AIM: To investigate the clinical effect of one-dose multiple point scanning panretinal photocoagulation (PRP) in the treatment of non-proliferative diabetic retinopathy (NPDR).METHODS:Selected 78 patients 132 eyes with NPDR in our hospital from January 2015 to June 2016,the patients were randomly divided into observation group (42 patients of 72 eyes) and control group (36 patients of 60 eyes).The observation group was given one-dose multi-point scanning PRP.The control group was given a single point scanning,3 to 4 times to complete the PRP.To observethe therapeutic effect in two groups,the average visual field threshold,the flash electroretinogram (F-ERG) a,b wave amplitude,the laser energy and so on were observed.RESULTS:The effective rate of the observation group and the control group were 84.7% and 83.3%,the difference was not statistically significant (P>0.05).In the observation group and the control group,at 6mo after treatment,the leakage area of retinal neovascularization was lower than that before treatment (P<0.05).The visual field threshold and F-ERG a wave amplitude differences were not statistically significant (P>0.05).F-ERG b wave amplitude were 221.94±70.18mV and 219.82±69.56mV in the observation group and the control group at 6mo after treatment which were significantly lower than that before treatment (P<0.05).The laser energy of the observation group was 541.23 ± 56.39mW,significantly higher than the control group 326.39±78.83mW (P<0.05),while the energy density was 0.34±0.14mW·ms/mm2,significantly less than the control group 2.01±0.97mW·ms/mm2(P<0.05).The incidence of complications in the observation group and the control group were 8.3% and 15.0%,the difference was statistically significant (P<0.05).CONCLUSION:The clinical effect of multiple point scanning PRP in the treatment of NPDR is better than single point multiple PRP with advantages of lower energy density and less laser damage.

The sulfonylureas as insulin secretagogues,have formed a cornerstone of pharmacotherapy for type 2 diabetes over 50 years.Although sulfonylureas are effective antihyperglycemic agents,individual variability exists in drug response (i.e.,pharmacodynamics) and adverse effects which may be related to some genes.This article mainly reviews the advance of recent studies about drug metabolism and pharmacogenomics of sulfonylureas.

Objective To explore the moderate effect of work control on the work stress-organizational citizenship behavior relationship and work stress-job satisfaction relationship.Methods 200 employees of five corporations in Shandong province were surveyed by questionnaires using work control scale,work stress scale,organizational citizenship behavior scale and job satisfaction scale.Results Work stress was negatively correlated with job satisfaction(r=-0.28,P＜0.01)and organizational citizenship behavior(r=-0.25,P＜0.01).Job satisfaction was significantly higher for employees who perceived high work control under certain work stress(β=0.16,P＜0.05).The relationship between work stress and organizational citizenship behavior was significantly stronger for employees who perceived high work control under certain work stress(β=0.19,P＜0.05).Conclusion The final results showed that work stress Was negatively correlated with organizational citizenship behavior.Work control had a positively moderate effect on the relationship between work stress and organizational citizenship behavior and the relationship between work stress and job satisfaction.

Background:The efficacy of traditional medicine on ulcerative colitis (UC) is often unsatisfactory, hence development of drug based on the pathogenic mechanism of UC becomes a hot topic in the research of UC.It has been revealed in recent studies that activation of nuclear factor-κB (NF-κB) is implicated as a key regulator in the immune and inflammatory responses in UC.Aims:To explore whether bortezomib, a potent proteasome inhibitor that inhibits NF-κB activation can be used for treatment of UC.Methods:Thirty-two BALB/c mice were used to induce acute experimental colitis by drinking 3%dextran sulfate sodium (DSS) freely for 7 days, and then randomly allocated into four groups injected intraperitoneally with 0.2 (low-dose group), 0.6 (medium-dose group), 1.0 mg/kg (high-dose group) bortezomib and normal saline (model control group), respectively.On the 7th day after treatment, the disease activity index (DAI) and histopathological change of colonic tissue were observed;the colitis-related parameters including peripheral blood hemoglobin (Hb), C-reactive protein (CRP) and colonic myeloperoxidase (MPO) activity were measured, and the nuclear translocation of NF-κB was assessed by electrophoretic mobility shift assay.Results:Compared with the model control group, the DAI, CRP, MPO activity, and injury score of colonic tissue were decreased gradually, and the Hb was increased gradually in mice treated with low-, medium-and high-dose bortezomib (P all <0.05).The efficacy of medium-and high-dose bortezomib was notable.In mice treated with medium-and high-dose bortezomib, nuclear translocation of NF-κB was inhibited obviously.Conclusions:Bortezomib can modulate the colonic inflammation in mice with experimental colitis by inhibiting NF-κB activation and subsequently improving the clinical manifestations, colitis-related parameters and tissue damage.Increasing the dosage of bortezomib in a safety range may enhance the treatment response.

Reference materials containing mixed degradation products of amoxicillin and ampicillin were developed after optimization of preparation processes. The target impurities were obtained by controlled stress testing, and each major component was identified with HPLC-MS and compared with single traceable reference standard each. The developed reference materials were applied to system suitability test for verifying HPLC system performed in accordance with set forth in China Pharmacopeia and identification of major impurities in samples based on retention and spectra information, which have advantages over the methods put forth in foreign pharmacopoeias. The development and application of the reference materials offer an effective way for rapid identification of impurities in chromatograms, and provide references for analyzing source of impurities and evaluation of drug quality.

Objective To preliminarily understand and evaluate the fertility ability of male infertility patients by analyzing the semen routine testing results to assist further clinical diagnosis and treatment .Methods 7 586 cases of semen routine detection re‐sults were performed the retrospective analysis and statistics by classification according to the indexes of semen volume ,pH value , liquefaction time ,sperm density ,sperm activity and sperm deformation rate .Results Among 7 586 cases of semen routine examina‐tion ,there were 1 450 cases(19 .1% ) of semen volume less than 2 .0 mL ;1 372 cases (18 .1% ) of pH < 7 .2 ;2 740 cases(36 .1% ) of liquefaction time > 60 min ;989 cases(13 .0% ) of sperm density lower than 20 × 106 /mL ;3 719 cases(49 .0% ) of sperm activity grade a + b less than 50% ;3 431 cases(45 .2% ) of sperm deformation rate less than 15% ;141 cases(1 .9% ) of aspermatism ;1 119 cases (14 .8% ) had no abnormality in each index .Conclusion Semen liquefaction ,sperm motility and sperm deformation rate are the indexes with the highest abnormal rate in the semen routine detection ;the semen routine detection provides the important exper‐imental data for clinical diagnosis and treatment of male infertility ,the objective and correct analysis of the detection result is of great significance to the judgment of male fertility .

Objective To compare the efficacy of total pelvic floor reconstruction with mesh patch and vaginal hysterectomy with anterior and posterior vaginal wall repair for presby-pelvic floor dysfunction patients.Methods We randomly chose 92 pelvic floor dysfunction inpatients of our hospital from September 2007 to February 2009 and divided them into 2 groups:total pelvic floor reconstruction group(n=70) underwent total pelvic floor reconstruction with mesh patch[12 patients of this group complicated with stress incontinence,4 patients received tension free vaginal tape-obturator(TVT-O) simultaneously];control group(n=22) received vaginal hysterectomy with anterior and posterior vaginal wall repair(3 patients of this group complicated with stress incontinence).A follow-up for 3 to 18 months after the surgery were taken in all the patients.Results In total pelvic floor reconstruction group,no recidivate case was found and the FOP scores and cure rate were 0 and 100% respectively.In control group,there were 7 recidivate cases,all of who were primary vagina anterior wall bulge patients.The cure rate of control group was 68%;For the patients complicated with stress incontinence,they were all cured by TVT-0 or by ascending the front patch in total pelvic floor reconstruction group,but 1 case was as before operation and 2 cases were even worse in control group.Conclusion Total pelvic floor reconstruction with vicarious materials had a definite efficacy for presby-pelvic floor dysfunction.

Objective To observe the dynamic changes of vascular endothelial growth factor (VEGF) levels in the patients with acute intracerebral hemorrhage during different phases and study the relationships between the VEGF levels and the volume of hemorrhage or stroke severity. Methods Fifty-six patients with acute intracerebral hemorrhage were divided into three groups by the volume of hemorrhage:group A(＜ 20 ml,23 caees),group B(20-30 ml, 18 cases),group C(＞ 30 ml, 15 cases) and light group (1-15 scores,22 cases),midrange group (16-30 scores,20 cases),severe group (31-45 scores, 14 cases)by China stroke scale (CSS) score in 24 hours of hospitalization. The serum VEGF level was determined at 24 h,48 h,72 h,7 d and 14 d after the onset of disease by enzyme linked immunosorbent assay(ELISA).Results The serum levels of VEGF during different phases (within 24 h,48 h,72 h,7 d,14 d) had statistical differences between group A and group C (P ＜0.01). Within 24 h,48 h,72 h,7 d,there was statistical difference between group B and group C (P ＜0.01).Within 48 h,72 h,7 d,14 d,there was statistical significance between group A and group B (P＜ 0.05). The serum levels of VEGF during different phases(24 h,48 h,72 h,7 d, 14 d) were higher in severe group than those in light group(P＜ 0.01). Within24 h,48 h,72 h,7 d,there was statistical difference between severe group and midrange group (P＜ 0.01).Within 48 h, 72 h, 7 d, 14 d, there was significant difference between midrange group and light group (P ＜0.05). The volume of hemorrhage had positive correlation with the CSS scores of neurologic impairment on admission (r = 0.916, P ＜ 0.05). Conclusion The serum VEGF levels might be related to the volume of hemorrhage and the severity of the disease.

AIM: To investigate the effect of Chinese propolis on the activity of phosphatidylcholine-specific phospholipase C (PC-PLC) and the expression of Toll-like receptor 4 (TLR4) in LPS-treated vascular endothelial cells (VECs). METHODS: Confluent VECs were stimulated with LPS at the concentration of 100 μg/L in the presence of 0.5% fetal bovine serum. The cells were treated with Chinese propolis at the concentration of 12.5 mg/L for 12 h and 24 h. The viability of VECs and the level of nitric oxide (NO) were detected by sulforhodamine B (SRB) assay and chemical method, respectively. The activity of PC-PLC was measured using L-α-phosphatidylcholine as substrate. The protein levels of TLR4, nuclear factor-Κb p65 (NF-Κb p65) and p53 were determined by Western blotting. The level of intracellular reactive oxygen species (ROS) was examined using a fluorescent probe, 2',7'-dichlorodihydrofluorescin (DCHF). For the measurement of mitochondrial membrane potential, the fluorescent dye JC-1 was used. RESULTS: Treatment with Chinese propolis for 24 h had no effect on the viability of VECs. However, the levels of NO and ROS were significantly decreased by Chinese propolis. PC-PLC activity and NF-Κb p65 expression were significantly depressed by Chinese propolis treated for 12 h, and the expression of TLR4 and p53 were dramatically decreased by Chinese propolis treated for 12 and 24 h. No effect of Chinese propolis on mitochondrial membrane potential was observed. CONCLUSION: Chinese propolis depresses the activity of PC-PLC and the expression of TLR4, and then inhibits the downstream signal molecules such as NF-Κb p65, p53, ROS and NO in VECs.

Objective To investigate the related clinical factors and homology of strains in Stenotrophomonas maltophilia (S. Maltophilia) infections in 15 patients with liver transplantation. Methods Fifteen patients with S. Maltophilia infection from September to December 2006 were enrolled and their clinical data were collected. Minimal inhibitory concentrations (MICs) of 10 antimierobial agents against S. Maltophilia were determined by Etest strips. Antibiogram was carried out by resistance analysis assembly with WHONET 5 software. The genomic DNA of all the isolates was digested with Xbal and subjected to pulse-field gel electrophoresis (PFGE). Results All patients received mechanical ventilation during the treatment and had a history of long-term use of extended-spectrum β-lactamases and quinolones. MICs of 10 antimicrobial agents indicated that S. Maltophilia were susceptible to several antimicrobial agents including compound sulfamethoxazole and ciprofloxacin, but the best active agent against these resistant isolates was minocycline in vitro. The results of all 15 S. Maltophilia antibiograms were accordance with PFGE patterns. All 15 S. Maltophilia isolates were classified as 2 PFGE patterns: 9 for pattern A and 6 for pattern B. Conclusion Mechanical ventilation might be associated with the S. Maltophilia septicemia in patients with liver transplantation.
