1.Accuracy of index of consciousness in monitoring depth of anesthesia with sevoflurane inhalation in children: a comparison with bispectral index

Objective To evaluate the accuracy of the index of consciousness (IoC) in monitoring the depth of anesthesia with sevoflurane inhalation in children,by comparing it with the bispectral index (BIS).Methods Forty-two ASA physical status Ⅰ or Ⅱ children,aged 8-12 yr,scheduled for elective abdominal surgery under general anesthesia,were studied.Anesthesia was induced with sevoflurane inhalation.The patients were endotracheally intubated.The concentration of sevoflurane was then adjusted to maintain the end-tidal concentration of sevoflurane (CETSev) at 1.6%,2.1%,2.6%,3.1%,3.6%,4.1% and 4.6% for5 min.The IoC and BIS values were recorded simultaneously.Results When CETSev < 3.1%,CETSev was negatively correlated with IoC and BIS values,and the correlation coefficients between CETSev and IoC and BIS values were-0.927 (r1) and -0.869 (r2),respectively (P < 0.01).When 3.1% ≤ CETSev ≤ 4.6 %,CET Sev was negatively correlated with IoC and BIS values,and the correlation coefficients between CETSev and IoC and BIS values were-0.808 (r3,P< 0.01) and-0.430 (r4,P < 0.05),respectively.Comparison of the absolute value of correlation coefficients:there was no significant difference between r1,r2 and r3 (P > 0.05) ; r1,r2 and r3 were significantly larger than r4 (P < 0.01).Conclusion IoC can accurately reflect the depth of anesthesia with sevoflurane inhalation in children and when 3.1% ≤ CET Sev ≤ 4.6 %,the accuracy of IoC in monitoring the depth of anesthesia is superior to that of BIS.

Protracted bacterial bronchitis(PBB)is one of the main causes of chronic cough in children.At present, the pathogenesis of PBB is mainly related to airway persistent bacterial infection, airway inflammatory response, immune dysfunction, airway deformity and airway microbial population disorder, especially airway persistent bacterial infection and airway inflammatory response.This reviwe summarizes the progress on the pathogenesis of PBB, in order to improve the understanding of clinical workers on PBB.
3.Role of JNK signaling pathway in hyperoxia-induced expression of connective tissue growth factor in A549 cells

Objective To investigate the role of c-Jun N-terminal kinase (JNK) signaling pathway in hyperoxia-induced expression of connective tissue growth factor (CTGF) in A549 cells.Methods The cultured A549 cells were seeded in 6-well culture plates at a density of 1 × 105 cells/ml (0.5 ml/well,24 wells in total) and were randomly divided into 4 groups (n =6 each) using a random number table:control group (group C);95% oxygen group (group 95% O2);room air plus SP600125 (JNK inhibitor) group (group C+SP);95% oxygen plus SP600125 group (group 95% O2+SP).SP600125 5 μmol/L was added to each well in C+SP and 95% O2+SP groups.The A549 cells were incubated for 48 h in an incubator filled with room air (C and C+SP groups) or 95% oxygen (95% O2 and 95% O2+SP groups).The expression of phosphorylated JNK (p-JNK) and CTGF mRNA was determined using Western blot and realtime reverse transcriptase polymerase chain reaction,respectively.Results Compared with group C,no significant change was found in the expression of p-JNK and CTGF mRNA in group C+SP (P>0.05),and the expression of p-JNK and CTGF mRNA was significantly up-regulated in group 95% O2 (P< 0.05).Compared with group 95% O2,the expression of p-JNK and CTGF mRNA was significantly down-regulated in group 95% O2+SP (P<0.05).Conclusion JNK signaling pathway activation is involved in up-regulation of hyperoxia-induced CTGF mRNA expression in A549 cells.
4.Efficacy and toxicity of S-1 in elderly patients with ad vanced non-small lung cancer

Objective To observe the efficacy and toxicity of S -1 monotherapy in elderly patients with advanced non -small lung cancer (NSCLC).Methods Fifty-six patients aged 70 or over with stage IIIb-IV NSCLC were enrolled.S-1 was given orally twice a day according to body surface area on days 1 to 14 for 21-day cycle, the evaluation of efficacy and toxicity were performed on all patients after 2 cycles.Results For all 56 patients, CR 0, PR 15, SD 29 and PD 23 were observed.The objective response rate (ORR) and the disease control rate (DCR) were 26.8% and 78.6%,respectively.The 1-year survival rate was 30.9%, with a medi-an survival of 10.0 months.The myelotoxicity and gastrointestinal toxicity were major side effects .The major toxicity could be tolerated by the patients.Conclusions S-1 monotherapy is an effective chemotherapy for advanced NSCLC in elderly patients with moderate side effects and safety in clinical practice .

6.Accuracy Assess of Five Criterims of Electrocardiogram for Locating Accessory Pathways in Patients With Wolff-Parkinson-White Syndrome

The resting 12 lead electrocardiograms (ECG) of 45 patients with Wolff-Parkinson-White syndrome and 30 normal persons were assessed by five criterias for locating the accessory pathway. Golden criteria was established by site of accessory pathway confirmed during epicardial mapping and absent accessory pathway from normal person. Compared five criterias of EGG with golden criteria, the idiosyncrasy of every criteria was 100% and the sensitivity of Rosenbaum's cri-terias, Gallagher's, WHO's, Lindsay's, Reddy's were respectively 93. 3% , 71. 1% , 64. 4% , 60% , 53. 3%. The preliminary feature for locating the accessory pathway of ECG was put forward from advantages of five criterias and observation of this report data.
7.The contents of Surfactant Protein A and D in BAL Fluids of Healthy Smokers and Healthy Nonsmokers

Objective To observe whether cigarette smoke alter the component of pulmonary surfactant pro-tein A and D. Methods In this study, we determined the contents of SP-A and SP-D in BAL fluids of healthy smok-ers and nonsmokers by enzyme linked immunosorbent assay(ELISA). Resluts The contents of SP-A and SP-D in BAL fluids were significantly decreased in smokers compared to those in nonsmokers[ (3.1±0.40) μg/ml vs (1.8± 0.4)μg/ml, (1.5±0.2) μg/ml vs (0.6±0.1)μg/m, all P <0.05]. Conclusion These results suggested that the decreased levels of SP-A and SP-D in smokers may impair the host defense functions of suffactant in the peripheral airways and might have a crucial role in the development of chronic obstructive lung disease.
8.An investigation and analysis for dietary pattern and action in 80 type 2 diabetics

Eighty patients with T2DM hospitalized at Tongren Hospital, Beijing during June to December 2009 were chosen for survey by FFQ (food frequency questionnaire).Their daily food intake and nutrient intake were collected and percentage of energy generated from carbohydrate, protein and fat were estimated for each participant, and analyzed based on the criteria of dietary intake for T2DM, the dietary guidelines and the recommendations for dietary nutritional structure for Chinese residents.Among 80 patients with T2DM, 27 (34%) were found insufficient daily protein intake, 39 (49%) insufficient carbohydrates,40 (50%) excessive fat, and 74 (93%) insufficient dietary fiber, respectively.There was a significant gap between their daily total energy intake and that recommended for patients with T2DM ( P < 0.01 or 0.05 ).No significant difference was found in daily intake of total energy, protein, fat carbohydrate and dietary fiber among T2DM patients with varied body mass index ( P > 0.05 ).Dietary pattern and actions and selfmanagement in the patients with T2DM surveyed are not so reasonable.
9.The relationship between vascular endothelial growth factor, endostatin and type 2 diabetic nephropathy

Objective To investigate the possible association between vascular endothelial growth factor (VEGF), endostatin (ES) and type 2 diabetic nephropathy. Methods The level of serum VEGF and ES were measured by ELISA in eighty type 2 diabetic patients including thirty subjects without diabetic nephropathy , twenty with microalbuminuria and thirty with macroalbuminuria, and the correlation was analyzed. Results The level of serum VEGF and ES in patients with macroalbuminuria were both significantly higher than that of patients with and without microalbuminuria (P < 0.05). The level of serum VEGF and ES in patients with microalbuminuria were significantly higher than that of patients without diabetic nephropathy (P<0.05). Both VEGF and ES level were positively correlated with the level of microalbuminuria (mAlb) (r=0.226, P<0.01; r=0.491, P<0.01), and negatively correlated with the level of estimated glomerular filtration rate (eGFR) (r=-0.237, P<0.05;r=-0.620, P<0.01). There was a significantly positive correlation between VEGF and ES in patients with microalbuminuria and macroalbuminuria (r1=0.633, P1<0.01; r2=0.532, P2<0.05). However, there was no correlation between VEGF and ES in patients without diabetic nephropathy (r3=0.175, P3>0.05). Conclusion The levels of VEGF and ES in type 2 diabetic nephropathy had varying degree of increasing in different diabetic nephropathy stage , and closely related to mAlb and eGFR. Disequilibrium of VEGF and ES may speed up the progression of diabetic nephropathy.
