1.Nursing of 72 patients with internal iliac artery coverage during endovascular abdominal aortic aneurysm repair

Coverage of internal iliac artery during endovascular abdominal aortic aneurysm repair may cause ischemic complications in buttock,intestine,lower extremity and genital.We reported nursing care of 72 patients in our center to investigate the key points of nursing.Our experience were as follows:individualized blood pressure management and doctor-nurse cooperation in pain management were key points in preoperative nursing;close postoperative observation was focused on ischemic colitis,buttock claudication and ischemic buttock skin;cooperation with ostomy care group improved pertinency of nursing care;rounding by integration of doctors and nurses obtained high satisfaction and improved patients' compliance.
2.EFFECTS OF ANGIOTENSIN Ⅱ ON PAI-1 GENE EXPRESSION IN HUMAN GLOMERULAR MESANGIAL CELLS AND ITS MECHANISM

To explore the effects of angiotensin Ⅱ on plasminogen activator inhibitor 1 (PAI 1) gene expression in human glomerular mesangial cells.Glomerular mesangial cells (GMCs) were cultured from a healthy adult human kidney (unsuitable for renal transplantation) and then treated with angiotensin Ⅱ at concentration of 10 -9 ,10 -8 , 10 -7 and 10 -6 mol/L, respectively. The mRNA expressions of angiotensinⅡAT1 and AT2 receptor, and PAI I were examined with RT PCR and Northern blot analysis, respectively. Plasminogen activator activity in the supernatants of the GMCs was measured with fibrin plate method. After the GMCs were stimulated by angiotensin Ⅱ, the mRNA level of the AT1 receptor was markedly enhanced, but the AT2 receptor mRNA level was still undetectable as that in normal condition. When GMCs were treated with angiotensin Ⅱ for 48 hours, PAI 1 mRNA expression was increased in a dose dependent manner. The plasminogen activator activity was significantly reduced in the supernatants of the angiotensinⅡ treated GMCs. the results suggested that angiotensinⅡ promote PAI 1 gene expression of the GMCs possibly through its AT1 receptor.
3.Evaluation of different methods in eliminating Mycoplasma contamination in cell culture of human hepatoma-derived cell line C3A

Objective To evaluate the effectiveness of several methods in eliminating Mycoplasma contamination in cell culture of human hepatoma-derived cell line C3A. Methods PCR was performed to detect the Mycoplasmas contamination in cell cultures. The contaminated samples were treated by ciprofloxa-cin, heating, Plasmocure or co-culturing with macrophages. Transmission electron microscope ( TEM) and Q-PCR were used to comparatively analyze the cell morphology and gene expression before and after Plas-mocure treatment. Results Plasmocure succeeded in eliminating Mycoplasma contamination, while cipro-floxacin showed temporary efficacy. Heating and co-culturing with macrophages failed to eliminate Mycoplas-ma contamination. No Mycoplasma contamination in the Plasmocure-treated group was observed under TEM and the expression of ALB, TF and CYP3A4 genes were higher than the genes expressed in the contaminated group (P<0. 01). Conclusion Plasmocure treatment was effective in eliminating Mycoplasma contamina-tion in cell culture. Moreover, the cell morphology and gene expression in Plasmocure-treated group were re-stored to normal.

Objective To investigate the association between the trace elements and senile dementia.Methods The trace elements including zinc,iron,copper,manganese,and lead elements and calcium in the hair of the elderly patients with Alzheimer' s disease(AD)admitted in our hospital from January,2006 to September,2008 were analyzed,and the results were compared with those from 46 control subjects.Results Zinc,calcium and manganese levels in the AD group were significantly lower than those in the control group(P<0.05).Conclusion Lowered contents of the trace elements zinc,calcium and manganese might be associated with the pathology of AD.
7.Enteral versus parenteral nutrition after gastrointestinal surgery: a meta-analysis of randomized controlled trials.

OBJECTIVETo compare the different prognosis between enteral nutrition (EN) and parenteral nutrition (PN) in patients after gastrointestinal surgery (GIS), and to investigate a reasonable regimen of enteral nutrition (EN) after GIS.
METHODSRandomized controlled trials (RCTs) on EN/PN after GIS from 1970 to 2008 retrieved from the data bank of Pubmed, EMBASE and Cochrane Library were analyzed. Evaluation endpoints were anastomotic dehiscence, infection (catheter sepsis, wound infection, pneumonia, intra-abdominal abscess and urinary tract infection), vomiting and abdominal distention, other complications, length of hospital stay and mortality rate.
RESULTSTwenty-three RCTs including 2784 patients met the entering criteria. Compared with PN, EN was beneficial in the reduction of anastomotic dehiscence (RR = 0.67, 95%CI: 0.50 - 0.91; P = 0.010), infections (RR = 0.72, 95% CI: 0.64 - 0.81; P < 0.001), other complication (RR = 0.82, 95%CI: 0.73 - 0.92; P < 0.001) and duration of hospital stay (weighted mean difference: -3.60; 95%CI: -3.88 - -3.32; P < 0.001). But the risk of vomiting was increased among patients with EN (RR = 1.39, 95%CI: 1.21 - 1.59; P < 0.001), and there was no significant differences in mortalities between the two groups (P = 0.400).
CONCLUSIONSThere is no advantage in treating patients 'nil by mouth' after gastrointestinal surgery. It indicated that early commencement of enteral feeding is beneficial.

8.The clinical outcome and complications of Ex-PRESS glaucoma shunt implantation

Objective To report operative course and relative complications in a group of patients who underwent Excessive Pressure Regulation System (Ex-PRESS) glaucoma shunt implantation in the real world,and discuss the ralated factors and possible interventions in clinical practice.Methods Preferred Ex-PRESS glaucoma shunt implantation were performed on 11 eyes with deverse types of glaucoma.Operative course and relative complications were analyzed from recorded tapes and documents.The correlations between complications,types of glaucoma,and previous procedures were analyzed,the complications were appropriately managed.Results 11 patients with diverse types of glaucoma who underwent Ex-PRESS glaucoma shunt implantation were recruited for the study.Among them,2 patients had combined phacoemulcification surgery,1 patient had repeat shunt implantation.During the procedure,1 patient had dislocated shunt into anterior chamber;1 patient had shunt entirely inserted into anterior chamber with plunger.4 patients had hypotony and flat anterior chamber,2 of them had hyphema,2 experienced subchoroid detachment.1 patient had intraocular lens pupil clamping after combined cataract and glaucoma surgery;4 cases had flat filtering blebs and uncontrolled intraocular pressure during the follow-up period,2 of them had blocked drainage and proliferative anterior synechia around the head,which was found in the patients with primary angle-closure glaucoma who had previous operation history.The common long-term complication was external ostium occlusion (4/11).The complications of drainage are related to the type of glaucoma and the history of operation.Conclusions Other than the complications which present commonly in trabeculectomy procedure,there were some complications related to the Ex-PRESS glaucoma shunt.History of ocular surgery,surgical techniques and the type of disease were associated with the incidence of complications.
9.The SARS-CoV 3a and 7a Protein May Enhance the Induction of IFN-?

3a and 7a are nonstructural proteins of SARSCoV, which are encoded separately by ORF 3a and ORF 7a in SARSCoV genome. The expression of 3a has been founded in cells infected by virus in vivo or in vitro. Firstly, the pGL3Control vector was reconstructed , the pGL3Enhancer vector deletious of SV40 promoter gene was obtained . Then the IFN? promoter gene was cloned into the pGL3Enhancer vector and pGLIP21, the Luciferase reporter plasmid with IFN? promoter was established. The availability of pGLIP21 was verified by NDV ,the inductor of IFN?, the Luciferase activity was assayed in cells transfected with pGLIP21 by Luminometer. In order to see the function of 3a and 7a protein of SARSCoV,CHO cells expressing 3a or 7a protein were transfected with pGLIP21, the intensity of luciferase activity was analyzed . By analysis, in vitro, 3a and 7a protein of SARSCoV had the similar ability in triggering the expression of Luceferase gene, i.e 3a and 7a protein of SARSCoV could effectively activate the promoter fragment of IFN? gene. This result will help studying the function of 3a and 7a protein and provide a method to study the nosogenesis mechanism of SARSCoV.
10.Relationship between gene polymorphisms of thromboxane A2 receptor and cerebral infarction

Objective To investigate the relationship between the gene polymorphisms of thromboxane A2 receptor (TXA2R) and cerebral infarction. Methods A genetic association study of one single nucleotide polymorphism (rs768963) in the human TXA2R gene was performed in 334 patients with cerebral infarction and 135 healthy controls by polymerase chain reaction-amplification and restriction fragment length polymorphism(PCR-RFLP) method. Results The levels of blood pressure,blood-fat and serum glucose were independent risk factors for cerebral infarction. No significant differences in the overall distribution of genotypes (T/T, T/C and C/C) were noted (P>0.05); however,significant differences in T or C gene frequency of rs768963 of TXA 2R gene between patients with cerebral infarction and noninfarction controls were found (P<0.05). Logistic regression analysis showed that no association between the mutant of rs768963 of TXA 2R gene and such factors as gender, age and levels of blood fat, blood pressure and serum glucose was noted. Conclusion The rs768963 of TXA 2R gene in human thromboxane A2 receptor may be a risk factor for cerebral infarction and patients carried C allele are much likely to have cerebral infarction.
