Objective To compare the effects of different dosage of esmolol on hemodynamics responses to tracheal extubation. Methods Fourty patients with ASAⅠ~Ⅱ were randomly divided into four groups.Group A as a control group,Group B,C and D received different dosage of esmolol.Postoperative patients of general anesthesia were commanded to open eyes, then received 5ml saline(Group A),esmolol 0 5mg?kg -1 (Group B),1 0mg?kg -1 (Group C) and 2 0mg?kg -1 (Group D) respectively. The changes in the HR,SBP and DBP before induction, after operation, during tracheal extubation and 1,3,5,10min after extubation were observed. Results After extubation,HR in Group A increased significantly(P

Drug-induced immune hemolytic anemia has complicated manifestations and pathogenesis, and therefore clinicians should know its etiology and pathogenesis for safe medication. In this paper, we made a discuss on the etiology and pathogenesis of drug-induced immune hemolytic anemia from both traditional Chinese and western medicine viewpoint hoping to provide references for clinical physicians.

Objective To investigate the effect of various doses of dezocine on the prevention of emergence agitation after sevoflurane anesthesia in children .Methods 100 children aged from 1 to 3 years old were randomly divided into 4 groups with 25 cases each :the 0 .03 mg/kg dezocine group (group D1) ,the 0 .05 mg /kg dezocine group (group D2) ,the 0 .10 mg/kg dezocine group (group D3) and the control group .Anaesthesia was induced with 6% sevoflurane and 1 μg/kg remifentanil .Anaesthesia was maintained with 1 .5% - 2 .5% sevoflurane and remifentanil .Different doses of dezocine 0 .03 mg/kg ,0 .05 mg/kg ,0 .10 mg/kg ,and the same volume saline were administered before surgery .The incidence of emergence agitation was assessed with 5 points scale and the severity of emergence agitation was assessed with Pediatric Anesthesia Emergence Delirium (PAED) scale .The time to remove the laryngeal mask airway ,the time to be discharged from the post-anesthesia care unit (PACU) ,FLACC and Ramsey scores ,post-operative nausea and vomiting were recorded and considered .Results Compared with the control group ,the incidence of emergence agitation and PAED scales of D2 and D3 group were significantly lower than it (P < 0 .05) .Compared with the control group , FLACC scores of D2 and D3 group were lower than it(P< 0 .05) .At the same time ,Ramsey scores of D2 and D3 were higher than that of the control group(P < 0 .05) .Moreover ,Ramsey score of D3 was higher than D2(P < 0 .05) .The time span of being dis-charged from the PACU of D3 was significantly longer than that of the other groups(P< 0 .05) .Conclusion Dezocine of 0 .05 mg/kg and 0 .10 mg/kg both can reduce the incidence of emergence agitation effectively ,and there is no significant difference between the effect of the two doses .However ,the dose of 0 .05 mg/kg has a better performance in the time span for being discharged from the PACU .

Objective To evaluate the use of LigaSure vessel sealant system on hemorrhoidectomy for the patients with grade 3 and grade 4 hemorrhoids. Methods Sixty five patients with hemorrhoids (41 males and 24 females), among whom 46 cases were grade 3 and 19 cases were grade 4, underwent hemorrhoidectomy under local anesthesia with the use of LigaSure vessel sealant system. The time consumed for operation and the blood volume lost during and after operation were recorded. Postoperative pain was assessed by means of visual analog score (1-10). The patients were followed up for 2 months. Results The operations were performed successfully in all patients. The time consumed was 5-25 minutes (9.7?4.6 minutes). The pain scores at 4 hours and 24 hours after operation were 1-7 (2.7?1.7) and 0-5 (1.9?1.3), respectively. On the third day after operation, only 4 cases had a pain score of 3 to 5, in the remaining 61 cases the score was under 3. On the seventh day after operation, only 5 patients had pain scores of 1 to 3. No patients complained pain about 2 months later. The volume of blood loss in 9 cases was less than 15ml (15.8%), and in the remaining 56 cases there was no apparent blood loss during the operation. Only one patient was re-operated for bleeding after operation, and in 18 (27.7%) patients there was only oozing of blood. Mild incontinence occurred in 6 patients shortly after operation, and no severe complications occurred. Conclusions LigaSure vessel sealant system shows the advantage of good haemostatic effect, less time consuming for operation, and less pain after operation for the treatment of patients with Grade 3 and Grade 4 hemorrhoids.
7.Recognition of anti-VEGF therapy base on the mechanism of VEGF in wet age-related macular degeneration
Chinese Journal of Experimental Ophthalmology 2012;30(4):289-292
Age-related macular degeneration (AMD) is the leading cause of visual impairment among older population worldwide, and wet AMD is more threatened to vision because of the choroidal neovascularization. Some physical therapies are thought to destroy the lesions but can not improve the visual acuity.Therefore,anti-VEGF drug therapy is becoming a new approach to the management of wet AMD.Vascular endothelial growth factor(VEGF) is thought to play an important role in the complicated pathogenesis, which can be addressed by disease reduction strategies.Among the anti-VEGF drug therapies,anti-VEGF monoclonal antibodies are proved to maintain and improve visual acuity.Other therapies have been or now being developed for the treatment of neovascular AMD with the goal of inhibiting VEGF.These inhibitors include VEGF receptor decoy aflibercept, small interfering RNA-based therapies (bevasiranib) and tyrosine kinase inhibitors (vatalanib),which could offer the potential for further advances. To completely realize the active mechanism of VEGF in wet AMD is helpful for the rational use of anti-VEGF drugs.

ObjectiveTo study the relationship between function of cardiac autonomic nervous system and 24-hourambulatory blood pressure in diabetes mellitus with normal blood pressure in the casual assay.Methods71 DMpatients with normal blood pressure in casual assay were measured with cardiac power spectral analysis and 24hAMBPassessment.ResultsThere was no difference in 24hAMBP between DM patients without AND and normal subjects.24-hour ambulatory systolic pressure values and the prevalence of blood pressure burden in DM patients with mild AND were significantly higher than that in the control group and the percentage of day-night change in AMBP weresignificantly lower than that in the control group. In DM patients with severe AND, 24-hour ambulatory systolicpressure values and the prevalence of 24hAMBP burden were higher and the percentage of day-night change was lowerthan that in DM patients with mild AND, however, there was no significant difference between them. Conclusion Blood pressure was in severe disorder as soon as cardiac AND appear in DM patients with normal casual blood pressure.Abnormality of ANSF and blood pressure in DM patients contributed to the onset and development of cardiovascularevents. They are very important for investigating cardiac ANSF in DM patients and early intervening and treating DMpatients with AND.
