1.Predictive factors for lateral lymph node metastasis in papillary thyroid microcarcinoma

Objective To find the clinical and histopathological predictive factors for lateral lymph node ( LN ) metastasis in thyroid papillary microcarcinoma( PTMC ).Methods From January 2007 to December 2010,141 patients with PTMC underwent central and lateral LN dissections were enrolled in the study.The data from the cases were analyzed retrospectively to determine the predictive factors for lateral LN metastasis.Results Among the aforementioned cases,37 cases ( 26.2% ) were PTMC with the presence of multifocality,24 ( 16.9% ) Hashimoto thyroiditis,84 ( 59.6% ) central LN metastasis,56 ( 39.7% ) lateral LN metastasis.Multifocality,central LN metastasis,underlying Hashimoto thyroiditis,and upper pole location were significantly related to lateral LN metastasis in univariate analysis( P<0.05 ).These four factors were also found to be independent predictive factors for lateral LN metastasis in multivariate analysis( P<0.05 ).9(6.4% ) of there patients were found to show skip metastasis in which there was lateral LN metastasis but no central LN metastasis.The upper pole location was statistically significantly associated with skip metastasis.Conclusions Patients with multifocality,central LN metastasis,underlying Hashimoto thyroiditis,and upper pole location should be paid more attention to the status of lateral LN.Skip metastasis occurs in a minority of patients with PTMC.Even if there is no central LN metastasis,patients with upper pole lesion should be searched carefully for the lateral cervical LN metastasis.
2.Determination of curcumol and germacrone in Zedoary Turmeric Oil and Glucose Injections by RP-HPLC

AIM: To establish a HPLC method for determination of curcumol and germacrone in Zedoary Turmeric Oil and Glucose Injections. METHODS: RP-HPLC was applied for quantitative analysis of curcumol and germacrone. Hypersil ODS2 column (250 mm? 4.6 mm, 5 ?m) and gradient elution was used. Mobile phase A was acetonitrile and mobile phase B was water. Phase A time-percentage composition was as follows:0-3 min,45%;3-30 min,45%→65%;30-38 min, 65%; 38-45 min, 65%→90%;45-55 min,90%→100%;55-65 min,100%. The flow rate was at 1.0 mL?min -1 . Diode array detector was set at 214 nm and column temperature was at 25 ?C. RESULTS: The calibration curves of curcumol and germacrone were linear in the range of 0.26 - 6.5 ?g(r= 0.999 9 ) and 0.112 5 - 2.812 5 ?g(r=1), The average recoveries of them were 102.1% and 98.8% (n=5), respectively. CONCLUSION: Curcumol and germacrone can be determined by the method, and this will improve the quality control of Zedoary Turmeric Oil and Glucose Injections.

Objective To analyze the correlationship of ACE I/D gene polymerphisms with the anti-hypertensive efficacy of irbesartan among essential hypertension (EH) patients in Yunnan han people.Methods One hundred EH patients hypertension were treated with irbesartan 150mg once daily for 4 weeks,and anti-hypertensive efficacy were monitored during the treatment.Results There was a significant difference between anti-hypertensive efficacy of irbesartan among different genotype groups,DD group >ID group >Ⅱ group (P < 0.05) Conclusions There is different therapy responsiveness in diffenent genotypes.ACE I/D polymorphism may be an important hereditary factor that impacts the efficacy of irbesartan.
4.The Appearances of Extraconal Orbital Diseases on High-resolution MR Using Surface Coils

Objective To summarize the important HR-MRI manifestations of orbital disease in extraconal compartment.Methods Forty-four patients with extraconal orbital disease identified pathologically were analysed retrospectively. MRI of the orbit at 1.5 Tesla was performed with surface coil, thin slice, suitable bandwidth, small field of view (FOV) and large matrix. The analyzed criteria of imaging appearance were: location, shape,size, margins, extent, adjacent structures, and signal intensity.Results Lymphoma, hemangioma, rhabdomyosarcoma, metastasis, pseudotumor, and phlegmon accounted for 70% of all extraconal orbital diseases. Three cases of haemangioma had round shape and clear margin; most lymphoma had irregular shape, unclear margin and big volume; while most rhabdomyosarcoma and metastasis had round shape and unclear margin. The effect of space-occupying of lymphoma was not obvious. One case of lymphangioma had high signal intensity on both T 1- and T 2-weighted images without enhancement. Melanomametastasis appeared hyperintense on T 1- weighted and hypointense on T 2-weighted images. The contrast-enhancement of haemangioma was much more prominent than others. There were characteristic of rhabdomyosarcoma, metastasis, pseudotumor, and phlegmon about their age,clinical history and examination. Conclusion Location criteria, morphological changes, MRI signal intensity, clinical history and the effect of space-occupying are very useful in the differential diagnosis of the orbital disease.
5.Cerebral fMRI study of the normal adult on electro-acupuncture Guangming point (GB37) under the vision stimulation condition

AIM:To study the human brain hemodynamics responses induced by acupuncturing foot shaoyang gallbladder meridian point GB37 and visual stimulation,and to analyze the effect of acupuncture to activate the cerebral functional area in visual stimulus. METHODS:Twenty five normal subjects were divided into two groups:15 subjects for electro -acupuncture GB37 in visual stimulus group and 10 subjects for electro-acupuncture GB37 groups. The blood oxygen level dependent functional magnetic resonance imaging (BOLD-fMRI) was carried out under the condition of two dif-ferent stimulation patterns (including a vision stimulation task and point GB37 acupuncture under the vision stimulation condition). A scintillating chessboard grid was applied as the vision stimulation task and the frequency was 8 Hz. The activation of cerebral functional areas of tested people under the conditions of the vision stimulation task and acupuncture were analyzed,respectively. RESULTS:Out of 15 subjects of electro -acupuncture GB37 in visual stimulus groups,twelve health volunteers were enrolled. When the vision stimulation task was carried out,the alarge area of brainstem and posterior lobe of cerebellum was seen to be activated. The areas of limbic lobe,hippocampus,middle temporal gyrus and inferior temporal gyrus were also activated. Under the condition of acupuncture only,the areas of posterior central gyrus,inferior parietal lobule,parietal lobe precuneus and the middle frontal gyrus were activated. When doublesided points of gallbladder meridian of foot-shao-yang were electroacupunctured under the visual stimulus status,the areas of superior frontal gyrus,inferior frontal gyrus,parietal precuneus and occipital gyrus lingualis were activated. CONCLUSION:The functional MRI shows the activation of cerebral functional area after acupuncture. Visual stimulus causes significant changes of brain conduction path connection. Under the visual stimulus status,the acupuncture to the points of gallbladder meridian of foot -shao -yang,GB37 has an extent of influence to conduction path conformity of the vision system and with the treat-ment of ophthalmocace. Furthermore,the external stimulation has prominent influence to the acupuncture therapy possibly.
6.Application and evaluation of a probe melting analysis-based assay for detection of Isoniazid-resistant mutations in Mycobacterium tuberculosis

Objective To validate the performance of a PMA-based assay for detection of INH-resistant mutations in MTB and investigate the mutation characteristic of INH-resistance.Methods The MTB standard strain H37Rv was from National Tuberculosis Reference Laboratory,1 wild-type strain and 1 katG S315T ACC mutant strain were from Xiamen CDC,7 MTB INH-resistant strains with known INH resistance mutations were from Shenzhen Center for Chronic Disease Control,Henan CDC,No.309 Hospital of PIA and Xiamen CDC.707 MTB clinical isolates were from Xiamen CDC,Xiamen No.1 Hospital and Zhangzhou CDC,126 sputum samples were from Xiamen Tongan CDC.The genomic DNA of the MTB standard strain H37Rv,7 MTB INH-resistant strains and 833 clinical samples,were extracted with Xiamen Zeesan Mycobacterium tuberculosis Isoniazid-resistance Mutation Test Kit using the thermal lysis method.The genomic DNA of 1 wild-type strain and 1 katG S315T ACC mutant strain were extracted with AxyPrep Bacterial Genomic DNA Miniprep Kit.The mutations were discriminated by the △Tm between the samples and wild type control in katG315 codon,inhA promoter -17 to -8 region,ahpC promoter region -44 to -30 and - 15 to 3,and inhA94 codon.A 10-fold dilution series of MTB DNA from 3 × 105 to 300 copies/ reaction obtained from a wild-type strain and a katG S315T ACC mutant strain,respectively,were prepared to determine the analytical sensitivity.Seven MTB INH-resistant strains with 9 predetermined mutations were used for the analytical specificity assay,and 5 mutants of which were used for the repeatability assay.The clinical detection performance of PMA assay were confirmed by the sequencing method in 833 samples.Results Results could be obtained within 3 hours from DNA extraction to PMA assay,including 46 samples in a standard 96-well real-time PCR instrument simultaneously.The analytical sensitivities of PMA were 300 copies/reaction for both the wild-type strain and katG S315T ACC mutant strain.Nine INH-resistant point mutations could be discriminated and 5 of which had standard deviations of melting temperature less than 0.5 ℃.Fully concordant results of mutant locus between PMA assay and sequencing were obtained in all 162 mutant samples.INH-resistant mutations in the four loci were found in 19.4% (162/833) samples by PMA assay in Xiamen and Zhangzhou.Among the 14 lNH-resistant mutant types detected,katG S315T ( AGC→ACC),inhA promoter - 15C→T and katG S315N (AGC→AAC) accounted for 83.3% (135/162) of the overall mutations.
7.Appearance of HR-MR imaging of non-Hodgkin lymphomas of the orbits in extraconal compartment

Objective To investigate HR-MR imaging characteristics of non-Hodgkin lymphomas (NHL) of the orbits in extraconal compartment. Methods The HR-MR images of 16 patients with NHL of the orbits identified pathologically were retrospectively analyzed. MRI of the orbit at a 1.5 T scanner was performed with 4-cm surface coil, thin slice, suitable bandwidth, small FOV, and large matrix. T1-weighted (before and after i.v. bolus injection of contrast medium) and fast spin-echo T2-weighted sequences were acquired in all patients. All cases were performed with fat suppression techniques. The analyzed criteria of imaging appearance included location, number, size, shape, margins, extension, adjacent structures, and signal intensity of the lesions. Results The lesions in 13 of 16 cases located in anterior part of extraconal compartment, posterior to the orbital septum, and with irregular cast shape. The other 3 patients showed diffuse lesions with infiltration in extraconal compartment and with irregular shape. Eleven lesions had the large volume above 3.0 ml, 4 lesions with medium volume ranging from 1.0 to 3.0 ml, and only one with small volume less than 1.0 ml. 13 cases had a poor-defined margin with infiltration of extraocular muscles, but without mechanical shift of the muscles. 7 patients had infiltration of eyeball, but only 3 cases with exophthalmos. Destruction of orbital bone was evident in 2 cases with diffuse lesions but never in isolated orbital lymphoma. On the T_1-weighted images, the signal intensity of the lymphoma was isointense compared with that of the muscle in all patients. The T_2-weighted fast SE images showed a hyperintense signal in 13 cases and intermediate in 3 cases. All lesions enhanced after intravenous Gd-DTPA administration, 15 cases with homogeneous enhancement, and 10 cases with reliably visible enhancement in the T_1-weighted fat-suppressed sequences. Conclusion Most NHL of the orbits in extraconal compartment have the characteristic findings on HR-MR imaging, including anterior part of extraconal compartment location, posterior to the orbital septum, irregular cast shape, large volume, poor-defined margin with infiltration but without mechanical shift of extraocular muscles, uniform signal characteristics and marked enhancement after injection of contrast medium, and without bony destruction in isolated orbital lymphoma. The understanding of HR-MRI features of orbital NHL in extraconal compartment is very useful for making the differential diagnosis with other orbital diseases.
8.The Posterior Contour Sign of the Lumbar Intervertebral Disc----A Sign to Show the Posterior Border of Disc in Plain Film

Objective:To eveluate and study the anatomic basis of the posterior contour sign of lumbar intervertebral disc.Methods:The most protruding length of the soft tissue mass behind intervertebral spase on lumbar plain film were measured in 100 cases and compared with CT or MR findings.Results:41 posterior contour sign of the interlumbar disc were fined in 39 cases,of which there were 25 in L4/5(61.0%),15 in L3/4(15.6%)and 1 in L2/3(2.4%).The protruding was less than 0.2 cm in 26 and larger than 0.2 cm in 15,there were 10 lumbar disc protruding confirmed by CT or MR ,7 of which the pesterior contour ≥0.2 cm,and 3>0.3 cm,none was ≤0.2 cm.Conclusion:The adipose tissue in the posterior border of lumbar body and disc was the anatomic basis of the sign,and it was an importent sign for diagnosing of lumbar disc hernia on plain film.
9.Free forearm flap and free iliac bone reconstruct the composite tissue defects of the floor of mouth and the lower part of face in the same period

ObjectiveTo probe into the clinical efficiency of reconstruction to person who had composite tissue defect of the floor of mouth and the lower part of face with the free forearm flap and free iliac bone in the same term.MethodsBefore the surgery, the surgeon should carefully evaluate the scope and shape of defects in 11 cases who had the composite tissue defects of the floor of mouth and the lower part of face, to design the individual forearm flap, so that it matches with the defect region. During surgery, the first resumption of defects using titanium plates forming the basic shape of mandible and the occlusal relationship,then used to re-sawing to take modeling of the iliac bone,transplanted free iliac bone and fixed after the inside of the titanium plate, thereby restoring the continuity of mandible missing.ResultsFree forearm flap and free iliac bone all survived in 11 cases, the success rate of 100%. The patients were followed up for 6 to 12 months, although the shape of restoration areas were different levels of fat, but eating, swallowing and other oral function had been well improved, 7/11 could enter normal diet, 4/11 to enter liquid diet, At the same time, the patients in social activities can communicate in a language daily, sensory function of skin flap and facial appearance has been satisfactory recovery.ConclusionConformal free forearm flap and free iliac bone is an ideal way to reconstruct the composite tissue defects of the floor of mouth and the lower part of face in the same period, not only safe, practical prognostic effect, and also can significantly improve the patients quality of life, which is worthy of reference and use.
10.Effects of mild hypothermia induced by pentobarbital sodium on hematology in male BALB/C mice

Objective To observe the effects of mild hypothermia induced by pentobarbital sodium on hematology in male BALB/C mice.Method Healthy male BALB/C mice were divided randomly into two groups:control group ( C) and mild hypothermia group(M).The body temperature of the mild hypothermia group was maintained between 28℃ to 30℃( anal temperature ) for 4 hours induced by pentobarbital sodium injected intraperitoneally , then recover unaffected . Anal temperature, coagulation, electrolytes, and blood cell indexes were examined in 2, 24, 72 hours after treated by mild hypothermia;Control group was given equal volume of saline volume at constant temperature .Results The body temperature and coagulation in mild hypothermia group showed no significant difference compared with the control group ( P﹥0.05),but the concentration of K +and Na +in mild hypothermia group were higher than control group (P﹤0.01), the number of WBC in mild hypothermia group was lower than control group ( P﹤0.01或P﹤0.05 ) , and the RBC、HGB、MCH、MCHC in mild hypothermia group were lower than control group transiently (P﹤0.01或P﹤0.05).Conclusion Mild hypothermia induced by pentobarbital sodium affects some of hematological values in mice considerably .
