1.Application of asymmetrical Z-plasty in correction of epicanthus

Objective To investigate a new method in correction of epicanthus.Methods Asymmetrical Z-plasty was used in correcting epicanthus and lengthening palpebral fissure in 42 cases with 84 eyes.It could be combined with double-eyelid-blepharoplasty to create beautiful eyes.Results All 42 cases with 84 eyes had satisfactory outcome after half a year to 2 years follow-up.The epicanthus scar was conspicuous in the first 6 weeks and it subsided gradually.In the three cases.the hypertrophic scar occurred in the epicanthus,but it softened and became inconspieuous 6 mouths after operation.Conclusion Asymmetrical Z-plasty is a new and reliable method for correction of epicanthus with inconspicuous scar and little chance of recurrence that can be widely used.
2.Safty analysis of cisapride combined with polyethylene glycol electrolytic solution in preoperative bowel prepara-tion for patients with colorectal cancer

Objective To explore the effect of cisapride combined with polyethylene glycol electrolytic solution on preoperative bowel preparation for patients receiving colorectal cancer operation. Methods One hundred and fifty-two patients received colorectal cancer opera-tion in our hospital from October 2010 to October 2014 were selected. Those who were treated with cisapride combined with polyethylene gly-col electrolytic solution for bowel preparation before operation were assigned to oberservation group(n=83),those who were treated with poly-ethylene glycol electrolytic solution for bowel preparation before operation were assigned to control group(n=69). The side effect after medi-cation, electrolyte concentration and incidence of incision infection between groups after operation were collected and compared. Results The incidence of nausea and vomiting was 7. 23% in observation group,which was significantly lower than 18. 84% in control group (χ2 =4. 64,P=0. 03). The difference of Na+,K+ and Cl- concentration between groups were not significant (t=1. 83,P=0. 07;t=0. 42,P=0. 68;t=1. 86,P=0. 07). The body temperature,leukocyte count and incidence of incision infection after operation in observation group were significantly lower than those in control group (t=2. 68,P<0. 01;t=3. 29,P<0. 01;χ2 =4. 90,P=0. 03). Conclusion Cisapride com-bined with polyethylene glycol electrolytic solution before operation can significantly decrease the incidence of gastrointestinal side effect after medicaiton and incision infection after operation and it is beneficial to recovery for patients with colorectal cancer operation.

4.Laparoscopic radiofrequency ablation for the treatment of hepatocellular carcinoma

Objective To investigate the efficacy and safety of laparoscopic radiofrequency ablation (RFA) therapy for hepatocellular carcinoma.Methods Clinical data of 78 hepatocellular carcinoma patients undergoing laparoscopic radiofrequency in Beijing China-Japan Friendship Hospital from May.2008 to July.2010 were reviewed retrospectively.Age ranged from 31 to 87 years,41 were male and 37 were female.Intraoperative ultrasound-guided needle biopsy was performed and diagnosis of hepatocellular carcinoma was established in all cases before RFA was carried out.Data analysis was performed using software SPSS or OriginPro7.Necrosis,local recurrence of the tumor and the cumulative survival rate were analyzed by Kaplan-Meier test and x2 test.Results The median follow-up time was 16 months after RFA treatment.Tumor size,proximity to intrahepatic vessels and combination with TACE were factors that influence local recurrence.The complete ablation (CA) rate was remarkably higher in tumors < 3 cm than in tumors > 3 -5 cm (90.5% vs 71.4%,x2 =4.291,P =0.038 ).Tumors adjacent to major vessels had a significantly lower CA rate as compared with those not adjacent to them (63.6% vs 91.9%,x2 =6.351,P =0.012).The CA rate were 88.9% in the TACE ± RFA group and 75.0% in the RFA group ( x2 =1.567,P =0.211 ).The mean overall survival were 48.7 ± 2.4 months,the cumulative survival rate was 86.1% at 1 year、76.9% at 2 years、60.3% at 3 years、51.8% at4 years and 33.1% at 5 years.Conclusions Laparoscopic RFA is safe,mini-invasive and effective for unresectable hepatic carcinoma with a favorable long term survival.

Hirsch index or h-index of a subject means that the subject has h published papers which are cited at least h times for each.This index is considered as both quantity and quality of the SHbiect's academic outcome.In this paper the data showed that the h-indexes of the Chinese medical SUbiects were different.The SUbiect's h-index could be used as a balance factor.which made the outcome of scientists from different SUbiects comparable.Using h-index and the other 2 indexes.Chinese medical snbiects were clustered into 4 groups,which could be used as a guideline for classified management and the evaluation of the medical subjects
6.Efficacy of Twin-block appliance in Class II division 1 of adolescents: A systematic review

Objective; To evaluate the efficacy of Twin-block appliance in Class Ⅱ division 1 of adolescents. Methods: Several e-lectronic databases (PubMed, The Cochrane library, Embase, CBMdisk, CNKI, VIP)were searched. Abstract that appears to fulfill the initial selection criteria were selected by consensus and original articles were retrieved. Five Chinese journals were hand searched for possible missing articles. Randomized controlled trials(RCTs) that evaluate the efficacy of Twin-block appliance in Class Ⅱ division I adolescents without any surgical intervention or syndromic characteristics were considered. A comparable untreated control group was required to factor out normal growth changes. The quality of included studies was evaluated according to Cochrane Reviewers Handbook 4.2.6 Meta-analysis was performed by using RevMan 5.0.0 software. Results: Four RCTs including 289 patients were identified. The results of our meta-analysis showed a significant increase on SNB angle, Ar-Gn, Nasolabial angle and Labial-mental fold compared with control group(P<0.05). Whereas overjet, overbite, ANB angle and Ls-E had a significant decrease. In addition, SNA angle and Li-E didnt show statistical significant changes(P>0. 05). Conclusion; In Class II division 1 of adolescents, Twin-block appliance can effectively decrease their overjet, overbite, ANB angle, and induce anterior-posterior growth of mandible. The effect on restrain forward growth of the maxilla was unclear. More RCTs are required to evaluate the efficacy of Twin-block appliance.

8.Evaluation of left ventricular dyssynchrony with two-dimensional speckle tracking echocardiography in acute myocardial infarction

Objective To evaluate the left ventricular (LV) systolic dyssynchrony after the acute myocardial infarction (AMI) with two-dimensional speckle tracking echocardiography (STE). Methods STE were performed in 65 patients within 72 h of AMI and compared with 60 age- and sex-matched healthy volunteers. The peak longitudinal strain (LS_(peak)) was measured at LV myocardium. LV dyssynchrony was defined as an interval ≥130 ms for the absolute difference in time to peak radial strain for the anteroseptal wall versus the posterior wall (TAS-POST). Size of myocardial infarction (MIS)was confirmed by wall-motion score index (WMSI). Results The LS_(peak) and LV ejection fraction (LVEF) were lower, and WMSI and TAS-POST were larger in AMI patients compared with controls. Forty-two patients had developed LV dyssynchrony (64.62%), and there were strong correlation between LV dyssynchrony (TAS-POST) and LS_(peak), LVEF, and WMSI (MIS). MIS was the most independent predictor for systolic dyssynchrony. Conclusion AMI with normal QRS can induce LV dyssynchrony, which is mainly determined by MIS. STE is a reliable technique for accurate evaluation of LV synchrony.
9.Association of CD+4 CDHigh25 regulatory T cells and the negative control factor IL-10 with the development of acute graft-versus-host disease after allogeneic hematopoietic cell transplantation

Objective To investigate the relationship between the CD+4 CDHigh25 regulatory T cells and cytokine IL-10 and acute graft-versus-host disease (aGVHD) after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Methods Flow cytometric was used to detect the percentage of CD+4 CDHigh25Foxp3High Treg cell and CD+4 CDHigh25 CDLow127 Treg cell in CD+4 T cells and at the same time ELISA was used to test the serum IL-10 levels in corresponding period. Results 13 patients have received hematopoietic function reconstruction. aGVHD group of CD+4 CDHigh25 CDLow127/CD+4 and CD+4 CDHigh25 Foxp3High/CD+4 ratio were significantly lower than non-aGVHD group, the difference was statistically significant (P <0.01); Ⅲ-Ⅳ degree of aGVHD subgroup was lower than Ⅰ - Ⅱ degree of aGVHD subgroup, but no statistical significance(P >0.05); the same as between-group CD+4 CDHigh25 CDLow127/CD+4 and the CD+4 CDHigh25 Foxp3High/CD+4 has no significant difference;aGVHD group of IL-10 concentration was significantly lower than non-GVHD group, the difference was statistically significant (P <0.01). Treg cell and IL-10 changes in correlation, r = 0.557, P <0.05. Conclusion The level of Treg cell was closely related to the occurrence of aGVHD after allo-HSCT. So it is very important to monitor the Treg cell level for clinical early diagnosis of aGVHD and predict prognosis of aGVHD and guide the application of immunosuppressant. CD127 can serve as a Treg cell surface-specific marker, to promote the detection of Treg cell and purification. IL-10 was an important negative regulator. Treg cell and IL-10expression in patients with aGVHD was correlation, which may provide some basis for Treg cell immunosuppressive mechanism.
10.Efficacy and adverse reactions of different stimulus dosage of modified electroconvulsive therapy for depressive episode

Objective To evaluate the antidepressant efficacy and adverse reactions of 3 different stimulus dosage of modified electroconvulsive therapy ( MECT) in patients with depressive episode. Methods 120 patients with depressive episode were randomized into low dosage group ( n=40) ,medium dosage group ( n=40) and high dosage group ( n=40) . Low dosage were 50%× age,medium dosage was 70%×age,high dosage was 80%×age . All patients received 6 treatments. Hamilton depression scale?17(HAMD?17) was used to evaluate the clinical symptoms at baseline,3 and 6 treatments. Effects and adverse reactions were compared among three groups.Results Compared with baseline the HAMD?17 scores of the 3 groups were significantly improved after 6 treatments( t=24.026, P=0.000;t=26.541, P=0.000;t=25.904, P=0.000) , but there were no statistically significant differences among the three dosage groups(F=0.409, P=0.665). Compared with low dosage group((27.2±5.4)%),the HAMD?17 scores reductive ratio of medium dosage group((34.3±6.8)%) and high dosage group((33.9±6.9)%)) were significantly improved after 3 treat?ments ( t=-5.513, P=0.000;t=-4.785, P=0.000). Compared with the low dosage group,the incidence rate of headache,nausea and vomiting,delirium were significantly higher in high dosage groups( headache:χ2=14.532, P=0.000;nausea and vomiting:χ2=13.333, P=0.000;delirium:χ2=14.907, P=0.000) . The re?covery time was significantly longer in medium dosage group ( ( 10. 5 ± 1. 6 ) min ) and high dosage group ((11.2±1.8)min) than that in low dosage group((8.8±1.2)min)( t=-5.144,=0.000;t=-6.889, P=0.000).Conclusion Different stimulus dosage of MECT for depressive episode has equivalent curative effect after 6 treatment . Medium dosage and high dosage treatments appear to have an early onset of efficacy,but may also be associated with more adverse reactions.
