1.Clinical analysis on 26 cases of imaging understated breast cancer

Objective To analyze the clinical data,radiological and pathological features of breast cancer which were understated by image,and summarize the causes.Methods The clinical features,radiological features and pathological data of 26 cases of breast cancer were retrospectively analyzed.The preoperative ultrasound and mammography BI-RADS grades of these 26 cases were both ≤ 3,and they were confirmed to be breast cancer by pathology.The clinical and pathological data of 1224 cases of breast cancer in the same period were compared.Results For the 26 cases,100% were early breast cancer,100% were HER2 negative breast cancer,and only 1 case had lymph node metastasis.26% were special type of breast cancer.Mammography showed glandular multi type,and ultrasound showed atypical or typical benign features.Conclusions Breast cancer understated by image shows features of young age,early stage,well differentiated,low malignant degree,not easy to be visualized by mammography,and their ultrasound features are usually atypical.In clinical setting,analysis of the ultrasound and mammography images should be combined with other examination,to decrease misdiagnosis rate.
2.The Clinical Contrast Study of Three X-ray Check Methods of Spinal Fracture

X-ray plane film. Three-dimensional CT could show the three-dimensional structure of spinal canal clearly. The postoperative efficacy of all patients was satisfactory. Conclusion The three checking methods of spinal fracture were complementary to each other, but three-dimensional CT had the advantage in reconstructing spinal canal.
3.Assessment of T cell subsets and sjTREC gene level in the sepsis children

Objective To evaluate T-lymphocyte subsets and sjTREC gene level in sepsis children and to provide a reasonable theoretical basis for immune regulation. Methods This prospective study was performed on children who were classified as sepsis group (n = 25), severe sepsis group (n = 23), and control group (n = 50). The T cell subsets were measured before the blood products,immune agents,and nutritional support were administrated. By real-time fluorescence quantitive-PCR method, the sjTREC levels of the patients with sepsis and healthy children were quantitatively detected respectively;then the sjTREC levels were absolutely quantified by two standard curve method. The results were demonstrated by sjTREC and endogenous reference gene (Cα) copies ratio (sjTREC/Cα× 2 × 1000) and statistically analyzed by SPSS 16. 0 software. Results The levels of CD3 +, CD4 +, and CD8 + T cells in severe sepsis group were lower than those in control group (P<0.01). The level of sjTREC of severe sepsis group was 173.86 +38.37,which was significantly lower than those of sepsis group (345. 15 ± 66. 21) and control group (389. 42 ± 50. 20) (P < 0. 01). Conclusion Children with severe sepsis have a range of T-lymphocyte subsets disorders and decreased thymic output function, so early immunotherapy can improve clinical outcome.
4.Research on clinic of patients with acute myocardial infarction.

Objective To study characteristics of clinical manifestation of patients with acute myocardial infarction (AMI).Methods From 1999 to 2002,the risk factors,symptoms,signs,and results of assistant check of 97 patients with AMI were analyzed.They were divided into thrombelytic group and control group.Results (1) The main risk factor are hypertention,smoking and diabetes.(2) There was no difference in complication of AMI between two groups.Thrombolytic group has higher curing rate.Conclusion AMI patients without contraindication should be treated by thrombolytion in time.
5.The effect of HBO on RhoA expression and nerve function in rat focal cerebral ischemic model

Objective To evaluated the effect of hyperbaric oxygenation therapy(HBO)on the RhoA ex- pression and nerve function after transient focal cerebral ischemia in a rat model of middle cerebral artery occlusion. Methods One hundred and twenty-six healthy Sprague-Dawley rats were used and randomly divided into a sham op- eration group(shame group,n=42),a treatment group(n=42),and a control group(n=42).The animal model of middle cerebral artery occlusion(MCAO)was established by using the Zea-Longa method with the animals in the treatment and the control groups,and sham operation was performed with those in the sham group.HBO was applied to the animals in the treatment group.The RhoA protein expression was observed by using immunohistochemistry technique,and the neurological function was evaluated by Bederson's scale at different time points after MCAO.Re- sults(1)Weakly positive expression of RhoA could be located in bilateral cortex and the basal ganglia in the sham group.The expression of RhoA in the treatment group and control group was increased as early as 6 hours after MCAO when compared with that of the sham group,and peaked at 48 h after MCAO and decreased after then,but was still higher than that of the shame group at 7th day to 14th day after MCAO.It was also found that the expression of RhoA of the treatment group was significantly lower than that of the control group(P

Obiective To elaborate the application of PCT/CT orientation Gamma Knife in the body.Methods From May, 2007 to May, 2008, PET/CT equipment was used on 120 cases with gamma knife body positioning and image fusion, Re-sults The PET/CT scan may not have iodine in the contrast agent to enhance the effective conduct of tumor imaging and CT position provides more simple and more accurate image information to guide target(GTV) of the outline. Conclusion The PET/CT in the gamma knife treatment can improve in the positioning accuracy of GIN, and improve treatment opportunity for iodine allergy patients.
7.Clinical observation of S-1 combined with γ-ray stereotactic body radiation therapy in treatment of locally advanced pancreatic cancer

Objective To evaluate the clinical toxicity and efficacy of S-1 combined with γ-ray body stereotactic radiation therapy in treatment of locally advanced pancreatic cancer.Methods Forty-five patients with locally advanced pancreatic cancer were randomly divided into two groups.The combination group received γ-ray stereotactic body radiation therapy which was given isodose curve of 50 %-60 %,tumor encircling dose of 3.0-4.5 Gy per fraction depended on dimension of tumors,9-12 fractions.Combined with S-1 40 mg/m2,2 times/d,for consecutive twenty-one days for four courses.The control group was given γ-ray stereotactic body radiation therapy only.Toxicities and effects were evaluated according to the criteria of WHO and RTOG.Results The CR rates in combination group and control group were 30.4 % (7/23) and 13.6 % (3/22),the response rates were 91.3 % (21/23) and 63.6 % (14/22) (x2 =4.980,P =0.026).The rates of gastrointestinal tract side reaction in combination group and control group were 82.6 % and 68.2 % (x2 =1.267,P =0.260),myelosuppression in combination group and control group were 78.3 % and 63.6 % (x2 =1.171,P =0.279).The rate of Ⅲ-Ⅳ grade myelosuppression in combination group were higher than that in control group (x2 =4.874,P =0.027).The median progression-free survival (PFS) rates of two groups were 8 months and 6 months respectively (x2 =1.357,P > 0.05),the median survival period were 17 months and 14 months (x2 =1.017,P > 0.05),1 year survival rates were 60.9 % and 54.5 % respectively (x2 =0.184,P > 0.05).Conclusions S-1 combined with body gamma system treatment can improve local control rate and effective rate for inoperable patients with local advanced pancreatic carcinoma,and the adverse reactions are well tolerated.This method can be used as locally advanced pancreatic cancer chemoradiation safe and effective choice.
8.Effects of Ca2+ and SA on physiological and photosynthesis of Platycodon grandiflorum under high temperature stress.

In order to reveal feasibility of different concentrations of Ca2+ and SA on Platycodon grandiflorum under high temperature stress, the effects of Ca2+ on physiological index and related photosynthetic parameters were studied. Pot cultured P. grandiflorum leaves under the same outdoor conditions were sprayed with CaCl2 and SA separately, and then placed in the high incubator [35 degrees C/25 degrees C (day/night), light intensity 3 600 lx], and sprayed with distilled water at 25 degrees C and under high temperature stress were set as the control. The related photosynthesis, relative conductivity, contents of proline, malondialdehyde, soluble protein, activities of SOD and CAT, ASA and GSH content were measured. The results show that the 6 mmol x L(-1) CaCl2 and 1.5 mmol x L(-1) SA enhanced the activities of SOD and CAT, the contents of proline and soluble protein, and effectively reduced the damage of heat stress on cell membrance. At the same time, the exogenous Ca2+ and SA increased the contents of chlorophyll and carotenoid, the efficiency of leaf photosynthesis and ASA and GSH content, and thus effectively resisted the oxidative stress caused by high temperatures, but with the increasing concentration of spraying, P. grandiflorum decreased the ability to resist high temperature stress. In conclusion, the foliage spraying CaCl2 and SA could reduce the damage of high temperature stress on P. grandiflorum leaves.

9.Studies on effects of calycosin-7-O-β-D-glucoside on prim-O-glucosylcimifugin and cimifugin in vivo pharmacokinetics.

Study on the effects of Astragali Radix main active flavone calycosin-7-O-β-D-glucoside on Saposhnikoviae Radix main active ingredients prim-O-glucosylcimifugin and cimifugin, a UPLC-MS/MS method for simultaneous determination of prim-O-glucosylcimifugin and cimifugin in rat plasma was established, and the comparative pharmacokinetics of prim-O-glucosylcimifugin and cimifugin after oral administration of prim-O-glucosylcimifugin and calycosin-7-O-β-D-glucoside-prim-O-glucosylcimifugin to rats were carried out, which might be conductive in exploring the rationality of Astragali Radix - Saposhnikoviae Radix herb couple. Twelve male SD rats were divided into two groups. Prim-O-glucosylcimifugin and cimifugin in rat plasma of different time points after oral administration of prim-O-glucosylcimifugin and calycosin-7-O-β-D-glucoside - prim-O-glucosylcimifugin to rats were determinated. And the main pharmacokinetic parameters were investigated using DAS 3. 2. 4. The established method was rapid, accurate and sensitive for simultaneous determination of prim-O-glucosylcimifugin and cimifugin in rat plasma. The analysis was performed on a Waters Acquity BEH C18 column (2.1 mm x 100 mm, 1.7 μm) with the mixture of acetonitrile and 0.1% formic acid/water as mobile phase, and the gradient elution at a flow rate of 0.3 mL x min(-1). The analytes were detected by tandem mass spectrometry with the electrospray ionization (ESI) source and in the multiple reaction monitoring (MRM) mode. Compared with prim-O-glucosylcimifugin group, the AUC(0-t)., and AUC(0-∞) of p-O-glucosylcimifugin as well as the C(max) of cimifugin significantly increased (P < 0.05) in calycosin-7-O-β-D-glucoside-prim-O-glucosylcimifugin group. Calycosin-7-O-β-D-glucoside could enhance the absorption of prim-O-glucosylcimifugin and cimifugin and improve the bioavailability, explaining preliminarily the rationality of Astragali Radix-Saposhnikoviae Radix herb couple.

10.Sepsis and severe sepsis in PICU infant: analysis of risk factors and clinical feature

Objective To analyze the clinical characteristics of PICU infant with sepsis,severe sepsis,and to explore the impact of prognostic factors.Methods We conducted a retrospective study of 141 cases (from January 2006 to December 2011) who were diagnosed as sepsis and severe sepsis to identify the clinical characteristics.There were 14 variables of the PICU admission day which concluded gender,age,underlying diseases,sepsis severity,organ involvement,critical score,blood biochemistry (lactate,albumin,glucose),blood gas (base excess,bicarbonate,pH),shock,culture-positive.The death or discharge as the outcome factor.The data were analyzed with the Logistic regression model.Results One hundred and forty-one cases aged from 1 to 12 months,of which 72 cases were sepsis and 69 cases were severe sepsis,29 cases died with the mortality of 20.6%.In 29 death children,13 cases had underlying diseases.In children with sepsis and severe sepsis,the most common infection site was lung,accounting for 47.2% (34/72) and 47.8% (33/69) ;blood cultures were positive for 34 cases,accounting for 47.2% (34/72) and 49.3% (34/69).Variables in the univariate analysis those were significantly associated with death from sepsis were gender,underlying diseases,age,sepsis severity,shock,organ involvement,the number of critical illness score,blood lactate,base excess,bicarbonate and pH.In the Logistic regression model,the variables significantly associated with death were the severity of sepsis (OR =22.5,95 % CI =5.089,99.475),and the number of organ involvement (OR=3.305,95%CI=2.152,5.075).Conclusion The heavier sepsis severity and the more number of organ involvement are,the greater the risk of death will be.
