1.Effect of surgery for spontaneous subconjunctival orbital fat prolapse via bulbar conjunctiva under topical anesthesia

AIM: To evaluate the clinical efficacy of surgery for spontaneous subconjunctival orbital fat prolapse via bulbar conjunctiva under topical anesthesia.
●METHODS: A total of 22 eyes of 11 patients received surgery for spontaneous subconjunctival orbital fat prolapse via bulbar conjunctiva under topical anesthesia were included. Objective and subjective duration of the operation, degree of cooperation during surgery, postoperative recovery, recurrence and complications were observed.
●RESULTS: In the process of surgery, patients without pain and being-cooperated were recorded in 17 eyes. The patients who occasionally felt slight pain, but within endurance after adding topical anesthesia once and the operation was completed successfully were recorded in 5 eyes ( the second eye surgery ) . The operation was successfully completed in all the patients. Compared the coordination degree during surgery of Gradel with GradeⅡ, the difference was statistically significant ( t=-3. 123, P<0. 01). All eyes were healed well after operation.
● CONCLUSION: The surgery for spontaneous subconjunctival orbital fat prolapse via bulbar conjunctiva under topical anesthesia can ensure satisfactory anesthetic effect and get better quality of operation. It′s a simple, safe and effective anesthesia method for spontaneous subconjunctival orbital fat prolapse.
3.Urinary injuries in laparoscopic-assisted vaginal hysterectomy: A report of 5 cases

Objective To investigate urinary tract injuries in laparoscopic-assisted vaginal hysterectomy(LAVH).Methods Clinical data of 5 cases of urinary tract injuries from 415 cases of LAVH from December 1995 to January 2006 in this hospital were retrospectively studied. Results Intraoperative bladder injury occurred in 2 cases and an immediate repair was given.Postoperative ureteral injury occurred in 3 cases: 2 cases of ureteral injury were found 4 days after operation because of abdominal pain,and 1 case was found on the 30th day after operation because of massive vaginal discharge.Conclusions Urinary tract injury is one of common complications in laparoscopic-assisted vaginal hysterectomy.High-risk factors include tumor size more than 5 cm,tumor protruding to the broad ligament,and tumor adjacent to the isthmus.Surgical repair is the major treatment.

5.Effect of Propofol Target-controlled Infusion on Stress Response during Nasoscopic Operation

Objective To observe the effect of propofol target-controlled infusion on stress response during nasoscopic procedures.Methods Totally 40 patients with ASA gradesⅠ-Ⅱ scheduled for the nasoscopic operation ware randomly divided into two groups:Group A(propofol continuously injection,2.5 mg?kg-1,n=20)and Group B(propofol target-controlled infusion,4 ?g?ml-1,n=20).The operations were all performed under general anesthesia.Venous blood samples were taken to measure cortisol and blood glucose at three time points:before operation,at 30 min after the operation started,and 60 min after the endotracheal catheter was withdrawn.Meanwhile,HR and MAP of the patients were recorded.Results At both 30 min after the operation started and and 60 min after the endotracheal catheter was withdrawn,Group A showed significantly higher MAP and serum levels of glucose and cortisol than Group B.At 30 min after the operation started:HR:(73?8)/min vs(65?13)/min,t=2.344,P=0.024;MAP:(74?7)mm Hg vs(68?7)mm Hg,t=2.711,P=0.010;blood glucose:(6.28?0.11)mmol/ml vs(5.31?0.15)mmol/ml,t=23.321,P=0.000;cortisol:(125.3?11.5)ng/ml vs(89.6?9.9)ng/ml,t=10.521,P=0.000.At 60 min after the endotracheal catheter was withdrawn:MAP:(79?6)mm Hg vs(73?8)mm Hg,t=2.683,P=0.011;blood glucose:(6.18?0.09)mmol/ml vs(5.62?0.16)mmol/ml,t=10.082,P=0.000;cortisol:(169.1?16.3)ng/ml vs(149.5?15.3)ng/ml,t=3.921,P=0.000.Conclusion Propofol target-controlled infusion can inhibit the stress response caused by nasoscopic operation.
6.Perioperative psychological nursing in patients with cerebral tumor of functional zone undergoing craniotomy under conscious state

Objective To explore and analyze perioperative psychological care and its effect in craniotomy patients under sober state.Methods 10 patients with cerebral tumor of functional zone undergoing craniotomy under conscious state were given pertinent psychological nursing and psychological counsehng,besides,perioperative comprehensive nursing wag given in order to reach the aim of effective psychological nursing.Results The perioperative behavioral control of patients wag better,they could cooperate with the operation as requested and reached the anticipated aim.All patients rehabilitated at discharge.Conclusions Effective psychological care plays a very important role to ensure the success of sober craniotomy and rehabilitation of patients.
7.Involvement of serine/threonine protein phosphatases 1/2A intolerance established by hypoxic preconditioning inhuman umbilical vein endothelial cells

AIM To investigate the role of serine/threonine protein phosphatases 1 and 2A (PP1/2A) in regulation of cell signal transduction involved in the tolerance of human umbilical vein endothelial cells (HUVEC) to hypoxia. METHODS HUVEC tolerance was established by hypoxic preconditioning. The tolerance of HUVEC was evaluated by the cell survival rate, lactic dehydrogenase (LDH) releasing and total antagonistic-oxidative capability (T-AOC). Subcellular localization of nuclear factor E2-related factor 2 (Nrf2) was determined by immunocytochemistry combined with Western blot. The expression of stress protein of heme oxygenase-1 (HO-1) was measured by Western blot. RESULTS Hypoxia 90 min decreased the survival rate and T-AOC of HUVEC significantly, increased the release of LDH in cultured HUVEC. Compared with the hypoxic group, hypoxic preconditioning (4, 8 and 24 h after hypoxia 10 min) up-regulated the tolerance against hypoxia in HUVEC, the survival rate of HUVEC and T-AOC increased and the release of LDH down-regulated when insulted with hypoxia (90 min) in HUVEC. Hypoxic preconditioning established the translocation of Nrf2 from cytoplasm to nucleus and up-regulated the expression of downstream protein HO-1. Pretreatment with okadaic acid (40 nmol·L-1), a powerful inhibitor of PP1/2A, for 10 min in hypoxic preconditioning HUVEC partly inhibited the translocation of Nrf2 from cytoplasm to nucleus and the expression of HO-1, abolished the tolerance of HUVEC established by hypoxic preconditioning. CONCLUSION PP1/2A at least partly take part in Regulation of translocation of Nrf2 and expression of HO-1, with is associated with the tolerance of HUVEC established by hypoxic preconditioning.
8.Cloning and expression of hexon protein of human respiratory adenovirus of three serotypes and analysis of antigenicity of the recombinant proteins

Objective To clone and express the hexon protein of three prevalent human adenovi -rus strains causing respiratory disease and analyze the antigenic characteristics of the recombinant proteins . Methods The full length genes encoding hexon protein of human adenovirus serotype 3(HAdV3), serotype 4(HAdV4) and serotype 7(HAdV7) were cloned by PCR and sequenced , respectively.The alignment anal-ysis was performed by using hexon gene sequences from GenBank .The major antigenic regions of hexon pro-tein of the three serotypes were expressed in E.coli and purified.The antigenicity, immunogenicity and cross reactivity of the recombinant proteins were determined by ELISA and Western blot assay .Results The full length gene sequence encoding hexon protein of human adenovirus serotype 4 was firstly reported in China , which showed more than 99%homology in both nucleotide and amino acid sequences with the human adeno-virus type 4 NHRC3 strain.The partial hexon protein sequence of HAdV 3, HAdV4 and HAdV7 containing all of the 7 hyperviriable regions ( HVRs) were expressed in E.coli, respectively .The purified recombinant proteins could be recognized by antiserum of the three serotypes of adenovirus .The antiserum samples against the three recombinant proteins could cross-react with particles of the three serotypes of adenovirus . The possible type-and species-specific epitopes were predicted .Conclusion The major antigenic regions of hexon protein of the three serotypes were successfully expressed .The purified recombinant proteins contai-ning both intertypes and type-specific epitopes showed a strong immunogenicity .
9.Cause and preventive measures of positive stump in gas-trectomy for gastric cancer

Objective: To investigate the cause and preventive measures of positive stump in gastrectomy for gastric cancer. Methods: Nine hundred forty-six patients performed with gastrectomy for gastric carcinoma had been collected from January 1998 to May 2009, the ratio of positive stump was analyzed retrospectively. Results: The rate of positive stump of gastrectomy was 6.24%(59/946). Statistical difference was found between the rate of radical and palliative gastrectomy(P0.05). Conclusion: The rate of positive stump was higher in palliative gastrectomy, Borrmann Ⅲ~IV type, low differentiated cancer and those cases with metastatic lymph nodes more than seven. To prevent the occurrence of positive stump in gastrectomy for gastric cancer, gastroscopy and barium meal should be carried out, enough length between incisal margin and tumor edge should be kept and frozen biopsy should be done.
