1.Features and nursing strategies for patients with nosocomial infection in ICU

Objective To analyze the clinical characteristics and nursing measures of infection in hospital ICU,and to provide the refer-ence for clinical nursing. Methods From January 2010 to June 2013,116 patients with nosocomial infection in surgical ICU were collected as observation group,and 72 patients with nosocomial infection in the surgery were as the control group. The risk factors of nosocomial infec-tion were retrospectively analyzed,and the problems related to nursing were discussed. Results The nosocomial infection rate was 8. 44% in ICU,which was significantly higher than the surgical infection rate(4. 43%) in general ward (P<0. 05). Whether patients in ICU or surgery ward,respiratory and urinary tract infections were the most common site of nosocomial infection. The mortality rate in ICU was 9. 48%,which was higher than that of nosocomial infection in surgical ward (4. 17%),P<0. 05. The nosocomial infections were obviously related to the age of patients,duration of hospitalization,the invasive operation,antibiotic and immunosuppressive agents and disturbance of consciousness in ICU and general wards (P<0. 01). Conclusion The ICU nosocomial infection rate was significantly higher than that in general ward,the main infection sites are respiratory tract and urinary tract,with a variety of factors,the key is to establish the mechanism of prevention and ef-fective nursing strategy.
2.Isolation and Identification of One Marine Actinomycete Strain Exhibiting Antitumor Activity

The fermentation broth of one actinomycete strain ACMA006 strongly inhibited growth of many tumor cells and some microorganisms, but its cytotoxicity to human normal cells were weak. Strain ACMA006 grow well on most tested media, producing exuberant vegetative hyphae and aerial hyphae. Its optimization temperature is 28?C. Phyloge-netic analysis based on 16S rDNA sequence showed that strain ACMA006 was closely related to one of the genus Streptomycetes (S.cavourensis subsp. washingtonensis) with 16S rDNA sequence similarity values of 100%, but had many differences in other features including its morphology, physiological and biochemical characteristics. The pre-liminary study supported the view that the strain ACMA006 represented a new strain of the S.cavourensis subsp. wash-ingtonensis.
3.Effects of mindfulness - based stress reduction on compliance in primary open angle glaucoma patients with topical medication

AIM: To explore the effects of mindfulness- based stress reduction ( MBSR ) on anxiety, compliance and visual function in primary open angle glaucoma( POAG) patients. ·METHODS:Totally 64 patients, who were treated with topical medication and were diagnosed with POAG in the department of ophthalmology at Shaanxi Provincial People's Hospital, were randomly divided into the intervention group ( n=32 ) and the control group ( n=32 ) . Both groups received conventional mental health education while the intervention group was complemented with MBSR intervention. The patient's anxiety status, compliance rates and MD before and after the intervention were evaluated. ·RESULTS:After the MBSR intervention, the scores of HAMA of the intervention group were significantly lower than that of the control group (P<0. 01). The compliance of patients in intervention group was higher than that of the control group (P<0. 01). ·CONCLUSION:The MBSR intervention can effectively relieve the anxiety of patients and improve the compliance in primary open angle glaucoma patients. It can be promoted and put into clinical application.
4.Endoscropic assist in rat orotracheal intubation

Objective To investigate endoscropic assist in rat orotracheal intubation.Methods 40 rats were randomly divided into two groups.The procedure time, total time, intubation frequency,the success rate of the first intubation and the survival rate in 24h were compared between the conventional group and the endoscropic group.Results The total time in two groups were nearly the same.But the procedure time and frequency of intubation were significantly less in the endoscropic group than those in the conventional group.The success rate of the first intubation in the endoscropic group was obviously higher than that of the conventional group.There was no obviously difference between the survival rate after extubation in 24h between groups.Conclusion Endoscropic assist is superior to the conventional method and worth promoting.

During the last decade a lot of new learning about asthma was arisen along with the development of modern immunology and molecular biology,and asthma had been one hotspot in the studies of molecular biological and administer medical field.One of the new viewpoints was that airway neurogenic inflammation took important role in asthma development.For years scholars had suspected that airborne pollutants had an synergistic effect on the attack of asthma and could cause asthma attack,but they did not know the molecular mechanism.In 1991 Denish scholar Nielsen GD advanced a new hypothesis that airway irritation was mediated by capsaicin receptor.The pathological process included a molecular chain of message transfer as air pollutants_capsaicin receptor_Ca2+_substance P_neurokinin receptor.Airway neurogenic inflammation was the result of activation of the receptors.In 1997 American scholar Caterina MJ and his colleagues successfully obtained the cloned capsaicin receptor in expressing cells,this will make Nielsen GD's hypothesis living.
6.Effect of Aluminium on Concentration of Intracellular Ca~(2+) in Hippocampus

Objective To explore the effect of aluminium on the concentration of intracellular Ca2+ in the hippocampus neurons of postnatal rat. Methods The hippocampus neurons of postnatal rats were primarily cultured and treated with Al3+ at doses of 0,10,100 and 1 000 ?mol/L. Fura-2-AM calcium ions fluorescence indicator was used to determine the concentration of the intracellular Ca2+ when they were cultured for 15 min,30 min and 45 min. Results The concentration of intracellular Ca2+ in the hippocampus neurons increased with the aluminum concentration increased and as the concentration of the aluminium was 1 000 ?mol/L,at the three cultured time points,the concentration of intracellular Ca2+ were (276.2?53.4) nmol/L,(306.5?73.7) nmol/L and (408.8?79.7) nmol/L respectively,significantly higher than the control group(P
7.What is significance of subtemporal decompression in the traumatic total anterior circulation infarcts

Objective To explore clinical significance of subtemporal decompression in traumatic total anterior circulation infarcts(TTACI).Methods 24 TTACI cases accepted subtemporal decompression were analyzed including CT scan feature, pre operative observation to temporal superficial artery, operative cranial pressure classification and blood circulation in cortical small artery.Results Clinical symptom compared between before and after operation: good 4 cases, unchanged 14 cases, dead 6 cases. CT scan showed infarct areas became smaller in 4 cases, unchanged in 9 cases, larger in 2 cases three months after operation.Conclusion Subtemporal decompression cannot reduce the areas of infarcts and mortality, nor improve neurological function and promote quality of survivor.
8.Progress in immunotherapy for uveal melanoma

Once uveal melanoma (UM) has distant metastasis, the median survival time of the patient is less than 12 months. There is currently a lack of standard treatment for metastatic UM. In recent years, immunotherapy is splendid in the field of oncology. Immune checkpoint therapy, cancer vaccine therapy and T cell adoptive therapy have been applied to UM therapy. However, most of the clinical effects are limited and the survival benefit is not high. The recent early research results of the new immunotherapeutic drug IMCgp100 are encouraging.
9.Relationship between the rehabilitative intervention of ischemic cerebrovascular diseases and the functional alteration of monomer polymerization of fibrinogen

BACKGROUND: The elevation of plasma fibrinogen(Fbg) is a key risk factor of cerebrovascular diseases. The evaluation of the monomer polymerization function of fibrin has even more important clinical merit than the detection of Fbg level.OBJECTIVE: To investigate the clinical significance of the monomer polymerization function of fibrin in patients with isehemie cerebrovascular diseases and its impacts on rehabilitative intervention.DESIGN: A case control study employing patients and healthy individual as subjects.SETTING: An Institute of Hematology and Department of Neurology of one university.PARTICIPANTS: Totally 110 patients with different ischemic cerebrovascular disease selected from the Department of Neurology, Union Hospital of Tongji Medical College, Huazhong Science and Technology University from September 2001 to March 2002, and 50 healthy individuals were included in the study.METHODS: Rehabilitative intervention was performed in 31 randomly selected cerebral infarct patients, and the parameters indicating the monomer polymerization functions of fibrin in the plasma were detected by the measurement system for the monomer polymerization function of fibrin.MAIN OUTCOME MEASURES: Abnormal condition of monomer polymerization function of fibrin in each parameter.RESULTS: Each parameter indicating the monomer polymerization functions of fibrin in plasma was significantly increased in ischemic cerebrovascular diseases patients than healthy individuals( P < 0.01 ) . The abnormal rate of Fbg leveland fibrin monomer polymerization velocity (FMPV) was significantly elevated in ischemic cerebrovascular disease patients than healthy individuals ( P < 0. 01 ) . The relative risk(RR) of ischemic cerebrovascular diseases in patients with abnormal FMP functions was 4 to 31 times more than healthy control group. In cerebral infarct group, FMPV of anterior circulation infarct subgroup was significantly elevated than that of posterior circulation infarct and lacunar cerebral infarct subgroups( P < 0.05). The FMP function of anterior cerebral infarct patients was significantly higher than that of healthy group before rehabilitative intervention. Although each FMP parameter reduced after rehabilitative intervention, the difference between was not significant compared with that of before therapy.CONCLUSION: FMP function analysis can completely and objectively reflect the coagulation status of the patients with ischemic cerebrovascular diseases, and it can also reflect the range and severity of infarct to some extent. Although common rehabilitative intervention cannot effectively improve the high-coagulation of the blood, the impacts of specific rehabilitative intervention on the coagulation mechanism deserve further investigation.
10.Effect of exogenous Smad7 gene transfected hepatic stellate cells on mRNA expression of transforming growth factor beta 1, collagen Ⅰ and collagen Ⅲ

BACKGROUND: Smad7 is a major repressible protein in transforming growth factor β(TGF-β1) signal transduction pathway,which possess antifibrotic effects.OBJECTIVE: To construct rat Smad7 eukaryotic vector and to observe the mRNA expression level of TGF-β1, collagen Ⅰ and collagen Ⅲ in rat hepatic stellate cells (HSC)-T6 cell.DESIGN, TIME AND SETTING: The gene recombination and cytology observation experiment was performed at the First Affiliated Hospital of Shihezi University School of Medicine.MATERIALS: pcDNA3.1 (+) plasmid was reserved in the laboratory. E coil DH5α was presented by Key Laboratory of Xinjiang Endemic and Ethnic Diseases, Shihezi University School of Medicine. The HSC T6 cell was provided by Cancer Institute and Hospital, Chinese Academy of Medical Sciences.METHODS: Rat Smad7 cDNA was cloned into eukaryotic plasmid pcDNA3.1(+) to construct Smad7/pcDNA3.1(+) plasmid and transfected it into HSC-T6 ceils by Lipofectmine 2000. The experiment was divided into normal control, empty vector and Smad7 transfected groups, and the positive cells were selected by G418.MAIN OUTCOME MEASURES: The levels of Smad7, TGF-β1, collagen Ⅰ and Ⅲ mRNA was detected by reverse transcriptase polymerase chain reaction, respectively.RESULTS: Smad7 eukaryotic vector was successfully constructed and confirmed by endonuilease digestion and sequencing. Compared to the control and empty vector groups, Smad7 mRNA expression was significantly higher in Smad7 transfected group (P < 0.01 ); and TGF-β1 and collagen Ⅰ mRNA expression was notably reduced (P < 0.01). There was no statistically significant difference of the change of collagen Ⅲ mRNA expression among the three groups (P>0.05). The difference of Smad7, TGF-β1,collagen Ⅰ and Ⅲ mRNA expression had no statistically significant between control and empty vector groups (P_(all) > 0.05)CONCLUSION: Smad7 eukaryotic expression vector is successfully constructed. The Smad7 gene can effectively expressed in transfected HSC-T6 cell, and decrease mRNA expressions of TGF-β1 and collagen Ⅰ.
