1.Risk factors for acute pancreatitis after minimally invasive pancreaticoduodenectomy and a prognostic analysis based on the 2022 International Study Group of Pancreatic Surgery criteria

ObjectiveTo investigate the risk factors for postpancreatectomy acute pancreatitis (PPAP) after minimally invasive pancreaticoduodenectomy (MIPD) and their association with major complications, to improve the understanding of PPAP and postoperative pancreatic fistula (POPF), and to reduce the incidence rates of PPAP and POPF. MethodsA retrospective analysis was performed for the clinical data of 114 patients who underwent MIPD in Department of Hepatobiliary and Pancreatic Surgery, The First Affiliated Hospital of University of South China, from March 2015 to December 2023, and the definition of PPAP and its diagnostic criteria provided by the International Study Group of Pancreatic Surgery were used to determine the presence or absence of PPAP. The independent samples t-test or the Mann-Whitney U rank sum test was used for comparison of continuous data between groups, and the Kruskal-Wallis H test was used for comparison between multiple groups, and the Dunn-test was used for further comparison between two groups.; the chi-square test or the Fisher’s exact test was used for comparison of categorical data between groups., and the Bonferroni-test was used for further comparison between two groups. The multivariate logistic regression analysis was used to identify the risk factors for PPAP after MIPD. ResultsOf all 114 patients undergoing MIPD, 27 (23.7%) experienced PPAP. The multivariate analysis showed that main pancreatic duct diameter ≤3 mm (odds ratio [OR]=5.083, 95% confidence interval [CI]: 1.703 — 15.172, P=0.004), soft pancreatic texture (OR=5.925, 95%CI: 1.986 — 17.677, P=0.001), and C-reactive protein ≥180 mg/L on day 1 after surgery (OR=5.419, 95%CI: 1.586 — 18.513, P=0.007) were independent risk factors for the onset of PPAP after MIPD. Clinical analyses showed that the patients with PPAP tended to have significantly higher incidence rates of POPF, peritoneal infection, Clavien-Dindo grade 3/4 complications, discharge with tube, and 90-day postoperative death (χ2=5.676, 5.460, 9.863, 5.439, and 4.207, all P<0.05). ConclusionMain pancreatic duct diameter, pancreatic texture, and C-reactive protein on day 1 after surgery are closely associated with the onset of PPAP after MIPD, and PPAP can lead to other postoperative complications and even death.
2.Variability of remnant cholesterol inflammation index exhibits a dose-response relationship with stroke risk:Evidence from the Chinese Kailuan cohort

Objective To investigate the association between the variability of remnant cholesterol inflammatory index(RCII),a novel composite biomarker,and the risk of stroke,in order to provide a theoretical basis for stroke prevention.Methods A prospective cohort study was conducted on 38 659 Kailuan individuals who took annual physical examinations in 2006,2008,and 2010.These subjects were grouped based on the quartiles of RCII variability,which was represented by standard deviation(SD)and average real variability(ARV),and were followed up every 2 years,with the occurrence of stroke(including ischemic and hemorrhagic strokes),death,or the end of follow-up on December 31,2022 as the endpoints.Kaplan-Meier method was used to calculate the cumulative incidence rate of endpoint events across different groups,and log-rank test was used to compare the difference of cumulative incidence of endpoint events in each group.Multivariate Cox proportional hazards regression model was adopted to analyze the association between RCII variability and risk of stroke.Results Among the 38 659 participants,a total of 2 539 strokes occurred during a mean follow-up period of 11.22±2.26 years.After adjusting confounding factors,when the participants were grouped by the quartiles of RCII-SD,the hazard ratio(HR)for stroke was 1.034(95%CI:0.917~1.167,P=0.584),1.146(95%CI:1.018~1.290,P=0.025),and 1.209(95%CI:1.066~1.370,P=0.003),respectively in the Q2,Q3,and Q4 groups,when compared with the Q1 group(Ptrend<0.05).When they were grouped by the quartiles of RCII-ARV,the HR for stroke was 1.008(95%CI:0.894~1.136,P=0.901),1.109(95%CI:0.986~1.248,P=0.085),and 1.152(95%CI:1.018~1.303,P=0.025),respectively,in the Q2,Q3,and Q4 groups,when compared with the Q1 group.Furthermore,both sensitivity and stratified analyses yielded similar results.Conclusion RCII variability is significantly associated with stroke,and the risk of stroke is gradually increasing with increment of the variability.Countermeasures Relevant authorities can focus on reducing RCII variability as a central objective by establishing regular monitoring mechanism,strengthening lifestyle interventions,and standardizing dietary,exercise,and weight management in order to suppress the index fluctuations.The principle of stable lipid-lowering in medication and optimization of therapeutic regimens with stable efficacy should be emphasized to prevent the risk of additional vascular damage.
3.Advances in Dual-response Adenosine Triphosphate Fluorescent Probes for Bioimaging

Adenosine triphosphate(ATP),as the core energy metabolism molecule in living systems,has dynamic changes closely related to fundamental physiological processes.To meet the urgent demand for spatiotemporal ATP detection in vivo and in situ,the development of highly sensitive multifunctional synchronous sensing fluorescent probes has become a recent research focus.These dual-function probes achieve fluorescence detection of dual targets by designing recognition sites for ATP alongside biological factors or microenvironment parameters such as reactive oxygen/nitrogen/sulfur species,metal ions,and enzymes,enabling physiological/pathological state correlation analysis through bioimaging.This paper systematically reviews recent advances in fluorescent probes for the collaborative detection of ATP and key biomolecules.It specifically examines probe construction strategies based on specific molecular recognition mechanisms(e.g.,metal coordination competition,electrostatic interactions,and host-guest recognition),multi-modal optical signal transduction mechanisms(ratiometric fluorescence,fluorescence lifetime,and photodynamic therapy),and their applications in pathological models such as oxidative stress monitoring,metal homeostasis imbalance,and enzyme activity co-detection.Finally,from the perspective of molecular probe engineering,current challenges and future research directions are proposed to provide methodological support for precise analysis of ATP-related life process regulation networks.
4.Analysis of HPV infection and subtypes in 21 316 patients in Dongguan

Objective To analyze the infection status and subtype distribution characteristics of human papillomavirus(HPV)patients in Dongguan area from 2017 to 2023,as well as the relationship between the subtype distribution of HPV-positive patients and their cervical histopathology.Methods A total of 21 316 patients who underwent HPV genotyping in Dongguan Maternal and Maternal and Child Health Hospital of Dongguan from 2017 to 2023 were selected as the research subjects.Cervical exfoliated cells were collected from females,and specimens from lesion sites were collected from males.The 23 types of HPV genotyping were performed on the exfoliated cell specimens of the patients by PCR-reverse dot hybridization.Statistically analyze the annual prevalence of HPV subtypes from 2017 to 2023,the viral infection status of patients of dif-ferent genders and age groups,as well as the relationship between the subtype distribution of HPV-positive patients and their cervical histopathology.Results The overall detection rate of HPV was 31.5%(6 719/21 316).The annual prevalence of HPV showed a significant downward trend from 2017 to 2023(P<0.001),and HPV types 52,16,58,53,and 51 were the most common.Female infections were mainly high-risk HPV types 52,16,58,53 and 51.Male infections were mainly low-risk HPV types 6 and 43 and high-risk HPV types 52,51 and 68.People under the age of 25 and those aged 55-<65 were high-risk groups for infection,with detection rates of 46.6%(668/1 432)and 42.3%(473/1 117),respectively.There was a statistically significant difference in the overall detection rate among different age groups(P<0.001).The HPV16 type increased significantly with the progression of cervical lesions from 15.0%(80/545)in patients with cervical intraepi-thelial neoplasia(CIN)Ⅰ to 36.3%(198/545)in patients with CIN Ⅱ-Ⅲ,and the difference was statistically significant(P<0.001).HPV16 is the main infectious subtype of CIN Ⅱ-Ⅲ.Conclusion The detection rate of HPV in Dongguan area shows a downward trend from 2017 to 2023,and it is age and gender-specific.HPV 16 is closely related to high-grade CIN in HPV genotyping.
5.Application prospects of POCT technology in the detection of infectious diseases in high-altitude areas

Point-of-care testing(POCT)has the characteristics of simple operation,low price,rapid detec-tion,no need for a fixed testing site,can be carried out next to the patient,and high applicability to operators and testing conditions.In recent years,POCT technology has been widely used in the medical field and has demonstrated certain clinical value,such as its application in disease screening,diagnosis,treatment monito-ring,and prognosis evaluation.The occurrence,spread and prevalence of infectious diseases in high-altitude ar-eas often have its particularity due to the special geographical,climatic and humanistic environment.Rapid and accurate detection of infectious diseases is not only an important part of the public health security system,but also crucial to the prevention and control of infectious disease outbreaks.This article discusses the characteris-tics of infectious diseases in high-altitude areas and the application of POCT technology in detection.

Intracranial atherosclerotic stenosis (ICAS) is a common cause of ischemic stroke. The evaluation of its structure and function is of great significance for formulating clinical intervention strategies. The indications for endovascular treatment of ICAS lesions in the past were mainly based on the degree of luminal stenosis showed by cerebral angiography, which had certain limitations. The translesional pressure ratio (PR), as an important indicator for functional assessment after arterial stenosis, has gradually received attention in the evaluation of ICAS lesions in recent years. This article reviews the evaluation methods and clinical significance of PR in ICAS lesions.
7.Study on quality evaluation of Alpiniae Katsumadai Semen based on HPLC specific chromatogram and chemometricsy

Objective:To determine the contents of four flavonoids by establishing HPLC specific chromatogram for Alpiniae Katsumadai Semen; To evaluate the differences of Alpiniae Katsumadai Semen from different producing areas.Methods:The specific chromatogram was developed on a column of Thermo Acclaim C18 with acetonitrile-0.1% phosphoric acid solution as the mobile phase by gradient elution at a flow rate of 1.0 ml/min. The detective wavelength was 260 nm, and the column temperature was 30 ℃. Similarity evaluation, PCA analysis, and OPLS-DA analysis were conducted. The contents of Alpinetin, Pinocembrin, Cardamonin, Alnustone in 16 batches of Alpiniae Katsumadai Semen.Results:There were 9 characteristic peaks in the specific chromatogram of Alpiniae Katsumadai Semen. Except the sample of S2 (Hainan producing area), the similarity of Alpiniae Katsumadai Semen in different producing areas was greater than 0.90; PCA analysis divided 16 batches of Alpiniae Katsumadai Semen into 2 categories, and OPLS-DA analysis identified 4 differential biomarkers, with the order of impact being peak 3>peak 5>Alpinetin>Cardamonin. Among them, the quality of Alpiniae Katsumadai Semen from Guangdong producing area was generally stable. Moreover, there were significant differences in the contents of Alpinetin and Cardamonin among the indicator components of Alpiniae Katsumadai Semen from different producing areas.Conclusion:This method can effectively analyze the differences in the quality of Alpiniae Katsumadai Semen from different producing areas, providing reference for the quality evaluation of Alpiniae Katsumadai Semen.
8.Relationship between polygenic risk scores for various psychiatric disorders and clinical and neuropsychological characteristics in children with attention-deficit/hyperactivity disorder.

OBJECTIVES:
To investigate the relationship between the polygenic risks for various psychiatric disorders and clinical and neuropsychological characteristics in children with attention-deficit/hyperactivity disorder (ADHD).
METHODS:
Using a cross-sectional design, 285 children with ADHD and 107 healthy controls were assessed using the Child Behavior Checklist, the Behavior Rating Inventory of Executive Function for parents, the Wechsler Intelligence Scale for Children, Fourth Edition, and the Cambridge Neuropsychological Test Automated Battery. Blood samples were collected for genetic data. Polygenic risk scores (PRSs) for various psychiatric disorders were calculated using the PRSice-2 software.
RESULTS:
Compared with the healthy controls, the children with ADHD displayed significantly higher PRSs for ADHD, major depressive disorder, anxiety disorder, and obsessive-compulsive disorder (P<0.05). In terms of daily-life executive function, ADHD-related PRS was significantly correlated with the working memory factor; panic disorder-related PRS was significantly correlated with the initiation factor; bipolar disorder-related PRS was significantly correlated with the shift factor; schizophrenia-related PRS was significantly correlated with the inhibition, emotional control, initiation, working memory, planning, organization, and monitoring factors (P<0.05). The PRS related to anxiety disorders was negatively correlated with total IQ and processing speed index (P<0.05). The PRS related to obsessive-compulsive disorder was negatively correlated with the processing speed index and positively correlated with the stop-signal reaction time index of the stop-signal task (P<0.05).
CONCLUSIONS
PRSs for various psychiatric disorders are closely correlated with the behavioral and cognitive characteristics in children with ADHD, which provides more insights into the heterogeneity of ADHD.

9.Taxifolin attenuates liver fibrosis by regulating the phosphorylation of NDRG1 at Thr328 via hepatocyte-stellate cell cross talk.

Taxifolin (TAX) is a natural compound known for its liver protection effect, but the mechanism remains unknown. Phosphorylated proteomics analyses discovered that the phosphorylation level of NDRG1 at T328 was a key event of TAX-improved liver fibrosis. We established models with NDRG1 knockout (KO) in vivo and in vitro, demonstrating that NDRG1 KO attenuated the development of hepatocyte injury, and combining NDRG1 KO and TAX administration did not result in a reduction in protection against liver injury. Cellular thermal shift assay and surface plasma resonance analysis showed that TAX directly binds to NDRG1 rather than its upstream kinase, subsequently demonstrating that TAX regulated phosphorylation of NDRG1 at T328 through binding to its C289 site. NDRG1 T328A (phosphorylated mutation) and T328E (mimic phosphorylation) in vivo and in vitro confirmed that pNDRG1T328 exacerbates hepatocyte injury along with DNA damage, inflammatory response, and apoptosis, thereby contributing to hepatic stellate cells (HSCs) activation. In contrast, TAX can inhibit the above pathological abnormalities and block hepatocyte injury-triggered HSCs activation and fibrosis. Overall, TAX is a potent liver protection drug primarily targeting NDRG1 and inhibiting pNDRG1T328 in hepatocytes.
10.Anterior Cingulate Cortex Contributes to the Hyperlocomotion under Nitrogen Narcosis.

Nitrogen narcosis is a neurological syndrome that manifests when humans or animals encounter hyperbaric nitrogen, resulting in a range of motor, emotional, and cognitive abnormalities. The anterior cingulate cortex (ACC) is known for its significant involvement in regulating motivation, cognition, and action. However, its specific contribution to nitrogen narcosis-induced hyperlocomotion and the underlying mechanisms remain poorly understood. Here we report that exposure to hyperbaric nitrogen notably increased the locomotor activity of mice in a pressure-dependent manner. Concurrently, this exposure induced heightened activation among neurons in both the ACC and dorsal medial striatum (DMS). Notably, chemogenetic inhibition of ACC neurons effectively suppressed hyperlocomotion. Conversely, chemogenetic excitation lowered the hyperbaric pressure threshold required to induce hyperlocomotion. Moreover, both chemogenetic inhibition and genetic ablation of activity-dependent neurons within the ACC reduced the hyperlocomotion. Further investigation revealed that ACC neurons project to the DMS, and chemogenetic inhibition of ACC-DMS projections resulted in a reduction in hyperlocomotion. Finally, nitrogen narcosis led to an increase in local field potentials in the theta frequency band and a decrease in the alpha frequency band in both the ACC and DMS. These results collectively suggest that excitatory neurons within the ACC, along with their projections to the DMS, play a pivotal role in regulating the hyperlocomotion induced by exposure to hyperbaric nitrogen.
