1.High intensity focused ultrasound is treatment of 14 hepatic metastases from colorectal cancer

Objective To evaluate the clinical safety and efficacy of HIFU treatment of hepatic metastases from colorectal cancer.Methods 20 tumor lesions in 14 patients with hepatic metastases from colonectal cancer were treated with HIFU therapy.The vital signs,main organs'functions,examination of MRI and complications were observed.Results Vital signs of all the 14 patients were stable.The function of liver and kidney were normal and life quality were improved with elevation of Karnofsky in 5 patients.On MRI examination,the MRI signals of treating area of 20 tumor lesion in 14 patients decreased remarkably with disappearance of blood supply and a thin layer of edematous intensified zone was detected on the margin of lesion.6 patients were found to have cord-like burning scar on skin.Conclusion HIFU treatment is safe and effective in treatment of hepatic metastases from colorectal cancer.

The chemical constituents of Poria cocos were studied by means of silica gel, ODS column chromatography, Sephadex LH-20 and preparative HPLC. Thirteen compounds were isolated from this plant. By analysis of the ESI-MS and NMR data, the structures of these compounds were determined as tumulosic acid (1), dehydrotumulosic acid (2), 3beta, 5alpha-dihydroxy-ergosta-7, 22-dien-6-one (3), 3beta, 5alpha, 9alpha-trihydroxy-ergosta-7, 22-diene -6-one (4), ergosta-7, 22-diene-3-one (5), 6, 9-epoxy-ergosta-7,22-diene-3-ol (6), ergosta-4,22-diene-3-one (7), 3beta, 5alpha, 6beta-trihydroxyl-ergosta-7,22-diene (8), ergosta-5, 6-epoxy-7,22-dien-3-ol (9), beta-sitosterol (10), ribitol (11), mannitol (12), and oleanic acid 3-O-acetate (13), respectively. Compounds 3-13 were isolated from the P. cocos for the first time.

6.Differentiation of adult mouse mesenchymal stem cells into hepatocytes cultured in a conditioned culture medium of injured hepatocytes.

OBJECTIVETo establish a method through which murine bone marrow mesenchymal stem cells (MSCs) can be induced into hepatocytes in vitro.
METHODSA conditioned medium of injured hepatocytes (with CCl4 in vivo) was used to culture the isolated MSCs. The differentiated cells were identified by morphological observation, reverse transcription polymerase chain reaction (RT-PCR), immunofluorescence assay (for AFP, Albumin, and CK18) and periodic acid schiff reaction (PAS) for glycogen.
RESULTSThe differentiated cells showed characteristics of hepatocytes. PT-PCR detected AFP mRNA on day 5 and it increased gradually until day 15, and then decreased; CK18 mRNA was detected on day 10; TAT was detected on day 20. Immunofluorescence assay for AFP, albumin and CK18 showed positive staining reactions on day 20. PAS positive glycogen granules appeared in the cytoplasm of the differentiated cells.
CONCLUSIONMSCs of adult mice cultured in a conditioned medium of injured hepatocytes can differentiate into hepatocytes. This method can be used in further studying of the mechanism of transdifferentiation of MSCs into hepatocytes.

7.Study on the prevalence and risk factors of depressive symptoms among ‘empty-nest' and non-empty-nest' elderly in four provinces and cities in China

Objective To investigate the prevalence and determinants of depressive symptoms among ‘empty-nest' and ‘non-empty-nest' elderly in four cities/provinces.Methods 4265 elderly aged 60 and over,were recruited with cluster sampling method in Shanghai,Heilongjiang,Guangdong and Shanxi province and interviewed,using the Geriatric Mental State Schedule and self-developed related questionnaire.Results ( 1 ) The prevalence of depressive symptoms for ‘empty-nest' elderly was (8.18%),significantly higher than that for ‘non-empty-nest' eldcrly (P=0.019) ; (2) the ‘empty-nest' elderly had a significantly higher proportion of the following factors:being male,married,with higher income ( ≥ 15 000 Yuan/year),living in city,with high education background,under employment etc.than the ‘non-empty-nest' elderly (P<0.0001) ; (3)the ‘empty-nest' elderly had significantly higher proportions on good self-rated health status and life (P=0.0001,P<0.0001 ) as well as heavier health problems and economic difficulties (P=0.00 1,P=0.002 ) ; (4) there were significantly negative associations between depressive symptoms and the following 10 factors:being female,single,having bad self-rated health and life status,having somatic disease ≥3,with big health problems in the last two years and loss of dearest persons,community engagement and involvement of religious activities.Conclusion The ‘empty-nest' elderly showed higher prevalence of having depressive symptoms than the ‘non-empty-nest' elderly.The ‘empty-nest' elderly had characteristics as being single,female,having adverse event etc.and should be under greater attention for care.
8.A multivariate analysis of prognostic determinants for stages II and III colorectal cancer in 141 patients.

BACKGROUNDPrevious prognosis analyses of colorectal cancer (CRC) patients with stage II and III disease were done as separate categories. The purpose of this study was to analyze prognostic factors associated with survival in a group of patients who underwent radical resection of stages II and III CRC.
METHODSA retrospective review was performed for 141 consecutive stages II and III patients who had undergone radical resection of colorectal adenocarcinoma between May 2003 and November 2003. Univariate and multivariate analyses were performed to assess the effect of record variables on disease free survival and overall survival.
RESULTSThe median follow-up time was 59 months, and the 3- and 5-year survival rates were 76% and 68%, respectively. Four factors were independently associated with a worse disease-free survival: diabetes (hazard ratio (HR) 2.338; 95% confidence interval (CI) 1.011 - 5.407), expression of cyclooxygenase-2 (Cox-2) (HR 0.335; 95%CI 0.126 - 0.888), expression of matrix metalloproteinases 2 (MMP-2) (HR 0.233; 95%CI 0.101 - 0.541), expression of vascular endothelial growth factor (VEGF) (HR 0.295; 95%CI 0.088 - 0.996). Four factors were independently associated with a worse overall survival: lymph nodes metastasis (HR 1.67; 95%CI 1.29 - 2.14), Cox-2 positive (HR 0.056; 95%CI 0.247 - 0.731), MMP-2 positive (HR 0.398; 95%CI 0.190 - 0.836), VEGF (HR 0.364; 95%CI 0.090 - 0.716).
CONCLUSIONSDiabetes, expression of Cox-2, MMP-2 and VEGF were independently associated with a worse disease- free survival. Lymph nodes metastasis, expression of Cox-2, MMP-2 and high level of VEGF predicted a poor overall survival.

9.Association of p53 codon 72 polymorphism with liver metastases of colorectal cancers positive for p53 overexpression.

OBJECTIVETo evaluate the association between p53 codon 72 polymorphism (R72P) and the risk of colorectal liver metastases.
METHODSThe p53 R72P genotype was identified by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method in 78 consecutive colorectal cancer patients with liver metastases and 214 age- and sex-matched cases with nonmetastatic colorectal cancer.
RESULTSThe R allele of the p53 R72P polymorphism was more frequently found in metastatic cases than in nonmetastatic cases (P=0.075). Carriers of the 72R allele had a 2.25-fold (95% CI (confidence interval)=1.05 to approximately 4.83) increased risk of liver metastases. On the stratification analysis, 72R-carrying genotype conferred a 3.46-fold (95% CI=1.02 to approximately 11.72) and a 1.05-fold (95% CI=0.36 to approximately 3.08) increased risk of liver metastases for p53 overexpression-positive and negative colorectal cancers, respectively.
CONCLUSIONThese results demonstrate for the first time that the 72R allele of the p53 polymorphism has an increased risk for liver metastases in colorectal cancers positive for p53 overexpression.

10.Neuroelectrophysiological characteristics of peripheral neuropathy in primary Sj?gren's syndrome:study protocol for a prospective case series and preliminary results

BACKGROUND: Sj?gren's syndrome (SS) is a chronic progressive autoimmune disease. The incidence of peripheral nervous system damage in patients with primary Sj?gren's syndrome (pSS) is 10%-30%. Previous studies have shown that there are multiple electrophysiological manifestations in patients with pSS presenting with peripheral neuropathy. However, there is no consensus on its neuroelectrophysiological manifestations. Peripheral neuropathy associated with pSS is easily confused with peripheral neuropathy caused by other etiologies. OBJECTIVE: To observe the neuroelectrophysiological manifestations of peripheral neuropathy associated with pSS to assist in the diagnosis of the disease. METHODS: A total of 100 pSS patients with peripheral neuropathy who receive treatment in the Department of Neurology, First Affiliated Hospital of Kunming Medical University, in China will be included in this study. Fifty-two patients included in the preliminary experiment presented with peripheral neuropathy associated with pSS. The primary outcome measure is the rate of abnormal motor nerve conduction velocity. The secondary outcome measures include the rate of abnormal terminal motor latency, the rate of abnormal compound muscle action potential amplitude, the rate of sensory nerve conduction velocity, the rate of abnormal sensory nerve action potential amplitude, the rate of abnormal F wave, and the rate of abnormal sympathetic skin response. RESULTS AND CONCLUSION: Results of 52 patients included in the preliminary study showed that the rate of each electrophysiological index was similar between upper and lower extremities; the rate of abnormal motor nerve conduction velocity was significantly higher than the rate of abnormal compound muscle action potential amplitude; the rate of sensory nerve conduction velocity was significantly higher than the rate of abnormal sensory nerve action potential amplitude; the rate of abnormal motor nerve conduction velocity was similar to the rate of abnormal sensory nerve conduction velocity; the rate of abnormal compound muscle action potential amplitude was similar to the rate of abnormal sensory nerve action potential amplitude; the rate of abnormal wave was significantly lower than the rate of abnormal motor nerve conduction velocity; the rate of abnormal sympathetic skin response was similar to the rate of abnormal motor nerve conduction velocity. Results from this study will reveal neuroelectrophysiological abnormality in peripheral neuropathy associated with pSS, which will help diagnose the disease.
