the local ovarian sympathetic nerve activity.
9.Effects of magnesium sulphate on the efficacy of postoperative patient-controlled intravenous analgesia with morphine

Objective To evaluate the effects of magnesium sulphate on the efficacy of postoperative patient-controlled intravenous analgesia (PCIA) with morphine. Methods Sixty ASA Ⅰ or Ⅱ patients of both sexes aged 45-60 yr weighing 48-70 kg scheduled for elective upper abdominal surgery were randomly allocated into 2 groups ( n = 30 each) : morphine group (group M) and magnesium sulphate-morphine group (group MS) . If the VAS score=3, PCIA was started. Patients in group M received morphine in a 0.015 mg/kg bolus dose. Patients in group MS received morphine 0.015 mg/kg and magnesium sulphate 0.9 mg/kg. The consumption of analgesic drugs was recorded at 4, 8, 16, 24 and 48 h after 1st attempt. The gastrointestinal function recovery time was recorded. Venous blood samples were taken before anesthesia induction and at the end of operation and analgesia to determine the serum magnesium and calcium ion concentrations. Results Compared with group M, the consumption of analgesic drugs was significantly decreased, and the gastrointestinal function recovery time after operation was significantly shortened in group MS (P < 0.05). The serum magnesium ion concentration in both groups was significantly lower at the end of operation and analgesia than before anesthesia induction ( P < 0.05) . The serum magnesium ion concentration in group MS was significantly higher at the end of analgesia than at the end of operation ( P <0.05) .There was no significant difference in the serum calcium ion concentration between M and MS groups. Conclusion Magnesium sulphate can ameliorate the efficacy of postoperative PCIA with morphine.
10.Health care professionals′attitude of patients involvement in patient safety and its influencing ;factors

Objective To investigate health care professionals′attitude of patient involvement in patient safety and analyze its influencing factors. Methods A convenience sample of 946 health care professionals recruited from twelve hospitals in Shanghai was employed for a cross-sectional survey. Results The general attitudes of patient involvement in patient safety scored 3.86 ±0.56, the perception of the content of patient involvement scored 3.97±0.54, the average score of caring involvement (4.12±0.59) was larger than the average score of decisive involvement (3.88±0.58), the average score of complaining involvement was the least (3.39±0.81), the difference had statistical significance (F=583.797, P<0.01). The regression analysis showed that identity, professional title, hospital grade influenced health care professionals′general attitude of patient involvement in patient safety and that identity, hospital grade influenced health care professionals′attitude of the content of patient involvement. Conclusions In health care professionals′opinion, patient involvement in patient safety has both advantages and disadvantages. They preferred to caring involvement, followed by decisive involvement, then complaining involvement. So, hospital managers should view patient involvement in patient safety discriminately and construct rational involvement model ofpatients willing, medical support, promote interaction and harmony build.
