Objective The severe hypospadias is present with complicated deformity,and the successful rate of treatment is low with high complication rate.Here is to present a kind of useful and reliable treatment for it.Methods From 2004 to 2005,the scrotal septal island skin flap combined with the scrotal split thickness skin graft to form the neourethra in a stage was used to treat the hypospadias without underdeveloped scrotum.Results A total of 15 patients including proximal penile,penioscrotum and scrotal hypospadias who were followed up for about 1 year healed well without the occurrence of urethral stricture and urethral fistula,the penile curvature was corrected and the urine line was direct,and the endoscopic examination showed the inner wall of the urethra was smooth and intact.Conclusions The application of the scrotum septum flap combined with the scrotal splitted thickness skin graft to form the neourethra in one stage is reliable to treat the severe hypospadias with the simple procedure.The combined technique has obtained both the advantages of the skin graft and the skin flap to form the neourethra,and it could be used to correct the curvature of the penile.Thus the satisfactory result of the appearance and the function could be achieved.The technique is an excellent procedure.

Objective To discuss the value of essential hypertension arteriosclerosis as a predictor of early renal damage.Methods 90 cases of essential hypertension with early renal damage were selected and statistical analysis to result of AASI,Scr,ACR and eGFR with SPSS17.0、Excel statistical analysis software were carried out.Group A was defined as AASI≥0.55 and group B was defined as AASI＜0.55.Compared the two groups with relevant analysis,and then built multivariate linear regression equation.Results ① The group A showed higher age(P＜0.01),24 h DBP(P＜0.05),ACR(P＜0.01),ACR(+) rate(P＜0.01)and lower eGFR (P＜ 0.01),compared with the group B.②AASI showed a positive correlation with age (r＝ 0.483,P＝ 0.000),24 h SBP (r＝ 0.350,P ＝0.001),ACR (r＝0.474,P＝ 0.000),and a negative correlation with eGFR (r＝-0.383,P＝ 0.000).Two multiple linear regressions showed that AASI was associate with eGFR (B ＝-0.001,P＝ 0.038) and ACR(B ＝ 0.025,P ＝ 0.000) independent of other factors.Conclusion ①Arteriosclerosis is closely related with age and blood pressure mutation rate day and night.②Arteriosclerosis has a closely relationship with essential hypertension early renal damage,and can predict early renal damage in hypertension patients.

Objective To observe the clinical efficacy of acupuncture at the trigger points on the back in treating cardioneurosis. Methods Fifty patients with cardioneurosis were selected to receive syndrome differentiation of traditional Chinese medicine, and then were evaluated for the improvement of symptoms respectively after first acupuncture treatment at the trigger points on the back and after 3~5 acupuncture treatments. Results Of the 50 cardioneurosis patients, 30 showed marked effect, 14 showed effective, 6 failed in the treatment, and the total effective rate was 88.0%. The total effective rate was 96.2%in patients of deficient heart yang, and was 75.0%in those of excessive fire due to yin deficiency, 83.3%in those of qi stagnation due to liver depression, and 83.3%in those of deficient heart yin. The improvement rate of subjective symptoms after 3~5 treatments was significantly different from that after the first treatment (P<0.05). Conclusions Acupuncture at the trigger points on the back is an effective approach in treating cardioneurosis, and it can achieve a marked efficacy with successive 3~5 sessions.

Objective To evaluate the value of detecting chlorophyll related genes of plankton in the diagnosis of drowning. MethodsEighteen rabbits were divided randomly into three groups: death by drowning (n=10), postmortem submersion (n=6) and control (n=2). The heart blood, lung, liver, kidney and brain tissues were taken from every rabbit. After isolated plankton from tissues with percoll and extracted their DNA, the chlorophyll-related genes, including EG (EG1 and EG2) and SK (SK1 and SK2), were detected using PCR technique. Meanwhile, diatom test was also performed from lung and liver tissues by nitric acid digestion method. ResultsFor the drowning group, the specific amplification products for EG1 were detected from 9 samples in heart blood, 10 samples in lung, 9 samples in liver, 7 samples in kidney and 8 samples in brain. The products for EG2 were detected from 8 samples, 10 samples, 7 samples, 5 samples and 7 samples accordingly. There were a small number of positives in heart blood, lung and kidney with SK1 and SK2 (≤2). For the postmortem submersion group, only one case was positive from heart blood and lung tissue respectively for EG1. No amplified product was detected for EG1 and EG2 in various tissues in control group, and also no product was detected for SK1and SK2 in other groups. In addition, diatoms were detected from 9 lung and 3 liver tissues in drowning group with the nitric acid digestion, and only one sample of lung was positive in the postmortem submersion group. ConclusionThe detection rate of the chlorophyll-related gene EG with PCR method was higher than that of diatom with nitric acid digestion method in drowning victims, and it can be used as a potentially useful tool for diagnosing drowning.

Objective To estimate the efficacy of super-early minimal invasive evacuation of hematoma via keyhole for treating hypertensive basal ganglionic hemorrhage. Methods 36 patients with hypertensive basal ganglionic hemorrhage(9 cases in grade Ⅱ, 17 in grade Ⅲ and 10 in grade Ⅳ) were treated by minimal invasive evacuation of hematoma via keyhole at super-early stage, and their efficacy was statistically compared with 35 patients with hypertensive cerebral hemorrhage underwent conservative therapy or lateral ventriculopuncture. Results In the 36 patients underwent minimal invasive evacuation of hematoma via keyhole, 22 cases had a good prognosis(61.1%), 9 had a poor prognosis(25.0%), and 5 died(13.9%). Among the 35 patients underwent conservative therapy or lateral ventriculopuncture, 5 cases had a good prognosis(14.3%), 22 had a poor prognosis(62.9%), and 8 died(22.9%). Objective To investigate the prevalence of macula choroidal neovascularization(CNV) in high myopia patients and the associated risk factors.Methods Fifty-six outpatients(112 eyes) with high myopia were medically examined from Mar.2008 to Jun.2009.The inspection items,including A scan,B scan,diopter and optical coherence tomography(OCT),were carried out to check whether CNV existed and its growth fashion.The patients' basic status,main symptoms and the pathologic change in macula were recorded.Results The prevalence of CNV in the 56 outpatients(112 eyes) was 14.3%.Gender showed no correlation with CNV(r=-0.015,P=0.879),while age(r=0.629,P=0.000),diopter(r=0.725,P=0.000),axial length(r=0.236,P=0.013),posterior scleral staphyloma(r=0.344,P=0.000),atrophy of choroid and retina in posterior pole(r=0.421,P=0.000),lacquer crack(r=0.421,P=0.000) and Fuchs spot(r=0.519,P=0.000) were significantly correlated with the prevalence of CNV.Conclusions CNV may highly occurred in the patients with high myopia.Posterior scleral staphyloma,atrophy of choroid and retina in posterior pole,lacquer crack,Fuchs spot,age,axial length and diopter seem to be the risk factors for the occurence of CNV.
