1.Marriage status, marriage quality and marriage adaptation of inpatients with depression

ObjectiveTo understand the marriage status marriage quality and marriage adaptation of inpatients with depression.Methods 100 inpatients with depression met the diagnostic criterion of major depression of CCMD-3 and 100 health people were recruited,and investigated by using the self-developed marriage status questionnaire,Olson's marriage quality questionnaire,Loche Wallace marriage adaptation scale and HAMD.Results ( 1 ) The marriage and remarriage rates of the depressive inpatients in female were significant higher than those in male(P< 0.05).(2) The score of ENRICH in the depressive inpatients group was significant lower than that in the health control group(P< 0.01 ).(3)The rate of the well adaptation of marriage in the depressive inpatients group was 32.0%,which was lower than that in the control group(P< 0.01 ).(4) The total score of the EN-RICH was negative correlation with the total score of HAMD( r=- 0.419,P< 0.01 ).ConclusionThe marriage status of the depressive inpatients is not fine and maladatative,which is influenced by the depressive symptoms.
2.Effects of Lipo-PGE1 on the expression of T-cell specific transcription factors T-bet/Gata-3 in acutelung injury

Objective To investigate the effects of Lipo-PGE1 on the expression of T-bet and Gata-3,and its potential mechanisms causing the shift of T cells from Th1 to Th2 on Acute lung injury(ALI)induced by Lipopolysaccharide(LPS)in mice.Methods Sixty male BALB-C mice were randomly divided into three groups(n =20 in each group):(1)control group,mice were treated with intravenous injection of NS in dose of 10 ml/kg,(2)LPS group,mice were exposed to LPS with dosage of 5 mg/kg(0.5 g/ml diluted in saline),and(3)LPS + PGE1 group,mice were treated with Lipo-PGE1 in dose of 15μg/kg.Sixhours after injection,the lungs were removed for observing the histopathological changes and determination of wet/dry lung weight(W/D)ratio.The levels of Th1 and Th2 were determined by flow cytometry,and the expressions of T-bet and Gata-3 mRNA were detected by using RT-PCR.One-way ANOVA was used for comparing differences between groups,and all data were presented in((x)± s).Results The histological changes of lung injury were lessened by PGEC ompared with the W/D ratio(5.74 ± 0.31)in LPS group,the one(4.92 ±0.27)in LPS +PGE1 group was lower significantly(P <0.01).The levels of Th1 and Th2 and their ratio Were higher in LPS +PGE1 group[(20.31 ±2.20)%,(10.50±0.80)%,(1.93±0.05)]than in LPS group[(16.65 ±1.70)%,(9.40 ±1.25)%,(1.73 ±0.03)](P<0.01).Compared with control group,the expressions ofT-bet mRNA(1.183 ±0.495),and Gata-3 mRNA(0.693±0.285),and their ratio(1.713 ± 0.131)were lower(P <0.01); compared with LPS group,PGE1 significantly increased the expressions of T-bet mRNA(1.827 ± 0.705)and the ratio of T-bet/Gata-3 (2.502 ±0.352)(P <0.01),while didn(t)increased the expressions of Gata-3 mRNA(0.7191 ±0.186)significantly(P > 0.05).Conclusions Lipo-PGE1 may up-regulate transcription factor T-bet which participates in the Th1 differentiation ratio,and then improve the inflammatorv svmntom.
3.Effects of different head positions during operation on wake-up time and quality among ankylosing spondylitis patients with thoracolumbar kyphosis following pedicle subtraction osteotomy

Objective To investigate the effects of different head positions during operation on wake-up time and quality among ankylosing spondylitis (AS) patients with thoracolumbar kyphosis following pedicle subtraction osteotomy (PSO).Methods Sixty patients were included in this study.All of them,treated with intravenous anesthesia,took the bow-prone position.The patients were randomly divided into two groups:the experiment group where the head was elevated by 10° and the control group where the head was not elevated.The two groups were compared in respect of wake-up time and quality of wake-up test.Results The wake-up time in the experiment group was significantly shorter as compared to that of the control group (1 3.7±2.0 min vs.24.2±2.7min,P<0.05).The wake-up quality was better as well.Conclusion Elevation of the head by 10° can shorten wake-up time and improve the quality of wake-up test during the procedure of PSO for AS patients with thoracolumbar kyphosis.

5.Ethical Reflections on Hospital Administration

Based on their multiple years of personal experiences in hospital administration,authors of this article point out that medical service,humanistic service,and emotional service should be the core aspects in the current hospital competition.Medical professionalism embodied in the three levels of consciousness,behaviors,and regulations should be the main subject in the research on the role of medical ethics in hospital administration and medical treatment behaviors.It is an urgent ethical requirement and the chief task for hospital administrators to achieve humanistic medical service through humanistic administration of hospitals.
7.Countermeasure and Practice of Improving Connotative Quality of Electronic Medical Records

Objective To explore connotative quality of how to improve the electronic medical records. Methods Combining with the ways of improving connotative quality in fact, the advantages and common defects of electronic medical records were reviewed and some suggests were brought forward. Results The connotative quality of electronic medical records could be enhanced by specific quality standards, the implementation of quality control of section links and the emphasis quality control suited to expert's specialty, at the same time, the connotative quality of electronic medical records was raised by monitor methods of informationazition platform. Conclusion Improving the whole medical level of hospitals is the crux of raising the connotative quality of medical records.
8.Immune effect of propylthiouracil and methimazole in the treatment of Graves' disease

ObjectiveTo observe the changes in TSH receptor antibody (TRAb) and soluble intercellular adhesion molecule-1 ( sICAM-1 ) levels after treatment of propylthiouracil ( PTU ) and methimazole( MMI ) in patients with Graves' disease (GD). MethodsOne hundred and six cases of clinically diagnosed patients with GD were divided into PTU and MMI groups( each group, n =53 ). The patients in two groups were regularly followed for 36 months. TRAb and sICAM-1 were measured with ELISA method. Results( 1 ) The general data of two groups were not significantly different before treatment( all P>0. 05 ). ( 2 ) There was no difference in TRAb positive rates between two groups before treatment. The clinical remission rates between PTU and MMI groups showed no difference (83.02% vs88.68% ). The cure rate was higher in MMI group than in PTU group( 58.49% vs 37.74%, P< 0. 05 ). (3) There existed significant differences in TRAb levels determined before and 6, 12, 24,30, and 36 months after treatment( all P<0. 01 ), being gradually decreased with time( F=275.48 ,P<0.01 ). TRAb levels between two groups were significantly different( F=5.86, P<0. 05 ). (4) sICAM-1 levels at 36 months after treatment compared with the baseline in both groups were statistically different (P<0. 01 ), but no difference was found between two groups.ConclusionsBoth PTU and MMI improve the immune status of patients with GD,and the immunosuppressive effect of methimazole is more evident.
9.Graft injury and expression of redox factor-1 during early period after liver transplantation in rats

Objective To study the relationship between graft injury and expression of redox factor-1 in early period after liver transplantation in rats. Methods One hundred and fifty adult male Wistar rats were randomly divided into three groups: liver transplant group, sham surgery group and control group. Animals were sacrificed in each group at different time points: 3,6,9, 12,24 hour after liver transplantation. The changes and significance of the expression of Ref-1 were explored by immunohistochemistry, serology and histopathology. Results Compared with sham surgery group and control group, the expression of Ref-1 protein in transplant group was higher than that in other two groups in early period after liver transplantation ( P < 0. 05 ). In pathology there were lots of inflammation cells infiltration around the portal veins, and cell damage of hepatic tissue, while that in sham surgery and control group was comparatively slight. Serum ALT and AST values reached the peak at 6 ~ 12 h, and decreaced significantly after 12 h ( P < 0. 05 ). Conclusions Graft injury after liver transplantation during early period reaches peak at 6 hour and then decreased. Hepatic ischemic reperfusion injury promotes Ref-1 expression, which in turn compensates for programme cell death induced by the injury.
10.Transgenic Production of Polyunsaturated Fatty Acids in Mammalian Cells

Linoleic acid (C18∶2n-6) and ?-linolenic acid (C18∶3n-3) are found widely in fungi, plants and some lower animals. However, they can not be synthesized in mammals due to lack of △12 and ?-3 fatty acid desaturases. To enable endogenous production of essential fatty acids in mammalian cells, here the stable expression of a Caenorhabditis elegans gene FAT-2 encoding △12 fatty acid desaturase in CHO cells was reported. First, the FAT-2 coding sequence was cloned by RT-PCR. To facilitate high level synthesis of heterogeneous protein, the codon usage of the fatty acid desaturase genes was optimized according to the codon preference of mouse by site-directed mutagenesis, 2 synonymous mutations were introduced into FAT-2 gene by overlapping PCR. The codon-modified gene was finally fused to pBudCE4.1 vector (Invitrogen) under the control of CMV promoter. The expression vector pBudCE-FAT2 was linearized with NheⅠ, and then transfected CHO cells, the cells were under Zeocin selection for nine days and then propagated, then the transfected cells were harvested. The genome and total RNA were isolated for PCR and Norhern blot ananlysis. The results revealed that FAT-2 gene has been integrated into the genome of CHO cells and expressed properly. Fatty acids of total cellular lipids were analyzed by gas chromatography. The results indicate that the expression and function of △-12 fatty acid desaturase resulted in accumulation of linoleic acid. The levels of linoleic acid in transgenic cells were 2.4-fold higher than those in wild-type cells. The moderate linoleic acid in CHO cells was derived from cell culture media uptaken by cell membrane. The results demonstrate that a heterogenous desaturase gene can function well in mammalian cells and prove that transgenic approach is an efficient strategy for changing fatty acid composition of mammals.
