1.Immunopharmacological actions of capsule bushenyanshou in mice

Aim The effect of capsule bushenyanshou (BSYS), a compound of traditional Chinesemedicine, on the immunopharmacological activities of mice was investigated. MethodsThe indexes of immunopharmacological activity, such as the clearance rate of charcoalparticles, the lymphocyte transformation and the content of serum hemolysin, were mea-sured. Results Capsule BSYS (400, 800 mg?kg-1, qd ? 12) markedly increased theclearance rate of iv charcoal particles and 1he lymphocyte transformation stimulated invivo by PHA in mice. In hydrocortisone -treated mice(15 mg? kg-1, sc, qd ? 5), capsuleBSYS significantly enhanced the content of serum hemolysin and the weights of spleenand thymus. The results also demonstrated capsule BSYS performed a sighted inhibition ofthe delayed type hypersensitivity in mice. Conclusion Capsule BSYS has the capacity ofimmunological intensification and regulation.

3.Comparison of Cellular Immunity Raised by a Novel Adjuvant DC-Chol with Different HBsAg Preparations

Objective: To screen candidate antigen for therapeutic HBV vaccine with a novel adjuvant DC-Chol. Methods: BALB/c mice were injected with DC-Chol liposome and HBsAg prepared from CHO and Yeast respectively. One week later, IL-4, IL-2, IFN-?were measured by ELISA or ELISPOT. Results: The levels of IL-2, IFN-?of HBsAg from Yeast with DC-Chol liposome were 20 and 119 times higher respectively than those of HBsAg from CHO with DC-Chol liposome. ELISPOT assay showed that the counts of spot-forming cells of IL-4 and IFN-?of HBsAg from Yeast with DC-Chol liposome were 2.8 and 46.3 times higher respectively than those of HBsAg from Yeast with Al(OH)3. Conclusion: HBsAg prepared from Yeast together with DC-Chol liposome may be an appropriate candidate for therapeutic HBV vaccine .
4.Dynamic observation of macular thickness after cataract operation by optical coherence tomography

AIM:To observe the changes of macular thickness in patients after cataract operation.
METHODS: A total of 126 patients ( 133 eyes ) were divided into two groups that were included in this study. The group (68 eyes of 64 case) with phacoemulsification combined intraocular lens implantation and ( 65 eyes of 62 cases ) with manual small incision cataract surgery ( MSICS) . There was no complication before and during the surgery. Optical coherence tomography ( OCT) was examined 1, 3mo before and after surgery. The visual acuity and macular thickness were compared after surgery. The SPSS 17. 0 software was used for statistical analysis. The paired t-test was used before and after surgery. Independent-samples t test was used in two groups before and after surgery,α=0. 05.
RESULTS: The cataract phacoemulsification group:preoperative and 1mo after surgery, the mean macular thickness was 241. 3±10. 9μm and 279. 7±16. 5μm, with significant difference ( P < 0. 01 ). The mean macular thickness was 245. 6±12. 6μm 3mo after surgery without statistical difference compared with preoperative ( P>0-05). The mean macular thickness was 279. 7±16. 5μm 1mo after surgery and 245. 6±12. 6μm 3mo after surgery with statistical difference(P<0. 05). Three eyes displayed cystoid macular edema at 1mo after surgery and 2 eyes of them was resolved 3mo after surgery. The group of MSICS: preoperative and 1mo after surgery, the mean macular thickness was 240-5±11. 9μm and 280. 7±16. 8μm, with significant difference (P<0. 01). The mean macular thickness was 246. 6±13. 2μm 3mo after surgery without statistical difference compared with preoperative ( P >0-05). One month after surgery and 3mo after surgery, the mean macular thickness were 280. 7±16. 8μm and 246-6±13. 2μm, with statistical difference (P<0. 05). Two eyes displayed cystoid macular edema at 1mo after surgery and 2 eyes were resolved 3mo after surgery. Between the two groups pre-operation or after operation for 1, 3mo respectively P values were without statistical difference (P>0. 05).
CONCLUSION: The macular thickness and macular edema can be found 1mo after cataract phacoemulsification and MSICS. Three months post-operatively, macular edema disappears and does not show statistical difference compared with preoperative. The increase of macular thickness has no obvious relationship with the choice of surgery.
5.Early persistent vacuum suction drainage in patients with high risk of pancreatic fistula after pancreaticoduodenectomy

Objective To study the function of early persistent vacuum suction drainage in patients with high risk of pancreatic fistula after pancreaticoduodenectomy (PD).Methods From Jul.2010 to Jun.2013,the clinical data of 286 patients undergoing PD were retrospectively evaluated.87 patients with high risk of pancreatic fistula were screened and then divided into early persistent vacuum suction drainage group (the observation group) and conventional drainage group (the control group) according to postoperative drainage manners.We statistically analyzed the two groups in terms of general information,blood loss,operative time,medical expenses,hospital stay,mortality and morbidity of complications such as pancreatic fistula.Results There were 40 patients screened into the observation group and 47 patients into the control group.No difference was found between the observation group and the control group in basic clinical data or surgical data.There was no statistical difference between the two groups in delayed gastric emptying,bile leakage,bleeding or the incidence of pancreatic fistula and intra-abdominal infection.The incidence of pancreatic fistula with grade B and C in the observation group was statistically lower than that of the control group(12.5% vs 34.0%,P<0.05).The incidence of intraabdominal infection in observation group was statistically lower than those in the control group (20.0% vs 40.4%,P<0.05).The incidence of total complications in observation group was statistically lower than that in the control group (60.0% vs 83.0%,P<0.05),but no difference was found between the observation group and control group in morality.Early persistent vacuum suction drainage could reduce hospital stay((21.93±7.14) days vs (28.70±12.45) days,P<0.05) and clinical expense ((64.8±12.0) thousands vs (75.2± 14.6) thousands,P<0.05) in patients with high risk of pancreatic fistula after PD.Conclusions Early persistent vacuum suction drainage can reduce the rate of grade B and C pancreatic fistula in patients with high risk of pancreatic fistula undergoing PD.The manner can also reduce the incidence of intra-abdominal infection,total complications and cost of hospital stay.This manner can accelerate postoperative recovery of patients undergoing PD and is worth of widely used.
6.Comparison of Clinical Effect of Anterior and Posterior Focus Resection and Bone Graft on Spinal Tuberculosis

Objective:To compare the clinical effect of anterior and posterior resection,bone grafting and internal fixation on the spinal tuberculosis.Methods:82 cases of patients with spinal tuberculosis in our hospital from February 2014 to August 2015 were selected and randomly divided into the control group and the observation group with 41 cases in each group.Both groups received conventional anti tuberculosis treament and underwent debridement bone graft and internal fixation treatment,the control group underwent anterior internal fixation,while the observation group underwent posterior internal fixation.The operation time,intraoperative blood loss,low back pain relief time,ambulation time and hospitalization time were compared between two groups.All patients were followed up for 6 months,the Frankel grade and serum Thl7 cell related factors levels were compared between two groups before and after operation.Results:The low back pain relief time,ambulation time and hospitalization time of observation group were significantly shorter than those of the control group(P<0.01).After operation,the Frankel classification grade A and B ratio of observation group were significantly higher than those of the control group (P<0.05),the serum IL-10,IL-17,IL-23 and TGF-beta 1 levels were significantly lower than those of the control group (P<0.01).Conclusion:Anterior resection combined with posterior bone graft inFixation could effectively promote the rehabilitation of spinal tuberculosis,reduce the severity of spinal injury,relieve the inflammatory response and promote the recovery of immune function.

8.Preliminary study on general safe medication regularity of Chinese patent orthopedic medicines based on adverse reaction/event literature analysis.

Chinese patent orthopedic medicines feature complex components, mainly including desperate and toxic herbal pieces, narrow safety window, more clinical contraindications and frequent adverse drug reaction/events (ADR/ADE). To study the general safe medication regularity of Chinese patent orthopedic medicines, define key points in the medication education and ensure rational clinical medication, the authors took 80 types of commonly used Chinese patent orthopedic medicines as the study objects, collect 237 cases from 164 ADR/ADE documents through a system retrieval strategy, make a multidimensional literature analysis to determine the common risk factors for safe and rational medication of Chinese patent orthopedic medicines and establish an ADR/ADE prevention regularity. First, in the aspect of clinical symptoms, skin allergy is the most common ADR/ADE and closely related to the toxic ingredients, particularly accumulated liver or kidney damage caused by some drugs. Second, there are three time nodes in the ADR/ADE occurrence; The ADR/ADE occurred in 30 minutes is closely related to the idiosyncrasy; the ADR/ADE occurred between several months and half a year is related to the drug-induced liver and kidney damages; The most common ADR/ADE was observed within 7 days and predictable according to the pharmacological actions; Third, toxicity is an important factor in the occurrence of ADR/ADE of Chinese patent orthopedic medicines. Fourth, emphasis shall be given to the special medication factors, such as the combination with western medicines and Chinese herbal decoctions, overdose and long-course medication and self-medical therapy. In conclusion, the general ADR/ADE prevention regularity for Chinese patent orthopedic medicines was summarized to provide supports for clinicians in safe and rational medication and give the guidance for pharmacist in medication education.

10.Health literacy levels in diabetes patients with poor glycemic status in communities of Shanghai

Objective To investigate the levels of literacy and numeracy in type 2 diabetes patients with poor glycemic status in communities of Shanghai,China,and to evaluate their associations with blood glucose level.Methods A total of 800 type 2 diabetes patients with recent HbA1c≥7.5% or fasting plasma glucose level ≥10 mmol/L were recruited from 8 communities in Minhang district and Changning district of Shanghai,China,and were interviewed using a structured questionnaire during February 2015 and March 2016.Literacy and numeracy levels of all patients were evaluated using the validated Health Literacy Management Scale (HeLMS) and the 5-item version Diabetes Numeracy Test (DNT-5),respectively.Results The patients included in this study were observed to have higher levels of health literacy,with a median score of HeLMS being 116 [interquartile range (IQR),108-120] and a median correct rate of DNT-5 of 80% (IQR,60%-100%).Age,educational level and occupation were significantly related with health literacy levels and numeracy.Sex and income were closely related with health literacy levels.HeLMS score was not significantly associated with HbA1c level (P =0.383),while the lower correct rate of DNT-5 was linked with a higher level of HbA1c.The median HbA1c level was 8.3% (IQR 7.7%-9.4%) in the patients with the lowest tertile of DNT-5 correct rate,significantly higher than 8.2% (IQR:7.5%-9.2%) in the medium and 8.0% (IQR:7.5 %-8.8 %) in the highest tertile group (P =0.009).Conclusions Diabetes patients with poor glycemic status in communities of Shanghai have high levels of health literacy,which was significantly related with age,sex,educational level,occupation and income.Ability in numeracy may be a more important influence factor than health literacy for glycemic status of diabetes patients.
