2.Environmental Pollution by Dioxin-like PCBs around a Disassembly of Obsolete Solid Waste

Objective To investigate the distribution and environmental pollution of dioxin-like polychlorinated biphenyls(PCBs) in a place of disused transformer and other electronic waste disassembled. Methods The ambient air, aerosol, water, soil and vegetable samples were collected to analyze the dioxin-like PCBs level with isotope internal standards and HRGC-HRMS techniques. Results The concentration of dioxin-like PCBs in ambient air was 4 031.77-4 942.34 pg/m3(namely 1.832-2.666 pg TEQ/m3), while in aerosol it was 325.45-3 499.77 pg/m3(0.370 9-1.536 3 pg TEQ/m3). In the soil sample, the value was 2 316.59-19 100.30 pg/g(1.358-15.264 pg TEQ/g) dry weight. In water it was 5.731-237.640 ng/L(2.585-73.046 pg TEQ/L). In vegetable it was 1 078.45-1 620.04 pg/g(0.778 3-1.126 2 pg TEQ/g) wet weight. In all air, aerosol, soil, water and vegetable samples, the content of PCB-118 was the most. PCB-126 had the highest TEQ value. In non-ortho PCBs, PCB-77 was of maximum content. In mono-ortho PCBs, PCB-118 was the most, and the followings were PCB-105, PCB-156 and PCB-167. All the 12 kinds of PCBs were investigated in all samples except PCB-169 absent in ambient air. Conclusions The investigated area around the disassembly of obsolete solid waste has been badly contaminated by dioxin-like polychlorinated biphenyls(PCBs), and PCBs already exist in the vegetables. According to the results of the present paper, some further studies should be carried out to evaluate the toxic effects on food and people health.
3.Application of plan-do-check-act cycle in quality management for loaner surgical instruments

Objective To investigate the management status of loaner surgical instruments,and evaluate the effect of plan-do-check-act cycle (PDCA)quality control on loaner surgical instrument management.Methods From July 2011 to June 2012,8 965 pieces of loaner surgical instruments before adopting PDCA quality control management was as control group;from July 2012 to June 2013,8 564 pieces of loaner surgical instruments adopting PDCA quality control was as ob-servation group.The defects of loaner surgical instruments during application process and effect of PDCA quality control on loaner surgical instrument management were analyzed.Results There were many problems in checking-tracking,cleaning quality and company personnel of loaner surgical instruments.The qualified rate of observation group was higher than that of control group(99.36% vs 96.27%)(χ2 =194.74,P <0.01).The main causes for unqualification of observation group were unqualified cleaning (n=21 ,38.18%)and incomplete function of instruments(n=8,14.55%);while the main causes for unqualification of control group were the loss of instruments(n=81,24.25%),lack of monitor and record (n=71, 21.26%),unqualified cleaning(n=55,16.47%)and the soaking of package(n=54,16.17%).Conclusion PDCA quality control is an effective method for loaner surgical instruments management,it is helpful for building long-term effective quali-ty control system for loaner surgical instruments,and make loaner surgical instrument management more scientific,system-atic,and standard.
4.Aerobic exercise combined with tiotropium inhalation in the treatment of stable chronic obstructive pulmonary disease complicated with pulmonary hypertension

Objective To observe effect of aerobic exercise combined with inhalation of tiotropium on patients with stable chronic obstructive pulmonary disease (COPD) complicated with pulmonary hypertension.Methods A total of 98 patients with stable COPD and grade Ⅱ or Ⅲ pulmonary function complicated with pulmonary hypertension were recruited.They were divided into an observation group (n =31) and two control groups (n =30 and 37).Patients in control group 1 inhaled shahmette and fluticasone powder,and antibiotics as well as apophlegmatisant were also used in accordance with the demands of the patient's condition.Patients of in control group 2 inhaled tiotropium dry powder and accepted other therapeutic measures equivalent to those of control group1.The patients in the observation group were given aerobic exercise in addition to other therapeutic measures equivalent to those of control group 2.Six-minute walk test distances,pulmonary arterial systolic pressure and chronic obstructive pulmonary disease assessment test scores were compared after 6 months of treatment,among the groups and against the baseline values before treatment.Results After 6 months of treatment the average 6-minute walk distance,chronic obstructive pulmonary disease assessment score and pulmonary arterial systolic pressure of the observation group were all significantly better than for either of the control groups.Conclusions Inhaling tiotropium dry powder combined with aerobic exercise can reduce pulmonary artery systolic pressure and improve the symptoms as well as the quality of life for patients with stable COPD complicated by pulmonary hypertension.
5.Effects of Astragalosides on Angiogenesis in Myocardium Infarction Rats

Objective To investigate the effects of astragalosides(AST)on angiogenesis of myocardium in rats after myocardial infarction.Methods Myocardial infarction(MI)was induced by ligation of the proximal left anterior descending coronary artery,30 postoperative rats were randomly divided into three same-size groups,i.e,medical group A(AST 2.5 mg · kg-1 · d-1),medical group B(AST 10mg · kg-1 · d-1)and control group(physiological saline).All of three groups were treated with intraperitoneal injection of 2ml dose for 4 weeks.The pathological changes of the heart tissue were observed by H-E staining and the micro-vascular count(MVC)/micro-vascular density (MVD)were calculated by CD34-staining.Results HE staining showed cardiac fabric disarrangement,granulation tissue generation,and fibroblast proliferation;The change of medical groups was less obvious than the control group; the change of group B with higher dose was less obvious than group A.CD34 staining showed that regeneration of neovascularization at the margin of myocaardium infarction was seen in all of three groups;for the MVC/MVD,medical groups were significantly higher than the control group,while group B is significantly higher than group A (all P <0.01).Conclusion AST can improve myocardial ischemia of rats after myocardial infarction.AST can promote angiogenesis in ischemic myocardium of rats,and the effect is positively correlated with AST dose.
6.The standardized perioperative treatment of chronic rhinosinusitis with nasal polyps and asthma.

OBJECTIVE:
To discuss the perioperative treatment of chronic rhinosinusitis with nasal polyps (CRSwNP) and asthma.
METHOD:
Retrospective analysis of perioperative clinical data of 43 cases with CRSwNP and asthma. The admitted and under endoscopic surgery. Patients with preventing perioperative asthma attacks and corresponding standardized treatment were Observed.
RESULT:
Thirty-five cases were stable during perioperative period and without asthma. Seven patients diagnosed as mild and moderate asthma attacks because of low pulse oximetry (SpO2 92%-95%) and scattered wheeze heard in the lungs. So these patients were sent to ICU for the treatment. They went back to ward after their conditions turned to stable and no asthma during perioperative. One patient diagnosed as severe asthma attack, because irritability and suffocation happened, SpO2 decreased from 99% to 84%-81%, diffuse wheeze could be heard in the whole lung . So we give him tracheal intubation and sent him to ICU for advanced treatment after breathing smooth. Five days later the patient retuned to the ward in stable condition and with no asthma attack again.
CONCLUSION
Before operation the patients should be give some corresponding standardized comprehensive treatment according to the nasal symptoms and the degree of asthma attack, such as the application of topical steroid and antiallergic medicine. And some special treatment should be given to reduce airway hyperresponsiveness mucosa during anesthesia. These methods can reduce the risk of the asthma attacks and improve perioperative safety, prevent serious complications.

7.Compliance with early Warfarin anticoagulation therapy after mechanical valve replacement and its influencing factors in elderly patients

Objective:To investigate the compliance with early Warfarin anticoagulation therapy after cardiac mechanical valve replacement(MHVR)and its related factors in elderly patients.Methods:The convenience sampling method was used to prospectively recruit 210 patients undergone MHVR at the Ningbo Medical Center of Lihuili Hospital from January 2017 to October 2019.Six months after discharge, face-to-face interviews or telephone follow-ups were conducted to assess general information, warfarin anticoagulation knowledge, anticoagulant treatment compliance and social support.Results:The overall compliance of early Warfarin anticoagulation therapy was excellent, with 99.5% of patients compliant with medication and 99.0% compliant with INR monitoring, both higher than the rate of compliance with advised lifestyle adjustment(92.1%). Anticoagulation knowledge and age were the main influencing factors for compliance in elderly patients after MHVR.Conclusions:The compliance with early Warfarin anticoagulation therapy after MHVR is good in elderly patients in the Ningbo area.The correlation analysis suggests that medical professionals need to promote education on anticoagulation knowledge and pay more attention to anticoagulation compliance in elderly people.
8. A discussion on the practice, effect and proposal of proficiency testing program for pharmaceutical dissolution determination

OBJECTIVE: To discuss the role of proficiency testing program for pharmaceutical dissolution determination in the capacity building of relevant laboratories. METHODS Based on the results of the two proficiency testing programs carried out in recent years, analyze the difference of the quality management and the level of dissolution determination among the participating laboratories and discuss on the way that the proficiency testing programs carried out in future. RESULT: There is difference among the laboratories in both the quality management and the level of dissolution determination, the "unsatisfactory" laboratories mainly concentrated in local drug control institutes and pharmaceutical manufacturers. The three "unsatisfactory" laboratories in the first program, took corrective and preventive measures, participated in the later program, and achieved "satisfactory" results. CONCLUSION: The proficiency testing program is effective in improving the dissolution determination capacity of the participating laboratories and is recommended to be conducted continually in the pharmaceutical industry.
9.Effect of patient-controlled intravenous analgesia with morphine and fentanyl after cardiac surgery

ObjectiveTo observe the efficacy and safety of patient-controlled intravenous analgesia (PCIA) with morphine and fentanyl after cardiac surgery.MethodsSeventy patients operated with cardiac surgery were randomly divided into morphine group (group M) and fentanyl group (group F). The beginning efficacy time of analgesia,efficacy of analgesia,patient's evaluation,heart rate,respiratory rate,mean arterial pressure,and incidence of nausea and vomiting were assessed.ResultsThere were no significant differences in efficacy and patient's evaluation between two groups. In group G,the beginning efficacy time of analgesia was significantly shorter than those in group M (P<0.05),and the times of nausea and vomiting were significant less than those in group M (P<0.05).ConclusionPCIA with fentanyl and morphine for postoperative pain relief after cardiac surgery is efficient and safe. Compared with morphine,the beginning efficacy time of fentanyl is significant shorter,and times of nausea and vomiting are little.
10.Endoscopic endonasal approach resection for anterior skull base, pterygopalatine fossa and infratemporal fossa tumors: retrospective analysis of 73 patients.

OBJECTIVE:
To summarize the clinical experience with endoscopic transnasal resection of nasal skull- base neoplasms, which involved anterior skull base, pterygopalatine fossa, nfratemporal fossa.
METHOD:
Clinical data from 73 patients performed on endoscopic transnasal resection of nasal skull-base neoplasms were analyzed retrospectively.
RESULT:
Total tumor removal was obtained in 54 cases, subtotal removal in 19 cases. In 16 cases of benign tumor, the postoperative survival rate was 100%; Malignancy in 57 cases, of which 16 patients were died, and half-year survival rate was 71.9%.
CONCLUSION
Endoscopic endonasal approach be able to fully reveal and re- moval of lesions involving the anterior skull base, pterygopalatine fossa and infratemporal fossa. The approach is feasible and safe.
