1.Active Immunotherapy with Anti-Idiotypic Antibody for Nasopharyngeal Cancer

According to the immune network hypothesis proposed by Jeme, certain anti-idiotypic antibodies (Ab2) express three dimensional shapes which resemble the structure of natural antigens. Here we propose a study of active immunotherapy by Ab2 for patients with Nasopharyngeal Cancer (NPC) . Two Ab2,designated 2H4 and 5D3,against two Abl (FC_2 and HNL5)that recognize NPC associated antigen were generated. They could substitute NPC antigen to induce humoral and cellular immune response against NPC cells in syngeneic mice. Nineteen patients with NPC at stage IV were chosen for active immunotherapy. They were treated with Alum-precipitated 2H4 or 5D3 accompanying radiotherapy. Nine patients with radiotherapy alone were as control. Both anti-anti-idiotypic antibodies (Ab3) and anti-NPC antibodies (Abl') were increased and human anti-mouse antibodies (HAMA) occurred in nineteen patients of the experimental group; whereas the levels of Abl' did not rise in control group.Serum IL-2, IFN-? and TNF-? level were increased in most patients in experimental group. While in the control group, there was no difference of cytokine level between pretherapy and post- therapy.
2.Messenger RNA expression of IL-6 and IL-8 in allergen-induced late-phase cutaneous reactions (LPR) in Schneiderian membrane subjects

Objective:To detect mRNA expression of IL-6 and IL-8 in allergen-induced late-phase cutaneous reactions in schneiderian membrane subjects. Methods:Cryostat sections from rhinitis biopsies from 24 h allergen-induced late-phase cutaneous reactions (LPR) in 10human atopic subjects were hybridization with 35S-labeled RNA probes for IL-6 and IL-8.Results:mRNA was detected for IL-6 (9/10) and IL8 (10/10).Compared with the control, there were significant increases in the numbers of ce11 expreasing mRNA expression for IL-6 and IL-8(P<0.05, P<0.01). Conclusion: The augmentation of mRNA expression of IL-6 and IL-8 maybe regarded as the mark of rhinits in IL PR.
3.Treatment of 185 cases of ureteral stones with ureteroscopic pneumatic ballistic lithotripsy

Objective To evaluate the clinical effectiveness of ureteroscopic pneumatic ballistic lithotripsy for the treatment of ureteral stones.Methods A total of 185 patients with ureteral stones(accompanying renal colic in 96 patients) were treated with ureteroscopic pneumatic ballistic lithotripsy from February 2004 to March 2005 in this hospital. Results The procedure failed in 12 patients,including conversions to open surgery in 6 patients and to extracorporeal shock wave lithotripsy 3 days later in 6 patients.The single-session success rate was 93.5%(173/185),and was 75.0%(24/32) in the upper segment,95.8%(46/48) in the middle segment,and 98.1%(103/105) in the lower segment.The success rate in patients with renal colic was 100%(96/96).The rate of intraoperative ureteral injury was 2.9%(5/173),with conversions to open surgery required in 3 patients(1.7%).Postoperative renal colic occurred in 1 patient.Follow-up checkups in all the patients for 6~12 months(mean,10.2 months) showed no recurrence. Conclusions Ureteroscopic pneumatic ballistic lithotripsy is safe and effective,being the first choice for patients with middle or lower ureteral stones,especially accompanying renal colic.
4.Combined inhibition of Stat3 and HIF-1α for enhancement of radio- and chemo-sensitivity in laryngeal squamous cell carcinoma.

OBJECTIVE:
To investigate the influence of signal transducer and activator of transcription 3 (Stat3) and hypoxia-inducible factor-lα (HIF-1α) on the resistance effect of laryngeal squamous cell carcinoma to radiation therapy and chemotherapy under the hypoxia circumstances.
METHOD:
Western blot was used to test the expression of p-Stat3 and HIF-1α in the Hep-2 cells under the hypoxia conditions. MTT assay was used to test the proliferation of Hep-2 cells after radiation therapy and chemotherapy; the Hep-2 cells were suppressed expression of Stat3 and/or HIF-1α.
RESULT:
(1) AG490 induced significant proliferation inhibition on Hep-2 cells and Hep-2HIF-1α-/- cells in vitro underthe hypoxia environments (P < 0.05); (2) Suppressing expression of Stat3 reduced the expression of HIF-1α protein (P < 0.05); (3) Combined inhibition of Stat3 and HIF-1α enhanced radio- and chemo-sensitivity in laryngeal squamous carcinoma cells under hypoxia.
CONCLUSION
Combined inhibition of Stat3 and HIF-1α can further enhance radio- and chemo-sensitivity in laryngeal squamous carcinoma cells under hypixia compare than inhibiting Stat3 or HIF-1α alone. Effectively blocking of HIF-1α pathway and suppressing the expression of Stat3, would be an effective method to enhance radio- and chemo-sensitivity in laryngeal squamous carcinoma cells, which provides a new thought to reduce the resistance to treatment.

5.Correlation between expression of ERCC1 and the treatment of cisplatin-based chemotherapy in local advanced nasopharyngeal carcinoma.

OBJECTIVE:
To study the expression of excision repair cross-complementing 1 (ERCC1) and the treatment of cisplatin-based chemotherapy in local advanced nasopharyngeal carcinoma (NPC).
METHOD:
The expression of ERCC1 protein in 107 cases with NPC and in 48 normal nasopharyngeal tissues adjacent to the cancer was detected by immunohistochemical method.
RESULT:
High expression of ERCC1 was observed in 52 cases with NPC, and 18 cases normal nasopharyngeal tissues, there was no statistically significant differences between them. The expression of ERCC1 protein was significant correlated with patient total TNM stage, but not significantly correlated with age, gender, histological type, T stage and N stage. The recent treatment efficiency in Low ER-CCl expression cases was higher than high expression cases. There was statistically significant difference between them. In 97 follow-up cases, 2 cases died, 5 cases with liver and lung metastasis, there was no statistically significant difference between them.
CONCLUSION
ERCC1 expression maybe regarded as indicator platinum based chemotherapy sensitivity prediction in nasopharyngeal carcinoma, and also helpful for formulating individualized treatment. The immunohistochemical detection is also simple and effective detection method for ERCC1 expression.

7.Effect of Ondansetron on postoperative nausea and vomiting in patients undergoing laparoscopic cholecystectomy with general anesthesia

Objective To investigate the influence of Ondansetron on postoperative nausea and vomiting(PONV)in patient undergoing laparoscopic cholecystectomy with general anesthesia and the nursins care. Methods Ninety patients undergoing laparoscopic cholecystectomy with general anesthesia were randomly divided into 3 groups(n=30):low-dose Ondansetron group(O1 group)received 4 mg Ondansetron before section,high-dose Ondansetron group(O2 group)received 8 mg Ondansetron before section and the control group(C group)received normal saline.All injections were diluted to 4 ml.The number and degree of nausea and vomiting wag recorded during 24 h after operation.Results Number of patients with PONV in O1 group and O2 group were less than that of C group,P<0.05,so was the PONV degree.No statistical difference was seen between O1 group and O2 group.Conclusion Intravenous injection of Ondansetron during operation can decrease the PONV in patient undergoing laparoscopic cholecystectomy with general anesthesia.
8.Expression of coagulation-and fibrinolysis-related molecules of endothelial cells induced by activated complement alternative pathway and intervention

Aim To investigate the change of molecu-lar expression related to coagulation and fibrinolysis in human microvascular endothelial cells ( HMEC ) in-duced by activated complement alternative pathway and effect of pyrrolidine dithiocarbamate ( PDTC ) and res-veratrol on intervention. Methods Normal human se-rum was activated by cobra venom factor ( CVF) . After exposure of HMEC to activated complement for various times, supernatant was removed and assayed for ex-pressions of P-selectin, VWF, t-PA, PAI-1, TF, TM, and NO by using reagent kits. The expressions of the above molecules in HMEC pretreated with PDTC and resveratrol were also investigated. Results P-selectin and VWF were rapidly released by endothelial cells and the expression reached the peak at the time point of 15 min. The expressions of t-PA, PAI-1, and TF were continuously upregulated, whereas NO and TM were decreased. PDTC and resveratrol inhibited the upregulation of P-selectin, VWF, t-PA, PAI-1 and TF, and intervened the downregulation of NO. Res-veratrol further downregulated the expression of TM. Conclusion Activated complement alternative path-way can influence the expression of molecules related to coagulation and fibrinolysis in HMEC, and PDTC and resveratrol can affect this change.
9.Preparation of rmOP-1 and its protective effect on renal ischemia reperfusion injury

Objective To investigate the protective effect of recombinant mouse OP 1 (rmOP 1) on renal ischemia reperfusion injury. Methods The full length open reading frame of mouse OP 1 was cloned by RT PCR. The rmOP 1 was prepared and its osteogenesis was assayed in vitro. Thirty Wistar rat models of renal ischemia reperfusion were randomly divided into 3 groups: preventive group, therapeutic group and control group. The former two groups were administered with rmOP 1 (250 ?g/kg) via the superior mesenteric vein at different time points. The renal function and structure were assessed by serum BUN, Cr, histological findings and the modified Miller's scoring. Results rmOP 1 showed osteoinductive activities in vitro. The level of serum BUN, Cr and Miller's scoring were higher in control group than the others ( P 0.05 ). Histological finding showed that rmOP 1 minimized the cell necrosis. Conclusion rmOP 1 has an protective effect on renal function and structure.
10.The clinical effect of high frequency repetitive transcranial magnetic stimulation combined with cognitive intervention for stroke patients with aphasia

Objective: To discuss the applied effect of high frequency repetitive transcranial magnetic stimulation combined with cognitive intervention for stroke patients with aphasia. Methods: 84 stroke patients with aphasia were divided into observation group (42 cases)and control group (42 cases). Patients of control group were treated by routine intervention of language training, while patients of observation group were treated by high frequency repetitive transcranial magnetic stimulation combined with cognitive intervention. A series of evaluated indicators, such as the language function score of Boston diagnostic aphasia examination(BDAE), the anxiety symptom score of Hamilton anxiety scale(HAMA), the quality of life and degree of satisfaction, were adopted to compare the post-treatment effect for the two groups. Results: After the treatment, there were statistical significance in the difference of understanding, retelling ability, reading ability and writing ability about BDAE scores between the two groups (t=3.572, t=4.482, t=6.238, t=3.013; P<0.05), and the HAMA scores of observation group was significant higher than those of control group (t=3.472, P<0.05). Besides, through different interventions, the series of indicators about quality of life, such as physical function, mental function, social function and material function, of observation group were higher than those of control group (t=4.513, t=3.923, t=4.473, t=4.033; P<0.05). And the satisfaction of observation group was significant higher than that of control group (x2=5.233, P<0.05). Conclusion: The clinical effect of high frequency repetitive transcranial magnetic stimulation combined with cognitive intervention for stroke patients with aphasia is favorable. And this method can increase quality of life and satisfaction for patients, and can extremely reduce medical dispute and improve the therapeutic level of hospital for stroke patients with aphasia.
