1.Effect of lidocaine gelatin fiber on analgesia and hemostasis after endoscopic sinus surgery

Objeetive To explore the analgesic and hemostatic effect of lidocaine gelatin fiber used after endoscopic sinus surgery.Methods 86 patients underwent endoscopic sinus surgery were randomly divided into A and B groups.In A group,20% lidocaine gelatin fiber and expansion hemostatic sponge was packed into 43 patients' nasalcavity,while in B group,only expansion hemostatic sponge was packed.Nasal bleeding in patients while packed material within 24 hours and when extracted the material were observed.The analgesic effects were evaluated after packing material 1,6,12,and 24 hours after surgery.Results In A group,the amount of bleeding was (16.30 ± 5.19)ml,while the amount was(32.30 ± 12.09) ml in group B.Statistical analysis showed significant difference(t =7.97,P <0.05).There were no significant differences in nasal bleeding when extracted the stuffing and in analgesic efficiency 1hour after surgery.But the analgesic efficiency of lidocaine gelatin fiber was 20%,which was better than expansion hemostatic sponge 6 ~24 hours after surgery (t =27.163,29.091,16.241,all P < 0.05).Conclusion Lidocaine gelatin fiber not only had better hemostasis,but also had better analgesia than expansion hemostatic sponge after endoscopic sinus surgery.
2.Baseline investigation of medical aid following Wenchuan Eathquake

Objective This paper aims to make clear the post-disaster medical needs of residents in the disaster areas and changes among the poor population, for the purpose of policy recommendations on post-disaster medical assistance. Methods Such methods as in-home questionnaire survey and literature study were used to study the income, prevalence rate and mental health of 4 380 households in 10 counties (cities and districts) in the disaster areas. Results In the hardest hit areas, 80. 4% and 52. 8% of residents in rural and urban areas are dependent on government relief respectively;the proportion of poor population resulting from the disaster rose from 5% before the disaster to over 75% afterwards in the hard-hit areas, while such a population rose to 15% in the hardest-hit areas and about 10% in general disaster areas;It is estimated that the new medical aid funds in Sichuan Province will reach 350 million yuan, two-fold that of the budget for 2009;In the hardest hit areas, the two-week prevalence rate per thousand people is twice that found in the third survey of health services in 2003, characterized of acute respiratory illness and rheumatoid;In the hardest-hit areas, nearly 70% of the residents are exposed to high mental health risks. Conclusion Recommendations: Strengthening the raising and use of medical aid fund;dynamic management for the population in need of post-disaster medical aid;Developing appropriate medical aid packages to fit post-disaster health needs and postdisaster disease characteristics;and consolidating the connection between medical aid and primary medical insurance system.
3.A Cross-sectional Study on Diabetes Mellitus in Rural Population in Kunming

Objective To study the epidemiological characteristics of diabetes mellitus and to provide effective prevention and intervention methods in rural area in Kunming.Methods A cross-sectional study was conducted among 3 440 permanent residents aged 20~74 years in kunming rural districts.Results The general prevalence rate of diabetes was 4.01% and the standardized rate was 2.90%.The standardized prevalent rate for male(3.55%) was higher than that for female(2.61%),and the prevalence rates varied among different groups of rural inhabitants.Conclusion It is necessary to take effective methods to prevent and control diabetes in rural area.
4.Effect of controlled hypotenston with nitroglycerin on intraocular pressure during gynecological laparoscoplc surgery

Objective To investigate the effect of controlled hypotension with nitroglycerin on intraocular pressure(IOP)during gynecological laparoscopic surgery.Methods Forty ASA Ⅰ or Ⅱ patients,aged 25-55 yr,weighing 55-70 kg,undergoing gynecological laparoscopic surgery,were randomly divided into 2 groups(n =20 each):control group(group C)and controlled hypotension with nitroglycerin group(group N).Anesthesia was induced with iv injection of midaaolam 0.1 mg/kg,fentanyl 3 μg/kg,vecuronium 0.1 mg/kg and etomidate 0.3 mg/kg and maintained with iv infusion of propofol at 8 mg· kg-1 · h-1 and vecuronium at 0.1 mg· kg 1 · h-1.Additional fentanyl 2 μg/kg was injected intravenously at 3 min before the starting of surgery.The patients was tracheal intuhated and mechanically ventilation.PETCO2 was maintained at 35-40 mm Hg.BIS value was maintained at 50-60.Controlled hypotension was induced by infusion of nitroglycerin at 2-4 μg· kg-1 · min-1 after the starting of surgery.CVP was maintained at 12-14 cm H2 O.Controlled hypotension was stopped at the end of operation.IOP and CVP were recorded before induction(To),at 3 min after tracheal intubation(T1),at 10,30,60,90 and 120 m in of pneumoperitoneum(T2-6)and at 10 min after pneumoperitoneum(T7).Intraocular hypertension(IOP > 21mm Hg)was also recorded.Results Compared with group C,IOP at T3-6 and CVP at T2-6 were significantly decreased,and the incidence of intraocular hypertension was also decreased in group N(P < 0.01).Conclusion Controlled hypotesion with nitroglycerin can prevent the intraocular hypertension effectively during gynecological laparoscopic surgery.
5.Genotypes and serotypes of avian infectious bronchitis viruses isolated during 2009-2011 in Guangxi, China.

In order to investigate the prevalence and track genetic and antigenic evolutions of infectious bronchitis virus (IBV) and their prevalence in Guangxi, China since 1985, gene amplification and sequencing and virus neutralization (VN) test on chicken embryo tracheal organ cultures were used in genotyping and serotyping of 28 IBV isolates during 2009-2011 in Guangxi. The results of N gene sequencing and comparison showed that the 28 isolates and reference strains were classified into three groups, and most isolates belonged to group Ill, while the isolates in 1985-2008 belonged to groups IV and II. The data of VN test indicated that the 28 isolates belonged to 6 serotypes; among them, 71. 4% belonged to serotypes 1, 2, and 3, and 11 (39.3%) shared the same serotype with the current vaccine strains. Given the data of our previous study, it is found that prevalent serotypes and their proportions varied in different areas of Guangxi and during different periods. These data lay a good foundation for developing an oil-emulsified inactivated polyvalent vaccine containing local dominant serotypes for the effective prevention and control of infectious bronchitis.

6.Yoga is found hazardous to the meniscus for Chinese women.

OBJECTIVEYoga is becoming more and more popular in the female society while the concomitant sports injury is seldom mentioned. Many clinicians have noted that yoga may result in knee problems, which though requires more researches to corroborate. This investigation was conducted to ascertain the relationship between yoga and meniscus injury as well as the extent of impairment according to variant yoga practice periods.
METHODSTotally 819 women aged 20-49 years who practiced yoga or other popular sports including badminton, jogging, climbing hills, etc for at least one hour per day were selected to participate in this research. These subjects were required to complete a questionnaire and receive relevant physical examination. Magnetic resonance (MR) scan of the knee was recommended for the suspicious subjects for ultimate diagnosis. The subject with abnormal meniscus MR signals was defined as a case and matched with two controls in terms of age and body mass index (BMI). Altogether there were 273 cases and 546 controls. The nested case-control model was adopted to assess the risk of meniscus injury between variant exposures in practicing yoga and several other popular sports. Moreover, the 181 yoga subjects were subdivided into three groups according to different exercise durations, followed by further analysis with the variables of age, BMI and Lysholm score.
RESULTSYoga was found associated with a higher risk (P equal to 0.008, OR equal to 1.621) of meniscus injury compared with badminton, jogging and climbing hills. The three yoga subgroups showed statistical difference between each other in terms of Lysholm score (P equal to 0.027) and BMI (P equal to 0.003). The subjects with longer-term yoga practice had lighter weight but lower Lysholm scores.
CONCLUSIONSYoga perhaps exerts destructive impact on the meniscus for Chinese women, yet it needs further verifications. Furthermore, the female yoga players with longer exercise duration are more susceptible to meniscus injury though they can become leaner.

7.Clinical observation on effects of routine acupuncture combined with Feiteng Bafa for chloasma.

OBJECTIVETo explore the relatively efficacious acupuncture treatment for chloasma, to promote its clinical application in medical settings.
METHODSForty cases were randomly divided into a Feiteng Bafa group (FB) and a selecting points based on differentiation of syndromes group (DS), 20 cases in each group and acupoints. Hegu (LI 4), acupoints in close proximity to chloasma and according to differentiation of syndrome were selected in DS group. The acupoints selected according to Feiteng Bafa principles were adhered in FB group. After two treatment courses, the effectiveness rate, the pro-and post-size and pro-and post-color of the chloasma area were compared between the two groups.
RESULTSThe size of the chloasma area decreased, and the color of the chloasma area became lighter in both groups (P < 0.01, P < 0.05), and the FB group was better than the DS group (both P < 0.05). The effectiveness rate in the FB group was 95.0%, better than that of 70.0% in the DS group.
CONCLUSIONThe acupuncture treatment adhered to Feiteng Bafa is better than the simple routine acupuncture for chloasma.

The modulation of ACh on delayed rectifier-like potassium currents (I(K)) was studied in freshly dissociated cerebral cortical neurons using the whole-cell patch-clamp technique. Wistar rats between 10- and 14-day old of both sexes were used. After rats were decapitated, their brains were quickly removed, iced, and then manually cut into 400 mum slices. Slices were then incubated for 0.5 h at 32 degrees C in a buffered artificial cerebrospinal fluid (ACSF) bubbled with 95% O2, 5% CO2. Slices were then removed into buffered ACSF containing protease (0.5 mg/ml) at 32 degrees C. After 30 min of enzyme digestion, tissue was rinsed three times in the buffered saline. Then the enzyme-treated slices were mechanically dissociated with a graded series of fire-polished Pasteur pipettes. The cell suspension was then plated into a 35 mm dish and placed on the stage of a Olympus inverted microscope. For whole-cell recordings of currents, standard voltage-clamp techniques were used. Neurons were held at -80 mV, and the I(K) was evoked by 2 000 ms depolarizing voltage commands to potential between -40 mV and +60 mV in 10 mV steps applied at a frequency of 0.5 Hz. It was found that the inhibitory effect of ACh (0.1, 1, 10, 100 mumol/L) on I(K) was dose-dependent. It was also found that ACh affected the activation process of I(K) significantly, i.e., the activation curve of I(K) was characterized by half-activation potential of (-41.8+/-9.7) mV and a slope factor of (30.7+/-7.2) mV in the cortical neurons and they were changed to (-122.4+/-38.6) mV and (42.4+/-7.0) mV, respectively, after giving ACh (10 mumol/L). Tubocurarine (100 mumol/L) antagonized the inhibitory effect of ACh on I(K), and the drop of currents varied from the control value of (36.5+/-7..8)% to (16.9+/-13.8)% (n=8, P<0.01). 4-DAMP (10 mumol/L) blocked the inhibitory effect of ACh on I(K), and the currents reduced from the control value of (36.5+/-7.8)% to (26.8+/-4.7) % (n=6, P<0.05). Pirenzepin did not antagonize the inhibition of ACh on I(K) (n=7, P>0.05). Chelerythrine (20 mumol/L) blocked the inhibitory effect of ACh on I(K) and the currents reduced from the control value of (36.5+/-7.8)% to (11.7+/-17.3)% (n=6, P<0.05). On the contrary, PDBu (10 mumol/L) strengthened the inhibition of ACh on I(K) and the drop of currents changed from the control value of (36.5+/-7.8)% to (59.2+/-14.0)% (n=5, P<0.05). PDBu abolished the antagonism of chelerythrine on ACh in cortical neurons. It is suggested that the ACh-induced depolarization of neurons in the cortex is attributed to the inhibition of I(K) that is most likely evoked by the activation of nicotinic ACh receptors and muscarinic M3 receptor via protein kinase C (PKC) signal transduction pathway.

10.Seroprevalence of Toxoplasma gondii Infection in Wild Boars, Wild Rabbits, and Wild Chickens in Hubei Province, China.

Toxoplasma gondii causes serious infection worldwide in humans and animals. In this study, the seroepidemiology of toxoplasmosis was investigated in wild boars (Sus scrofa) (n=377), wild rabbits (cape hare, Lapus capensis) (n=331), and wild chickens (red junglefwol, Gallus gallus) (n=571) in 4 forested and country sided area of Hubei province of China. For this, blood samples were collected and tested by indirect hemagglutination test (IHA). The seroprevalence was found to be 7.2%, 5.1%, and 12.6% in wild boars, rabbits, and chickens, respectively, with significant differences among these species. The prevalence of T. gondii infection in male and female wild boars was found to be 7.9% and 6.5% (P<0.01), in male and female rabbits was 5.6% and 4.9% (P<0.01), and in male and female chickens was 17.1% and 7.7% (P<0.01), respectively, with significant differences between 2 genders of chickens (P<0.01). The findings of this study may help in planning of the prevention measures against T. gondii infection in wild animals in this area.
