1.The significance of expression of CK19 in patients with colon cancer

Objective To study the expression of red blood cell keratin 19(CK19) mRNA in the peripheral blood of the pa‐tients with colorectal cancer and the relationship between the CK19 mRNA and the pathological characteristics ,to investigate the relationship between CK19 and colorectal cancer .Methods Clinical data of patients with colorectal cancer received treatment at our hospital from 2012 to 2014 was retrospectively analyzed .A total of 160 patients were retrospective analyzed ,including 52 patients with colorectal cancer ,55 with colon polyps and 53 normal persons .The control group was the healthy subjects and the patients with colon polypsc who accept the physical examination at our hospital at the same period .The different expressions of CK19 mR‐NA of colorectal cancer ,colon polyps and the normal persons were compared .The relationship between the CK19 mRNA and the pathological characteristics of colorectal cancer was analyzed .The different diagnostic values of CK19 mRNA and CEA ,CA199 to colorectal cancer were compared .Results There were significant differences in the positive rate of CK19 mRNA between three groups(χ2 =54 .53 ,P<0 .01) .The positive rate of CK19 mRNA in the patients with colorectal cancer was the highest .The positive CK19 mRNA in the patients with colorectal cancer of different Dukes typing(χ2 =16 .14 ,P<0 .01) ,differentiation(χ2 =8 .155 ,P=0 .017) ,liver metastasis(χ2 =13 .68 ,P<0 .01) were different .And the differences were significant .The result of ROC area under the curve was 0 .947 which described the diagnostic value of CK19 mRNA to the patients with colorectal cancer .The sensitivity and specificity of CK19 mRNA were higher than both of CEA and CA199 .Conclusion The expression of CK19 mRNA in colorectal cancer is significantly increased .There is significant correlation between the expression level of CK19 mRNA and the staging ,grad‐ing and transferring of the tumor .The diagnostic value of CK19 mRNA to the patients with colorectal cancer is better than CEA and CA199 .
2.The relationship between helicobacter pylori eradication and curative effect of gastroesophageal reflux disease

Objective To investigate the relationship between helicobacter pylori(HP)eradication and curative effect of gastroe-sophageal reflux disease(GERD) .Methods Two hundred and fifty four patients with GERD were prospectively recruited for endo-scope and acid reflux evaluation .According to the results of gastroscope ,the eligible patients were divided into non-erosive reflux disease (NERD) and erosive esophagitis (EE) group ,and each divided into HP positive and HP negative group ,the HP positive group were randomly divided into the anti HP treatment group and not .These patients were assigned to rabeprazole triple therapy (the anti HP treatment group) or rabeprazole(20 mg ,twice per day) single(other groups) for 10 d ,then rabeprazole (20mg ,twice per day) was given to all patients until two months .Then all patients underwent acid reflux evaluation ,EE group underwent endos-copy ,and all patients underwent a 13 C urea breath test 4 weeks after cessation of rabeprazole to determine HP status .Results One hundred and seventy nine GERD patients were included in the study(NERD group 119 cases ,including anti HP treatment group 47 cases ,not anti HP treatment group 32 cases ,HP negative group 40 cases ;EE group 60 cases ,including anti HP treatment group 24 cases ,not anti HP treatment group 15 cases ,HP negative group 21 cases) .HP was eradicated in 76 .6% of the NERD anti HP treatment group and in 75 .0% of the EE anti HP treatment group .Overall ,there is no difference between the anti HP treatment group and other groups on these aspects ,including symptom、esophagitis and acid reflux improvement .Conclusion There are no significant correlation between HP eradication and GERD .
3.Clinical analysis of infection in severe acute pancreatitis patients

Objective:To investigate the distribution of infection pathogens and their drug resistance in acute severe pancreatitis patients. Methods:Retrospective analysis was made on all pathogenic bacteria and their drug resistance of infection among 78 patients with acute severe pancreatitis patients. Results:Among 78 patients with acute severe pancreatitis 75 patients were cured,3 patients were dead.Of 286 clinical isolates,Gram negative organisms accounted for 61.19%,Gram positive ones 32.17%,fungi(6.64%).They resisted to antibiotics at high degrees.Among 92 strains of gram positive organism,60 were drug-resistance;also 172 strains of gram negative organism,above half were drug-resistance. Conclusion:Patients with acute severe pancreatitis are at high risk of infection.Due to drug-resistance are at high degrees in infection pathogens,effective drainage should be adopted to accelerate treating the infection besides selecting correct antibiotics according to the antibiotic sensitive essay results.

Iguratimod' s median toxic concentration to 3T6 cell was about 1142.6mg/L. Taking into the consideration of the cells performance after administration under spectacles, we chosen the administration concentration as 334.8 mg/L(DMS0:1?l/256?l) ,167.4mg/L,83.7mg/L and 41.85 mg/L. 6.Clinical Observation of Lactulose Combined with Bacillus subtilis Bifarous Viable Bacteria in the Treatment of Functional Constipation

OBJECTIVE:To observe the clinical efficacy and safety of lactulose combined with Bacillus subtilis bifarous via-ble bacteria in the treatment of functional constipation(FC). METHODS:In a randomized double blind controlled study,60 FC pa-tients were randomly assigned into treatment group(28 cases)and control group(32 cases). Both groups received lactulose orally. Treatment group additionally receive B. subtilis bifarous viable bacteria,and control group additionally received placebo continually. 2 weeks later,both groups stopped taking lactulose but continued to take adjunctive drug till 4th week. The defecate frequencies, stool property,constipation symptoms,quality of life and ADR were compared between 2 groups before treatrnent and 2nd,4th week cofter treatment. RESULTS:In the second week ,the defecate frequencies,stool property and constipation symptoms of both groups were significantly improved,with statistical significance (P<0.05). Compared with the control group in the fourth week, these constipation symptoms of treatment group were improved more significantly,with statistical significance (P<0.05). In the fourth week,aspects of quality of life in both groups were significantly improved,with statistical significance(P<0.05);physical discomfort scores,satisfaction scores and total scores of treatment group were improved significantly,compared with the control group,there was statistical significance (P<0.05). There were no severe ADR in both groups. CONCLUSIONS:Lactulose com-bined with B. subtilis bifarous viable bacteria can effectively relieve FC,improve abdominal symptoms and slow the recurrence of constipation,which is safe and reliable.
7.Microanatomy and histological study of the fascicular group to deltoid muscle in axillary nerve at the level of quadrilateral zone

Objective To observe and study the distribution and histological feature of the fascicular groups to deltoid muscle in axillary nerve at the level of quadrilateral zone. Methods 12 adult human cadavers (24 upper limbs)embalmed by formalin were used,, and microsurgical longitudinal contradissection of the axillary nerves by tracing their terminal branches were processed. At the level of quadrilateral zone,distrabution of the fscicular groups to deltoid muscle in the never trunk were recorded and their diameters were measured;Another 6 upper extremities (3 right and 3 left) of fresh-frozen human cadavers were obtained.Axillary nerves and their fascicular groups were exposured, and the segments at the level of qusdrilateral zone gotten, then histological cross section was done, acetycholinesterase (ACHE) stained with the method of Karnovsky Roots and myelin stained with the method of Loyezs. Defferent never fibres were distinguished under microscope and the fiber number was counted with IAS . Results At the level of quadrilateral zone,fasciculars were divided into two groups.The fascicular group of anterior branch to deltoid muscle was in the lateral of the nerve trunk . Its cross sectional area was (2.449?1.327)mm2 ,occupied 55.4%?9.3%of the axillary nerve trunk ;Its most fibers were motor fibers;The number of fibers is (2112?631), occupied 45.6 %?1.1%of all fibers of axillary nerve . Conclusions In the treatment of root avulsion of brachial plexus injury, selective suture donor nerve with the lateral fascicular group should be processed to repair the function of shoulder abduction,fibers loss would be reduced and rate of functional restoration improved .
8.Mucopolysaccharidosis type Ⅱ: a progress on molecular genetics

Mucopolysaccharidosis type Ⅱ is a rare single gene inherited disorder. In the earlier studies, more attention was paid to the molecular analysis of the hot spots of the mutations. With the detection of more and more novel mutations,more studies are trying to analyze the effect on splicing of different types of mutations and the genotype-phenotype correlations. This review will introduce the progress on molecular genetics, the gene mutation,analysis of the female patients and the genotype-phenotype correlations. With the study of these progress,it suggested that more attention should be paid to the possibility of gene-pseudogen recombinations, trying to study the effect on splicing from the cDNA level, which will benefit the investigation of the genotype-phenotype correlations.
9.Current status of diagnosis and management of rectal cancer in China

Colorectal cancer is one of the most common malignancies in the world, and rectal cancer accounts for a large percentage of this disease in Chinese patients. Surgical extirpation is the mainstay of therapy, Intraluminal ultrasound is highly accurate in detecting depth of invasion and lymph node metastasis, and is also helpful in preoperative staging of rectal cancer. Preoperative adjuvant therapy could down-stage tumors, improve the radical resection rates of advanced lower rectal cancer, increase the rate of sphincter-saving surgery, and decrease the local recurrence rate. Total mesorectal excision could signifi-candy decrease the local metastasis of rectal cancer, and rectal cancer resection with preservation of pelvic autonomic nerve is effective in preventing urinary or reproductive dysfunction. Recently, minimally invasive surgery has been introduced for the management of rectal cancer, and transanal endoscopic microsurgery is a new method with advantages in rectal cancer treatment.
10.Selective regulation of peroxisome proliferator-activated receptors on fatty acid binding protein-4 in human syncytiotrophoblast cells

Objective To observe the selective regulation of peroxisome proliferator-activated receptors (PPAR) on fatty acid binding protein-4 (FABP4) in human syncytiotrophoblasts.Methods Cultivate normal human syncytiotrophoblast cells,and put in the specific antagonists and agonists of PPAR each subtypes receptors,then observe the different expression of FABP4 mRNA and protein.Results Pretreated the human syncytiotrophoblast cells with the agonists (GW7647,GW0742) and antagonists (GW6471,GSK0660) of PPARα and PPARβ receptors,the expression of the FABP4 was not significantly change (P>0.05).However pretreated with PPARγ agonists (rosiglitazone,1 × 10-9,1 × 10-8,1 × 10-7 and 1 × 10-6 mol/L),the expression of FABP4 mRNA and protein could be dose dependent-promoted significantly (mRNA:1.27 ±0.12,1.45 ±0.14,1.57±0.14,1.72 ±0.12,protein:1.10 ±0.08,1.37 ±0.09,1.60 ±0.13,1.79 ± 0.14 ; P < 0.05),furthermore,the promotion can be dose dependent-reversed by specific antagonists GW9662 (mRNA:0.92 ± 0.06,0.77 ± 0.06,0.64 ± 0.05,0.55 ± 0.05,protein:0.91 ±0.03,0.78 ±0.06,0.70±0.07,0.55 ±0.06; P < 0.05).Conclusions In normal human syncytiotrophoblast cells,FABP4 is a target factor of PPARγ.PPARγ regulated the expression of FABP4 mRNA and protein selectively.And the regulation will not be influenced by the other two PPAR subtypes.
