Objective To observe the preventive effect of dexmedetomidine on penile erection(PE) after general anesthesia induction and urinary bladder irritation during recovery(UBIR) with urethral catheterization in male patients.Methods A total of 1000 male patients with ASA grade Ⅰ ～ Ⅱ were randomly divided into control group(group C) and dexmedetomidine group (group D,n =500).Dexmedetomidine (1 μg/kg) was intravenous pumped in group D and saline was given in group C 15 minutes before anesthesia induction.Both groups started catheterization within 10 minutes after endotracheal intubation.The incidence and grade of PE before infusion(To),before induction(T1),before catheterization(T2) and during catheterization (T3) were observed.The degree and extent of UBIR were observed and recorded.Results The incidence and grade of PE at T3 time-point in group C were highest (Compared with T2 and other time-points,P ＜ 0.05),but there was no statistically differences in incidence and grade of PE in group D between at T3 and T2time points (P ＞ 0.05).The incidence of PE at T2 and T3 time-points were 4.0％ and 5.6％ in group D,which was significantly lower than those in group C (15.4％,77.8％,P＜0.05).The incidence of PE at grade 1,2 and3 was 4.4％,1.0％ and0.2％ respectively in group D,which was significantly lower than those in group C (62.6％,11.4％,3.8％,P ＜ 0.05).The incidence of UBIR was significantly lower (28.4％ vs 63.0％,P ＜ 0.05),among which the incidence of UBIR at grade 1,2 and 3 were 15.6％,10.4％ and 2.4％,which was significantly lower in group D than those in group C (22.0％,21.0％,20.0％,P ＜ 0.05).Conclusion 1μg/kg dexmedetomidine pumped before anesthesia induction could prevent the occurrence of PE during catheterization and UBIR,which can improve patients' comfort.

Objective To evaluate anesthesia efficacy of sevoflurane combined midazolam and remifentanil and propofol in patients undergoing short surgery. Methods Fifty ASA Ⅰ or Ⅱ patients who scheduled for elective surgery under general anesthesia were enrolled in study. The anesthesia was induced with intravenous injection of midazolam 0.03 mg/kg and sevoflurane inhalation. Induction of anesthesia was started with 6% sevoflurane and fresh gas flow rate of 6 L/min. The inspired concentration of sevoflurane was reduced to 2% as soon as eyelash reflexes of these patients were abolished. At the same time, remifentanil 2 μg/kg was injected slowly over 1 min,and 30 s later,patients were mechanically ventilated after tracheal intubation. 2% sevoflurane was kept inhaling, and remifentanil and propofol were kept pumping for anesthesia maintenance. The mean arterial pressure (MAP), heart rate, pulse oxygen saturation (SpO_2) and bispectral index (BIS)were recorded before administration,at the time of the disappearance of eyelash reflex, immediately before tracheal intubation,and immediately after tracheal intubation. The time period from sevoflurane inhalation to the disappearance of eyelash reflex was also recorded. And conscious recovery, spontaneous breathing recovery,the endotracheal extubafion time and OAAS score were recorded. Results All patients were successfully intubated at the first attempt. The satisfactory intubation conditions were found in 82% (41/50) of cases. The time period from sevoflurane inhalation to the disappearance of eyelash reflex was (73 ± 12) s. MAP,heart rate and SpO_2 remained stable and BIS was maintained at 45-55 before and after tracheal intubation, and all patients were quick recovery. Conclusion The anesthesia efficacy of sevoflurane combined midazolam and remifentanil and propofol without muscle relaxant in patients undergoing short surgery is smooth, safe and reliable.

Objective To explore the features and influencing factors of ambulatory blood pressure in chronic kidney disease (CKD) patients.Methods A total of 540 CKD patients from May 2010 to May 2012 in our department were enrolled in this study.Ambulatory blood pressure monitoring was carried out.Blood pressure (BP),proteinuria and other clinical parameters were measured regularly.Ultrasonography was used to evaluate cardiac structure and function,carotid intima-media thickness and plaque.Univariate and multivariate analysis were used to examine the association between BP and clinical parameters.Results 63.9％ of CKD patients was non-dipper BP pattern,and 36.1％ was dipper BP pattern.As compared to dipper BP patients,those with non-dipper BP had higher ratio of nighttime/daytime proteinuria (0.51±0.29 vs 0.42±0.21,P ＜ 0.01),lower estimated glomerular filtration rate (eGFR) [(56.2±48.2) vs (75.5±56.5) ml· min 1 · (1.73 m2)-1,P ＜ 0.01],higher serum cystatin C[(2.8±2.0) mg/L vs (2.1±2.0) mg/L,P ＜ 0.01],higher left ventricular mass index [(53.7±23.2) vs (45.1± 16.3) g/m2,P ＜ 0.01] and severely damaged left ventricular diastolic function and higher carotid intima-media thickness [(0.7±0.3) vs (0.6±0.2) mm,P＜ 0.01].Nighttime blood pressure was independent predictor for proteinuria,eGFR and left ventricular mass index.Conclusions Nondipper blood pressure pattern is very common in CKD patients.Nighttime pressure is closely associated to renal damage and cardiovascular injuries.

OBJECTIVE:To improve community pharmaceutical care so as to promote the rational drug use of community resi-dent and improve the quality of life. METHODS:By analyzing the situation of community pharmaceutical care,the pharmaceutical care of community pharmacists was improved by changing pharmaceutical care mode,actively developing the propaganda of ratio-nal drug use,strengthening retraining of clinical pharmacists. RESULTS:With the help of Hongkou district quality control group, many hospitals of the district signed thepharmaceutical linkage assistance agreement. Through the efforts of Hongkou district quality control group and many hospitals,community pharmaceutical care was improved and the propaganda of rational drug use has achieved certain results. CONCLUSIONS:Through exploration and practice,the pharmaceutical care levels of community phar-macists have been improved and the rational drug use of community residents has been promoted.

Purpose To explore the difference of expression and prognostic significance of SP1,KLF4 and p21 in low grade ovarian serous carcinoma (LGSC) and high grade ovarian serous carcinoma (HGSC).Methods The expression of SP1,KLF4 and p21 protein was examined with immunohistochemistry EliVision method in cases with LGSC and HGSC.Kaplan-Meier analysis and Cox multivariate survival analysis were used to assess the impact of SP1,KLF4 and p21 expression on prognosis of LGSC and HGSC.Results SP1,KLF4 and p21 expression were detected respectively in 74.5％,17.0％ and 11.7％ HG-SC cases,and in 65.2％,34.8％ and 26.1％ LGSC cases.Compared to control group,the expression level of SP1 was significantly higher (P ＜ 0.05),but the expression level of KLF4 and p21 were significantly lower (P ＜0.05).There was no significant difference of SP1,KLF4 and p21 expression between HGSC and LGSC (P ＞ 0.05).The expression of SP1,KLF4 and p21 were associated with FIGO stage,meanwhile SP1 associated with residual tumor size in HGSC (P ＜ 0.05).There was a significant negative correlation between SP1 and KLF4,p21 proteins in HGSC (P ＜ 0.05).Kaplan-Meier analysis revealed that there were significantly poor overall survival (OS) of 5 years for patients with HGSC displaying high expression of SP1,or low expression of KLF4 and p21 (P ＜0.05),but no significantly improved OS for patients with LGSC (P ＞ 0.05).Cox analysis showed that SP1 overexpression is an independent prognosis factor for HGSC.Conclusion Overexpression of SP1 and low expresion of KLF4 and p21 contribute to carcinogenesis of HGSC and LGSC.They are associated with a poor prognosis of HGSC,but not LGSC,meanwhile SP1 is an independant prognosis factor for HGSC.

OBJECTIVETo study in situ intestinal absorption kinetics of baicalin contained in Tiangou Jiangya capsules, and the effect of different intestinal segments, pH value, drug concentration and P-gp inhibitor on the absorption.

METHODThe in situ intestinal perfusion test was adopted, and HPLC method was used to determine the content of baicalin in samples at different time points. Ultra-violet (UV) spectrophotometry was used to determine the content of phenol red in samples at different time points.

RESULTWhen pH value was at 5. 0, 6. 5, 7. 4, the absorption of baicalin was not impacted. P-gp inhibitor verapamil could enhance the absorption of baicalin. When the quality concentration of the test solution ranged between 5-20 g L -1 , the linearity of the absorption amount of baicalin increased. The absorption kinetic equation of baicalin was Y = -0. 073 7X +0. 118 7 (r = 0. 994 8) , K. 0. 073 7 h -1 , t1/2 9. 40 h.

CONCLUSIONBaicalin is mainly absorbed in colon. The absorption of baicalin shows the first-order kinetics process, with the absorption mechanism of passive diffusion. Baicalin is a substrate for P-gp.

Aim To evaluate the anti-tumor activity of the extract from the root of Actinidia chinensis planch in vitro and in vivo. Methods Active components from the root of Actinidia chinensis planch were isolated by traditional phytochemical techniques. The in vitro anti-tumor activity was determined by sulforhodamine B assay and the in vivo anti-tumor activity was evaluated using experimental mouse tumor models and human tumor xenografts in nude mice. Results Powdered air-dried roots of Actinidia chinensis planch were percolated with methanol at room temperature thrice. The filtrate was concentrated to dryness in vacuo and then was further extracted with ethyl acetate, n-butanol , and chloroform. The fraction extracted by chloroform displayed the most potent activity against several tumor cell lines including hepatocellular carcinoma Bel-7402 cells, non-small cell lung cancer A549 cells, lymphoma Ramos cells, and breast cancer MCF-7 cells. Further more, the anti-tumor efficacy of the chloroform fraction was confirmed in Bel-7402 xenografts in nude mice with the percentage inhibition of 38.0 %. Conclusion The extract of the root of Actinidia chinensis planch has anti-tumor activity, and the active components are mainly in the fraction extracted by chloroform.

Objective To design radioactive biliary stents and to evaluate the feasibility and safety of the stents.Methods Plastic stents with inserted iodine-125 seeds were designed and tested in sixteen normal pigs. In the brachytherapy group,the pigs were implanted radioactive stents in the common bile duct (CBD) and then divided into three groups on the basis of radiation dose of the reference point,50 Gy group (n=4),100Gy group (n=4),and 150 Gy group (n=4).In the control group,the same plastic stents with non-radioactive seeds were implanted (n=2),whilst in the blank control group,no stents were implanted (n=2).Blood routine,serum amylase,liver and kidney function were tested before and 1,7,14,30,60 days after the implantation of stents. Animals were sacrificed on the 14,30 and 60 days after stem implantation.Some relevant index such as perito- neal bleeding and inflammation,ascites,injury of adjacent organs,as well as perforation,stricture and dilatation of bile duct were investigated.Bile duct tissues were stained with H-E,and observed under microscopy. Results The radioactive plastic biliary stents were successfully prepared and implanted.There was no effusion, hemorrhage or necrosis in the adjacent organs of radioactive biliary stent implanted group.Perforation of the CBD wall was not observed in the brachytherapy group.By pathological examination in the CBD,obvious hyperplasia of the mucosa and mucosal glands were seen in the control group.Necrosis of mucosal layer existed in brachy- therapy groups.In 50 Gy group,mucosal layer was incomplete and mild hyperplasia of mucosal glands was seen. In 100 Gy group,mucosal layer disappeared and almost no hyperplasia of the mucosal glands could be found.In 150 Gy group,mucosal layer disappeared and mucosal glands obviously decreased.There were no obvious abnormalities noted in blood tests after implantation in each group.Conclusions The design of radioactive biliary stents are feasible and safe.The radioactive stents have obvious radiation effect besides the mechanical effect on the mucosal layer of CBD.

Objective To observe the sequence of fat deposit and its relationship with insulin resistance in SD rats fed by high fat diet.Methods Normal 8-week-old male SD rats were randomly divided into normal chow (NC,n=40)and high fat diet(HF,n=40)groups.Triglyceride(TG)in serum,liver and muscle were measured;glucose infusion rate(GIR)and the mRNA level of genes related to lipid metabolism in liver and muscle were determined in different stages.GIR was detected by eugiyeemic-hyperinsulinemic clamp for evaluating the insulin sensitivity.Gene expression was determined by real-time PCR.Results(1)As compared with NC group,serum TG was not increased after high fat feeding for4 and 8 weeks,it began to increase after 12 weeks [0.52(0.15-1.00) mmol/L vs O.31(0.09-0.53)retool/L, P0.05)in skeletal muscle.After 8 weeks,the expression of ACC1 in liver in HF group was increased by 20.6%,CPT-1 was decreased by 27.1%(P
