1.Role of multimedia teaching in clinical teaching of pediatric surgery

Multimedia teaching was not often used in pediatric surgery teaching in department of pediatrics.We used multimedia courseware to assist teaching in clinical teaching for 2007 grade seven-year-program students in Chongqing medical university.Results showed that this method improved students' learning enthusiasm,initiative and interests and laid a good foundation for the smooth development of their clinical practice.
2.Clinical Significance of Serum Cardiac Troponin in Patients with Dilated Cardiomyopathy

Objective To observe the serum cardiac troponin I (cTnI) level and to analysis the relations between the serum cTnI level and prognosis in patients with dilated cardiomyopathy (DCM) and Cardiac Dysfunction.Methods Serum cardiac cTnI level was measured by enzyme-linked immunosorbant assay. Results The serum cTnI level in DCM patients with class IV cardiac function (0.53 ?0.31) ?g/L was significantly higher than in DCM patients with class Ⅲ cardiac function (0.45?0.27) ?g/L.There was significantly difference in serum cTnI levels between DCM patients with class Ⅲ cardiac function and DCM patients with class Ⅱ cardiac function(0.29?0.27) ?g/L.Conclusion The higher serum cTnI level is correlated with the severity of cardiac function and may be useful for evaluating prognosis in patients with DCM.
3.Influence of laser positioning system to position repeatability in the use of improved belly board

Objective To investigate influence of laser positioning system to position repeatability of patients treated with radiotherapy for pelvic tumors.Methods Forty patients with pelvic tumors were divided into two groups,using laser positioning system positioning (20 cases) and conventional positioning (20 cases),respectively,CBCT was used to verify and analyze the setup errors of two groups.Results The setup errors on X,Y and Z axes in laser positioning system positioning group were (2.63±1.07) mm,(2.38±1.28) mm and (2.46±1.21) mm,they were (3.04±1.22) mm,(2.55±1.35) mm and (3.09±1.37) mm in conventional positioning group.It has significance reduce on X and Z axes (P =0.040,0.003,respectively) by using laser positioning system,and has no significance on Y axis (P =0.432).Conclusion It can improve patient positioning repeatability effectively by using laser positioning system for patient positioning.
4.Effects of recombinant human growth hormone on tumor growth and access relevant to Janus kinase 2-signal transducer and activator of transcription 3 of human gastric carcinoma xenografts in nude mice

Objective To investigate the effects of recombinant human growth hormone (rhGH) on tumor growth and tumor angiogenesis factor relevant to Janus kinase 2-signal transducer and activator of transcription 3 of human gastric carcinoma xenografts in nude mice with different expressions of growth hormone receptor (GHR).Methods Immunocytochemical method was used to pick out one GHR-positive and one GHR-negative cell line. The cells were subcutaneously injected into 26 nude mice separately, then the patterns of xenografts in nude mice with different expressions of GHR were established. The nude mice bearing two different kinds of human gastric caicinoma were equally randomized into control group, low-dose rhGH group, and high-dose rhGH group,and were treated with drugs for 14 days. Changes of tumor volumes and body weight of nude mice were record. The protein and mRN A expressions of tumor angiogenesis factor in tumor tissue were detected by RT-PCR and Western blot, respectively. Results GHR was highly expressed in SGC-7901 celk but negative in MKN-45 cells. For nude mice bearing GHR+ SGC-7901 xenografts, the tumor volumes were significantly larger in low-dose rhGH group [(1. 141 ±0. 234) cm3] and high-dose rhGH group [(2. 106 ±0. 260) cm3] than in control group [(0.612±0. 156) cm3] (P = 0. 034, P = 0. 001), and the high-dose rhGH group revealed greater effect (P =0. 043 ). Body weight was not significantly different among three groups. Compared with the control group, the mRNA expressions of tumor angiogenesis factor were significantly increased in low-does rhGH group, and the P values of GHR, Janus kinase 2, signal transducer and activator of transcription 3, vascular endothelial growth factor (VEGF), hypoxia-inducible factor-1α (HIF-1α), fibroblast growth factor, and matrix metalloproteinases-2 (MMP-2) was 0.001, 0.011, 0.042, 0.045, 0.040, 0.002, and 0.003, respectively; however, the high-does rhGH group did not show the greater effects. The protein expressions were significantly increased in low-does rhGH group, and the P value of phosphorylation-signal transducer and activator of transcription 3, signal transducer and activator of transcription 3, VEGF, HIF-1α, and MMP-2 was 0. 015, 0.003, 0.010, 0.008,and 0. 005, respectively; furthermore, the high-does group revealed the further greater effects, and the P value of VEGF, HIF-1α, and MMP-2 was 0.012, 0.025, and 0.046, respectively. On the contrary, for nude mice bearing GHR- MKN-45 xenografts, the body weights of low-dose rhGH group [(24.94 ±0. 517) g] and high-dose rhGH group [(26.97 ±0.686) g] were significantly higher than that of control group [(22.78 ±0.418) g] (P =0. 040, P = 0.012 ) , while tumor growth as well as the expressions of mRNA and protein of tumor angiogenesis factor in tumor tissue were not significantly different Conclusions rhGH can promote tumor growth and up-regulate the expression of tumor angiogenesis factor in the GHR-highly-expressed SGC-7901 xenograft tumor model; However,such effects do not exist in GHR-negatively-expressed MKN-45 xenograft tumor model. The existence of GHR may be a key target by which rhGH influences the tumor growth and the expressions of tumor angiogenesis factor, which is probably achieved through Janus kinase 2-signal transducer and activator of transcription 3 activation.
5.Effect of compound ezhu powder on serum levels of CA125 and CA19-9, and the expression of cyclin D protein in endometriosis patients.

OBJECTIVETo study the effect of Compound Ezhu Powder (CEP) on serum levels of CA125 and CA19-9, and the expression of cyclin D protein in endometriosis patients, thus providing theoretical evidence for clinical application of CEP.
METHODSTotally 69 all endometriosis patients underwent surgical treatment at Department of Gynecology and Obstetrics, Tianjin Nankai Hospital from January 2011 to January 2013 were randomly assigned to group A (35 cases) and group B (34 cases). Meanwhile, 30 patients with uterine fibroids who prepared for surgical treatment during the same period were recruited as the control group. Patients in group A took EZP 3 months before surgery. No treatment was given to patients in group B and the control group. The serum CA125 level and the expression of cyclin D in the ectopic endometrium and the eutopic endometrium were detected in the 3 groups before surgery.
RESULTSThe expression of cyclin D was higher in group A and group B than in the control group (P < 0.05). The serum levels of CA125 and CA19-9 were significantly lower in group A than in group B (P < 0.05). The expression of cyclin D in the ectopic endometrium was lower in group A than in group B, but with no statistical difference (P > 0.05). The expression of cyclin D in the eutopic endometrium was significantly lower in group A than in group B with statistical difference (P < 0.05). Meanwhile, the serum CA125 level was positively correlated with the serum CA19-9 level (r = 0.45, P < 0.05).
CONCLUSIONSThe expression of cyclin D obviously increased in endometriosis patients, which was associated with the occurrence of endometriosis. CEP could lower serum levels of CA125 and CA19-9, and down-regulate the expression level of cyclin D, indicating its roles in inhibiting the cell cycle.

6.Clinical therapy and pathological observation of cyclosporine A-induced gingival overgrowth

Objective To explore the clinical therapy for cyclosporine A(CsA)-induced gingival overgrowth (GO) and the pathological changes in gingival overgrowth tissues.Methods Nine cases of CsA-induced GO after renal transplantation were subjected to periodontal non-surgical treatment and surgical treatment.Under light and electron microscopy,the pathological changes in CO tissues were observed.Results The bleeding index(BI) and the plaque index(PLI) of patients were declined after periodontal treatment.GO recurred in 2 patients 6 months later and happened to recur in all 9 patients 12 months later(GOD≤1).At 18th month after transplantation,an obvioUS GO(GOD≥2)occurred in one patient,and re-operation was done to cut hyperplastic gingiva.At 48th month during the observation period,GO existed continuously but no more than 2 in GOD.There were 3 other patients who had their GO(GOD≥2)at 24th month after peridental treatment and re-operation was carried out to remove the hyperplasic gingivaL Under a light microscope,epithelial pegs constituted of basal cells and prickle cells elongated and presented as cancellation structure;spinus layer thickened:hyperkeratosis or parakeratosis occurred in cuticular layer where inflammatory cells infiltrated:collagen increased in proper layer.Under the transmission electron microscopy,the volume of fibroblasts in hyperplastic gingival tissues was increased,rough endoplansmic reticula in the intracytoplasm were abundant and expanded slightly,and there were a few of the apoptotic fibroblasts in the early stage.Conclusion BI and PLl were declined in patients taking CyA for a long-term who were subjected to periodontal and surgical treatments.GO recurred in some patients.The proliferation and differentiation of fibroblasts was not observed in hyperplastic gingival tissues.
7.Experimental study of rabbit segmental radial defects repaired by mesenchymal stem cells composite grafts

[Objective]To discuss the tissue engineering method of treating bone defects through rabbit bone marrow mesenchymal stem cells(MSCs)'culture in vitro and make composite grafts with anti-extracted bovine cancellous bone(BCB).The composite grafts were implanted into segmental radial defects in rabbits was compared on the curative effect of repairing segmental bone defects in rabbits with the composite grafts.[Method]The purified,culture-expanded MSCs were combined with BCB in vitro according to condition of cell culture.The composite grafts were tested by scanning electron microscope.An bone defect(15mm in length) was created at two radii in each rabbit.The composite grafts were implanted into segmental radial defects in rabbits through open operation.The curative effect was evaluated by radiographic examination,histologic analysis,biomechanical test after surgery in experimental groups and control's and blank's.[Result]Roentgenographically,the bone defects that had been treated with grafts exhibited new bone formation increased with time.But the result of experimental groups were apparently superior to that of the control groups′s at 2、4、8、12、16 week after operation(P
9.Mediating effects of self feeling of inadequacy between resilience and alexithymia in depressive undergraduates

Objective To study the relationship among resilience,self feeling of inadequacy and alexithymia in depressive undergraduates,and to provide a theoretical basis for the prevention and intervention of depression among undergraduates.Methods 500 undergraduates of two universities in Hebei Province were collected by cluster sampling method.Beck depression scale,adolescent resilience scale,Toronto alexithymia scale and self feeling of inadequacy scale were applied to undergraduates.The survey results were analyzed statistically.Results The detection rate of depression was 28%(119/426) in undergraduates.Total score of Alexithymia Scale(69.99±9.43) was higher than the norm(65.70±7.98),and the difference was statistical significant (P<0.05).The score of self feeling of inadequacy had a significant difference between male (149.88±28.00) and female(138.58±28.79) (P<0.01).Neither gender nor grade had significant difference in depression,alexithymia and resilience(P>0.05).Self feeling of inadequacy was positively correlated with alexithymia (P<0.05).Resilience was negatively correlated with self feeling of inadequacy and alexithymia(P<0.05).For the depressive undergraduates,self feeling of inadequacy served as a full mediator between the resilience and alexithymia,whereas for the non depression undergraduates it was a partial mediator.Conclusion Depression undergraduates have serious alexithymia,serious self feeling of inadequacy and poor resilience.Self feeling of inadequacy serves as a mediator between the resilience and alexithymia in depressive and non-depressive undergraduates.
10.An electrophysiological study of Riche-Cannieu anastomosis

Objective To explore the optimal electrophysiologieal approach for detecting Riehe-Cannieu anastomosis(RCA),an anomalous anastomosis between the deep branch of ulnar nerve and the recurrent branch of the medial nerve in the palm of the hand,and to estimate its incidence. Methods One hundred subjects(56 male,44 female,mean age 37.8 years)without any hand motor or sensory dysfunction were selected randomly.The ulnar nerve was stimulated at both the elbow and wrist,and recordings were made from the abductor pollicis brevis,which is normally innervated by the medial nerve,to document any compound muscle action potentials(CMAP).CMAP recorded from both points during stimulation is an accepted indicator of RCA.Group A comprised 40 hands of 20 subjects,while group B included 160 hands of 80 subjects.Surface electrode stimulation was used in both groups.Surface and needle electrode recording was used in group A,while only needle electrode recording was used in group B.Results In group A,31 hands of 16 subjects were found to have RCA by means of surface electrode recording,but only 6 hands of 3 subjects were found to have RCA by means of concentric needle electrode recording.There was a difference of up t0 80.6% between results obtained by the 2 recording methods.In group B,35 hands of 20 subjects were found to have RCA.A total of 41 hands of 23 subjects among the 100 were found to have RCA when concentric needle electrode recording was used(20.5%incidence). Conclusion The type of recording electrode influences the accuracy of RCA examination.An accurate and reliable result can be obtained by using a concentric needle electrode.The abductor pollicis brevis can be anomalously innervated by the ulnar nerve because of RCA.When both the medial and ulnar nerve have been injured.RCA might result in anomalous clinical symptoms and electrophysiological findings.Thoroughly understanding this anomaly is of crucial importance in the clinical evaluation and diagnosis of medial or ulnar nerve injury,as well as to avoid mistakenly interpreting the electrophysiological data when Riche-Cannieu anastomosis is present.
