3.Application of parenteral nutrition in the elderly postoperative patients with gastric carcinoma

Objectives:To observe the role of parenteral nutrition(PN) in the elderly postoperative patients with gastric carcinoma. Methods:Forty cases were randomly divided into two groups. One group received PN after operation as PN group and another group was supported only with the general treatment without PN as control group. Results:①The rate of postoperative complications was 15% (3/20) with no death in PN group, and it was 45%(9/20) with mortality rate of 5%(1/20) in control group. ② Between the two groups, there were very significant differences in nitrogen balance, body weight, creatinine height index(CHI), serum albumin, serum transferrin and blood urea nitrogen(BUN) at the 8th postoperative day( P
4.Study on Purification of Swertiaman from Swertia pesudochinensis Hara. with Macroporous Adsorbent Resin

Objective To explore the suitable type of macroporous adsorbent resin to purity swertiaman from Swertia pesudochinensis Hara., and determine the parameters and conditions for its purification procedures. Method Purification effect of 8 types of macroporous resins used to purify the Swertia pesudochinensis Hara. were inspected, with the content of swertiaman as index. Result HPD-100 type resin showed better effect than others, it had optimum adsorption and elution parameters with its dynamic saturated adsorption ratio was 70.48 mg/g. After eluted with 2 BV of distilled water, 4 BV 30% ethanol and collected, the content of swertiaman in the spray dried powder was 52.29%. Conclusion HPD-100 types macroporous resin showed a better comprehensive adsorption property and can be used to enrich and purify the swertiaman included active fraction of iridoid glycosides for industrialization.
5.Design and clinical application of “exclamation mark” shaped pedicle flaps

Objective To investigate effects of pedicle processing methods on the survival and appearance of the pedicle flaps. Methods The study involved 17 patients with soft tissue defects of extremities treated with the “exclamation mark” shaped pedicle flaps from October 2009 to January 2011.The traditional pedicled rotation flaps was designed to be a tennis racket-like shape,then the rotational pedicle was designed to small round tension reduction flap and pedicled rotation flaps,which contributed to formation of the “exclamation mark” shaped pedicle flaps.The soft tissue defects was located at the upper extremity in three patients and at lower extremity in 14,with defect areas of 4.0 cm× 7.2 cm-5.8 cm × 14.0 cm.There used two superficial radial nerve vasotrophic flaps,one ulnar artery supra-carpal cutaneous flap,eight sural neurocutaneous vasotrophic flaps and six saphenous nerve vasotrophic flaps.Results All flaps survived at Ⅰ stage except that one flap had partial distal necrosis postoperatively,which was cured after dress change.The follow-up ranged from six months to one year,which showed unobvious swelling of the survived flaps and good appearance of the rotation point of the pedicle. Conclusion The “ex(c)lamation mark” shaped pedicle flaps can reduce entrapment of the distal pedicle,promote venous drainage,improve survival and modify the appearance of the flaps.
6.Clinical study on treatment of adult T lymphoblastic lymphoma with Hyper-CVAD regime

Objective To improve the recognition of treatment of adult T lymphoblastic lymphoma (TLL) with Hyper-CVAD regime.Methods The turnovers of 7 cases treated with Hyper-CVAD regime were summarized.Results Among the 7 cases,1 case died after the first unfinished chemotherapy,1 case gained complete remission before autologous stem cell transplantation but relapsed after transplantation,3 cases gained complete remission until now,2 cases without transplantation relapsed and then gave up following treatments after treating with ICE regime or the virgin regime,but the therapeutic effects were poor.Conclusions Adult TLL should get early chemotherapy treatments,and some special characteristics should be noted,the treatment does not relate to the stage,the chemotherapy treatment should be carried out regularly and enough,radiation of mediastinum mass can be applied after 4 cycles of Hyper-CVAD regime,choose suitable transplantation method.
7.Investigation on Chikungunya virus infection in humans in Cangyuan County, Yunnan Province

Objective To preliminarily understand the infection of Chikungunya virus (CHIKV) in Cangyuan County, a southern border area of Yunnan Province, and provide a reference basis for the prevention and control of Chikungunya fever. Methods In April 2020, a total of 400 serum samples from individuals seeking medical care at the People's Hospital of Cangyuan County in Yunnan Province were collected. Among these, 121 samples were from healthy individuals undergoing physical examinations, and 279 samples were from patients with fever. The serum samples collected underwent CHIKV neutralizing antibody testing using a serum micro-neutralization assay. Real-time fluorescence quantitative RT-PCR was used to detect CHIKV nucleic acid in the samples, followed by analysis of the test results. Results The results of neutralizing antibodies showed that 18 of the 400 human serum samples were positive for neutralizing antibodies against CHIKV, with an overall positivity rate for serum samples of 4.5% (18/400). Among the 279 serum samples collected from patients with fever, 18 were positive for neutralizing antibodies against CHIKV, with a positive rate of 6.45% (18/279), and the neutralizing antibody titers ranged from 1∶10 to 1∶320. The results of 121 healthy human serum samples were negative for neutralizing antibodies against CHIKV. The results of real-time fluorescence quantitative RT-PCR showed that 3 of the 400 human serum samples were positive for CHIKV nucleic acid, and the positive rate was 0.75% (3/400). Among the 279 serum samples collected from patients with fever, 3 samples were positive for CHIKV nucleic acid, with a positive rate of 1.08% (3/279), and Ct values ranged from 36.58 to 37.74. While all healthy human serum samples were negative for CHIKV nucleic acid. Conclusions The findings indicate that infection of CHIKV exists in the population of Cangyuan County, a southern border area of Yunnan Province, and an outbreak of the disease is occurring. Therefore, it is necessary to strengthen the monitoring, prevention, and control of CHIKV in this area.
8.Application study of sequential severity evaluation in emergency nursing for stroke patients

Objective To explore the clinic value of sequential severity evaluation in emergency nursing for stroke patients. Methods 138 of stroke patients were enrolled in study group, which completed sequential severity evaluation in emergency medical and nursing provision drawn up according to the results. Another 207 of stroke patients were enrolled in control group, which emergency nursing provision drawn up according to general procedure. Length of time from emergency call to special therapeutic, mortality compared between the two groups respectively. Results In study group, length of time from emergency call to special therapeutic (48.9?34.1) min was significantly shorter than that of control group (73.1?46.7) min; mortality (11.1%) was significantly lower than that of control group (24.3%),cure rate (34.7%) was significantly higher than that of control group (26.9%). Conclusion Sequential severity evaluation in acute nursing may be a worthy procedure for proving reaction ability of nurse reaction ability in emergency medical and nursing, proving outcome for stroke patients and it should be commended.
9.Forensic Psychiatric Assessment for Organic Personality Disorders after Cranio-cerebral Trauma

Objective T o explore the occurrence and the differences of clinical m anifestations of organic personality disorder w ith varying degrees of craniocerebral traum a. Methods A ccording to the Interna-tional C lassification of D iseases-10, 396 subjects w ith craniocerebral traum a caused by traffic accidents w ere diagnosed, and the degrees of craniocerebral traum a w ere graded. T he personality characteristics of all patients w ere evaluated using the sim plified N euroticism E xtraversion O penness Five-Factor Inventory (N E O-FFI). Results T he occurrence rate of organic personality disorder w as 34.6% w hile it w as 34.9%and 49.5% in the patients w ith m oderate and severe craniocerebral traum a, respectively, w hich signifi-cantly higher than that in the patients (18.7% ) of m ild craniocerebral traum a (P<0.05). C om pared w ith the patients w ithout personality disorder, the neuroticism , extraversion and agreeableness scores all show ed significantly differences (P<0.05) in the patients of m ild craniocerebral traum a w ith personality disorder; the neuroticism , extraversion, agreeableness and conscientiousness scores show ed significantly differences (P<0.05) in the patients of m oderate and severe craniocerebral traum a w ith personality disor-der. T he agreeableness and conscientiousness scores in the patients of m oderate and severe craniocerebral traum a w ith personality disorder w ere significantly low er than that of m ild craniocerebral traum a, and the patients of severe craniocerebral traum a had a low er score in extraversion than in the patients of m ild craniocerebral traum a. Conclusion T he severity of craniocerebral traum a is closely related to the in-cidence of organic personality disorder, and it also affects the clinical features of the latter, w hich pro-vides a certain significance and help for forensic psychiatric assessm ent.
10.Laparoscopic surgery for benign splenic lesions

Objective To study the safety and feasibility of laparoscopic total splenectomy (LTS) and laparoscopic partial splenectomy (LPS) in the diagnosis and treatment of benign splenic tumors by comparing the perioperative and follow-up data between these two operative approaches.Methods We retrospectively analyzed the clinical data of 57 patients who underwent laparoscopic surgery for benign splenic tumors from December 2009 to April 2016.These patients were divided into the LTS and the LPS groups.The clinical data including the preoperative,intraoperative,postoperative and follow-up data were analyzed.Results When compared with the LPS group,the LTS group had shorter operation time [(97.0 ± 22.1)min vs.(135.0 ± 24.6) min,P < 0.05] and less blood loss [(33.3 ± 19.5) min vs.(90.6 ± 55.1)min,P < 0.05],but there were no significant differences between the two groups in the length of hospital stay,the duration of drainage tube placement,the amount of postoperative analgesics as well as the incidences of pulmonary infection,postoperative pancreatic fistula and other complications after surgery.These incidences were low in the two groups.Conclusion Laparoscopic surgery,including LTS and LPS,were safe and feasible in treating benign splenic tumors.
