2.Formation mechanism of high-energy metabolism along meridians.

According to the analysis and integration on prior research results regarding meridian essence, it is believed that high-energy metabolism is one of the main characteristics of along-meridian specificity. With discussion on the formation mechanism of along-meridians high-energy metabolism as entry point, it is found out that proteins of voltage-gated calcium channel along the meridians are likely to play an essential role of starting and coupling during the along-meridians functional activity. Thus, the hypothesis "proteins coupling in the meridians" is modified to the hypothesis "calcium channels proteins coupling in the meridians", which opens new path to reveal material basis and action mechanism of meridians.

4. Protective effects of Acanthus ilicifolius alkaloids a and its derivatives on immune hepatic fibrosis in rats

OBJECTIVE: To research the protective effects of Acanthus ilicifolius alkaloids A and its two derivatives in the rats with immune hepatic fibrosis. METHODS: Immune hepatic fibrosis model was established by intraperitoneal administration porcine serum. SD rats were treated intragastric administration with [4-hydroxy-2 (3H)-benzoxazolone, HBOA], [4-acetoxy-2 (3H)-benzox-azolone, AcO-BOA], (3-acetyl-4-acetoxy-2-benzoxazolone, TC-3). The activities of ALT, AST in serum and the levels of HA and LN in serum were measured. RESULTS: Compared with the normal control group, the activities of ALT, AST and the levels of HA and LN in the model group were significantly increased. Compared with the model group, animals treated with the HBOA, AcO-BOA or TC-3 could decrease the activities of ALT and AST and the levels of HA and LN markedly. Histopathological results showed that treatment groups could relieve the porcine serum-induced liver damage. CONCLUSION: HBOA and its derivatives have protective effect on the rats with immune-mediated hepatic fibrosis.
5.Basic and clinical studies of the gene product-targeting therapy based on leukemogenesis--editorial.

In the last twenty years, using all-trans retinoic acid (ATRA) as a differentiation inducer, Shanghai Institute of Hematology has achieved an important breakthrough in the treatment of acute promyelocytic leukemia (APL), which realized the theory of reversing phenotype of cells and provided a successful model of differentiation therapy in cancers. Our group first discovered in the world the variant chromosome translocation t(11;17)(q23;q21) of APL, and cloned the PML-RAR alpha, PLZF-RAR alpha and NPM-RAR alpha fusion genes corresponding to the characterized chromosome translocations t(15;17); t(11;17) and t(5;17) in APL. Moreover, establishment of transgenic mice model of APL proved their effects on leukemogenesis. The ability of ATRA to modify the recruitment of nuclear receptor co-repressor with PML-RAR alpha but not PLZF-RAR alpha caused by the variant chromosome translocation elucidated the therapeutic mechanism of ATRA from the molecular level and provides new insight into transcription-modulating therapy. Since 1994, our group has successfully applied arsenic trioxide (As(2)O(3)) in treating relapsed APL patients, with the complete remission rate of 70% - 80%. The molecular mechanism study revealed that As(2)O(3) exerts a dose-dependent dual effect on APL. Low-dose As(2)O(3) induced partial differentiation of APL cells, while the higher dose induced apoptosis. As(2)O(3) binds ubiquitin like SUMO-1 through the lysine 160 of PML, resulting in the degradation of PML-RAR alpha. Taken together, ATRA and As(2)O(3) target the transcription factor PML-RAR alpha, the former by retinoic acid receptor and the latter by PML sumolization, both induce PML-RAR alpha degradation and APL cells differentiation and apoptosis. Because of the different acting pathways, ATRA and As(2)O(3) have no cross-resistance and can be used as combination therapy. Clinical trial in newly diagnosed APL patients showed that ATRA/As(2)O(3) in combination yields a longer disease-free survival time. With the median survival of 18 months, none of the 20 cases in combination treatment relapsed, whereas 7 relapsed in 37 cases in mono-treatment. This is the best clinical effect achieved in treating adult acute leukemia to this day, possibly making APL the first adult curable leukemia. Based on the great success of the pathogenetic gene target therapy in APL, this strategy may extend to other leukemias. Combination of Gleevec and arsenic agents in treating chronic myeloid leukemia has already make a figure both in clinical and laboratory research, aiming at counteracting the abnormal tyrosine kinase activity of ABL and the degradating BCR-ABL fusion protein. In acute myeloid leukemia M(2b), using new target therapy degradating AML1-ETO fusion protein and reducing the abnormal tyrosine kinase activity of c-kit will also lead to new therapeutic management in acute leukemias.

6.A study on background、 suicide and comorbidity in anxiety neurosis and obsessive- compulsive disorder

Objective To investigate the differences of background in anxiety neurosis and obsessive- compulsive disorder,and their suicide rate and comorbidity.Method 68 anxiety neuroses and 62 obsessive- compulsive disorders were investigated by the selfconstructed scule.Rusult Age of onset,style of onset,marriage and education were significantly differences between anxiety neurosis and obsessive- compulsive disorder(P< 0.01~ 0.001).The rates of suicide behavior and suicide intent in anxiety neurosis and obsessive- compulsive disorder were 10.3% and obsessive- compulsive disorder with depressive disorders were 70.6% and 35.5% respectively.Conclusion Anxiety neurosis and obsessive- compulsive disorder two different disorders,they had higher suicide rates and comorbidity.
7.Investigation in adverse event reporting of nurses and its influencing factors

Objective To investigate the adverse event reporting of nurses and analyze the influenc-ing factors. Methods A sample of 801 nurses were assessed by completing AHRQ Hospital Survey On Patient Safety Culture (Chinese version). Results 82.65% of respondents within hospital didn't reported a single event in the past 12 months. The positive responsive rates for "frequency of event reported"and "nonpunitive response to error"was 29.92% and 29.38% respectively, where higher scores were found in the respondents possessing bachelor degrees and the average score of the respondents from out-patients unit was lower than those from other units. Conclusions The findings illustrate that it is necessary to improve the performance of event reporting of nurses and the influence factors lie in the level of the nurses' educa-tion degree and whether the event reporting system is punitive or not.
9.Comparison of efficacy of pressure-controlled ventilation and volume-controlled ventilation in patients undergoing lumbar surgery in prone position

Objective To compare the efficacy of pressure-controlled ventilation and volume-controlled ventilation in patients undergoing lumbar surgery in prone position.Methods Sixty ASA physical status Ⅰ or Ⅱ patients of both sexes,aged 40-64 yr,weighing 45-90 kg,scheduled for lumbar surgery in prone position,were randomly divided into 2 groups (n =30 each) using a random number table:pressure-controlled ventilation group (group P) and volume-controlled ventilation group (group V).Anesthesia was induced with dexamethasone 10 mg,etomidate 0.3 mg/kg,sufentanil 0.4 μg/kg,and rocuronium 0.6 mg/kg and maintained with propofol 2-4 mg· kg-1 · h-1,remifentanil 6-10 μg· kg-1 · h-1 and vecuronium 0.08 mg· kg-1 · h-1.The i-gel laryngeal mask airways were inserted after induction and the patients were mechanically ventilated.A gastric tube was inserted through the drain tube of i-gel.The maximum inspiratory pressure was adjusted to reach the tidal volume (VT) of 8 ml/kg in group P and the VT was set at 8 ml/kg in group V.PTrCO2 was maintained at 30-40 mm Hg.The mean airway pressure (Pmean) and peak airway pressure (Peak) were recorded immediately after insertion of i-geal (T0),immediately after the patients were turned to prone position (T1),immediately before skin incision (T2),30 min after the beginning of surgery (T3),immediately after the end of surgery (T4) and immediately after the patients were turned to supine position (T5).While dynamic lung compliance (Cdyn) was calculated.Arterial blood samples were taken at the same time points for blood gas analysis.Oxygenation index (OI) and respiratory index (RI) were calculated.Results Compared with group V,Pmoan and Ppeak were significantly decreased at T0-5,Cdyn and OI were increased,and RI was decreased at T1-4 in group P (P < 0.05).Conclusion Compared with volumecontrolled ventilation,pressure-controlled ventilation can better improve the ventilatory efficacy and reduce prone position-induced effect on respiratory function in patients undergoing lumbar surgery.
10.Detection of peripheral blood NK cell subsets in patients with malignant hematological diseases

Objective To study the clinical value of detection of peripheral blood natural killer(NK) cells subsets in patients with malignant hematological diseases.Methods A total of 100 patients with malignant blood disease were enrolled from March 2014 to April 2016 as the observation group,including 50 cases in acute stage and 50 cases in mild stage.At the same time,100 health subjects were enrolled as the control group.The number of NK cells in peripheral blood and the changes of subgroups were compared.Results The number of NK cells in the different groups were compared,the absolute numbers and relative numbers of NK cells in the acute phase of leukemia and lymphoma were much lower than those in the control group(P<0.05).While the absolute number and relative number of NK cells in mild stage of leukemia and lymphoma were higher than that in acute phase(P<0.05).After treatment,the absolute number and relative number of NK cells in patients with leukemia and lymphoma were higher than before treatment(P<0.05).Before treatment,levels of CD56bright and CD56dim in patients with leukemia and lymphoma were statistically different with those of control group(P<0.05).After treatment,CD56bright and CD56dim levels were significantly improved in both groups(P<0.05).Conclusion The number of peripheral blood NK cells in patients with hematological malignancies could be reduced,and the cells function might be compromised.Detection of NK cell subsets could be with guiding significance in the treatment of malignant hematologic disease.
