China Journal of Chinese Materia Medica 2013;38(23):4181-4184
As a characteristic tumor therapy in China, Chinese medicine (CM) plays an important position in comprehensive treatment of tumor. It's a critical issue of objective realization, analysis and evaluation of CM safety for scientific decision-making in tumor safe medication and it also is a pivotal issue which affects the international communication. The safety evaluation of CM includes three phases: pre-clinical safety evaluation, clinical trials (micro-dose studies and traditional clinical trials) and post-marketing CM safety assessment. The key point of evaluation should be distinguished among different stages and various types of CM (such as classic formulas, Chinese herbal extracts, etc). Emphasis should be given to chronic toxicity when evaluating oral Chinese herbal , microdose studies and quality control must be underlined while injection is evaluated and more attention should be pay to the dose-effect relationship and time-effect relationship when turned to toxic Chinese medicine , and so as for the toxicity grading study. Moreover, we should constantly improve CM safety assessment method in various stages of tumor treatment, such as introducing the concept of syndrome classification theory, bringing in metabonomics and real-world research method which are similar to the CM therapeutic concept. Most importantly, we must keep its own feature of CM theory when we learn the concept of safety evaluation from abroad. Actively exploring the anti-tumor medicine safety evaluation methods and strategies is of great significance for clinical and experimental research, and it can provide supportability platform to CM's international communication.

Journal of Jilin University(Medicine Edition) 2014;(6):1197-1200
Objective To explore the effects of trimetazidine on myocardial free radical inj ury induced by pirarubicin, and to clarify the protective effect and mechanism of trimetazidine on myocardial inj ury induced by pirarubicin. Methods 3 6 Wistar rats were randomly divided into pirarubicin group (n= 1 3 ), trimetazidine intervention group(n=13)and control group (n=10).The rats in pirarubicin group and trimetazidine intervention group were inj ected with pirarubicin 2.5 mg · kg-1 by the vena caudal once a week for 6 weeks. The rats in trimetazidine intervention group were intragastricly infused with trimetazidine 5.4 mg · kg-1 · d-1 one day for 8 weeks before making the model. At the end of the experiment, the malonaldehyde (MDA)level, nitrogen oxide (NO)level, superoxide dismutase(SOD)activity, and nonprotein sulfhydryl (NPSH)level in myocardium tissue were measured. The histological changes of myocardium tissue were detected by electron microscope. Results Compared with control group ,the levels of MDA and NO in pirarubicin group were increased(P<0.05), and the SOD activity and NPSH level in pirarubicin group were decreased(P<0.05).Compared with pirarubicin group, the levels of MDA and NO in trimetazidine intervention group were decreased(P<0.05),the SOD activity and NPSH level in trimetazidine intervention group were increased(P<0.05).Under electron microscope, the myocardiocytes of the rats in pirarubicin group showed irregular arrangement in sacromere structure, shrinkage in nuclear membrane, vacuolation in nuclear matrix, obvious mitochondria swelling, deposition of metachromatin throughout the nucleus, and an indistinct view of intercalated disc with isolation;while in trimetazidine intervention group the nucleus was round and nuclear membrane was indented, myofilament bundles were decreased slightly with a regular arrangement, intercalated disc oriented transversely with partial vague in cell j unction structure, and mitochondria slightly swelled. Conclusion Trimetazidine has the protective effects on the damaged myocardiocytes caused by pirarubicin, and its mechanism may be related to reducing the production of free radicals and decreasing the injury of structures within the cells, such as the nucleus, mitochondria and intercalated disc.

Chinese Journal of Clinical Oncology 2016;43(13):585-588
Small bowel adenocarcinoma (SBA) is a relatively uncommon neoplasm with poor prognosis. However, the incidence rate of this condition increases. SBA is usually diagnosed at the late stages, and the majority of patients present with the advanced stage. Data are limited when making decisions for treatment because of the lack of randomized trials for SBA. Radical surgery is considered necessary when possible. Adjuvant chemotherapy is predicted to be beneficial, but this procedure has not yet been investigated through randomized trials. Platinum-based chemotherapy is apparently the most effective treatment regimen used in retrospective tri-als for advanced SBA. Targeted therapies, such as those against the angiogenetic pathway or the epidermal growth factor receptor pathway, have not yet been established for this type of cancer. This article reviews the progress in the diagnosis and treatment of SBA.
